Abstract

Background: Vitamin D deficiency during pregnancy is associated with poor pregnancy outcomes. Research shows a complex interaction between iron and vitamin D. Pregnant women are susceptible to iron deficiency due to increased iron requirements during pregnancy. Ferritin reflects body iron stores and may decrease before serum iron. This study aims to analyze the relationship between ferritin levels and vitamin D status in pregnant women. Methods: This retrospective comparative cross-sectional analytical observational study involved pregnant women in the 1st, 2nd, and 3rd trimesters who underwent antenatal care at Hermina Hospital Padang from February to August 2023. Vitamin D (25(OH)D3) and ferritin were measured using enzyme-linked fluorescence assay (ELFA). Univariate and bivariate analysis used the Chi-square and odds ratio (OR) tests, with a significance of p<0.05. Results: Of 163 pregnant women (mean age 30.4 years), median ferritin levels were 25.85 ng/mL and 25(OH)D3 15.5 ng/mL. Low ferritin was found in 63.80% of subjects and sufficient vitamin D in 31.90%, insufficiency in 33.74%, and deficiency in 34.36%. There was no correlation between low ferritin and vitamin D insufficiency (OR=2.04; 95% CI 0.94-4.42; p=0.700). However, there was a significant correlation between low ferritin and vitamin D deficiency (OR=6.59; 95% CI 2.68-16.18; p=0.000). Conclusion: Pregnant women with low ferritin have a 6.59 times higher risk of experiencing vitamin D deficiency.

Abstract

Background: The co-occurrence of tuberculosis (TB) and acquired immunodeficiency syndrome (AIDS) presents a formidable clinical challenge due to the synergistic impact on the immune system. This case report describes a patient with TB/AIDS co-infection exhibiting profound immunosuppression characterized by a severely diminished CD4 count and an unusual reversal of the CD4:CD8 ratio. Case presentation: A 39-year-old male presented with symptoms indicative of both TB and advanced HIV infection, including shortness of breath, weight loss, and oral thrush. Physical examination revealed bilateral lung crackles. Laboratory investigations confirmed pulmonary TB and revealed a critically low CD4 count (6 cells/µL), and a CD8 count of 71 cells/µL, resulting in a reversed CD4:CD8 ratio of 0.08. The patient's HIV viral load was markedly elevated at 598,403 copies/mL. This case underscores the complex interplay between TB and HIV, highlighting the profound impact of co-infection on immune system function. The patient's severely depleted CD4 count and the atypical CD4:CD8 ratio reflect the advanced stage of HIV infection and the superimposed TB. The findings emphasize the need for close monitoring and aggressive management of co-infected patients to mitigate the risk of opportunistic infections and disease progression. Conclusion: This case report documents a rare and severe presentation of TB/AIDS co-infection with profound immunosuppression and a reversed CD4:CD8 ratio. It serves as a reminder of the significant morbidity and mortality associated with advanced HIV and TB co-infection, particularly in cases of delayed diagnosis or suboptimal treatment adherence.

Abstract

Background: Hepatocellular carcinoma (HCC) is a primary liver cancer that often arises in the setting of chronic liver disease, most commonly cirrhosis. Alpha-fetoprotein (AFP) is a tumor marker frequently elevated in HCC, but its diagnostic performance varies. We present a case of HCC with concomitant cirrhosis and a significant elevation of AFP. Case presentation: A 30-year-old male presented with hematemesis, abdominal distension, and jaundice. He had a history of heavy alcohol consumption. Physical examination revealed anemia, icterus, hepatomegaly, and palmar erythema. Laboratory investigations showed elevated AFP (>400 IU/mL), reactive hepatitis B surface antigen (HBsAg), anemia, coagulopathy, and liver dysfunction. Imaging studies confirmed HCC and cirrhosis. This case highlights the diagnostic value of AFP in HCC, particularly when combined with clinical and imaging findings. The patient's history of alcohol abuse and HBsAg positivity are well-established risk factors for both cirrhosis and HCC. The marked elevation of AFP, along with the characteristic imaging features, strongly supported the diagnosis of HCC. Conclusion: AFP remains a valuable tool in the diagnosis and monitoring of HCC, especially in patients with cirrhosis. However, it is essential to interpret AFP levels in conjunction with other clinical and laboratory data to ensure accurate diagnosis and timely management.

Abstract

This research explored whether vitamin D status is related to Insulin Resistance (IR) and pancreatic β-cell performance among non-diabetic obese employees at Dr. M. Djamil Padang Hospital, West Sumatra, Indonesia. Insulin resistance and β-cell activity were assessed using the Homeostatic Model Assessment—specifically Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) as an indicator of insulin resistance and Homeostasis Model Assessment of Beta-Cell Function (HOMA-B) to evaluate β-cell function. The study applied a cross-sectional design conducted from January to July 2020 and involved 81 obese hospital staff without diabetes. Measurements included Fasting Blood Glucose (FBG), Fasting Insulin (FI), and Serum 25-Hydroxyvitamin D [25(OH)D], all analyzed with automated instruments. HOMA-IR and HOMA-B indices were derived from FBG and FI using standard formulas. Statistical analysis used the Spearman correlation test with a significance threshold of p<0.05, followed by multivariate linear regression. The participants were predominantly female (58%), aged 18–58 years, with a mean Body Mass Index (BMI) of 31.46±3.99 kg/m². Average vitamin D levels were 14.84±5.48 ng/mL, while median values for HOMA-IR and HOMA-B were 2.30 and 193.09%, respectively. The results indicated that vitamin D concentrations did not show a meaningful correlation with either HOMA-IR (r=0.071; p=0.530) or HOMA-B (r=−0.106; p=0.347). BMI demonstrated the strongest association with HOMA-IR (β=0.165), and HOMA-IR was the variable most strongly related to HOMA-B (β=21.83). Overall, this study concludes that baseline vitamin D levels are not significantly linked to insulin resistance or pancreatic β-cell function, as reflected by HOMA-IR and HOMA-B measurements, among non-diabetic obese staff at Dr. M. Djamil Padang Hospital.

Abstract

Latar Belakang: Lupus Eritematosus Sistemik (LES) adalah penyakit autoimun kompleks yang dapat menyerang berbagai pertahanan sistem tubuh. Penyakit ini berhubungan dengan pembentukkan autoantibodi sehingga dapat menyebabkan inflamasi kronis yang berujung pada kerusakkan jaringan tubuh. Aktivitas Penyakit LES diduga dapat mempengaruhi kadar albumin di dalam tubuh.    Objektif: Penelitian ini bertujuan untuk mengetahui apakah terdapat perbedaan rerata kadar albumin pasien LES berdasarkan derajat aktivitas penyakit di RS M. Djamil Padang. Metode: Penelitian ini menggunakan metode analitik dengan pendekatan cross-sectional study. Sampel penelitian berjumlah 59 pasien LES rawat jalan dan rawat inap di RS M. Djamil Padang yang dikumpulkan secara consecutive sampling. Analisis data penelitian menggunakan uji one way annova. Hasil: Pada penelitian ini didapatkan mayoritas pasien LES berada di rentang usia 26−35 tahun (44,1%), berjenis kelamin perempuan (100%). Lebih dari setengah pasien LES mengalami hipoalbuminemia (76,3%), sebagian besar pasien memiliki derajat aktivitas penyakit sedang (39%). Rerata kadar albumin terendah ditemukan pada pasien dengan derajat aktivitas penyakit berat sebesar 2,25 g/dL. Kesimpulan: Terdapat perbedaan yang bermakna pada rerata kadar albumin pasien LES. Rerata kadar albumin terendah pasien LES ada pada kelompok derajat aktivitas penyakit berat. Rerata kadar albumin tertinggi pasien LES ada pada kelompok derajat aktivitas penyakit ringan.

Abstract

Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan kontrol glikemik dengan kadar kreatinin serum pada pasien diabetes melitus tipe (DMT2); Metode: Penelitian cross-sectional ini dilakukan pada bulan November 2022 hingga Januari 2024. Sebanyak 1.130 rekam medis pasien DMT2 yang melakukan pemeriksaan kadar HbA1c dan kadar kreatinin serum di Instalasi Laboratorium Patologi Klinik RSUP Dr. M. Djamil Padang pada tahun 2019–2021 diikutsertakan. Uji Mann-Whitney digunakan untuk menganalisis hubungan antara kontrol glikemik dan kadar kreatinin serum; Hasil: Sebanyak 52,7% pasien DMT2 adalah laki-laki dengan rata-rata usia 55,8 tahun. Lebih dari separuh (52,7%) pasien DMT2 memiliki kontrol glikemik yang buruk dan ditemukan bahwa median kadar kreatinin serum pada pasien DMT2 dengan kontrol glikemik buruk adalah 5,3 (1,3-15,3) mg/dL, sedangkan pasien dengan kontrol glikemik baik adalah 4,2 (0,4-8,0)mg/dL. Hasil analisis bivariat menunjukkan terdapat hubungan bermakna antara kontrol glikemik dengan kadar kreatinin serum pada pasien DMT2 (p<0,05); Kesimpulan: Temuan pada penelitian ini menunjukkan adanya hubungan antara kontrol glikemik dengan kadar kreatinin serum pada pasien DMT2.

Abstract

Glycemic control is important to prevent organ damage in Type 2 Diabetes Melitus (T2DM) Patients, especially liver and kidney. One of parameter used to see the decreasing function of both organs is serum albumin. Aim : The aim of this study was to identify the association between glycemic control and serum albumin level on T2DM patoents. Method :  This is an observational analytical study using cross sectional design. This study was conducted at RSUP Dr. M. Djamil Padang using T2DM patients medical records on 2019-2021. The level of HA1c <7% was cathegorised as good glycemic control and Hb1c >7% was cathegorised as poor glycemic control. Results : The results showed that there was an association between glycemic control and serum albumin level on T2DM patients at RSUP Dr. M. Djamil Padang. Conclusion : Glycemic control was inversely associated with serum albumin levels in patients with T2DM in RSUP Dr. M. Djamil Padang

Abstract

Nutritional status is an important indicator that determines the quality of health and development of school-age children. Adequate nutrition supports children's physical growth and cognitive abilities, and contributes to academic achievement. This community service activity aims to analyze the nutritional status of Bustanul Ulum Elementary School students by measuring height, weight, and Body Mass Index (BMI). The method used is quantitative descriptive with a sample of 150 students from grades 1 to 6. Measurements were carried out using standard and calibrated measuring instruments, while BMI was calculated using the formula body weight (kg) divided by the square of height (m²). The results showed that 75% of students had normal nutritional status, 10% were underweight and stunted, and 15% were overweight and obese. Factors that influence nutritional status include an unbalanced diet, lack of physical activity, and family socio-economic background. Children from families who have access to healthy food and good nutrition education tend to have better nutritional status. Based on these results, a comprehensive intervention program is needed in the form of nutrition education for students and parents, provision of healthy food at school, and promotion of regular physical activity. Collaboration between schools, health workers, and parents is essential to create an environment that supports optimal and sustainable child growth and development.

Abstract

Background: Down syndrome is a global health problem of particular concern because people with Down syndrome have a wide variety of clinical disorders and are one of the causes of mental retardation and serious physical growth disorders.Down syndrome can occur at all socioeconomic, ethnic and demographic levels. The incidence of Down Syndrome will increase as the mother's age increases at the time of pregnancy and the incidence varies in different populations: 1 in 319 to 1 in 1000 live births. Each year, an estimated 3,000 to 5,000 children are born with Down syndrome. The APOE gene is located in the long arm (q) of chromosome 19 at position 13.2 (19q13.2). The APOE gene consists of four exons and three introns, a total of 3597 base pairs. In melanocyte cells APOE gene expression can be regulated by MITF. APOE is a form of polymorphic, which translates into three gene alleles: normal: allele ?3 and dysfunctional: allele ?2 and allele ?4.  The polymorphism of the APOE gene had a strong effect on the level of allele production, a high concentration of APOE indicated that the production of ?4 allele was increased and a low concentration of APOE was associated with the production of ?2 allelePurpose:To Analyze of Allele Frequency Distribution Apoliprotein E Gene  in Patients With Down Syndrome Trisomy  21Methods: This research is an analitic observasional study with a comparative  study design. The sample used was the result of DNA extraction patients with Down's Syndrome Trisomy 21 as many as 33 samples and 33 controls stored in the Biomedical Laboratory, Faculty of Medicine, Andalas University, Padang, Indonesia. The next step is to examine the APOE gene polymorphisms using PCR and sequencing techniques.Results: Samples of Down syndrome patients had variations in the distribution of alleles, where alleles ?4 and ?2 were found even though the allele frequency ?3 was still the highest allele frequency. Meanwhile, in the samples representing the normal control population, ?3 and ?2 alleles were found and no ?4 allele was found. Although the allele is not associated with Down Syndrome, Down Syndrome sufferers have a 2.26 times greater risk for e2 and e4 alleles than e3.Conclusion: There was a difference in the frequency distribution pattern of the APOE gene allele in patients with Down Trisomy Syndrome 21 compared to the control.

Abstract

This study aims to investigate the perceptions of the community in Puskesmas Tuntungan, Jl. Kutalimbaru, Desa Tuntungan I, Kecamatan Pancur Batu, Kabupaten Deli Serdang, Sumatera Utara, regarding the impact of political dynamics on BPJS Health policy in Puskesmas Tuntungan. Using a quantitative approach with 100 respondents, the results show that the majority of respondents are aware of the frequent changes in BPJS policy. This study concludes that the community has diverse and critical perceptions of the dynamics of BPJS Health policy and its relation to political conditions.
