Abstract

Neonatal sepsis is a leading cause of morbidity and mortality worldwide, and one of the most common causes of neonatal death (15%). Early-onset neonatal sepsis (ESNAD) has a higher case-fatality rate than late-onset neonatal sepsis (SNAL). The gold standard for neonatal sepsis diagnosis is bacterial detection through blood culture, but this test has limitations such as being time-consuming and not being available in all health facilities. This study aims to determine the relationship between perinatal risk factors, hematological parameters in infants, procalcitonin, IL-18 with the incidence of SNAD. The population of this study were all babies who had risk factors for SNAD according to the PP Idai UKK Neonatology Clinical Practice Guidelines. The research sample was infants from an accessible population who met the inclusion and exclusion criteria. To fulfill statistical rules and fulfill the requirements for data analysis, in this study a minimum of 30 of each will be taken. The use of umbilical cord blood is a promising diagnostic tool in the diagnosis of SNAD. The IT and CRP ratios are elevated in umbilical cord blood from birth and are good indicators for detecting SNAD in infants at risk of infection. Umbilical blood procalcitonin is elevated in infants born >32 weeks with proven sepsis and low in infants with risk factors but without proven sepsis. Therefore, it is concluded that umbilical blood procalcitonin is a promising biomarker for detecting SNAD. IL-18 serves as a risk variable for neonatal sepsis. IL-18 has good discriminatory ability in the diagnosis of neonatal sepsis.

Abstract

Lung cancer remains the leading cause of cancer-related mortality worldwide, with persistently low survival rates. Bronchoscopy is an essential diagnostic procedure in patients with lung tumors and often requires anesthesia to improve comfort and procedural success. Emerging evidence suggests that anesthetic agents and techniques may influence tumor biology through modulation of immune responses and inflammatory mediators. Transforming Growth Factor-Beta (TGF-β) is a multifunctional cytokine involved in cell proliferation, differentiation, immune regulation, and extracellular matrix remodeling, and plays a critical role in cancer progression. This literature review aims to evaluate current evidence regarding the effects of anesthetic agents and techniques on TGF-β levels and their implications for tumor biology in patients with lung tumors undergoing bronchoscopy. A narrative literature review approach was used to analyze scientific publications exploring the relationship between anesthesia, immune modulation, cytokine alterations, and cancer progression, particularly focusing on inhalational and intravenous anesthetics. Surgical stress and anesthetic exposure can alter immune responses through activation of neuroendocrine pathways, leading to cytokine imbalance and immunosuppression. TGF-β exhibits dual roles in cancer, acting as a tumor suppressor in early stages and a tumor promoter in advanced stages. Inhalational anesthetics may enhance tumor progression, whereas intravenous anesthetics such as propofol may have immunomodulatory effects. Anesthetic techniques may influence tumor-related pathways, including TGF-β; however, evidence remains limited and inconsistent, requiring further well-designed studies.

Abstract

Latar Belakang: Lupus eritematosus sistemik (LES) merupakan penyakit autoimun yang berhubungan dengan berbagai sistem tubuh dan berhubungan secara signifikan dengan morbiditas dan mortalitas. Anemia pada penderita LES banyak terjadi akibat penyakit kronis. Anemia dapat menggambarkan tingkat keparahan penyakit yang mendasarinya sehingga evaluasi terhadap derajat aktivitas penyakit, salah satu caranya dengan menggunakan MEX-SLEDAI dan status anemia pasien LES diperlukan untuk menentukan rencana pengawasan dan tatalaksana dari pasien tersebut. Objektif: Tujuan dari penelitian ini adalah untuk mengetahui hubungan antara derajat aktivitas penyakit dengan kejadian anemia pada pasien LES di RS Dr. M. Djamil Padang. Metode: Penelitian ini merupakan penelitian analitik dengan pendekatan cross-sectional menggunakan data sekunder dari rekam medik pasien LES di RS Dr. M. Djamil Padang dan derajat aktivitas penyakit didapatkan berdasarkan MEX-SLEDAI. Teknik pengambilan sampel dengan consecutive sampling dengan total 69 sampel. Analisis data yang dilakukan menggunakan uji Fisher’s Exact dalam SPSS versi 29. Hasil: Hasil penelitian didapatkan mayoritas pasien LES berjenis kelamin perempuan (98,6%) dengan kelompok usia 18-25 tahun (40,6%), dan mayoritas dengan derajat aktivitas penyakit berat (53,6%). Penelitian ini didapatkan angka kejadian anemia pada derajat aktivitas penyakit ringan-sedang (87,5%) dan derajat aktivitas penyakit berat (89,2%) dengan hasil uji statistik didapatkan nilai p >0,05. Kesimpulan: Penelitian ini menunjukkan bahwa tidak terdapat hubungan yang signifikan antara derajat aktivitas penyakit dengan kejadian anemia pada pasien LES di RS Dr. M. Djamil Padang.

Abstract

This study was a laboratory-based experimental study using a post-test control group design to evaluate the effect of curd administration on body weight and blood glucose levels in hyperglycemic rat model. Male Wistar rats (Rattus norvegicus) aged 8–10 weeks with body weights of 250–300g were used and randomly divided into four groups: a negative control group without diabetes induction or treatment, a positive control group with diabetes induction without therapy, and two treatment groups receiving curd at different doses, P1 (1.8750 g/250 g body weight) and P2 (3.750 g/250 g body weight). The mean body weight was 277.67 g in the negative control group and 227.50 g in the positive control group. Treatment groups showed increased body weights of 246.83 g in P1 and 262.50 g in P2. Blood glucose levels were 125.56 mg/dL in the negative control group and increased to 487.00 mg/dL in the positive control group, while curd administration reduced levels to 122.17 mg/dL in P1 and 101.83 mg/dL in P2. These results indicate that curd improves body weight and lowers blood glucose in hyperglycemic rats, with greater effects at higher doses, suggesting its potential as a functional food for metabolic and glycemic control.

Abstract

This research was motivated by the low argumentation skills of fourth-grade students at SDN 09 Koto Baru, who tended to be passive and faced difficulties in expressing logically structured opinions. This study aimed to examine and describe the effect of using the Talking Chips cooperative learning model on students' argumentation skills based on Toulmin's argumentation components (Claim, Data, Warrant, and Rebuttal). The research utilized a quantitative approach with an experimental method through a Pre-Experimental Design in the form of a One-Group Pretest-Posttest Design. The research sample consisted of a saturated sample of 23 fourth-grade students. The data collection instrument employed an oral test based on contextual dilemma stories evaluated using an observation sheet of argumentation skills adapted from Toulmin's model with a 1-4 scale. The data were analyzed using the Paired Samples T-Test and Normalized Gain (N-Gain) Score analysis. The results indicated a significant increase in the classical mean score of argumentation skills, rising from 31.39 in the pre-test to 77.65 in the post-test. Based on the hypothesis testing, the obtained was -29.969 with a significance value (2-tailed) of 0.000 < 0.05, thereby rejecting the null hypothesis ( ) and accepting the alternative hypothesis ( ). Furthermore, the classical N-Gain analysis yielded a score of 0.682 (68.2%), which falls into the moderate category and is interpreted as quite effective. In conclusion, the Talking Chips cooperative learning model has a significant effect on improving the argumentation skills of fourth-grade students at SDN 09 Koto Baru in Pancasila Education subject.
