Abstract

Overactive Bladder (OAB) is a chronic medical condition that has a negative impact on an individual's health and quality of life. Cardiovascular problems are considered to be one of the risks associated with OAB where one of the higher cardiovascular parameters is metabolik syndrome. Aim to determine the relationship between  metabolik syndrome and Overactive Bladder in female patients at Dr. M. Djamil Hospital Padang. Quantitative research with a case control design using a questionnaire according to the Overactive Bladder Symptom Score (OABSS) and examination based on metabolik syndrome symptoms. The research was conducted at Dr. M. Djamil Hospital Padang in November 2022 to September 2023. There were 60 respondents who were divided into the OAB (28/60) and no-OAB group (32/60). Most respondents were aged 50-59 years (45,0%), graduated from high school (53.3%), worked as housewives (76.7%) and had diabetes mellitus (78,3%). In this study it was also found that 48,3% of respondents suffered from hypertension and 45.0% suffered from obesity. A total of 46.7% of respondents experienced OAB symptoms with a higher percentage of OAB found in the metabolik syndrome group (76,7%) than no-metabolik syndrome group (16.7%). Bivariate analysis found a relationship between metabolik syndrome and overactive bladder symptoms (p = 0.000) with metabolik syndrome affected the incidence of overactive bladder by 16.4 times (OR 16.42, (CI 95% 4.56-59.07). There is a relationship between metabolik syndrome and the incidence of OAB.

Abstract

Introduction: Many studies have been done on vitamin D, but there is still a lack of data regarding Vitamin D receptor (VDR) level and infertility, whereas VDR has a vital role in the action of vitamin D. Objective: To determine the differences in VDR levels between women with primary infertility and women without fertility problems. Method: This is a case-control study on women with and without fertility problems in Padang City, conducted from March 2023–January 2024. Serum VDR levels (ng/ml) were examined using the ELISA method in the Biomedicine Laboratory at Andalas University. Differences in VDR levels were calculated using the independent T-test. Results: There were 60 subjects divided into 30 subjects with infertility and 30 subjects without fertility problems. Subjects with primary infertility had more 31-35 years and >35 years old patients (54.8% and 66.7%), more overweight and obese BMI patients (50% and 83.3%), and had experienced infertility for 1-4 years (73.3%). The mean VDR levels in infertility subjects were lower than in subjects without fertility problems (1.73±0.92ng/ml and 2.35±1.30ng/ml), where this difference was statistically significant (p=0.036). Conclusion: VDR levels influence the incidence of primary infertility.

Abstract

Latar Belakang: Limbah medis padat Bahan Berbahaya dan Beracun (B3) rumah sakit terjadi peningkatan hingga 30% dimasa pandemi covid-19. Pengelolaan limbah medis B3 di Fasyankes diatur dalam Peraturan Menteri Lingkungah Hidup dan Kehutanan RI nomor 56 Tahun 2015.Tujuan: Penelitian ini bertujuan menganalisis implementasi pengelolaan limbah medis padat B3 pada masa pandemi covid-19 di RSUD Arosuka Kabupaten Solok Tahun 2021.Metode: Penelitian bersifat deskriptif menggunakan metode kualitatif melalui wawancara mendalam, FGD, observasi dan telaah dokumen. Komponen yang diteliti meliputi aspek input (kebijakan, SDM, dana, sarana prasarana), proses (pengurangan dan pemilahan, penyimpanan, pengangkutan), output (implementasi pengelolan limbah B3 sesuai dengan aturan yang ada) serta pengawasan oleh DLH Kab. Solok sedangkan pengolahan data dengan triangulasi sumber.Hasil: Hasil penelitian menunjukkan rumah sakit belum memiliki SOP, cold storage, merek dan label TPS, rak penyimpanan limbah, AC, jalur khusus pengangkutan limbah, tidak ada desinfeksi limbah covid-19, belum dilaksanakannya pengawasan berkala oleh DLH Kab. Solok, output pengelolaan limbah medis B3 pada masa pandemi covid-19 belum memenuhi persyaratan.Kesimpulan: implementasi pengelolaan limbah medis B3 di RSUD Arosuka Kab. Solok belum sesuai PermenLHK RI No 56 Tahun 2015. Disarankan agar dibuat kebijakan internal, SOP, penambahan dan peningkatan SDM, perbaikan dan kelengkapan sarana prasarana TPS B3, dilaksanakannya pengawasan dan pembinaan secara berkala oleh DLH terhadap pengelolaan limbah medis B3.

Abstract

LatarBelakang: Tingginya jumlah pekerja dibidang pertanian, berbanding lurus dengan tingginya penggunaan pestisida. Penggunaan pestisida dalam jumlah besar dan dilakukan secara terus menerus dapat menimbulkan dampak negatif bagi kesehatan dan lingkungan.Tujuan : Penelitian ini bertujuan untuk memberikan deskripsi tentang peran puskesmas dalam pengendalian paparan pestisida pada petani melalui aspek input, proses, dan output.Metode:Penelitian ini merupakan penelitian kualitatif dengan teknik pengambilan sampel purposive sampling. Teknik penelitian dilakukan dengan wawancara mendalam, Focus Group Discussion (FGD), observasi dan telaah dokumen.Hasil: Hasil penelitian menunjukkan upaya pengendalian paparan pestisida diwilayah kerja Puskesmas Sungai Tutung belum terlaksana. Hal ini dipengaruhi oleh beberapa faktor seperti belum ada kebijakan dari pemerintah daerah, SDM yang belum memadai, tidak ada alokasi dana kegiatan, tidak ada sosialisasi terkait upaya pengendalian paparan pestisida serta pengetahuan petani yang masih rendah dalam menggunakan pestisida sesuai aturan.Kesimpulan: Diharapkan kepada Dinas Kesehatan dan SKPD terkait untuk dapat membuat kebijakan sesuai dengan kebutuhan dalam upaya pengendalian paparan pestisida, menyediakan anggaran kegiatan, melakukan sosialisasi, menambah jumlah pos UKK dan mengaktifkan kembali kader Pos UKK serta melakukan pembinaan terhadap petani dalam menggunakan pestisida.

Abstract

Background: The stunting rate in Indonesia in 2022 was 21.6%. I was reduced when compared to the SSGI data in 2021 of 24.4%. The stunting rate appears to be decreasing from year to year, but a joint commitment is still needed to achieve the target of reducing stunting to 14% by 2024. Stunting prevention is a much more effective step than stunting treatment itself. Prevention of stunting can be done by optimizing the role of mothers in the golden phase of child development. stunting prevention in Indonesia still focuses on pregnancy and toddlers, while prevention during the preconception period is still limited.Purpose:To synthesize and critically effective steps that can be taken to prevent stunting in the preconception periodMethods:The research method employed was a literature review, conducted by using primary studies from PubMed, google scholar, and ScienceDirect with the keywords “stunting; prevention; praconception” published between 2017-2023, in English, open access, and relevant to the topicResults:A total of 397 research articles were screened for eligibility, and 15 articles met the inclusion criteria. Based on the 15reviewed articles, devide into three themes: Nutritional status during preconception, Supplementation during preconception and Lifestyle during preconceptionConclusion: This review found that there are several efforts made in preventing stunting in the preconception period. it is recommended that health workers can optimize the prevention of stunting in the preconception period.

Abstract

The prevalence of diabetes mellitus is rising globally. Oxidative stress, which can result from hyperglycemia in diabetes, might have negative consequences. An antioxidant is needed to prevent hyperglycemia. Cathechin isolates are derived from gambir, which has many antioxidants. This study examines catechin isolates from gambir (Uncaria gambir Roxb.) effect on glucose homeostasis in rats induced by alloxan. For this experiment, 35 male rats were employed. Male rats were given alloxan (150 mg/BW, IP), and after 72 hours, blood glucose levels were assessed. If blood glucose levels exceeded 200 mg/dl, three oral catechin isolates were administered (T1=10 mg/kg/day, T2=20 mg/kg/day, and T3=40 mg/kg/day). Following blood collection on the experiment's last day, fasting blood glucose, glucagon and insulin levels were measured. Catechin isolates decreased blood glucose levels in all treatment groups compared to the positive control group (T1 = 150.750 ± 14.359 mg/dl; T2 = 159.750 ± 15.434 mg/dl, and T3 = 153.375 ± 20.207 mg/dl vs 385 ± 60.989 mg/dl) significantly (p value-0.05). A decrease in glucagon serum level was also observed in the treatment group vs positive control (T1: 193.855 ± 36.009 pg/ml, T2 = 286.689 ± 20.313 pg/ml, and T3 = 319.462 ± 30.060 pg/ml vs  529.825 ± 74.279 pg/ml), significantly. Catechine isolates in the T3 group showed an increase in insulin serum level compared to the positive control group significantly (T3 = 216.640 ± 38.230 µIU/ml vs 69.833 ± 3.071 µIU/ml). In conclusion, catechin isolates from gambir decreased blood glucose levels by reducing glucagon and increasing insulin serum levels.

Abstract

Background: Colorectal cancer (CRC) is a major health burden globally. The prognosis of CRC is strongly influenced by the presence of lymphovascular invasion (LVI) and lymph node (LN) metastasis. Transforming growth factor-beta (TGF-β) is a cytokine with a complex role in CRC progression. This study aimed to evaluate the significance of TGF-β expression in predicting LVI and LN metastasis in CRC. Methods: This cross-sectional study involved 50 patients diagnosed with CRC. The expression of TGF-β was assessed using immunohistochemical staining and the Allred scoring system. The relationship between TGF-β expression and the presence of LVI and LN metastasis was analyzed using the Chi-square test. Results: High TGF-β expression was significantly associated with both LVI (p = 0.011) and LN metastasis (p = 0.012) in CRC. Patients with high TGF-β expression had a higher risk of LVI and LN metastasis compared to those with low TGF-β expression. Conclusion: TGF-β expression is a significant predictor of LVI and LN metastasis in CRC. This finding has potential implications for risk stratification and treatment decisions in CRC patients.

Abstract

According to Globocan data, in 2022, cervical cancer caused 348,874 deaths among women, and cases of cervical cancer in Indonesia increased to 36,633 people. WHO recommends early detection through the VIA (Visual Inspection with Acetic Acid) examination. However, the VIA examination coverage in Kampar Regency in 2023 was low, at only 1.2%. This study aims to analyze the Acetic Acid Visual Inspection (VIA) program for women of reproductive age (WRA) based on healthcare service accessibility in Kampar Regency, Riau Province, in 2024. This research uses a cross-sectional method, and the results show that information about VIA services is lacking, the acceptance of VIA services is low, service hours are not always available, and community empowerment in supporting VIA services is insufficient. The factors that most influence the participation of women of reproductive age in the VIA examination, based on healthcare service accessibility, are service acceptance (p=0.008 and exp(B)= 19.342) and community empowerment (p=0.017 and exp(B) = 8.272). Variables such as service location distance, knowledge, attitude, transportation ease, and environmental support did not significantly affect the situation. It is recommended that service acceptance be improved through promotion, education, and service socialization, as well as strengthened community empowerment by involving local health workers, the Family Welfare Movement (PKK), and related cross-sector stakeholders in an integrated manner.

Abstract

Improving effective communication is the second point in patient safety targets. Based on initial data, effective communication at RSU X Padang City in its implementation has not reached the target of 100% and the weakness of this hospital is that risk management has not been implemented properly. The purpose of this study was to analyze the implementation of effective communication by strengthening risk management in the inpatient ward of RSU X Padang City. This study used a qualitative research design with a phenomenological approach. Researchers conducted semi-structured interviews with 16 informants, document reviews, and observations. The results of the study showed that the role of HR factors in effective communication was not optimal, cooperation within the unit was quite good, the role of management and the hospital environment was not optimal, the role of risk management was not optimal, and the impact of effective communication could improve the achievement of IMP-RS, facilitate the delivery of patient conditions, and minimize errors in providing therapy or medication to patients so that patient quality and safety are guaranteed. It is recommended that the implementation of effective communication always be improved with improvement efforts, namely completing facilities and infrastructure, implementing socialization and training, monitoring and evaluation, and implementing risk management programs.

Abstract

Background: Family planning is a government programme designed to address population needs and balance demographic pressures. It generally aims to regulate the number of births to reduce the risks associated with pregnancy. Contraception serves as a tool for managing pregnancy and controlling childbirth. Women’s participation in family planning programmes is influenced by both internal and external factors. Methods: This research is an analytical study with a cross-sectional design, conducted at Puskesmas Alai, Kota Padang, with a total sample of 105 married women. The study utilized a questionnaire to assess internal and external factors related to women's involvement in the family planning programme. Data were analyzed using the chi-square test. Results: The results of this research indicate that the number of respondents who use contraception is greater than those who do not. Regarding internal factors, most respondents fall into the risk age group, have a high level of education, are not employed, possess a good level of knowledge, and have 0 to 2 children. For external factors, the majority of respondents receive support from their husbands, and more than half of the respondents have a low economic status. Conclusion: The conclusion of this research is that there is a significant correlation between age, knowledge level, husband's support, and economic status with participation in the family planning programme. However, no significant correlation was found between education, occupation, and the number of children.
