Abstract

Background: Colorectal cancer (CRC) is a major health burden globally. The prognosis of CRC is strongly influenced by the presence of lymphovascular invasion (LVI) and lymph node (LN) metastasis. Transforming growth factor-beta (TGF-β) is a cytokine with a complex role in CRC progression. This study aimed to evaluate the significance of TGF-β expression in predicting LVI and LN metastasis in CRC. Methods: This cross-sectional study involved 50 patients diagnosed with CRC. The expression of TGF-β was assessed using immunohistochemical staining and the Allred scoring system. The relationship between TGF-β expression and the presence of LVI and LN metastasis was analyzed using the Chi-square test. Results: High TGF-β expression was significantly associated with both LVI (p = 0.011) and LN metastasis (p = 0.012) in CRC. Patients with high TGF-β expression had a higher risk of LVI and LN metastasis compared to those with low TGF-β expression. Conclusion: TGF-β expression is a significant predictor of LVI and LN metastasis in CRC. This finding has potential implications for risk stratification and treatment decisions in CRC patients.

Abstract

According to Globocan data, in 2022, cervical cancer caused 348,874 deaths among women, and cases of cervical cancer in Indonesia increased to 36,633 people. WHO recommends early detection through the VIA (Visual Inspection with Acetic Acid) examination. However, the VIA examination coverage in Kampar Regency in 2023 was low, at only 1.2%. This study aims to analyze the Acetic Acid Visual Inspection (VIA) program for women of reproductive age (WRA) based on healthcare service accessibility in Kampar Regency, Riau Province, in 2024. This research uses a cross-sectional method, and the results show that information about VIA services is lacking, the acceptance of VIA services is low, service hours are not always available, and community empowerment in supporting VIA services is insufficient. The factors that most influence the participation of women of reproductive age in the VIA examination, based on healthcare service accessibility, are service acceptance (p=0.008 and exp(B)= 19.342) and community empowerment (p=0.017 and exp(B) = 8.272). Variables such as service location distance, knowledge, attitude, transportation ease, and environmental support did not significantly affect the situation. It is recommended that service acceptance be improved through promotion, education, and service socialization, as well as strengthened community empowerment by involving local health workers, the Family Welfare Movement (PKK), and related cross-sector stakeholders in an integrated manner.

Abstract

Improving effective communication is the second point in patient safety targets. Based on initial data, effective communication at RSU X Padang City in its implementation has not reached the target of 100% and the weakness of this hospital is that risk management has not been implemented properly. The purpose of this study was to analyze the implementation of effective communication by strengthening risk management in the inpatient ward of RSU X Padang City. This study used a qualitative research design with a phenomenological approach. Researchers conducted semi-structured interviews with 16 informants, document reviews, and observations. The results of the study showed that the role of HR factors in effective communication was not optimal, cooperation within the unit was quite good, the role of management and the hospital environment was not optimal, the role of risk management was not optimal, and the impact of effective communication could improve the achievement of IMP-RS, facilitate the delivery of patient conditions, and minimize errors in providing therapy or medication to patients so that patient quality and safety are guaranteed. It is recommended that the implementation of effective communication always be improved with improvement efforts, namely completing facilities and infrastructure, implementing socialization and training, monitoring and evaluation, and implementing risk management programs.

Abstract

Background: Family planning is a government programme designed to address population needs and balance demographic pressures. It generally aims to regulate the number of births to reduce the risks associated with pregnancy. Contraception serves as a tool for managing pregnancy and controlling childbirth. Women’s participation in family planning programmes is influenced by both internal and external factors. Methods: This research is an analytical study with a cross-sectional design, conducted at Puskesmas Alai, Kota Padang, with a total sample of 105 married women. The study utilized a questionnaire to assess internal and external factors related to women's involvement in the family planning programme. Data were analyzed using the chi-square test. Results: The results of this research indicate that the number of respondents who use contraception is greater than those who do not. Regarding internal factors, most respondents fall into the risk age group, have a high level of education, are not employed, possess a good level of knowledge, and have 0 to 2 children. For external factors, the majority of respondents receive support from their husbands, and more than half of the respondents have a low economic status. Conclusion: The conclusion of this research is that there is a significant correlation between age, knowledge level, husband's support, and economic status with participation in the family planning programme. However, no significant correlation was found between education, occupation, and the number of children.

Abstract

Background: Abnormal fat accumulation affects the compliance of the respiratory system which contributes to the process of normal lung expansion. Lung function can be evaluated through an examination using spirometry with the parameters assessed, namely FVC, FEV1, and FEV1/FVC. This study aims to determine the relationship between obesity and lungfunction in the spirometry examination of Andalas University Medical Education students. Methods: This study is an observational analytic study with a cross-sectional approach. The Number of samples in this study was 34 subjects. The sampling technique used wasconsecutive sampling from students of Medical Education at the Faculty of Medicine, AndalasUniversity, Class of 2018-2020. Evaluation of lung function was carried out using aspirometry examination with parameters assessed for FVC, FEV1, and FEV1/FVC while the evaluation of obesity was carried out using the BMI with a classification according toKemenkes RI. Data analysis was carried out using univariate and bivariate qualitative methods. Results: In this study, the majority of respondents were women (70.6%) with normal nutritional status (67.6%) followed by obesity (32.4%). The relationship between obesity and lung function was tested using fisher's exact test. The results showed that there was no relationship between obesity and lung function parameters FVC, FEV1, and FEV1/FVC on spirometry tests.

Abstract

Collaboration between health professionals was needed to improve health services. The collaboration can be applied to the education system through the Interprofessional Education (IPE) concept. The purpose of this study was to examine students’ perception of IPE in the Family Oriented Medical Education (FOME) II program. This research is a descriptive study with a cross-sectional design. The research samples were 162 Medical Faculty Universitas Andalas students who took part in the FOME II program in the 2021-2022 academic year. FOME II is a learning activities created by FK Unand with the aim of producing competent health workers with a family approach. There were 142 medical study program students and 20 undergraduate midwifery study program students. Study data was collected by using the Interdisciplinary Education Perception Scale (IEPS) questionnaire.  The vast majority of students(153 students,94.4%) had a good perception on IPE with the evidence of working together’s component having the highest percentage (90.7%) and needing to work together’s component had the lowest percentage (59.9%). Medical Study Program students have the highest percentage of the application of IPE perception (95.7%) while midwifery study program students have the lowest percentage of the application of IPE perception (85%).  Most of FK Unand students’ perceptions of IPE are in the good category. However, the aspect of the need to cooperate is still lacking so this needs to be considered and improved

Abstract

Prevalence of asthma in children increase especially in the industrial city. Cement factory in Padang city is located near from community, cause air pollution, especially to children. We aim to know the prevalence and risk factor of asthma and allergic disease in this area. A cross sectional study to children age 0-15 years old has been done on May-June 2015, around the Cement factory, with distance from factory <5 km (exposed area) and >10 km (un-exposed area). We used International Study of Asthma and Allergy in Childhood (ISAAC) questionnaire to determine asthma and allergic disease. Data was taken from the parent about asthma, allergic rhinitis and atopic dermatitis symptoms and some risk factors. Data was analyzed with chi-square test. This study found 90 children, 43 (47.8%) from exposed area. Asthma prevalence and atopic dermatitis was found higher in un-exposed area than exposed area (19.5% vs 13.9% and 39.1% vs 37.2%), allergic rhinitis was higher in exposed area (60.4% vs 58.6%), though no significance difference. This study concluded that there were no significant differences in asthma and allergies disease in exposed and un-exposed area of semen factory emissions.

Abstract

One of the most important elements in implementing smoking cessation health promotion and being an example for society. This study aimed to obtain in-depth information regarding the implementation of health promotion strategies in reducing smoking behavior and to determine the relationship between knowledge and the role of health tasks with smoking behavior in Airwarm Timur District, Kerinci Regency in 2022. This study applied a mixed method with a combination model design. Sequential Explanatory. The samples in this study were 110 people living in Airwarm Timur District, Kerinci Regency, who were taken using the quata sampling technique and 21 informants who were taken by the purposive sampling technique. The variables in this study were knowledge, motivation and the role of health workers. Data on these variables were analyzed using the regression test, with the result that the most influential factor on smoking behavior was motivation. At the qualitative study stage, advocacy activities, especially health promotion regarding smoking, were not carried out alone for smoking behavior, but also together with other activities such as “clean and healthy lifestyle” program activities, community development, or integrated service posts. The atmosphere development given to community leaders which was carried out to village head staff or village officials and community empowerment activities did not provide specific knowledge about smoking.Keywords: cigarette; knowledge; motivation ABSTRAK Salah satu elemen terpenting dalam pelaksanaan promosi kesehatan berhenti merokok dan menjadi contoh bagi masyarakat. Penelitian ini bertujuan untuk mendapatkan informasi yang mendalam mengenai pelaksanaan strategi promosi kesehatan dalam menurunkan perilaku merokok  dan untuk mengetahui hubungan antara pengetahuan dan peran tugas kesehatan dengan perilaku merokok di Kecamatan Air Hangat Timur Kabupaten Kerinci pada tahun 2022. Penelitian ini menerapkan mixed method dengan desain kombinasi model Sequential Explanatory. Sampel dalam penelitian ini adalah 110 masyarakat yang berdomisili di Kecamatan Air Hangat Timur Kabupaten Kerinci yang diambil dengan teknik quata sampling dan 21 informan yang diambil dengan teknik purposive sampling. Variabel dalam penelitian ini yaitu pengetahuan, motivasi dan peran petugas kesehatan. Data tentang variabel-variabel tersebut dianalisis menggunakan uji regresi, dengan hasil bahwa faktor yang paling berpengaruh terhadap perilaku merokok yaitu motivasi. Pada tahap studi kualitatif, kegiatan advokasi khususnya promosi kesehatan tentang rokok tidak dilakukan tersendiri untuk perilaku merokok saja, tetapi juga bersamaan dengan kegiatan lainnya seperti kegiatan program perilaku hidup bersih dan sehat, pengembangan masyarakat, atau pos pelayanan terpadu. Bina suasana yang diberikan kepada tokoh masyarakat yang dilakukan kepada staf kepala desa atau perangkat desa dan kegiatan pemberdayaan masyarakat belum memberikan pengetahuan spesifik tentang rokok.Kata kunci: rokok; pengetahuan; motivasi

Abstract

Diabetes mellitus is a metabolic disorder characterized by hyperglycemia, which causes damage to tissues and organs through the process of oxidative stress. Malondialdehyde (MDA) as a biomarker of oxidative stress has a significant increase in hyperglycemia. Stem cell therapy can be an option for diabetes mellitus such as Wharton's jelly mesenchymal stem cells. This study aimed to determine the effect of MSC-WJ administration on serum MDA levels of hyperglycemic Wistar rats. This experimental study applied a post-test only design with a control group. The sample consisted of 21 male Wistar rats divided into 3 groups, including the negative control group (K-), namely the group that was only given a standard feed diet, the positive control group (K+), namely the group that was injected with alloxan with a dose of 0.2 ml intraperitoneally, and the treatment group (P), which was the alloxan-induced group, and was given MSC-WJ at a dose of 3 million cells intravenously in the rat tails. In all three groups, blood was taken from the retro orbital plexus to measure serum MDA levels with the TBARS test. The results of the analysis showed that the mean serum MDA levels of the K-, K+, and P groups were 1.83 nmol/ml, 2.97 nmol/ml, and 1.73 nmol/ml, respectively. The results of the comparison test showed a value of p = 0.000. It was concluded that there was a significant difference in serum MDA levels between the three groups, which indicated that MSC-WJ administration could reduce alloxan-induced hyperglycemia Wistar rats serum MDA levels.Keywords: hyperglycemia; malondialdehyde; Wharton's jelly mesenchymal stem cells ABSTRAK Diabetes mellitus adalah gangguan metabolik yang ditandai dengan hiperglikemia, yang menyebabkan kerusakan jaringan dan organ melalui proses stress oksidatif. Malondialdehid (MDA) sebagai biomarker stress oksidatif mengalami peningkatan signifikan pada hiperglikemia. Terapi stem cell dapat menjadi pilihan untuk diabetes mellitus seperti mesenchymal stem cell wharton’s jelly. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian MSC-WJ terhadap kadar MDA serum tikus wistar hiperglikemia. Penelitian eksperimental ini menerapkan rancangan post-test only with control group. Sampel adalah 21 ekor tikus wistar jantan yang dibagi menjadi 3 kelompok, di antaranya kelompok kontrol negatif (K-) yaitu kelompok yang hanya diberi diet pakan standar, kelompok kontrol positif (K+) yaitu kelompok yang diinjeksi aloksan dengan dosis 0,2 ml secara intraperitoneal, dan kelompok perlakuan (P) yaitu kelompok yang diinduksi aloksan, serta diberi MSC-WJ dengan dosis 3 juta sel secara intravena pada ekor tikus. Pada ketiga kelompok dilakukan pengambilan darah dari pleksus retro orbitalis untuk pengukuran kadar MDA serum dengan tes TBARS. Hasil analisis menunjukkan kadar rerata MDA serum kelompok K-, K+, dan P secara berturut-turut adalah 1,83 nmol/ml, 2,97 nmol/ml, dan 1,73 nmol/ml. Hasil uji perbandingan menunjukkan nilai p = 0,000. Disimpulkan bahwa terdapat perbedaan kadar MDA serum yang bermakna di antara ketiga kelompok, yang menunjukkan bahwa pemberian MSC-WJ dapat menurunkan kadar MDA serum tikus wistar hiperglikemia yang diinduksi aloksan.Kata kunci: hiperglikemia; malondialdehid; mesenchymal stem cell wharton’s jelly

Abstract

Food poisoning is a serious problem that can have an impact on public health. This incident occurs when the food consumed is contaminated by bacteria, viruses, parasites or hazardous chemicals. Medan City is an area where food poisoning incidents occur every year. The purpose of this study is to examine the importance of the role of health workers in preventing extraordinary incidents of food poisoning. This study is an exploratory qualitative study through a phenomenological approach. The location of the qualitative research was conducted at the Medan City Health Office, in the working area of the Medan Area, Medan Deli, and Tuntungan Health Centers which were designated as research areas. Data collection was carried out through several main techniques, namely in-depth interviews with interview guidelines, observation, and focus group discussions. While The data processing process involves several stages, namely starting with data transcription, data reduction, and data presentation. The final stage is drawing conclusions and verification. Data analysis uses a triangulation approach to increase the credibility of research results, including data triangulation, observer triangulation, theory triangulation, and triangulation Methods. This study was conducted in 2024 with informants including the Disease Prevention and Control Team and the Public Health Team, the head of the health center. The results of this study indicate that efforts to prevent food poisoning outbreaks have not been going well. Where the cause is the suboptimal role of health workers in monitoring and evaluation due to limited human resources, in addition to the lack of training related to prevention and handling and the absence of a special policy for food poisoning outbreaks. This prevention can only be implemented if there is collaboration between health workers and catering owners to add a shared commitment to food safety. and it is necessary to carry out comprehensive coaching and supervision efforts.
