Abstract

Health, as a human right, requires fair, equitable, and quality services for the entire community. The Indonesian government is committed to improving health by providing facilities, including hospitals. The Health Law emphasizes the right of every individual to access safe and affordable health care. Hospitals play a strategic role in improving public health, but have evolved into "business" institutions with a more focused approach. Modern healthcare requires a new paradigm, with hospitals actively engaged in marketing communication and promotion. Integrated Marketing Communication (IMC) becomes a crucial approach to achieving promotional goals and enhancing the hospital's publicity. Patient visit data at RS Efarina Etaham Berastagi for the period 2019-2021 shows fluctuations influenced by factors such as the COVID-19 pandemic. This study proposes the implementation of IMC at RS Efarina Etaham Berastagi, considering the characteristics of internal and external informants. The research method used is qualitative descriptive, combining in-depth interviews, observations, and secondary data analysis. The results indicate that RS Efarina Etaham Berastagi has implemented some elements of IMC, but there are still internal and external challenges to overcome. By strengthening IMC strategies, it is expected that RS Efarina Etaham Berastagi can be more effective in reaching the community, building a positive image, and increasing the number of inpatient visits. This research provides insights for hospital management and stakeholders to optimize marketing strategies to achieve better public health goals.

Abstract

The most common long-term diabetic complication is diabetic nephropathy which, in most cases, falls into the End-Stage Kidney Disease. Numerous studies on the nephroprotective effects of citrus fruit have been conducted recently. This study was performed to investigate the phytochemical properties of Sunkist peel ethanol extract and their possible effects on improving diabetic nephropathy. To get the extract, the Sunkist peels were macerated and the extract was analyzed by GC-MS. An in vivo assay of five different groups of 25 male Wistar rats was then performed using the extracts. The groups included control, standard, 500 mg/kg extract, 750 mg/kg extract, and 1,500 mg/kg BW extract. All rats were sacrificed after 28 days of treatment and VEGF level serum and kidney histology analysis were performed. The VEGF data were first analyzed descriptively, followed by one-way ANOVA or Kruskal-Wallis test based on the data distribution. Meanwhile, the kidney histology was observed and narratively described. Results showed that the Sunkist peel ethanol extract has a potential effect to suppress the VEGF level (p-value < 0.05). The extract at the highest dose revealed the mildest pathology damage in the form of tubular structure degeneration without any glomerular damage. This study indicates that Sunkist peel extract has nephroprotective effects from its various phytochemical compounds that work as antioxidants and anti-inflammation.

Abstract

Background: The excessive use of chemotherapy drugs such as doxorubicin might induce cardiovascular diseases. Heart-specific biomarker enzymes such as LDH and BNP indicated the early signs of cardiotoxicity. However, there is no report on the effect of bay leaf on LDH and BNP nor its bioactive compounds and histopathology analysis. Objective: This study aims to determine the cardioprotective effect of Ethanol Extract of Bay Leaf (EEBL) based on phytochemical analysis, LDH and BNP levels, and cardiac histology of rats administered with doxorubicin. Methods: About 24 Wistar rats were divided into six groups treated with 100 mg/kg BW, 300 mg/kg BW, and 500 mg/kg BW of EEBL with CMC-Na 0.5 %, vitamin E, and doxorubicin. Preconditioning was 14 days, followed by 21 days of intraperitoneal administration. LDH and BNP parameters were measured on days 1st, 7th, 14th, and 20th. Histology analysis was conducted on day 21st.Results: Dark green EEBL showed sufficient physical quality and properties for practical application in pharmacy. EEBL has significantly affected LDH, BNP, and cardio recovery dose-dependent. The most effective doses were observed at 500 mg/kg BW, and its performance is not statistically different from those of the commercial vitamin E. The histopathological images revealed significant improvements in interstitial edema, wavy fibers, hemorrhage, intracellular vacuole, and inflammatory cell infiltration. Here we report six bioactive compounds obtained from EEBL have cardioprotective effects; Neophytadiene, squalane, phytol, methyl palmitate, stigmasterol, and  9,12-Octadecanoic acid methyl ester. Conclusion: This study has shown the promising potential of bay leaf extract as a cardioprotector with sufficient quality pharmacological standards.

Abstract

Diabetes mellitus (DM) is a significant global health issue, with its prevalence continuing to rise in Indonesia. Self-care is a key component in managing DM, yet many patients face various obstacles in its implementation. This study aims to deeply explore the challenges faced by individuals with type 2 diabetes in managing their condition independently. Using a descriptive qualitative approach, the study analyzes the experiences of seven diabetes patients receiving care at Royal Prima Hospital. The social ecological model serves as the analytical framework to identify factors influencing self-care behavior, including individual internal factors, interpersonal relationships, healthcare systems, and the social environment. Data was collected through in-depth interviews and analyzed using thematic content analysis. The findings reveal several barriers to effective self-care in diabetes management. Firstly, at the individual level, a lack of knowledge, limited resources, non-adherence to medication, and financial constraints were major obstacles. Secondly, insufficient social support and misinformation about alternative treatments also influenced patient behavior. Thirdly, at the healthcare service level, long waiting times and inadequate social support were identified as challenges. Lastly, limited accessibility due to transportation issues and community resource constraints further hindered self-care. Patients require comprehensive education, easy access to medical tools, and psychological support to manage their condition effectively. Families and communities must be equipped with accurate knowledge about diabetes and trained to support patients. Healthcare providers should improve service quality by adopting a holistic approach and collaborating with communities to ensure optimal care. With collective efforts, it is hoped that the quality of life for diabetes patients can improve, and complications resulting from the disease can be prevented.

Abstract

In the National Health Insurance (JKN) era, hospitals in Indonesia rapidly grew in number and also developed.  In order to stand out amongst the competition, hospitals often rely on marketing strategies through attractive advertisements, offering unique services, and also introducing their services to the public. One method often utilized in the 5P marketing mix consists of product, price, place, promotion, and people. This study aims to identify the impact of 5P marketing mix strategies on the patients’ interest in visiting Bandung Hospital in the city of Medan. Bandung Hospital in Medan conducted this quantitative study using a cross-sectional approach. This study involved 150 patients of Bandung Hospital’s inpatient unit, which is the whole outpatient clinic patient (total sampling) during the May 2023 period. Analysis was carried out using frequency distribution, chi-square, and binomial logistic regression analysis. This study found that the majority of patients visiting Bandung Hospital belong to the 30-39 years age group (62.0%), are female (58.7%), hold a bachelor's degree (48.0%), and are visiting Bandung Hospital for the first time (62.7%). Most of the patients in this study have a good perception toward all aspects of 5P marketing strategies in Bandung Hospital and are interested in visiting the hospital again in the future (p<0.05), with place playing the biggest role in influencing patients’ interest (OR = 4.082). According to all findings in this study, it can be concluded that marketing strategies such as the 5P marketing mix strategy positively influence patients’ interest in visiting Bandung Hospital. It can also be inferred that this strategy would work in similar types of hospitals, not only in Bandung Hospital. Keywords: Marketing mix, Hospital, Product-Price-Place-Promotion-People (5P), Patients
