Abstract

This research describes the lives of the people around the buffer village of Alas Purwo National Park, Kalipait Village. Traditional communities have local wisdom in the form of a number of traditions, rules and restrictions that still apply for generations. This local wisdom has the value of ecological intelligence that is maintained and developed about the relationship of human activities with its ecosystems. Local wisdom owned by traditional communities is used as a reference in the management of forest areas and coastal waters, both in the form of myths and abstinence. The center of attention of ecological studies according to Julian Steward is the process of cultural adaptation to the environment, by instilling values contained in local culture. This process is seen as a form of dialectical relationship in the context of inter-influencing relationships with others. The type of research used is qualitative descriptive with case study design. Analysis of social change will be adapted to Talcott Parsons' AGIL theory. The results showed that, the community around the Alas Purwo National Park area has local wisdom in the form of a number of traditions in the form of rules, abstinence that still applies for generations which is then maintained and adhered to until now. The abstinence is in the form of a ban on killing peacocks and abstinence in the payang system. The ecological value contained in local wisdom can help human awareness in environmental management so as to shape ecological attitudes.  Keywords: Local Wisdom, Conservation, Alas Purwo National Park AbstrakAbstrakPenelitian ini menggambarkan kehidupan masyarakat sekitar desa penyangga Taman Nasional Alas Purwo, Desa Kalipait.

Abstract

The patron-client relationships were based on unequal exchange, reflecting differences in status. A client (santri), receiving many services from patrons (kiai) so that the client is bound and dependent on patrons. The patron-client relationship is personal. The pattern of the personal reciprocity between kiai and santri create a sense of trust and reliance on the mechanism of the relationship. It can be seen from a culture of respect santri to kiai who tend to be cult. Patron spread thorough, flexible and limitless time frame. The patron-client relationship is put kiai at a high position, influential and authoritative in front of santri. Relation to relation kiai and santri, the santri should be obedient to the kiai. Keywords: Relationships, Patron Klien, Pesantren

Abstract

The severity of diseases, the complexity of treatment, and the use of medical devices in the intensive care unit (ICU) may change the pharmacokinetics (PK) profile of antibiotics among critically ill patients. This narrative review aims to explain the PK profile of critically ill patients compared to other group of patients and to describe the pharmacokinetic-pharmacidynamic (PK-PD) target attainment among this group of patients. Only articles published less than 10 years ago were included in this narrative review. Evidences have indicated that critically ill patients have relatively larger volume distribution (Vd) of hydrophilic antibiotics compared to patients with stable conditions. The fluid shifting to interstitial space, hypoalbuminemia, and aggressive fluid treatment may contribute to the increase value of Vd in critically ill patients. The clearance (CL) of hydrophilic antibiotics in critically ill patients is highly determined by dynamic changing of renal function compared to patients in other wards. The phenomenon of augmented renal clearance and the use of high intensity of renal replacement therapy can increase the CL of hydrophilic antibiotics. The different PK profile of antibiotics may lead to the failure of attaining the PK-PD target if the dose of antibiotics is not adjusted according to such differences

Abstract

Instruments which can be used to identify factors affecting the medication adherence of elderly with diabetes mellitus (DM) have not been discovered in Indonesia. The objective of this study was to develop a questionnaire to identify factors affecting elderly patients’ medication adherence. The development of the questionnaire was conducted based on published references attained through the exploration of research evidence with several keywords and databases. Content validity was conducted using experts’ assessment (two experts). Face validity was conducted in 13 subjects including seven (7) non-elderly subjects and six (6) elderly subjects. Construct validity test was conducted based on calculated R against table R while reliability test was conducted with Cronbach’s Alpha. There were 30 elderly subjects included in the construct validity. Present study produced a valid and reliable questionnaire comprising 35 questions in 14 subcategories from 4 factor domains with the calculated R = 0.357-0.788 and Cronbach’s Alpha = 0.934. The questionnaire can be used for further research throughout Indonesia. Hence, a complete overview of factors affecting elderly DM patients’ medicine consumption adherence can be obtained. The identification of factors related to the usage of DM patients’ medicine is expected to serve as a base for the government, both of state and local government, to produce policies related to health interventions to be implemented.

Abstract

INTRODUCTION: The main treatment modality for squamous cell carcinoma of the head and neck is by surgical wide excision. Post-excision reconstructive surgery is essential for restoring aesthetics and function. Fasciocutaneous free-flap is an acceptable method for reconstructing a wide lession of the face since the technique does not cause significant morbidity.CASE REPORT: A 41 year-old female was diagnosed with squamous cell carcinoma (T3N0M0) of the inferior lip after achieving tumor-free margin radical excision. Reconstruction of the defect was made with radial forearm freelap followed by a three-phase refinement flap. The patient was left with a aesthetically acceptable lip without other facial morbidity.DISCUSSION: Reconstructive surgery following a widely excised tumor of the lips requires special attention because of its important physical and aesthetical role. Radial forearm freeflap causes minimal facial morbidity since the technique does not require donor tissue from the face. Other benefits include adequate tissue thickness, minimal contracture and unaltering skin color. Further refinement flaps is needed to complete the reconstruction process.CONCLUSION: Radial forearm free flap is a good choice for reconstruction a wide lession following a wide excision of lip tumor.

Abstract

The Covid-19 pandemic had a dangerous impact on early-childhood education, lost learning in almost all aspects of child development. The house-to-house learning, with the name Moving Home Learning Program (MHLP), is an attractive offer as an emergency remote teaching solution. This study aims to describe the application of MHLP designed by early-childhood education institutions during the learning process at home. This study used a qualitative approach with data collection using interviews, observation, and documentation. The respondents involved in the interview were a kindergarten principal and four teachers. The research data were analyzed using the data content analysis. The Findings show that the MHLP has proven to be sufficiently in line with the learning needs of early childhood during the Covid-19 pandemic. Although, the application of the MHLP learning model has limitations such as the distance from the house that is far away, the number of meetings that are only once a week, the number of food and toy sellers passing by, disturbing children's concentration, and the risk of damage to goods at home. The implication of this research can be the basis for evaluating MHLP as an adaptive strategy that requires the attention of related parties, including policy makers, school principals, and teachers for the development of new, more effective online learning models.
