Abstract

The global COVID-19 pandemic has posed unprecedented challenges to global health. Dr. Soedomo Regional General Hospital in Trenggalek, which is involved in the management of COVID-19 patients, has witnessed a long journey in responding to and treating individuals infected with the SARS-CoV-2 virus. The use of acetaminophen has been a primary focus in efforts to improve standards of care and understanding symptom management in COVID-19 patients. Chronopharmacological analysis of acetaminophen use in COVID-19 patients at Dr. Soedomo Hospital aims to provide insight into how the timing of drug administration can affect therapeutic responses. There is evidence that the body's circadian rhythm can influence the metabolism and pharmacological effects of drugs, but this perspective has not been fully explored in the context of COVID-19. This study involved an analytical observational approach with a Cross-Sectional design and retrospective data collection method from the medical records of COVID-19 patients treated at Dr. Soedomo Regional Hospital in Trenggalek District. The results of the study showed that acetaminophen therapy at 16.00 WIB resulted in a more significant reduction in body temperature compared to other times. A deeper understanding of the body's circadian rhythm can help optimize the benefits of therapy and reduce potential side effects. Although there are limitations to this study, these results provide a basis for the development of more targeted and personalized treatment strategies. The clinical implications of this research can help in determining the optimal timing for administering therapy to achieve desired effects in COVID-19 patients.

Abstract

The emergence of the SARS-CoV-2 virus in 2020 led to the onset of the COVID-19 pandemic, posing significant challenges globally. As of December 19, 2021, the worldwide confirmed cases surpassed 273 million, with a devastating death toll exceeding 5.3 million. Indonesia reported its first case on March 2, 2020, and has since faced escalating numbers, necessitating urgent attention.Chronic kidney disease (CKD) has emerged as a commonly occurring comorbidity among COVID-19 patients, drawing attention due to the vulnerability of CKD patients to medical complications. Understanding the use of antiviral drugs in CKD patients with COVID-19 is essential, considering the risks of drug toxicity and interactions. This study aims to investigate the frequency of antiviral drug usage in COVID-19 patients with CKD, providing insights into treatment strategies and guiding medical practitioners. Conducted at Dr. Soedomo Trenggalek Regional Hospital, the research utilizes a cross-sectional, non-experimental design and purposive sampling method. Data collection involves patients' medical records, focusing on demographics, antiviral drug usage, disease severity, treatment duration, mortality rates, and potential side effects. The demographic analysis reveals that the majority of COVID-19 patients with CKD are aged 50 and above, with males comprising 74% of the cohort. Age and gender influence disease severity, with older individuals and males at higher risk. Antiviral drug usage predominantly includes favipiravir (64%) and remdesivir (36%), underscoring their importance in treatment protocols. Decisions regarding antiviral therapy in CKD patients with COVID-19 require careful consideration of benefits, risks, and patient characteristics.

Abstract

Type 2 DM can be a risk factor for arteriosclerotic cardiovascular disease complications (coronary heart disease, stroke, and peripheral arterial disease), heart failure, chronic renal failure, and cardiovascular risk. This cross-sectional study was conducted on 100 type 2 diabetic patients who were prescribed anti-diabetic therapy. The prescription pattern was analyzed and adherence to treatment guidelines was done by comparing with the 2021 PERKENI guidelines. The results of the distribution of the characteristics of T2DM respondents at X Hospital 2022 were mostly aged> 45 years as much as 87%, the most gender was male as much as 54%, the most comorbidities were 97% of respondents, the most treatment pattern was monotherapy as much as 64%, the highest number of drugs received by patients was 81% with criteria> 5 drugs.

Abstract

Research on the bioactivity of Antarctic organisms has garnered significant attention due to the continent's unique environmental conditions and the potential therapeutic value of its inhabitants. This study delves into the bioactivity of Pagodroma nivea, commonly known as the Snow Petrel, found in Antarctica, aiming to explore its medicinal potential. Despite previous research highlighting the bioactivity of various organisms in extreme environments, specific investigations into Pagodroma nivea remain scarce. Farmacoantarctic Research offers a promising avenue for exploring the therapeutic properties of Antarctic organisms, including Pagodroma nivea, traditionally used for medicinal purposes by local communities. Understanding the biochemical adaptations of organisms to Antarctica's extreme conditions is crucial, as these adaptations may yield bioactive compounds with pharmacological potential. The research not only seeks to uncover the bioactivity of Pagodroma nivea but also aims to contribute to drug development and environmental conservation efforts. In a parallel discussion, the study examines Mumijo, a traditional remedy found in various regions, including Antarctica. Chemical analysis of Mumijo extract, particularly the ester wax fraction, reveals its potential biomedical applications, including wound healing, skin diseases, and neuroprotection. The presence of glycerol-related ethers in Antarctic Mumijo suggests further avenues for exploration in neuroprotection and antimicrobial properties. By investigating the bioactivity of Antarctic organisms like Pagodroma nivea and Mumijo, this research offers insights into their therapeutic potential and underscores the importance of conservation efforts to preserve these unique ecosystems.

Abstract

Penelitian ini bertujuan untuk menganalisis perencanaan dana anggaran pada pengelolaan pelayanan ibu hamil di Puskesmas Suruh tahun 2023. Kesehatan ibu hamil merupakan indikator penting dalam penilaian kesehatan masyarakat, mempengaruhi kesejahteraan ibu serta kesehatan dan perkembangan bayi yang dikandung. Penyediaan layanan kesehatan yang optimal bagi ibu hamil menjadi prioritas dalam program kesehatan di berbagai daerah, termasuk di Puskesmas Suruh. Berdasarkan data dari Peraturan Daerah Kabupaten Trenggalek Nomor 5 Tahun 2022, alokasi anggaran untuk layanan kesehatan ibu hamil adalah Rp 71.812.500, dengan persentase 11,20% dari total anggaran sebesar Rp 640.747.000 yang dialokasikan untuk penyediaan layanan tingkat daerah kabupaten. Layanan ini mencakup pemeriksaan rutin, penyuluhan kesehatan, pemberian suplemen, dan penanganan komplikasi kehamilan. Implementasi anggaran dilakukan dengan mempertimbangkan berbagai aspek, termasuk pengadaan sarana dan prasarana, pelatihan tenaga kesehatan, serta kegiatan monitoring dan evaluasi. Melalui analisis SWOT (Strengths, Weaknesses, Opportunities, Threats), penelitian ini mengidentifikasi kekuatan seperti dukungan dari pemerintah dan ketersediaan tenaga kesehatan yang kompeten, serta kelemahan seperti keterbatasan anggaran dan masalah administratif. Peluang yang dapat dimanfaatkan meliputi program pemerintah yang mendukung dan kerjasama dengan sektor swasta, sedangkan ancaman termasuk perubahan kebijakan anggaran dan bencana alam. Kesimpulan dari penelitian ini menekankan pentingnya perencanaan dana anggaran yang baik dan implementasi yang transparan untuk meningkatkan kualitas pelayanan kesehatan ibu hamil di Puskesmas Suruh, yang diharapkan dapat memberikan dampak positif bagi kesejahteraan ibu dan anak serta masyarakat secara keseluruhan.

Abstract

This study aims to evaluate the cost-effectiveness of the immunosuppressant drug methylprednisolone at Gembleb Farma Pharmacy in Trenggalek, East Java, Indonesia. The research was conducted on May 30, 2025, using a descriptive quantitative approach supported by interviews with the responsible pharmacist. Data collection involved daily sales records of methylprednisolone and clinical response outcomes among patients. Each patient typically received five tablets per treatment, and the pharmacy recorded an average daily sale of 250 tablets. Therefore, it was estimated that approximately 50 patients were served per day. From those, 45 patients—equivalent to 90%—experienced favorable clinical responses to methylprednisolone therapy. The cost per tablet was IDR 500, resulting in a total daily cost of IDR 125,000. The calculated Cost-Effectiveness Ratio (CER) per day was IDR 2,777.78 per effectively treated patient. This value remained consistent when scaled to monthly (IDR 3,750,000 for 1,350 effective patients) and annual estimates (IDR 45,625,000 for 16,425 effective patients), yielding the same CER value of IDR 2,777.78. The findings indicate that methylprednisolone provides a stable and predictable cost-effectiveness profile in outpatient pharmacy settings. These results may support decision-making in therapeutic planning, particularly in community pharmacies that aim to deliver effective yet affordable immunosuppressive therapies. Additionally, this cost-efficiency aligns with the goals of rational drug use and sustainable pharmaceutical services at the primary healthcare level.

Abstract

Fenbufen Butanamine is a structural derivative of the non-steroidal anti-inflammatory drug (NSAID) Fenbufen, characterized by the addition of a butanamine group to its core structure. This modification is intended to improve the pharmacokinetic and pharmacodynamic properties of the parent compound, potentially enhancing its therapeutic efficacy. In this study, we conducted a detailed computational analysis of Fenbufen Butanamine’s chemical structure and molecular descriptors using data retrieved from the PubChem database. The chemical identity was confirmed by its IUPAC name, InChI, InChIKey, and SMILES notation, which provide precise molecular characterization essential for database referencing and further computational modeling. Key computational descriptors, such as molecular weight, logP, topological polar surface area (TPSA), and the count of hydrogen bond donors and acceptors, were analyzed to assess drug-likeness and predict pharmacokinetic behavior. Our findings indicate that Fenbufen Butanamine possesses a biphenyl core that contributes to hydrophobic interactions, while the butanamine side chain introduces hydrophilic properties. This amphiphilic nature is likely to influence the compound’s solubility, membrane permeability, and binding affinity to biological targets. The computed descriptors suggest favorable properties for oral bioavailability and potential interactions within biological systems. These results serve as a foundational step for in silico drug design and optimization of Fenbufen derivatives. Further experimental and pharmacological studies are warranted to validate these computational predictions and explore the therapeutic potential of Fenbufen Butanamine.

Abstract

Training in the skill of administering oral medication to test animals is a form of community service aimed at enhancing pharmacy students' competencies in pharmacology laboratory practices. This activity will take place on May 19, 2025, in the Pharmacology Laboratory at Kadiri University. It will consist of direct training that includes theoretical presentations, demonstrations, and independent practice by students, accompanied by guidance from instructors and laboratory staff. The materials presented include the anatomy of the mouse digestive system, the basic principles of oral gavage, the ethics of using test animals, and safety aspects. The evaluation results indicate that this training has enhanced the participants' understanding of oral gavage techniques as well as their skills in performing the procedure safely and accurately. Based on the comparison of pretest and posttest scores, as well as practical observations, there is a significant improvement in the technical skills and ethical attitudes of the students. This training also enhances the understanding of the importance of animal welfare in pharmacological experiments. This activity is expected to serve as a sustainable model for enhancing the quality of practical learning in the field of pharmacy education.

Abstract

The Professional Pharmacy Practice Experience (PKPA) at Community Health Centers and Health Offices plays a crucial role in developing the professional competence of pharmacy students before they enter the actual workforce. To facilitate this preparation, an interactive discussion activity has been conducted as a means of training and collective reflection among students, supervising lecturers, and resource persons from the Health Office and community health centers. This discussion aims to enhance students' understanding of the roles and responsibilities of pharmacists within primary healthcare facilities (FKTP), while also identifying the challenges and solutions involved in the implementation of PKPA. The execution of the discussion demonstrated a high level of enthusiasm among students in exploring both the technical and non-technical aspects of the pharmacy profession, including the integration of clinical and managerial roles. The evaluation of the questionnaire reveals that over 85% of participants believe this activity significantly aids in preparing them to confront the PKPA with greater confidence. Group discussions highlight the significance of interpersonal communication skills, professional ethics, and teamwork collaboration as essential factors for the success of practice. A representative from the Health Department assessed that this activity is effective in aligning the perspectives between educational institutions and practical venues, thereby fostering ongoing synergy.

Abstract

Beyond Use Dates (BUD) obat merupakan informasi penting terkait batas waktu keamanan penggunaan obat setelah kemasan dibuka atau sediaan dibuat, yang seringkali belum dipahami secara optimal oleh masyarakat. Kegiatan pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan publik mengenai BUD melalui edukasi interaktif pada acara Apothecary Fun Run 2025 yang diselenggarakan Ikatan Apoteker Indonesia (IAI) di Stadion Menak Sopal, Trenggalek, pada 10 Agustus 2025. Metode kegiatan meliputi presentasi visual, diskusi interaktif, simulasi penandaan BUD pada kemasan obat, serta pembagian leaflet informasi. Evaluasi dilakukan menggunakan pre-test dan post-test, disertai survei kepuasan peserta. Hasil menunjukkan peningkatan skor pengetahuan rata-rata dari 54,3 menjadi 88,5 poin, dengan persentase pemahaman konsep BUD naik dari 40% menjadi 92%. Tingkat kepuasan peserta sangat tinggi, dengan 94% menyatakan “Sangat Memuaskan” atau “Memuaskan”. Edukasi interaktif berbasis praktik nyata terbukti efektif dalam meningkatkan literasi kesehatan terkait keamanan obat. Program ini direkomendasikan untuk diimplementasikan secara berkelanjutan dalam kegiatan komunitas, guna meminimalkan risiko penggunaan obat yang sudah melewati BUD.
