Abstract

Choosing a location for delivery by pregnant women and deliveries assisted by skilled health workers are important keys in reducing MMR and IMR. The low birth rate at health facilities in Mandrehe Utara Subdistrict, Nias Barat Regency is a serious challenge to achieve the success of the maternal and child health service program. This study analyzes the factors that influence pregnant women who do not give birth in health facilities. This study uses a quantitative method with a cross-sectional approach. The samples were 180 people. The data were collected using a questionnaire and then tested using the Chi-Square test and logistic regression test. The results showed that the reason why mothers did not choose to give birth in health facilities was influenced by knowledge (p=0.022), attitude toward birth attendants (p=0.002), perception of distance (p=0.004), husband and family support (p=0.000), and access to health information (p=0.007). The role of health workers had no significant effect in this study (p=0.756). The results of the logistic regression test showed that husband and family support was the most dominant variable affecting mothers who did not give birth in health facilities. Mothers who perceive the lack of support from their husbands and families are 5,668 times more to choose to give birth outside a health facility. Education and socialization by health workers to mothers and their husbands and families are suggested to increase knowledge, awareness and change attitudes. No less important are the government's efforts to provide health facilities and infrastructure development to reduce access difficulties.Pemilihan lokasi persalinan oleh ibu hamil dan persalinan yang ditolong tenaga kesehatan terampil menjadi kunci penting dalam menurunkan AKI dan AKB. Rendahnya angka persalinan di fasiltas kesehatan di Kecamatan Mandrehe Utara Kabupaten Nias Barat menjadi tantangan berat untuk mencapai keberhasilan program pelayanan kesehatan ibu dan anak. Studi ini bertujuan menganalisis faktor-faktor yang mempengaruhi ibu hamil tidak bersalin di fasilitas kesehatan. Studi ini menggunakan metode kuantitatif dengan pendekatan cross-sectional. Sampel yang terlibat sebanyak 180 orang. Pengumpulan data menggunakan kuesioner dan kemudian diuji dengan menggunakan uji Chi-Square dan uji regresi logistik. Hasil penelitian menujukkan penyebab ibu tidak memilih bersalin di fasilitas kesehatan dipengaruhi oleh pengetahuan (p=0,022), sikap terhadap penolong persalinan (p=0,002), persepsi tentang jarak (p=0,004), dukungan suami dan keluarga (p=0,000), dan akses terhadap informasi kesehatan (p=0,007). Peran petugas kesehatan tidak berpengaruh signifikan dalam studi ini (p=0,756). Hasil uji regresi logistik menunjukkan dukungan suami dan keluarga merupakan variabel yang paling dominan mempengaruhi ibu tidak melahirkan di fasilitas kesehatan. Ibu yang menganggap kurangnya dukungan suami dan keluarga berpeluang 5,668 kali memilih melahirkan di luar fasilitas kesehatan. Edukasi dan sosialisasi oleh tenaga kesehatan terhadap ibu beserta suami dan keluarga disarankan untuk peningkatan pengetahuan, kesadaran dan perubahan sikap. Tidak kalah penting upaya dari pemerintah untuk penyediaan fasilitas kesehatan dan pembangunan infrastruktur guna memangkas kesulitas akses.

Abstract

The incidence of cancer is still high in the world, regionally and in Indonesia, where the incidence of new cases of breast cancer, uterine cervical cancer and lung cancer is the highest in Indonesia. The use of anthracycline doxorubicin as an anti-neoplastic agent is highly effective in various types of cancer in adults and children, including hematologic and solid tumors, but doxorubicin has serious organotoxicity, including pancreatic toxicity. The content of various compounds from Balakka fruit extract (Phyllanthus emblica L.) which has been identified through various studies, especially its potential as a pancreatic protective agent, through its antioxidant, anti-diabetic/hyperglycemic, and anti-hyperlipidemic activities. To determine the potential protective effect of ethanolic extract of Balakka fruit (Phyllanthus emblica L.) against doxorubicin-induced pancreatic toxicity in rats. This study used an experimental study with a completely randomized design to determine the potential protective effect of the ethanolic extract of Balakka fruit (Phyllanthus Emblica L.) against doxorubicin-induced pancreatic toxicity in rats (Rattus norvegicus). The results of qualitative phytochemical tests on the ethanolic extract of Balakka fruit contain alkaloids, flavonoids, phenols, steroids/triterpenoids, terpenoids and tannins. From the results of the different test results in the mean serial blood glucose levels more than 2 Kruskal Wallis groups, a significance value of p less than 0.05 was found, meaning that there were at least 2 groups that had a significant difference in fasting glucose levels (p = 0.003*) and TTGO test results at the 30th minute, 60th minute and 120th minute (p = 0.000*; p = 0.001*; p = 0.007*). The average fasting blood glucose levels of all treatment groups were in the normal range of 90 – 110 mg/dL. At the 120th minute of OGTT, it was found that the mean blood glucose level in the doxorubicin group reached 149.00 ± 7.99 mg/dL, still higher than the average level of the extract group, treatments 1 and 2 (p less than 0.05). And the mean blood glucose levels in the treatment groups 1 and 2 were significantly lower than the mean blood glucose levels in the doxorubicin group (p = 0.009*; p = 0.009*).The ethanolic extract of Balakka Phyllanthus Emblica L. fruit contains flavonoids, phenols, tannins, triterpenoids, terpenoids and alkaloids. Administration of ethanolic extract of Balakka fruit at a dose of 400 mg/kgBW can overcome pancreatic toxicity by maintaining a balance of glucose levels by decreasing blood glucose levels at the 30th minute, 60th minute, and 120th minute close to normal and can improve the pancreatic tissue of rats in a group of mice with doxorubicin-induced hepatotoxicity.

Abstract

Measurement of the service quality is very important, especially to improve service quality and get loyal customers in a private hospital using a lean service approach. This research aims to determine the effect of a lean service approach based on variables including overproduction, waiting time, transportation, excess processing, inventory, motion and satisfaction defects. This study used a quantitative method approach with a cross-sectional design. The population were treated in the inpatient ward of Malahayati Hospital, Medan. The sample size was 110 patients, determined by Rao formula. The sampling technique was a purposive sampling technique with inclusion criteria of patients who were hospitalized for more than three days on March 2021. Data obtained were analyzed with the logistic regression tests. The results showed all variable have significant effect on the inpatient satisfaction including over production (p = 0.019), followed by waiting time (p = 0.012), transportation (p = 0.011), excess processing (p = 0.017), inventory (p = 0.010), motion (p = 0.015), and satisfaction defects (p = 0.010). Based on multivariate analysis concluded that hospital service quality affects inpatient satisfaction based on the waste inventory (4.567 times) as the highest and waste transportation (2.786 times) as the lowest. This study has strongly suggested that the hospital management should maintain over production waste and other waste variables in order to improve the service quality.

Abstract

Nurses play an important role in the administration of nursing services because nurses are the main key to the organization. However, there are still many roles and functions of nurses that have not been going well. The aim of this research is to determine the relationship between the management function of nurses and the work function of executive nurses in non-surgical inpatient care at Efarina Etaham Hospital. Type of quantitative research with a cross sectional approach. The study sample was 66 executive nurses in a non-surgical inpatient hospital at Efarina Etaham Hospital. The data was analyzed using the Spearmen's Rank test. The results of the study showed that there was a significant relationship between the management function of nurses and the work function of executive nurses in non-surgical inpatient care at Efarina Etaham Hospital, p value = 0.000. The level of strength of the relationship between the variables Nurse Management Function and Executive Nurse Work Function is 0.716 or strong.  The results of this research can be used as a source of information and analytical reports that can be useful for hospital management in terms of improving nurses' work functions. The results of this research are used as input and consideration for Efarina Etaham Hospital in making decisions, especially in addressing management function issues so that Efarina Etaham Hospital continues to develop and can carry out management functions well. Future research should direct research with a larger survey scope with more in-depth interviews or experiments and with a larger sample, so that the causal factors can be identified. Keyword: Function management, Nurse work

Abstract

Tenaga kesehatan yang bekerja di rumah sakit memiliki risiko tertinggi untuk terpajan dan terinfeksi oleh virus SARS-CoV-2 selama pandemi COVID-19. Hal ini tercermin dari jumlah tenaga kesehatan yang terinfeksi. Oleh karena itu, dibutuhkan perilaku pencegahan infeksi yang baik ketika bekerja, termasuk dalam hal pengetahuan dan sikap. Studi ini bertujuan mengetahui faktor-faktor yang mempengaruhi perilaku pencegahan infeksi oleh tenaga kesehatan (nakes) ketika bekerja dan membandingkan pengetahuan, sikap dan perilaku pencegahan infeksi antara nakes di ruang rawat inap kelas III dan ruang isolasi COVID-19 di Rumah Sakit (RS) Universitas Royal Prima Indonesia. Studi cross-sectional komparatif dengan total sampling dilakukan pada 30 nakes dan 40 nakes ruang isolasi COVID-19. Data diperoleh melalui kuesioner yang diberikan melalui Google form dan data sekunder dari manajemen RS. Uji korelasi Spearman dan Mann-Whitney U digunakan untuk analisis statistik. Hasil studi menunjukkan terdapat hubungan signifikan antara perilaku terhadap COVID-19 dengan kualitas alat pelindung diri (APD) (r = 0,698) dan pelatihan APD (r = 0,698) pada nakes di ruang inap isolasi. Terdapat perbedaan perilaku (p = 0,22) dan pengetahuan (p = 0,00) terhadap COVID-19 antara nakes ruang inap kelas III dengan nakes ruang inap isolasi. Kesimpulan perilaku dan pengetahuan nakes ruang isolasi COVID-19 berbeda dengan nakes di ruang rawat inap kelas. Perilaku nakes di ruang isolasi COVID-19 berhubungan dengan kualitas dan pelatihan penggunaan APD. Disarankan terdapat monitoring ketersediaan APD berkualitas dan pelatihan penggunaan APD untuk meningkatkan perilaku pencegahan infeksi.

Abstract

This study aims to determine the effect of a doctor's explanation and duration of drug intake on BPJS patient loyalty, with patient satisfaction as an intervening variable. The research method used cross-sectional. The results showed that there was an effect of doctor's explanation on BPJS patient loyalty at RSU Royal Prima Medan P value <0.05, there was an effect of drug intake duration on BPJS patient loyalty at RSU Royal Prima Medan P value <0.05, there was an effect of doctor's explanation on patient satisfaction, there was an effect of drug intake duration on patient satisfaction, there was an effect of patient satisfaction mediating doctor's explanation on patient loyalty, there was an effect of patient satisfaction mediating drug intake duration on patient loyalty, and there was an effect of patient satisfaction on patient loyalty. Abstract

This study aims to analyze the factors that support the readiness to implement electronic medical records at the Royal Prima Medan General Hospital. The research method uses cross-sectional. The results of this study indicate that there is an influence of the perception of benefits on the readiness of the implementation of electronic medical records at the Royal Prima Medan Hospital with a p-value = 0.041, there is an influence of behavioral interest factors using the behavioral attention hospital management information system on the readiness for the implementation of electronic medical records at the Royal Prima Medan Hospital with a p-value = 0.000, there is an influence of the use of the hospital management information system on the readiness of submitting electronic medical records at the Royal Prima Medan Hospital with a p-value = 0.016. The conclusion is that there is an influence of the perception of benefits, behavioral interest, and use of information systems on the implementation of electronic medical records is a behavioral variable. Abstract

This study aims to determine the role of hospitals in preventing and controlling infections at Royal Prima Indonesia Hospital. The research method uses a cross-sectional study. The study results showed that the chi-square statistical test analysis of the role of hospitals in preventing and controlling infections at Royal Prima Medan Hospital showed a p-value = 0.283, which means the p-value> 0.05. In conclusion, there is no relationship between the hospital's role and the PPI team's compliance in preventing and controlling infections. Suggestions should be made so the hospital can provide ongoing training and education to improve compliance with standard precautions every three months. Abstract

This study aims to see the effect of animation media screening counseling on knowledge and attitudes about healthy snacks as an effort to prevent diarrhea in school-age children. The research method is quantitative with a pre-experimental design with one group pre-and post-test. Data were collected using observation sheets. Data were analyzed using the Wilcoxon test. The results showed that the mean knowledge score about healthy snacks before and after animation media screening counseling was 20.27 ± 1.461 and 23.07 ± 0.740. The mean score of attitudes about healthy snacks before and after animation media screening counseling was 38.43 ± 2.648 and 43.83 ± 2.291. The conclusion of this study is that animation media screening counseling has an effect on knowledge and attitudes about healthy snacks as an effort to prevent diarrhea in school-age children. Abstract

This study aimed to see the relationship between service quality and the accreditation status of the Health Center. The research method used a cross-sectional study. Data were collected using a questionnaire sheet. The collected data were analyzed using the Chi-Square test. The study's results showed a significant relationship between the service quality dimensions (responsiveness, assurance, tangible, empathy, and reliability) and the accreditation status of the health center. This study concludes a relationship between responsiveness, assurance, tangible, empathy, and reliability with accreditation status. Keywords: Accreditation, Service Quality, Health Center
