Abstract

Latar Belakang: Hipertensi merupakan salah satu penyakit kronis dengan prevalensi tinggi dan menjadi faktor risiko utama penyakit kardiovaskular. Terapi farmakologis menggunakan obat antihipertensi harus dilakukan secara rasional agar efektif mencapai target tekanan darah serta mencegah komplikasi. Tujuan: Mengetahui pola dan rasionalitas penggunaan obat antihipertensi pada pasien hipertensi rawat inap di RSUD Daha Husada Kota Kediri tahun 2024. Metode: Penelitian ini bersifat observasional dengan desain cross sectional menggunakan data rekam medis pasien hipertensi periode Januari–Desember 2024. Sampel dipilih secara purposive dengan kriteria inklusi pasien hipertensi yang mendapat terapi antihipertensi tunggal maupun kombinasi. Data dianalisis secara deskriptif untuk melihat distribusi penggunaan obat. Hasil: Dari 103 pasien yang memenuhi kriteria, mayoritas berjenis kelamin perempuan (62,14%) dengan kelompok usia terbanyak 56–65 tahun (47,57%). Lama rawat inap terbanyak adalah 3 hari (64,08%). Variasi terapi meliputi monoterapi (32,04%), kombinasi dua obat (49,51%), dan kombinasi tiga obat (18,45%). Obat terbanyak pada monoterapi adalah amlodipin (51,52%), sedangkan kombinasi terbanyak adalah amlodipin + candesartan (78,43%).Kesimpulan: Pola penggunaan obat antihipertensi di RSUD Daha Husada Kota Kediri menunjukkan dominasi terapi kombinasi, terutama ARB + CCB, sesuai dengan pedoman klinis nasional

Abstract

Hypertension is a chronic disease that requires long-term therapy with continuous use of drugs. This study aims to analyze the cost-effectiveness comparison between generic and branded antihypertensive drugs in patients at Gembleb Farma Trenggalek Pharmacy. The study was conducted by comparing demographic characteristics, therapy costs, clinical outcomes in the form of blood pressure reduction, and patient compliance levels. The results showed that the basic characteristics of patients in both groups were relatively equivalent so that comparisons could be made objectively. In terms of cost, branded drugs were on average almost three times more expensive than generic drugs (IDR 163,000 vs IDR 67,000 per month), while clinical outcomes showed no significant difference. The level of patient compliance in both groups was also relatively similar. Pharmacoeconomic analysis showed that generic drugs had a lower cost-effectiveness ratio, with the incremental cost-effectiveness ratio indicating the additional cost of branded drugs was not worth the minimal additional clinical benefit.

Abstract

This study aims to analyze the cost-benefit of simvastatin therapy compared to atorvastatin in dyslipidemia patients at Apotek 71 Kediri City using the Human Capital Method. Using a cross-sectional study design, this study analyzed 60 patients (30 simvastatin users, 30 atorvastatin users). Productivity loss data was collected through a patient self-report questionnaire and valued monetarily using the assumption of a uniform daily wage. Results showed that direct costs of simvastatin were lower (IDR 75,000/month) than atorvastatin (IDR 210,000/month), but lost productivity in the simvastatin group was higher (IDR 384,000) than atorvastatin (IDR 180,000). The Net Benefit value of simvastatin therapy was -Rp 39,000 with a Benefit Cost Ratio (BCR) of 0.91, while atorvastatin produced a Net Benefit of +Rp 210,000 with a BCR of 1.54. The difference in Net Benefit was statistically significant based on an independent t-test (p=0.002). Thus, atorvastatin is more economically efficient than simvastatin because it provides higher productivity benefits despite higher direct costs.

Abstract

Air merupakan komponen krusial dalam pembuatan media mikrobiologi, khususnya untuk pengujian sediaan kosmetik. Penggunaan air yang tidak memenuhi standar dapat menyebabkan kontaminasi, menurunkan validitas hasil uji, serta menghambat pertumbuhan mikroorganisme target. Oleh karena itu, laboratorium mikrobiologi wajib menggunakan air yang sesuai standar internasional seperti United States Pharmacopeia (USP) dan European Pharmacopoeia (EP), dengan pengawasan parameter seperti TOC, konduktivitas, dan cemaran mikrobiologi. PT. Triszie Lab Indonesia menyadari pentingnya edukasi mengenai standar mutu air, namun masih terdapat kesenjangan pengetahuan di antara personel laboratorium. Kegiatan pengabdian kepada masyarakat ini bertujuan memberikan pelatihan dan pendampingan terkait standar penggunaan air laboratorium. Metode yang digunakan meliputi observasi awal, pelatihan interaktif, penyusunan SOP, serta evaluasi melalui pre-test dan post-test. Hasil kegiatan menunjukkan peningkatan pemahaman teknis sebesar 50%, peningkatan praktik higiene, serta penerapan SOP yang sesuai standar USP dan WHO. Kegiatan ini juga mendorong perubahan budaya kerja dan kesadaran mutu di lingkungan laboratorium. Produk luaran seperti buku saku dan modul pelatihan mendukung keberlanjutan edukasi internal. Secara keseluruhan, kegiatan ini berkontribusi nyata terhadap peningkatan mutu pengujian mikrobiologi dan kesiapan laboratorium dalam menghadapi audit serta sertifikasi.

Abstract

The global COVID-19 pandemic has posed unprecedented challenges to global health, impacting healthcare institutions worldwide. Dr. Soedomo Regional General Hospital in Trenggalek has been actively involved in treating COVID-19 patients, emphasizing the need for improved care standards and effective symptom management. Paracetamol, a commonly used medication, is a key focus in this endeavor. This study investigates the chronopharmacology of paracetamol in COVID-19 patients, emphasizing the importance of timing in therapeutic interventions. The research, conducted at Dr. Soedomo Regional General Hospital, involves 100 COVID-19 patients undergoing paracetamol therapy. The analysis includes demographic data, paracetamol administration at different times, and the resulting body temperature changes. The findings reveal that the body's response to paracetamol varies significantly depending on the time of administration. Notably, at 4:00 PM, a more pronounced decrease in body temperature is observed, suggesting a heightened effectiveness of therapy. This phenomenon is attributed to the body's circadian rhythm, influencing physiological functions over a 24-hour period. This study lays the groundwork for the "right time, right dose" concept in clinical practice. Considering the circadian rhythm is essential in designing personalized and effective treatment protocols, aiming for optimal outcomes. Despite limitations, including a small sample size, this research provides valuable insights into the circadian aspects of drug efficacy, paving the way for more tailored treatment approaches.

Abstract

Pain management is a critical aspect of pharmaceutical research, and Acorus calamus L., commonly known as jeringau, has garnered attention for its potential analgesic properties. This study focuses on evaluating the analgesic activity of the chloroform fraction from ethanol extract of jeringau leaves (Acorus calamus L.) on male white mice induced by acetic acid. Chronic and acute pain significantly impacts individuals' quality of life, emphasizing the need for effective and safe analgesic alternatives. While Acorus calamus leaf extract has shown pharmacological potential, especially the chloroform fraction, a dedicated investigation into its analgesic effects on animal models is lacking. Male white mice, recognized for their reproducible and measurable responses to painful stimuli, serve as relevant subjects in this study. The population of 25 mice, aged 2-3 months and weighing 20-30 grams, is divided into 5 treatment groups, including negative and positive controls. The study employs three dosages of the chloroform fraction, administered orally, with observations made on writhing behavior following intraperitoneal injection of 0.5% acetic acid. Statistical analysis will evaluate significant differences between treatment groups, providing insights into the analgesic potential. Results indicate that Dosage III (44.8 mg/40g BW) exhibits the highest analgesic efficacy at 73.99%, showcasing a dose-dependent response. The mechanism involves flavonoids inhibiting the cyclooxygenase enzyme, aligning with established analgesic actions. Ibuprofen, the positive control, and jeringau leaf extract demonstrate analgesic effects, reinforcing traditional knowledge. This research contributes to scientific understanding, establishing a foundation for further exploration.

Abstract

This study explores the clinical characteristics, treatment patterns, and outcomes of COVID-19 inpatients without comorbidities, aiming to contribute valuable insights to the management of the disease. Through meticulous data collection from medical records of 468 confirmed COVID-19 patients admitted to Gambiran Regional General Hospital between April and June 2020, 36 patients meeting inclusion criteria were identified. The research findings reveal a higher incidence of COVID-19 among males and a prevalent age group of 31-40 years. Notably, elderly individuals and those with underlying health conditions face a greater risk of severe outcomes, underscoring the importance of tailored interventions for vulnerable populations. The study highlights a diverse treatment approach, with a significant proportion of patients receiving supplements to bolster immune function. Antiviral medications like Favipiravir are also utilized to inhibit viral replication, although definitive treatment for COVID-19 remains elusive. Antibiotics play a crucial role in preventing bacterial respiratory infections, while supportive medications help alleviate symptoms commonly associated with the disease. Importantly, the findings emphasize the multifaceted nature of COVID-19 management and the need for targeted interventions to optimize patient outcomes. By elucidating the therapeutic landscape and clinical course of COVID-19 inpatients without comorbidities, this study contributes to the growing body of knowledge surrounding the disease. These insights can inform clinical decision-making, resource allocation, and public health strategies aimed at combating the ongoing pandemic.

Abstract

This study investigated the potential anti-diarrheal effects of ethanol extract from Centella asiatica leaves using a castor oil-induced diarrhea model in mice. The extract, obtained from Centella asiatica leaves, was administered orally to mice at various doses. Parameters including the frequency, duration, and onset of diarrhea were measured and compared with control groups. Results indicated a dose-dependent reduction in diarrhea frequency and duration in treated mice. Higher doses of Centella asiatica ethanol leaf extract demonstrated efficacy comparable to loperamide, a standard anti-diarrheal drug. Histopathological analysis further revealed protective effects on intestinal tissue integrity, suggesting a potential mechanism of action for the observed anti-diarrheal activity. These findings underscore the potential of Centella asiatica ethanol leaf extract as a natural alternative for managing diarrhea. The study contributes to the growing body of evidence supporting the traditional use of Centella asiatica in folk medicine for gastrointestinal ailments. However, further research is warranted to elucidate the underlying mechanisms of action responsible for its anti-diarrheal effects and to conduct clinical trials for human validation. In conclusion, this research highlights the significance of exploring natural remedies derived from medicinal plants like Centella asiatica for combating gastrointestinal disorders. The study underscores the potential of Centella asiatica ethanol leaf extract as a therapeutic agent for managing diarrhea and improving public health outcomes.

Abstract

The research delves into the multifaceted realm of psychoactive drugs, spirituality, and the exploration of past life memories, known as "pharmacoreincarnation." Psychoactive substances like psilocybin, LSD, and DMT have gained attention for their profound alterations in consciousness and subjective experiences, often described as mystical or spiritual. Belief in reincarnation persists globally, fueled by anecdotal testimonials and cultural traditions. The study aims to explore pharmacoreincarnation through a comprehensive literature review and interdisciplinary perspective. Academic databases such as PubMed, PsycINFO, Scopus, and Google Scholar will be utilized to access relevant articles published in English within the last two decades. Thematic analysis will identify recurring themes and patterns across the literature, synthesizing findings to develop a coherent understanding of pharmacoreincarnation. The methodology involves systematic literature search, screening, and review processes to extract pertinent information aligned with the research objective. The synthesized findings will be organized into logical sections within the literature review, ensuring clarity and rigor. Peer review and revision will validate the quality and accuracy of the review before finalization. The study anticipates contributing significantly to knowledge on pharmacoreincarnation and its implications for consciousness studies, spirituality, and therapeutic practices. By embracing an interdisciplinary approach, it aims to unravel the underlying mechanisms of such experiences and their broader implications for personal identity and reality.

Abstract

Hypertension, often referred to as "The Silent Disease," presents a significant global health challenge due to its asymptomatic nature and association with severe cardiovascular complications. Effective management of hypertension primarily relies on the administration of antihypertensive drugs, either as monotherapy or combination therapy. This study investigates the utilization patterns of single antihypertensive drugs among hypertensive patients in the outpatient setting of Puskesmas "X" in Solo City. Utilizing a retrospective approach, data from patient medical records were analyzed to examine drug usage patterns and patient responses to therapy. Results revealed a noteworthy preference for combination therapy, with approximately 64.18% of patients opting for this approach. Combination therapy strategies were favored due to their additive and synergistic effects, potential for reducing side effects, and enhanced blood pressure control. Specifically, amlodipine from the calcium channel blocker (CCB) group emerged as the most commonly prescribed single antihypertensive drug, while the combination of amlodipine with candesartan from the angiotensin receptor blocker (ARB) group was the predominant combination therapy. These findings align with previous research, emphasizing the significance of combination therapies in hypertension management..
