Abstract

Data from the International Diabetes Federation (IDF) in 2021, the number of people with Diabetes Mellitus (DM) in the world reached 537 million adults. Indonesia is ranked 7th with 10.7 million. Lampung Province in 2021 there is an increase in the prevalence of DM to 88,518 people. The aim of application of Evidence Based Medicine (EBM) based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on the patient problem solving framework with a patient centered and family approach. This study analysis is a case report. Primary data were obtained through anamnesis (autoanamnesis), physical examination and home visits to complete family data, psychosocial and environmental data. Assessment was done based on holistic diagnosis from the beginning, process and end of the study quantitatively and qualitatively. Patient Mrs. M, 59 years old has internal risks, the patient rarely exercises, physical activity is relatively light, taking medicine irregularly, overweight nutritional status, Nutritional Adequacy Level (TKG) is less, still consuming foods that are not recommended for DM patients (consumption of fried foods), and lack of knowledge. Management of patients is holistic and comprehensive, patient-centered, family approach, and community-based in literature based on EBM. Patients were intervened with material in the form of power points. Patients experienced an increase in knowledge by 50 points, a decrease in blood sugar levels by 103 mg/dL and changes in patient behavior to consume food according to TKG.

Abstract

Scabies is a dermatological disorder caused by the parasitic mite Sarcoptes scabei which is capable of digging tunnels in the skin and causing itching that gets worse at night. The aim of this study is to apply family doctor services based on evidence-based medicine to patients by identifying risk factors, clinical problems, and patient management based on a framework for solving patient problems using a patient centered and family approach. This study is a case report. The data obtained were primary data obtained through alloanamnesis, physical examination, and home visits and secondary data obtained from the patient's medical records. Patients and their families have poor personal hygiene. The patient's family has insufficient knowledge about scabies, the habit of using towels together, and there is no regular schedule for washing and changing bed sheets, pillowcases and blankets. After carrying out holistic and comprehensive management using a family medicine approach, patient evaluation results were obtained in the form of increased knowledge of the patient's family regarding scabies, reduced complaints, and changes in behavior in maintaining personal and environmental hygiene.

Abstract

Tuberculosis cases in children at 2022 reach 1.1 million in the world and 100,726 cases in Indonesia. Management of Tuberculosis in children needs holistic treatment with a family doctor approach to achieve Tuberculosis elimination targets. The aim of this study is  to apply the principles of family medicine services to patients  and carry out holistic management based on the patient problem found with patient-centered approach, family focused, and community oriented based on Evidence Based Medicine. This study is a case report with Primary data obtained through history taking, physical examination and home visits to complete family, psychosocial and environmental data. Secondary data was obtained from the patient's medical record. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study. Diet and health behavior of patients classified as lacking. Knowledge about Tuberculosis disease and the implementation of healthy behavior in the family still poorly. After the intervention, there was an improvement in the diet and the patients diet behavior. Then there is an increase in knowledge through pretest and posttest as well as improvement in family life patterns to become healthier.  After holistic management was carried out based on the problems identified through family doctor approach, there was an increase in knowledge, nutritional intake, and better healthy behavior.

Abstract

Carpal Tunnel Syndrome (CTS) is a clinical syndrome that arises due to the depression of the median nerve in the carpal tunnel (tunnel carpal) on the wrist, which causes pain, numbness, and tingling in the distribution of median nerve. CTS risk factors include obesity, monotonous wrist activity, pregnancy, genetic offspring, and rheumatic inflammation. So it is necessary to handle proper and carried out holistically with a family medicine approach to achieve the success of therapy. The purpose of this study was to implement the service of an evidence-based medicine based family doctor in patients by identifying risk factors, clinical problems, and patient management based on a framework for solving patient problems with the Patient Centered and Family Approach approach. This study is a case report. The data obtained are primary data obtained through autoanamnesis, alloanamnesis, physical examination, and home visits and secondary data obtained from the patient's medical record. Patient Ny. T 56 years old comes with complaints of thumb, index finger, and middle finger of the right hand feels numb since one month ago. Patients also often complain of his fingers feel like shock and spread to the arms, until lately the patient cannot hold or hold objects. Patients work as washerwashers who work daily to wash and make the same movements repeatedly. Clinically the patient is diagnosed with CTS. Furthermore, management, intervention and evaluation are carried out. After holistic and comprehensive management, patients experience increased knowledge about their disease, and complaints of numbness in patients are reduced.

Abstract

Diabetes Mellitus is a metabolic disorder characterized by chronic hyperglycemia. Diabetic ulcer is a state of ulcer, infection, and/or tissue damage, which is associated with neurological disorders and peripheral vascular disease in the lower extremities. The family approach in the management of diabetic ulcers and Diabetes Mellitus helps to identify factors that influence both clinically, personally and psychosocially in the family so that management will be more comprehensive. The purpose of this study was to application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient problem-solving framework using a patient-centered and family approach. This study is a case report. Primary data were obtained through anamnesis (autoanamnesis), physical examination, supporting examination and physical environment. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. The patient complained of an open wound on the leg, numbness on both left, frequent urination and easy thirst. The patient has been diagnosed with DM since 2019. The patient was diagnosed with DM based on history, physical examination and supporting examinations. Knowledge of patients and families about the disease they suffer is lacking. After the intervention, the patient showed clinical improvement and there was a change in behavior. In the quantitative assessment, there was an increase in all aspects assessed, namely knowledge, diet and physical activity. Diagnosis and management of these patients is carried out in a holistic and comprehensive manner, patient center, family appropriate with regular DM treatment according to EBM is needed in order to control blood sugar by changing the patient's behavior so as to prevent other complications from arising.

Abstract

Diabetes Mellitus is a degenerative disease with a high prevalence and increasing every year. There is a possibility that there will be an increase in the number of people with Diabetes Mellitus in the future, so that comprehensive management must be carried out which is expected to prevent complications and improve the quality of life of patients. Application of evidence-based medicine-based services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered problem-solving framework with a patient-centered, family-focused, and community-oriented approach. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits to assess the physical environment. The assessment is based on a holistic diagnosis of the initial, process, and final studies qualitatively and quantitatively. After the intervention, there was an increase in the knowledge of patients and their families regarding the importance of controlling health workers, taking medication regularly, eating patterns and physical activities that are suitable for people with Diabetes Mellitus. Holistic management has been carried out using the family doctor approach, Mrs. S, 52 years old with Type 2 Diabetes Mellitus adjusted based on initial holistic diagnostics. The interventions carried out have increased patient and family knowledge regarding the importance of control to health workers, regular drug consumption, dietary patterns and physical activity as indicated by improvements in the final holistic diagnostic.

Abstract

Stress is a bodily response to a stressor, which refers to any condition perceived as threatening or causing pressure. Stress can have both positive (eustress) and negative (distress) effects. Prolonged distress can affect various body systems through the activation of the Hypothalamus-Pituitary-Adrenal (HPA) axis and the Sympathetic-Adreno-Medullary (SAM) axis. Activation of these axes triggers the release of cortisol, epinephrine, and norepinephrine, leading to physiological and behavioral changes. Stress impacts multiple organ systems, including the cardiovascular, respiratory, gastrointestinal, hepatobiliary, urinary, reproductive, musculoskeletal, integumentary, sensory, and immune systems. It also affects sleep quality and mental health. In the cardiovascular system, stress can cause hypertension, atherosclerosis, and an increased risk of heart disease. In the respiratory system, stress worsens asthma and increases the risk of airway infections. In the gastrointestinal system, stress triggers digestive disorders, such as gastritis and peptic ulcers, due to increased stomach acid production and inflammatory mediators. Stress affects the hepatobiliary system by impairing liver function through increased inflammation. In the urinary system, stress can cause dysfunctions such as enuresis and increased urination frequency. In the reproductive system, stress can result in menstrual disorders in women and decreased testosterone levels in men. The musculoskeletal system can be affected by excessive muscle contractions, leading to pain and muscle fatigue. Stress impacts the integumentary system, triggering conditions such as acne, psoriasis, and alopecia areata. It can also cause sensory system disorders, affecting hearing, taste, and vision. In the immune system, stress suppresses immune responses, increasing susceptibility to infections and prolonging wound healing. Prolonged stress negatively impacts both physical and mental health, highlighting the importance of understanding the mechanisms through which stress affects the body to reduce the risk of serious health problems.

Abstract

Appendicitis is inflammation of the vermiform appendix that can lead to perforation if not treated promptly. This study aims to identify the characteristics of appendicitis patients at RSUD Dr. H. Abdul Moeloek Lampung Province from January 2023 to July 2024. The method used was a descriptive observational study using secondary data from medical records of appendicitis patients. The sample was taken using total sampling, which was then selected based on inclusion and exclusion criteria, involving 84 patients with complete medical records and histopathology results. The results showed that the majority of appendicitis patients were in the 21-60 age range (49%), followed by the 0-20 age range (44%) and the >60 age group (7%). Based on gender, more male patients (56%) were found compared to females (44%). The most common type of appendicitis was simple acute appendicitis (48.8%), followed by perforated acute appendicitis (36.9%) and chronic appendicitis (14.3%). These findings are consistent with other studies showing a higher incidence of appendicitis in young adults and a higher prevalence in males. This study provides important insights into the characteristics of appendicitis patients, which may aid in early detection and more effective management.

Abstract

Students are a very valuable asset for the future of the nation. Good nutritional status is essential to support student achievement. However, currently Indonesia is experiencing a double nutritional problem in the form of malnutrition and overnutrition at school age. In addition, there are still few students in Indonesia who are accustomed to having breakfast with quality food in the morning. One of the factors that influences students' nutritional status is breakfast habits. This study aims to determine the relationship between breakfast habits and students' nutritional status. The method used is a literature review of 13 journals or articles from the Google Scholar database related to the Relationship between Breakfast Habits and Students' Nutritional Status published from 2015 to 2025. The results of this study indicate that the 12 journals or articles reviewed have a significant relationship between breakfast habits and nutritional status in students. Students who are accustomed to having breakfast will find it easier to meet their daily nutritional needs so that their nutritional status tends to be normal. However, students who are not accustomed to having breakfast will miss the opportunity to meet 15-30% of their daily nutritional needs so that their nutritional status is lacking. In addition, students who do not have breakfast tend to consume excessive lunch and high calories during the day so that their nutritional status is more. The conclusion of this study is that there is a relationship between breakfast habits and nutritional status in students.

Abstract

A normal menstrual cycle reflects good reproductive health and plays an important role in preparing the body for pregnancy. Based on the 2018 Basic Health Research, around 13.7% of women aged 10-59 years experienced irregular menstruation. One of the factors that can interfere with the normalcy of the menstrual cycle is abnormal nutritional status. This study aims to determine whether there is a relationship between nutritional status and the menstrual cycle in University of Lampung Medical Education students class of 2021. The type of research used was analytical observational with a cross-sectional approach, which was carried out from November 2024 to January 2025, involving 123 female students selected through simple random sampling technique. Data on nutritional status was obtained from the calculation of Body Mass Index (BMI), while menstrual cycle data was obtained using a menstrual cycle sheet. The results of the analysis using the mann-whitney test showed a p-value of 0.083 (p-value>0.05), which indicated that there was no significant relationship between nutritional status and menstrual cycle among University of Lampung medical education students Class of 2021.
