Abstract

Purpose: This study aimed to analyze the knowledge, attitudes, and compliance with antiretroviral (ARV) antidotes among HIV patients at the Central Bangka Regional General Hospital. Research Methodology: This study used a qualitative method with a phenomenological approach. The technique for selecting respondents was a non-probability sampling technique, purposive sampling. Nine informants participated in 9 people. Data were collected using an in-depth interview guide. Results: Findings from the research show that several informants had insufficient knowledge about compliance in taking ARV antidotes. Even though the informants' attitude towards compliance with antiretroviral (ARV) antiretrovirals was relatively good, several key informants stated that they had stopped taking antiretroviral (ARV) antidotes for several months because they felt healthy. However, several months later, they experienced adverse effects that resulted in declining health. Conclusions: The study concludes that Inadequate knowledge and misconceptions about ARV therapy remain significant obstacles to sustained adherence. Strengthening education, family involvement, and consistent supervision are essential for enhancing compliance and improving health outcomes for people living with HIV. Limitations: The study was limited to a small number of participants from a single hospital, which restricts broader generalization. Contribution: This research provides practical insights for healthcare providers and policymakers to design targeted interventions, improve supervision, and foster cross-program collaboration to ensure consistent ARV adherence among patients with HIV, ultimately supporting public health goals in HIV management.

Abstract

Introduction: Sarcopenia is a common problem found in the elderly. It is characterized by loss of muscle mass, muscle strength, and/or low physical performance. Currently, there’s little research on the association between serum albumin and muscle mass, muscle strength, and physical performance in the elderly, especially in Indonesia. This study aimed to find the correlation between serum albumin and muscle mass, muscle strength, and physical performance in the elderly. Method: A cross-sectional study was conducted from July 2022 to December 2022 towards elderly patients (>60 years) from the geriatric polyclinic at RSUP Moh. Hoesin Palembang. Serum albumin was examined for its correlation with muscle mass, muscle strength, and physical performance using Spearman’s test. Result: 41 elderly patients were included in this study (mean age 70.75 ± 7 years, 56.1% women). Correlation analysis showed that there’s a significant positive correlation between serum albumin with muscle strength (r=0.354, p=0.012) and a negative correlation with physical performance (r=-0.5, p <0.001). In comparison, there was no significant correlation between serum albumin and muscle mass (r=- 0.05, p=0.367). Conclusion: Muscle strength and physical performance have a significant correlation with serum albumin in the elderly. Therefore, even in the normal range, lower serum albumin is closely related to sarcopenia in the elderly. Key Words: albumin, muscle mass, muscle strength, physical performance, sarcopenia.

Articles searched from Google Scholar, PubMed, Science Direct, and EMBASE/ Elviser with use relevant keywords , then chosen based on criteria that have been set , and objectives study . Results: Increased levels of IL-10 and Reactive Oxygen Species (ROS) in patients results in a decrease in the immune system, this is characterized by increased virus replication, deactivation macrophages and depletion of the number of lymphocytes, so that the progression of the infection towards AIDS can occur. Conclusion: HIV infection causes the occurrence dysfunction mitochondria and enhancement production Reactive Oxygen Species (ROS) in cells CD4 + T lymphocytes. Various viral proteins, such as the envelope proteins Gp120, Tat, Nef and Vpr , are able to increase ROS production through various different pathways. The increase in ROS that occurs can increase mRNA expression through activation Nuclear Factor Kappa B ( NFκB ), increased IL-10 mRNA expression ultimately will increase IL-10 levels

Abstract

Background: Chronic hepatitis B (CHB) disease is a major global health problem with a high burden in Asia, including Indonesia. Liver stiffness measurement by transient elastography is a non-invasive key tool to assess fibrosis severity. The immune response in CHB involves T cell exhaustion marked by elevated soluble PD-1 (sPD-1) and changes in T lymphocyte subsets, which may influence fibrosis progression. This study aimed to assess the correlation between serum sPD-1, CD4+, and CD8+ T lymphocyte counts and liver stiffness in antiviral-naïve CHB patients. Methods: A cross-sectional study was conducted among 60 untreated CHB patients at Mohammad Hoesin Hospital, Palembang, Indonesia. The sPD-1 was measured by ELISA, while CD4+ and CD8+ T cell counts were determined via flow cytometry. Liver stiffness was assessed by transient elastography. Spearman correlation and multivariate linear regression were used to analyze associations between immune markers and liver stiffness. Results: Serum sPD-1 showed a significant positive correlation with liver stiffness (r = 0.481, p 0.001), while CD8+ T lymphocyte count was negatively correlated (r = –0.553, p 0.001). CD4+ T lymphocyte count showed no significant bivariate correlation (r = –0.189, p = 0.149). In the multivariate analysis, sPD-1 (β = 0.050, p 0.001), CD4+ T cells (β = –0.018, p = 0.001), and AST (β = 0.061, p = 0.005) were independent predictors of liver stiffness. Conclusion: Serum sPD-1 and CD4+ T-lymphocyte count are independently associated with liver stiffness in antiviral-naïve CHB patients. These findings suggested the involvement of immune exhaustion and liver injury in HBV-related fibrosis.

Abstract

Background: Diabetic kidney disease (DKD) progresses through inflammation and fibrosis, with transforming growth factor-β (TGF-β) as the principal profibrotic mediator and albuminuria as a clinical surrogate of glomerular injury. Methods: We conducted a 12-week, double-blind, placebo-controlled randomized trial at Dr. Mohammad Hoesin General Hospital, Palembang, between October 2025 and January 2026 to evaluate adjuvant resveratrol on serum TGF-β, urinary albumin-to-creatinine ratio (UACR), and estimated glomerular filtration rate (eGFR). Results: Of 61 randomized adults with DKD on standard care, 54 were analyzed (resveratrol n=27 received 25 mg twice daily, derived from Polygonum cuspidatum in 95% lecithin; placebo n=27). Within the resveratrol group, serum TGF-β fell from 123.7 to 77.1 pg/mL (p=0.008) and UACR from 94.1 to 89.8 mg/g (p=0.017); within placebo, UACR rose from 81.9 to 112 mg/g (p=0.029) while TGF-β change was non-significant (p=0.428). Between-group ΔUACR was significant (p<0.001), whereas ΔTGF-β (p=0.303) and ΔeGFR (p=0.567) were not. Multivariable linear regression identified resveratrol as an independent predictor of UACR reduction (B=−394.12 mg/g; 95% CI −659.53 to −128.71; p=0.004; adjusted R²=0.129). Baseline TGF-β was the dominant predictor of ΔTGF-β (B=−0.81; p<0.001; adjusted R²=0.701), and baseline LFG stage predicted ΔeGFR (B=−4.76; p=0.021). Mild bloating was reported in 14.8% of resveratrol versus 11.1% of placebo recipients; no serious adverse events occurred. Conclusion: Adjuvant low-dose resveratrol reduces albuminuria and serum TGF-β over 12 weeks in DKD without short-term improvement in eGFR, supporting an antifibrotic biomarker signal that warrants longer trials.
