Abstract

Gout arthritis is a progressive chronic inflammatory disease due to deposition of monosodium urate (MSU) crystals in the joints due to long-lasting hyperuricemia with several associated risk factors including excessive consumption of purine compounds and lack of physical activity. Gout arthritis needs to be managed comprehensively and holistically through the role of the family doctor in order to improve the patient's quality of life and prevent complications that can occur. Apply the principles of family medicine services to patients based on the problems found and carry out holistic and comprehensive management using the principles of patient-centered and family approach based on Evidence Based Medicine. This study is a case report. Primary data were obtained from anamnesis, physical examination, and home visits to assess the physical environment. Secondary data was obtained from the patient's medical record. Assessment based on holistic diagnostics at the beginning, process, and end of the study. Mrs. M, 67 years old, came with complaints of joint pain in both knees since 1 week ago. The patient was diagnosed with gout arthritis. In this case, diagnosis and treatment have been carried out according to the theory and related journals. Management has been carried out with the approach of family doctor Ny. M 67 years old with Gout arthritis adjusted based on initial holistic diagnostics. The interventions carried out have increased patient knowledge and improved the healthy lifestyle of patients and their families as indicated by improvements in the final holistic diagnostic.

Abstract

Neurodermatitis Circumscripta is a chronic inflammatory skin condition that often occurs in individuals aged 30 - 50 years, characterized by itching and thickening of the skin due to repeated friction. Applying the principles of family medicine and implementing holistic management based on the patient-centered, family-focused, and community oriented approach grounded in Evidence-Based Medicine. This study is a case report utilizing primary data obtained through anamnesis, physical examination, and home visits to get family, psychosocial, and enviromental data. Secondary data were obtained from the patient’s medical records. Patient Mrs. I, 50 years old, came with a complaint of itching on the back of the right leg since the last 3 months, the itching increased when the patient was resting or at night. The patient often scratched it, causing thickening and darkening of the skin. The patient was diagnosed with neurodermatitis circumscripta. Holistic management was performed with a family physician-designed approach for Mrs I, 50 years old, with neurodermatitis based on the initial holistic diagnosis. The intervention has improved the patient’s knowledge and changed some behaviours of the patient and her family.

Abstract

Nurses are often interacted with patients for a long time, thus nurses have a high enough risk factor. Therefore, Nurses need Personal Protective Equipment (PPE) to protect themselves from various potential hazards in the workplace and addition to prevent work accidents. This study aims to determine the relationship between knowledge, attitudes, and behavior of using PPE in inpatient nurses with the incidence of work accidents in RSUDAM Lampung Province. This study used an observational method with a cross sectional approach with a proportional random sampling technique. The research subjects were 158 RSUDAM inpatient installation nurses and the measuring instrument was a questionnaire. Data analysis was performed using Fisher's exact test. The results of this study showed a good level of knowledge as much as 81.6%, 50.6% positive attitude, 70.3% good behavior in the use of PPE. Meanwhile, the category of low incidence of work accidents was 93.7%. The results of statistical tests showed that there was a significant relationship between knowledge (p = 0.001), attitude (p = 0.001), and behavior (p = 0.001) using PPE with the incidence of work accidents. There is a relationship between knowledge, attitudes, and behavior of using PPE on inpatient installation nurses with work accident at RSUDAM Lampung Province.

Abstract

Migraine is a neurovascular disease which is one of the main factors of disability in adults under 50 years. Migraines are generally characterized by headache attacks that are unilateral, recurrent, hereditary and multifactorial. The prevalence of migraine in the world reaches 10-14% with the highest incidence in North America, Central America and South America. In Indonesia alone, the prevalence of migraine has reached 3.5 million people with the highest coming from individuals aged 15-24 years. Until now, the exact cause of migraines is not known. However, there are many risk factors that can influence the occurrence of migraines, including work-related factors. Occupational factors related to migraines include work stress, environmental factors, and excessive use of electronic screens. Work-related stress can be caused by workload, time pressure, and relationships with coworkers. Work environmental factors can include excessive lighting, noise, changes in temperature and air pressure, and strong odors. Meanwhile, the use of electronic screens can influence migraines because they are related to the use of blue light, computer flashes, light reflections, and prolonged exposure to light. The diagnosis of migraine can be made through the results of the history and physical examination, and if necessary, supporting examinations can also be carried out to rule out other secondary causes that may influence migraine.

Abstract

Menstruation is a natural process in which the inner uterine lining of the uterus is regularly released every month in women. The most vulnerable period for a person to experience menstrual disorders is during the first year, about 75% of adolescent girls experience menstrual disorders. According to the World Health Organization (WHO), 75% of adolescent girls experience menstrual cycle disorders. Risk factors that can cause disruptions in the menstrual cycle include hormonal imbalances due to environmental stressors and excessive physical activity and low energy intake. In addition, exposure to pollutants such as cigarette smoke, stress, depression, short sleep, Body Mass Index (BMI), and certain medical conditions can also affect the menstrual cycle. This study is an analytical study with cross-sectional design. The study was conducted at SMAN 5 Bandar Lampung in October - December 2023. The research instruments used in this study were questionnaires to assess sleep quality, namely PSQI, questionnaires from who, namely GPAQ to measure physical activity, and menstrual cycle questionnaires to assess menstrual disorders in respondents. Chi-square test results with a confidence level of 95% showed a p-value of 0.265 (p-value > 0.05) on sleep quality variables and p-value of 0.006 (p-value < 0.05) on physical activity variables. There is no relationship between sleep quality and physical activity with the menstrual cycle in adolescent girls at SMAN 5 Bandar Lampung in 2023. Abstract

The female reproductive system undergoes regular cycles in preparation for pregnancy and fertilization, known as the menstrual cycle. This cycle involves hormonal changes and complex physiological processes, with the follicular phase dominated by estrogen and the luteal phase dominated by progesterone. Menstrual cycle disorders, such as amenorrhea, polymenorrhea, and oligomenorrhea, can be caused by various factors including hormonal imbalances, unhealthy lifestyles, and certain medical conditions. Management of menstrual disorders varies depending on their causes, which may require a holistic and individualized approach. A good understanding of the menstrual cycle and its disorders is crucial for accurate diagnosis and appropriate treatment. It is also important to promote overall women's reproductive health.

Abstract

Kusta atau Morbus Hansen (MH) adalah penyakit menular kronis dengan keterlibatan yang menonjol pada kulit dan saraf yang disebabkan oleh Mycobacterium leprae. Penyakit kusta merupakan salah satu penyakit tropis yang terabaiakan yang masih banyak di dunia khususnya di Indonesia. Distribusi penyakit kusta di Provinsi Lampung pada tahun 2021 terdapat 136 kasus dan 21 kasus kusta di Kota Bandarlampung. Hipertensi adalah penyakit tidak menular yang merupakan masalah kesehatan utama di dunia maupun Indonesia. Kusta dan hipertensi dapat dihindari dan dikontrol melalui pola hidup sehat yang membutuhkan dukungan dari keluarga maupun komunitas maka dari itu penting dilakukan penatalaksanaan holistik melalui pendekatan keluarga pada pasien. Tujuan penulisan untuk menerapkan pendekatan dokter keluarga secara holistik dan komprehensif sesuai masalah yang dialami oleh pasien dan melakukan penatalaksanaan berbasis Evident Based Medicine yang bersifat patient-centered, family oriented dan community oriented. Studi ini merupakan sebuah laporan kasus. Data primer diperoleh melalui anamnesis (autoanamnesis), pemeriksaan fisik dan kunjungan rumah, untuk melengkapi data keluarga, data psikososial dan lingkungan. Data sekunder didapat dari rekam medis pasien. Penilaian berdasarkan diagnosis holistik dari awal, proses, dan akhir studi secara kualitatif dan kuantitatif. Setelah dilakukan intervensi, terdapat peningkatan pengetahuan pasien dan keluarganya mengenai penyebab, faktor risiko, serta mencegah kekambuhan pada penyakit kusta dan hipertensi. Penatalaksanaan secara holistik dapat meningkatkan pengetahuan, sikap, dan perilaku pada pasien. Selain itu, peran keluarga amat penting dalam perawatan dan pengobatan pasien serta dukungan juga diperlukan.

Abstract

Aging is a natural process experienced by all living beings, marked by physiological, psychological, and social changes in individuals over the age of 60, as well as a decline in functional capacity and structural changes in the body. Many elderly individuals reside in social care facilities, which, although providing care for the neglected or less fortunate elderly, often face negative views from society, especially those that adhere to strong family values, which can affect their quality of life. This study examines the culture and quality of life of elderly individuals living in social care facilities through a systematic review. Data were gathered from relevant articles using related keywords from various databases. The findings of this study indicate that elderly individuals residing in social care facilities tend to have lower quality of life, due to limited social interactions and health-related issues. Factors influencing the quality of life of the elderly include physical, social, psychological, and environmental aspects, which are often affected by health conditions, physical limitations, and the lack of social interactions in social care facilities. On the other hand, cultural values also play a role in shaping societal views of social care facilities, particularly in regions with strong family-oriented values. Recommendations to improve the quality of life for the elderly include increasing attention to both their physical and psychological health, while ensuring that cultural values are respected in elderly social care services.

Abstract

Acute ischemic stroke is a leading cause of morbidity and mortality worldwide, significantly impacting patients' quality of life and healthcare systems. Understanding prognostic factors that predict clinical outcomes in acute ischemic stroke patients is crucial, especially with the rising prevalence of risk factors such as hypertension, diabetes, and dyslipidemia. Recent studies have focused on hematological biomarkers, particularly the platelet-to-white blood cell ratio (PWR), which reflects the interaction between immune system components and hemostasis. This literature review examines the prognostic value of PWR in acute ischemic stroke, correlating higher PWR with better clinical outcomes measured by the National Institute of Health Stroke Scale (NIHSS). Lower PWR values have been associated with increased mortality risk within 90 days post-stroke. The dynamics of PWR provide insights into the body's response to ischemia and inflammation, highlighting its potential as a valuable prognostic marker. While PWR shows promise as a prognostic tool, limitations such as individual variability in inflammatory response, timing of measurement, and lack of standardization must be considered. Further research is needed to explore PWR's potential across diverse populations and address existing limitations to enhance its clinical application in managing acute ischemic stroke.

Abstract

Water is an essential necessity for humans on Earth. Its role is crucial for daily activities, including household needs, industry, offices, and food production. In Indonesia, each person is estimated to require approximately 30–60 liters of water per day, whereas in developed countries, the demand per person ranges between 60–120 liters per day. The feasibility of water usage is regulated based on quality characteristics, referred to as water quality. One of the key indicators related to household water quality is biological parameters, particularly concerning the presence of coliform bacteria and Escherichia coli. These bacteria have a fecal-oral entry route, meaning that if consumed in excessive amounts, they can cause gastrointestinal diseases, primarily diarrhea. This study aims to examine the impact of coliform and Escherichia coli contamination in household water sources, leading to diarrhea cases. The research method used is a literature review and analysis. The findings indicate that the presence of Escherichia coli in household water sources, especially in areas with poor sanitation, poses a high health risk, particularly increasing the likelihood of diarrhea. Several factors contribute to this contamination, including the proximity of wells to pollution sources, inadequate waste management, and improper well construction that does not meet safety standards. These conditions suggest that environmental sanitation and the distance between water sources and potential contaminants play a significant role in water quality and public health. Through this study, it is expected that the community will be more aware of the importance of maintaining clean water used for household needs to prevent diarrhea-related diseases.
