Abstract

Severe malaria, caused by Plasmodium falciparum infection, has a high mortality rate and is the main cause of death in malaria. Since clinical autopsy is unpopular in Indonesia, autopsy examination in malaria cases is rarely done. We reported a forty three year old woman from non endemic area that was dead because of severe malaria. Diagnosis was concluded from autopsy, histopathology, and toxicology. Abstract

Aim The place of ethics in undergraduate medical curricula is essential but the methods of teaching medical ethics did not show substantial changes. “Basic principles of bioethics” is the best knowledge to develop student’s reasoning analysis in medical ethics In this study, we investigate the effects of an additional small group discussion in basic principles of bioethics conventional lecture methods to cognitive achievement and retention. This study was a randomized controlled trial with parallel design. Cognitive scores of the basic principles of bioethics as a parameter was measured using basic principles of bioethics (Kaidah Dasar Bioetika, KDB) test. Both groups were attending conventional lectures, then the intervention group got an additional small group discussion.Result Conventional lectures with or without small group discussion significantly increased cognitive achievement of basic principles of bioethics (P= 0.001 and P= 0.000, respectively), and there were significant differences in cognitive achievement and retention between the 2 groups (P= 0.000 and P= 0.000, respectively).Conclusion Additional small group discussion method improved cognitive achievement and retention of basic principles of bioethics. Abstract

Several large placebo-controlled trials have confirmed that angiotensin converting enzyme (ACE) inhibitors significantly reduce mortality aid morbidity in all functional grades of congestive heart failure (CHF), nevertheless only a proportion of patients who may benefit from treatment are priscribed an ACE inhibitor. One of the perceived difficulties is the occurrence of first-dose hypotension in susceptible patients. A double-blind, randomised, single-dose therapy, parallel-group study was conducted with the aim to compare the first-dose responses to low dose ACE inhibitors captopril and perindopril in patients with stable chronic heart failure. Seventy patients (New York Heart Association class I-IV) were included. Blood pressure was recorded every 15 minutes 2 hours before starting treatment. The mean of these readings was taken as the baseline blood pressure. Patients were randomised to receive a single-dose of captopril 6.25 mg or perindopril 2 mg. After taking the drug, blood pressure was monitored every 15 minutes for 2 hours, every 30 minutes during 5 hours then hourly after 2 hours. The maximum mean arterial pressure fall from baseline of perindopril was 0.85 mmHg compared to captopril 4.60 mmHg. The maximum mean systolic fall from baseline of perindopril was 3 31 mmHg compared to captopril 6.76 mmHg while the maximum mean diastolic fall from baseline of perindopril was 1.08 mmHg compared to captopril 2.63 mmHg. The hypotensive effect of the captopril group started soon after dosing and reached its maximum after 1 to 2 hours while perindopril showed slight reduction of systolic after 1 hour and slight reduction of diastolic after 4 hours. Compared to captopril, perindopril seemed to be less likely to cause first-dose hypotension in patients with heart failure. Abstract

This research was conducted between February and April 2016 and is housed in RS PB. This study aims to analyze the medical solid waste minimization. The method used is survey method with qualitative descriptive research. Based on this research, medical solid waste minimization efforts that have been done of medical waste segregation, House keeping, preventive maintenance, and management of materials, while only limited efforts to use waste reuse (reuse). Recycling (recycle) and recovery (recovery) of medical waste has not done RS PB. Based on 2004 and 1204 Kepmenkes PP 18 jo 85, 1999, sorting and storage of medical waste is not eligible.

Abstract

ABSTRACTThe successfulness of a doctor in giving health services are based on how they applied Indonesian Medical Codes of Ethics or Kode Etik Kedokteran Indonesia (KODEKI) everyday. In January 1st 2014, Jaminan Kesehatan Nasional or National Health Coverage (NHC) started and applied gatekeeper concept for the general practitioner in primary healthcare facilities. By this program, the general practitioners have to give services for the patients that have grown in number in FKTP (primary healthcare facility) maximally. This program has been applied in Indonesia including in Rokan Hilir Regency. The purpose of this research was to know the implementation of KODEKI in NHC’s era in Rokan Hilir Regency. This research used the design of exploration method with qualitative approach. The sample was selected by using snowball sampling method. There were four general practitioners who suited the inclusion’s criterias and willing to be interviewed. The interview was done by the researcher with question’s lists and was recorded by audio recorder. The result of this research showed that the general practitioners in NHC’S era in Rokan Hilir Regency possess altruism, responsibility, professional idealism, accountability, scientific integrity, and social integrity. The results was not only seen from the informants’statements but also their attitude while giving services in publicKeywords : KODEKI values, NHC, Rokan Hilir Regency.

Abstract

ABSTRACTThe Indonesian Code of Medical Ethics or Kode Etik Kedokteran Indonesia (KODEKI) is a guideline for Indonesian doctors that consist of in the form of a set of norms of conduct as reference standards in carrying out everyday medical practice. National Health Insurance (NHI) system has the potential violations of the application of value KODEKI performed by a general practitioners as to make a primary diagnosis according to the coding International Classification of Diseases (ICD-10) that is not in accordance with professional standards. This study aims to determine how the implementation of the KODEKI values in the NHI era in Pelalawan. This study uses exploration method with a qualitative approach. The sample consisted of four doctors who were selected using snowball sampling technique. The data collection was done by in depth interview and recorded with the tape recorder. The result showed that all informants to contain the value of altruism, responsibility, idealism profession, accountability, integrity, scientific, and social integrity. This result shows that the general practitioners in Pelalawan have good implementation of KODEKI values. It can be seen from the statement and the general attitude of the doctors every day.Keywords: KODEKI value, NHI, Pelalawan Regency

Abstract

ABSTRACTGood quality is necessary given the crucial role in the proceedings of a criminal offense. Approximately 50-70 percent of cases that come to the Pelalawan Selasih General Hospital during the Period from January 1 2009-31 December 2013 were an injury cases and require a Visum et Repertum (VeR) and usually requested by investigators as evidence. This study was conducted to obtain a VeR quality at the Pelalawan Selasih General Hospital during the period from January 1 2009-31 December 2013. In order to achieve the intended objectives, retrospective descriptive approach has been used in this observational study. All VeR data that match to the inclusion criteria were sampled as many as 288 injury. Herkutanto scoring method is used to analyze 13 elements. The results of this study showed that 66 percent of survivors (190 cases of injury) is a type of highest violence in the age group 22-40 years male sex. 93.8 percent (270 cases) of the highest hardness is a result of blunt force. Quality introduction Ver were classified Good (76 percent). While the main and Conclusions section of each classified Medium (64 percent and 75 percent). The result of quality VeR in Pelalawan Selasih Gneeral Hospital is 50%. Based on these findings it can be concluded that in general the quality VeR injury in the Pelalawan Selasih General Hospital during the period from January 1 2009-31 December 2013 were categorized Medium.Keywords: Visum et Repertum, injury, quality of VeR of injury

Abstract

Background: Domestic violence (DV) is still a significant public health problem, especially in women’s health. Few studies have reported the prevalence and domestic violence in Indonesia. The aim of this study was to identify the prevalence, type of violence, and forensic examination on domestic violence victims in emergency departments.Methods: This study was a retrospective analysis of domestic violence victims observed in the Emergency Department at the Bhayangkara Hospital, Pekanbaru, Indonesia, between 2010 and 2014. The determinations of DV cases are based on the medico-legal reports (visum et repertum) and the police’s official inquiry letters.Results: Out of 6,876 medico-legal injury reports of living victims were reviewed, and 755 (10,9%) cases were DV. The majority of victims in DV were women (93.8%) with childbearing age group as the highest frequency (77.9%). Most of the DV victims were housewives (67.0%). Moreover, physical assault was the most common DV types (98.7%). Bruise was the predominant type of wound among the DV victims (76.2%), and almost half of the victims had abrasions (48.1%). Head and limbs were the predominant sites of wound. Blunt injury was found in more than three-quarters of the victims (88.5%).Conclusion: The prevalence of domestic violence was high among living victims in the emergency department, with women as the majority of victims.

Abstract

Autopsi diperlukan dalam upaya menentukan penyebab kematian dan cara kematian. Autopsi seringkali tidak dapat dilakukan karena penolakan oleh keluarga. Laporan kasus ini menyajikan investigasi forensik pada kasus kematian yang diduga akibat cedera kepala pada korban yang telah dikremasi. Investigasi pada tempat kejadian meninggal, perhitungan indikator efek trauma pada kepala dan studi literatur dilakukan untuk membuktikan probabilitas kematian akibat cedera kepala dan probabilitas kemungkinan penyebab kematian pada korban.  Dari metode tersebut didapatkan probabilitas terjadinya cedera otak traumatik ringan adalah 8,33% dan probalitas kematian jantung mendadak 80,95%. Kasus ini menunjukkan pentingnya memahami aspek medikolegal trauma, memahami biomekanika trauma dan melakukan investigasi forensik.

Abstract

Sidik bibir telah lama digunakan sebagai salah satu metode identifikasi individu.  Penelitian hubungan pola sidik bibir dengan jenis suku belum banyak diteliti, khususnya di Indonesia. Penelitian ini bertujuan mendapatkan hubungan pola sidik bibir antara galur murni suku Melayu Riau dan suku bukan Melayu Riau.Penelitian studi potong lintang dilakukan di Fakultas Kedokteran Universitas Riau antara bulan Juli sampai Agustus tahun 2015.  Sebanyak 160 mahasiswa memenuhi kriteria sampel penelitian dengan metode pengambilan sampel secara acak sederhana.  Kriteria eksklusi adalah mahasiswa Fakultas Kedokteran yang mengalami kelainan anatomi pada bibirnya (labiochisis/ labiopalatochisis), pascatrauma, dan alergi terhadap lipstik. Metode Vahanwal-Nayak-Pagare digunakan untuk mencetak pola sidik bibir. Pola sidik bibir ditetapkan dengan menggunakan klasifikasi Suzuki dan Tsuchihashi pada kedelapan kuadran bibir.  Jenis suku terbanyak adalah suku bukan Melayu Riau 111 (69,4%) dibanding dengan galur murni suku Melayu Riau 49(30,6%). Uji statistik menunjukkan hubungan yang signifikan hanya pada kuadran 3 (p = 0,04).  Dapat disimpulkan bahwa pola sidik bibir dapat digunakan untuk metode alternatif identifikasi personal, akan tetapi tidak dapat digunakan untuk membedakan galur murni suku Melayu Riau dengan bukan Melayu Riau pada populasi penelitian.Kata kunci: Klasifikasi Suzuki dan Tsuchihashi, sidik bibir, suku melayu RiauAssociation between Lip Print Pattern and Riau Malay EthnicsLip print have long been used as one of the personal identification methods. Studies on the lip print pattern associated with ethnicity has not been widely available, especially in Indonesia. This was a  study aimed to identify association between lip printi patterns and pure strains of Riau Malay and non-Malay Riau. A cross-sectional study was conducted at the Faculty of Medicine, University of Riau, between July to August 2015. A total of 160 students who met the criteria were selected through simple random sampling. Exclusion criteria were lip anatomic abnormalities (labiochisis/labiopalatochisis), post trauma experience, and allergic to lipstick. The Vahanwal-Nayak-Pagare method was used for printing the lip pattern. Lip print pattern was established using Suzuki and Tsuchihashi classification on eight lip quadrants. Most subjects were non-Malay Riau 111 (69.4%) with the remainings were identified as (n=49, 30.6%). The statistical test showed a significant relation only in Quadrant 3 (p=0,04). It is concluded that lip print pattern can be used as an alternative method in personal identification; however, it cannot be used to distinguish the pure strains of the Riau Malay ethnics from the non-Riau Malay ethnics in the study population. Key words: Lip print, Riau Malay ethnic, Suzuki and Tsuchihashi classification
