Abstract

Adiponectin and HDL cholesterol have a protective cardiovascular effect. Adiponectin levels decrease in obese people. This causes ATP-binding cassette transporter A1 expression and apolipoprotein A-I synthesis in hepatocytes to decrease, resulting in impaired HDL cholesterol synthesis. Objectives: To determined the correlation between adiponectin levels with HDL cholesterol levels in obese. Methods: This was the analytic study with a cross-sectional design of 59 obese people who met inclusion and exclusion criteria at RSUP dr. M. Djamil Padang, from January to August 2019. Research subjects were grouped into two groups based on WHO Asia Pacific body mass index (BMI) classification. First-degree obesity group (25.0 £ BMI <30.0 kg/m2) and second-degree obesity group (BMI ≥ 30.0kg/ m2). Adiponectin levels were examined by the enzyme-linked immunosorbent assay (ELISA) method and HDL cholesterol levels by the enzymatic colorimetric method. Data analyzed by Pearson correlation test, significant if p <0.05. Results: Research subjects 59 people (17 men, 42 women), average age 35 (8.04) years. Adiponectin levels in the first-degree obesity group were higher than second-degree obesity group, with a mean difference between groups 1.03 μg/mL (CI: 95%: 0.14 - 1.92, p = 0.02). Results showed adiponectin levels positively correlated with HDL cholesterol levels of moderate strength (r = 0.45) and statistically significant (p = <0.001). The results of the population subanalysis show relatively similar patterns. First-degree obesity population (r = 0.55; p = 0.002), second-degree obesity population (r = 0.41; p = 0.026). Conclusions: Adiponectin levels correlate with HDL cholesterol levels.Keywords: adiponectin, HDL cholesterol, obesity

Abstract

Atherogenic dyslipidemia is one of the risk factors for the acute coronary syndrome (ACS). Castelli's risk index-1 (CRI-1) is one of which risk markers for ACS. Castelli's risk index-1 is the ratio of total cholesterol and HDL cholesterol. The test is cheap and easy to do in the hospital setting. Objectives: To described CRI-1 in ACS patients in Dr. M. Djamil Padang Hospital. Methods: This descriptive study was carried out in the central laboratory and the CardioVascular Care Unit (CVCU) of Dr. M. Djamil Hospital Padang from September 2017 to September 2018. The population was all ACS patients who have been diagnosed by the clinician. The samples were part of the population that meet the inclusion and exclusion criteria. Colorimetric enzymatic method using automated clinical chemistry used to measure total cholesterol and HDL cholesterol. Results: This study used CRI-1> 4. Data were presented descriptively in a frequency distribution table. Seventy ACS patients were consisting of 50 (71.43%) males and 20 (28.57%) females, with a median age of 60.1 (8.93) years old. The median total cholesterol and HDL cholesterol levels were 178.66 (46.84) mg / dL respectively and 35.71 (10.86) mg / dL. CRI-1 mean is 5.43 (2.27). 81.43% CRI-1 subject results were more than four. Conclusion: The low levels of HDL cholesterol and within normal median total cholesterol level made CRI-1 value increased.Keywords: Acute coronary syndrome, Castelli’s risk index-1

Abstract

Rapid and accurate measurement of capillary glucose level using point-of-care testing (POCT) is needed to maintain the patient’s normoglycemic status in obtaining adequate management. The glucose POCT method should be evaluated to determine analytical performance by comparing with the hexokinase method as a reference method to provide accurate and reliable results. Objectives: To determined the correlation between capillary glucose using glucose dehydrogenase-nicotinamide adenine dinucleotide (GDH-NAD)  and serum glucose hexokinase methods. Methods:  This analytic cross-sectional study on 42 outpatients who underwent fasting blood glucose examination at Dr. M Djamil Padang General Hospital. This study was conducted from February until September 2020. Capillary fasting blood glucose was measured using glucose POCT GDH-NAD method and serum glucose with hexokinase method. The Spearman correlation test was used to analyze data, significant if p<0.05. Results: The subjects were 28 male (66.7%),14 female (33.3%) with mean age and hematocrit level was 56.12±12.97 years and 40.90±2.42%, respectively. The median capillary glucose GDH-NAD method and serum glucose hexokinase method were 100.00 mg/dL each, with a median difference was 3.00 mg/dL. Spearman correlation test showed very strong positive correlation and statistically significant (r=0,961;p=<0,001). Conclusion: Capillary glucose GDH-NAD method had a very strong positive correlation with serum glucose hexokinase method.Keywords:  hexokinase, fasting glucose level, GDH-NAD, glucose POCT

Abstract

Liver injury can be caused by coronavirus disease 2019 (COVID-19), characterized by increased levels of transaminase enzymes (aspartate aminotransferase/AST and alanine aminotransferase/ALT). Objective: To determined the correlation between transaminase enzyme levels and the severity of COVID-19 in Dr. M. Djamil Central Public Hospital, Padang. Methods: This was an observational analytic study with a cross-sectional design. Data were obtained randomly from medical records of confirmed COVID-19 patients hospitalized in Dr. M. Djamil Hospital from June to August 2021, with moderate, severe, and critical severity based on Indonesian Ministry of Health criteria. Data were analyzed using the Chi-square test to examine the distribution and correlation between variables and a simple logistic regression test to assess the level of risk between variables. Results: 57,80% had abnormal AST, and 44,95% had abnormal ALT. The median AST was highest in the critical group, while the median ALT was highest in the severe group. The distribution of sex and age group were significantly different based on AST and ALT levels, and the distribution of patients with coronary heart disease was significantly different based on ALT levels. Transaminase enzyme levels had a significant correlation with the severity of COVID-19 (p-value = 0,00). Patients with critical degrees had the highest risk for abnormal AST and severe degrees had the highest risk for abnormal ALT. Conclusion: Transaminase enzymes have a significant correlation with the severity of COVID-19.Keywords:  COVID-19, liver injury, transaminases, disease severity

Abstract

Abstrak Latar Belakang: Leukemia Granulositik Kronik (LGK) merupakan penyakit mieloproliferatif yang ditandai dengan adanya proliferasi seri granulosit tanpa ada gangguan diferensiasi. Direct Coomb’s Test (DCT) digunakan untuk mendekteksi adanya antibodi imun, baik IgG maupun komponen komplemen (umumnya C3d) yang menutupi atau mensensitisasi sel eritrosit. Pemeriksaan coomb’s test pada pasien LGK dilakukan apabila adanya kecurigaan anemia dan terjadinya retikulositosis tanpa ditemukan adanya sumber perdarahan yang jelas. Objektif: Tujuan penelitian ini untuk mengetahui karakteristik pasien LGK berdasarkan hasil direct coomb’s test di RSUP Dr. M.Djamil Padang. Metode: Penelitian deskriptif dilakukan terhadap semua pasien yang didiagnosis LGK yang memiliki hasil direct coomb’s test. Pengambilan sampel dilakukan secara consecutive sampling dengan 30 sampel yang memiliki data berupa usia, jenis kelamin, hasil direct coomb’s test, derajat anemia, hitung retikulosit, dan jenis terapi. Hasil: Penelitian berdasarkan jenis kelamin pada pasien leukemia granulositik kronik didapatkan lebih banyak perempuan yaitu 18 pasien (60%) dan laki-laki 12 pasien (40%) dengan rerata usia 44 tahun. Direct coomb’s test menunjukkan hasil positif pada 21 pasien (70%) dan negatif pada 9 pasien (30%). Anemia berat paling banyak ditemukan pada direct coomb’s test negatif dengan 55,6%. Retikulositosis lebih banyak ditemukan pada hasil direct coomb’s test positif. Kesimpulan: Pasien LGK dengan hasil direct coomb’s test positif lebih banyak ditemukan anemia berat, anemia sedang, dan retikulositosis. Abstract

Sungai Pisang Village is a village that has only been accessible by smooth road roads in the past year in Padang City. Previously this village was only accessible by sea and by lousy road. This condition causes the Pisang River community to lag in socio-economic and health aspects. This activity aims to screen for worms in elementary school students in Sungai Pisang village because worms are closely related to environmental health and affect the quality of human resources experiencing growth and development. Screening for worms is carried out by examining fecal preparations. This activity also aims to check visual acuity in elementary students and make references for sharp vision correction to learn well. Visual acuity checks were carried out using a Snellen card. The results of the examination showed that 3/143 students had worms and 3/273 students had visual disturbances. Students with worms are recommended to go to the Puskesmas to be given deworming medicine, and students with visual impairments are facilitated for examination by an ophthalmologist and given glasses if needed.

Abstract

Objective: To determine differences in the results of hematological examinations based on the severity of COVID-19 patients at Semen Padang Hospital in 2021. Methods: The type of research was observational analytic with a cross-sectional design. We took secondary data from the medical records of COVID-19 patients. A total of 100 patients were included in this study was conducted at Semen Padang Hospital in January-December 2021. Bivariate analysis used a one-way ANOVA with the Kruskal-Wallis alternative. The results are considered as significant if p <0.05. The significance of the parameters was tested using the Post Hoc Scheffe (p<0.05). Results: From 100 patients found the: 60% Male and 40% female, mean age of 51.95 ± 12.82 years, mild COVID-19 patients 36%, moderate 44%, severe 14%, and critical 6%. The mean hematological parameters were within normal limits. Hemoglobin levels (g/dL) of male patients based on severity (mild, moderate, severe, and critical) were 15,02 ± 1,10, 14,20 ± 1,29, 14,14 ± 1,21, 14,50 ± 1,99 respectively (p<0.05); hemoglobin levels (g/dL) of female patients were 13,73 ± 0,94, 12,96 ± 0,76, 14,20 ± 1,27, 12,16 ±0,47 respectively (p<0,05); leukocyte count (/mm3) were 5600, 5950, 6250, 7850 respectively (p>0.05); absolute neutrophil count (103/µL) were 3.51, 3.79, 4.72, 6.56 respectively (p<0.05); absolute lymphocyte count (103/µL) were 1.67±0.60, 1.42±0.44, 1.14±0.44, 1.14±0.59, 1.06±0.65 respectively (p<0.05); eosinophil count (103/µL) were 0.058, 0.057, 0.031, 0.023 respectively (p>0.05); and platelet count (/mm3) were 251,000 ± 61,077, 229,522 ± 59,900, 173,071 ± 57,596, 133,350 ± 44,724, respectively (p<0.05). Conclusion: The conclusion of this study is that there are differences in hemoglobin levels, absolute neutrophils, absolute lymphocytes, and platelet counts between mild, moderate, severe, and critical COVID-19 patients.

Abstract

Introduction. Non-communicable diseases as a cause of death in Indonesia are increasing. Hyperglycemia, hyperlipidemia, and obesity are some of the factors that contribute to an increased risk of non-communicable diseases. Education workers are a group of office workers who are at risk of obesity and non-communicable diseases because they have low physical activity. The study aimed to determine a correlation between body mass index with fasting blood glucose levels and total cholesterol levels in the health screening of educational staff faculty of medicine of Andalas University.  Methods. This research was an analytical study with a cross-sectional design using secondary data for the faculty of medicine of Andalas University health screening that was held on September 08, 2021. The tools used in the study were a calibrated glucometer to measure fasting blood glucose and a calibrated auto-check for total cholesterol examination. 37 data met the inclusion and exclusion criteria. Data were processed and analyzed using univariate and bivariate analysis using the Pearson correlation test.  Results. The result showed that the educational staff of the medical faculty of Andalas University are mostly in the late adult category, most are female and most of them are obese. The mean of body mass index is 25.3 kg/m2, the mean of fasting blood glucose level is 126.57 mg/dL and the mean of total cholesterol level is 205, 14 mg/dL. The bivariate analysis results showed no correlation between body mass index and fasting glucose levels (r = 0.316; p = 0.057) and no correlation between body mass index and total cholesterol levels (r = 0.091; p = 0.591). Conclusions. This study concludes that there is no correlation between body mass index with fasting blood glucose levels and total cholesterol levels in the health screening of educational staff faculty of medicine of Andalas University.

Abstract

Latar Belakang: Interaksi radiasi sinar-X berupa ionisasi dan eksitasi terhadap materi yang dilewatinya,     menyebabkan perubahan DNA, sel, jaringan, dan organ. Jenis paparan (langsung, tidak langsung) dan masa kerja (5 tahun) memengaruhi efek paparan yang terjadi. Tujuan: Mengetahui hubungan antara jenis paparan radiasi sinar-X dan masa kerja dengan kadar SGOT, SGPT, ureum, dan kreatinin pekerja radiasi. Metode: Penelitian analitik rancangan potong lintang 56 pekerja radiasi RSUP Dr. M. Djamil Padang yang melakukan pemeriksaan SGOT, SGPT, ureum, dan kreatinin (Maret 2022). Data dianalisis dengan uji parametrik t tidak berpasangan, bermakna jika p<0,05. Hasil: Subjek penelitian 30 laki-laki (53,6%), 26 perempuan (46,4%), rerata umur 34 tahun (23-55 tahun). Distribusi frekuensi paparan langsung 32(57,1%), tidak langsung 24(42,9%). Kadar SGOT, SGPT, ureum, kreatinin paparan langsung berturut-turut 19,6U/L; 25,5U/L; 28,8mg/dL; 0,8mg/dL, paparan tidak langsung 18,3U/L; 20,9U/L; 27,8mg/dL; 0,8mg/dL. Tidak ditemukan perbedaan bermakna antar 2 kelompok (p>0,05). Pembahasan: Tidak terdapat hubungan paparan radiasi sinar-X dengan kadar SGOT, SGPT, ureum, dan kreatinin disebabkan dosis paparan dalam nilai batas dosis dan penerapan proteksi radiasi sesuai standar. Simpulan: Tidak terdapat perbedaan bermakna kadar SGOT, SGPT, ureum, dan kreatinin antara pekerja radiasi yang terpapar langsung dengan yang tidak terpapar langsung.

Abstract

Objective : This research aims to analyze the effect of dadih probiotics on MDA levels in alloxan-induced diabetic rats. Methods : This study used an experimental design with a post-test-only control group. Wistar strain white rat samples were divided into 4 groups. The negative group was not given any treatment, K(+), P1, and P2 were induced by alloxan 150 mg/kg BW on the first day. Groups P1 and P2 were given dadih probiotic doses of 1.87 g/200 gBW and 3.74 g/200 gBW on days 4 to 17. Results : Measurement of MDA levels was carried out on the 18th day. Data were analyzed with One Way Anova and a post-hoc test. The research results are statistically significant if p-value <0.05. The average levels of MDA K- 2.29 nmol/mL, K+ 3.77 nmol/mL, P1 2.97 nmol/mL, and P2 2.53 nmol/mL. Based on statistical tests, there was an effect of dadih probiotics on MDA levels as evidenced by a p-value < 0,05. Conclusion : Dadih probiotic doses of 1.87 g/200gBW and 3.74 g/200gBW can reduce MDA levels in rats.
