Abstract

Tympanic membrane perforation and continuous discharge are hallmarks of chronic suppurative otitis media, a chronic inflammation of the middle ear. Through the malfunction of the Eustachian tube brought on by allergic inflammation, allergic rhinitis may contribute to the development of CSOM. This study aimed to analyze the relationship between allergic rhinitis and CSOM, identify common allergens, and assess the association between eosinophil levels and CSOM. A case-control study was conducted at RSUP H. Adam Malik and RS Murni Teguh Medan involving 72 participants: 36 CSOM patients and 36 healthy controls. Skin prick tests (SPT) and eosinophil counts were used to assess allergic status. Statistical analysis used Chi-square and Fisher’s exact test with significance set at p < 0.05. The results showed a significant association between allergic rhinitis and CSOM (p = 0.001; OR = 6.192). Common allergens found in CSOM patients were D. pteronyssinus (66.7%), D. farinae (63.9%), and cockroach (47.2%), with significant associations (p < 0.05). No significant relationship was found between allergic rhinitis and CSOM sides or type (p > 0.05). Elevated eosinophil levels were more common in the CSOM group but were not statistically significant (p = 0.624). In conclusion, allergic rhinitis is significantly associated with CSOM. Allergen screening is recommended for CSOM patients to improve diagnosis, treatment, and recurrence prevention.

Abstract

This study aims to determine the relationship of attitudes and physical activity levels with the incidence of noncommunicable diseases in educators and educational staff at SMAN 15 Medan. This type of research uses quantitative research with a Cross Sectional approach. The sampling technique in this study used total sampling, the number of samples was 90 respondents. Data analysis using univariate analysis and bivariate analysis using chi-square statistical test. The results of this study indicate that there is a relationship between age and the incidence of noncommunicable disease in educators and education staff at SMAN 15 Medan with a ρ-value = 0.001, there is no relationship between gender and the incidence of noncommunicable disease in educators and educational staff at SMAN 15 Medan with a p-value = 0, 274, there was no relationship between attitude and the incidence of noncommunicable disease in educators and educational staff at SMAN 15 Medan with p-value = 0.552, there was no relationship between physical activity level and the incidence of noncommunicable disease in educators and educational staff at SMAN 15 Medan with p-value = 0.922.

Abstract

Household contacts of Drug-Resistant Tuberculosis patients potential to develop latent tuberculosis infection (LTBI) or active tuberculosis (TB) infection. This research aims to determine the prevalence of LTBI and Pulmonary TB and the characteristic of household contact with Drug-Resistant Tuberculosis (DR-TB) patients. This is a descriptive analytical study using a cross-sectional approach in people aged > 18 years who have household contact with DR-TB patients whom confirmed by sputum GeneXpert and treated at Adam Malik Hospital Medan. The research subjects then underwent examinations including symptom anamnesis, chest X-ray, sputum GeneXpert, and IGRA blood test From 105 people who had household contact with DR-TB patients, the most common cases found were LTBI cases (55%), followed by uninfected cases (40%). Meanwhile, the only cases of Pulmonary TB found were Drug-Sensitive Pulmonary TB were 6 cases (5%) which are bacteriologically confirmed drug-Sensitive TB was 4 cases and clinically confirmed drug-sensitive TB was 2 cases. Majority of patients were in 18-30 year old group (30.5%), female (70%), high school graduates (61%), housewives (41%), normoweight (80%), no smoking habit (78%), no consuming alcohol (97%), and no comorbid diseases (87.5%). Based on contact duration, 70 people (67%) were found with > 5 hours and 35 patients (33%) with < 5 hours. In addition, the majority of samples (70%) did not sleep in the same room with MDR-TB patients.  LTBI cases were the most common cases found among household contact with DR-TB patients.

Abstract

Lung cancer is one of the leading causes of cancer-related deaths globally. Cigarette smoking remains the primary risk factor. The Fagerstrom Test for Nicotine Dependence (FTND) and the Brinkman Index are used to assess nicotine addiction and smoking intensity, respectively. These indices are associated with pulmonary function, typically assessed through spirometry. Objective: To determine the relationship between Fagerstrom scores and Brinkman Index with spirometry results in lung cancer patients. Methods: This quantitative observational study applied a cross-sectional design involving 68 smoking lung cancer patients at RSUP Haji Adam Malik and RS Prof. CPL Universitas Sumatera Utara. Data were collected via FTND and Brinkman Index questionnaires and spirometry testing. Data were analyzed using the Kruskal-Wallis test with a significance level of p<0.05. Results: Most patients exhibited moderate nicotine dependence (44.1%) and moderate smoking history per Brinkman Index (52.9%). Spirometry results indicated a high prevalence of obstructive and restrictive impairments. There was a significant correlation between FTND/Brinkman Index and lung function (p<0.001). Conclusion: Both Fagerstrom scores and Brinkman Index show a strong relationship with spirometry outcomes, indicating impaired pulmonary function among lung cancer patients.

Abstract

Hearing loss is a public health problem that increases with age and can be exacerbated by comorbidities such as type 2 diabetes mellitus (DM) and hypertension. This study analyzed the relationship between age and comorbidities (DM type 2 and hypertension) on hearing loss in women over 40 years old. A cross-sectional analytical study of 34 women over 40 years old in Kotamatsum IV Village, Medan. Hearing examinations were performed using pure tone audiometry and otoacoustic emission (OAE). Statistical analysis was performed using the Chi-Square, Mann-Whitney, and Kruskal-Wallis tests. There was a significant relationship between age and the degree of hearing loss (p=0.047), with the majority of subjects aged >50 years experiencing mild deafness. However, no significant relationship was found between comorbidities of type 2 DM and/or hypertension on the degree, type of hearing loss, or OAE results (p>0.05). Age was significantly associated with the degree of hearing loss, while type 2 DM and hypertension did not show a significant relationship. Further research is needed considering disease duration and treatment adherence to further understand the contribution of comorbidities to hearing loss.

Abstract

One of the main complications in patients with COPD is muscle weakness or atrophy, which becomes more pronounced during exacerbations, leading to a decline in quality of life, prognosis, and an increased risk of future exacerbations. This study compared upper limb muscle strength between stable COPD patients and those experiencing exacerbations. A cross-sectional study design was used, with an independent sample t-test to compare the mean values of two unpaired groups. Sampling was conducted using a non-probability consecutive sampling technique. Researchers assessed upper limb muscle strength using a Hand-grip Dynamometer on a predetermined sample size that met the inclusion and exclusion criteria. The results showed a significant correlation between the history of exacerbations within the past year and COPD in both groups (p < 0.05). Upper limb muscle strength in the exacerbation group was lower than in the stable COPD group. The independent t-test yielded a p-value of 0.001, indicating a significant difference in upper limb muscle strength between the stable and exacerbation COPD groups.

Abstract

Introduction: Retinal neurodegeneration may be an early indicator of diabetic retinopathy, the second most common complication after nephropathy. Tumor Necrosis Factor-Alpha (TNF-α) plays a role in the pathogenesis of inflammatory and neovascular eye disorders and is associated with intraocular inflammatory diseases like macular edema and proliferative diabetic retinopathy. This study aims to find the relationship between TNF-α levels and Retinal Nerve Fiber Layer (RNFL) thickness in diabetic retinopathy patients at Prof. CPL Universitas Sumatera Utara General Hospital and affiliated hospital.   Methods: This cross-sectional study included 45 patients with type 2 DM and diabetic retinopathy at Prof. CPL Universitas Sumatera Utara General Hospital and affiliated hospital from March to June 2024. RNFL and posterior segment examinations were conducted. TNF-α levels were measured from blood samples.Blood sugar data were taken from medical records.   Results: Of the 45 participants, 24 were men (53.3%) and 21 women (46.7%). 32 (71.1%) had DM for >5 years, and 27 (60%) experienced PDR. Highest location for RNFL thickness examination was superior (19, 42.2%). Average TNF-α with thin RNFL was 65.67 ng/L, and with thick RNFL was 64.78 ng/L. Mean TNF-α with thin superior RNFL was 65.74 ng/L, and thick was 63.70 ng/L. Mean TNF-α with thin inferior RNFL was 65.73 ng/L, and thick was 63.9 ng/L. Mean TNF-α with thin temporal RNFL was 68.73 ng/L, and thick was 67.19 ng/L. Mean TNF-α with thick nasal RNFL was 67.06 ng/L.   Conclusion: There was no statistically significant relationship between TNF-α and RNFL thickness in diabetic retinopathy patients.

Abstract

The demand for shoes must be met by the worldwide shoe manufacturing business, but the inhalation dangers associated with its chemical processes—which use benzene, toluene, and xylene (BTX)—raise health issues for workers. Initial surveys showed that most shoe manufacturing workshops do not have adequate ventilation. Most workers often complain of symptoms such as coughing, shortness of breath, and headaches. This study examined the relationships between worker symptoms and chemical levels at shoe factories in Medan, Indonesia, in 2017. This was an observational cross-sectional study. The total population was 72 workers in shoe workshops in the small industrial center area in Medan city. The sample size in this study was 47 people in seven shoe workshops. BTX concentrations were measured in seven workshops. Statistical tests used in this study were chi-square test and Fisher’s exact test with a significance level of p<0.05. According to the findings, the most common symptoms among employees were coughing (40.4%), dizziness (55.3%), and eye discomfort (55.3% of respondents). The range values were 0.09 ppm to 2.83 ppm, 1.20 ppm to 178.98 ppm, and 0.11 ppm to 50 ppm for benzene, toluene, and xylene, respectively. The two compounds most commonly exposed to levels exceeding the ACGIH (American Conference of Governmental Industrial Hygienists) threshold level value were toluene (55.3%) and benzene (25.5%), according to ambient readings. Of the workshops that were inspected, only two (28.57%) had a functional ventilation system. Exposure to benzene exceeding the quality standard showed a significant association with dizziness (p = 0.003) and watery eyes (p = 0.003). Meanwhile, exposure to toluene exceeding the permissible limit was significantly associated with dyspnea (p = 0.005). Our research showed that over half of the workforce reported subjective complaints and that levels of benzene and toluene exceeded recommendations for occupational exposure.

Abstract

Mangan yang secara alami dapat ditemukan di air, tanah, dan udara adalah zat nutrisi esensial bagi manusia dan hewan. Asupan yang tidak mencukupi atau yang berlebihan dapat gangguan kesehatan. Pajanan kronik mangan pada dosis yang tinggi dapat mengakibatkan gangguan pada sistem saraf. Tujuan penelitian ini adalah mengetahui perkiraan risiko kesehatan akibat asupan mangan dari air sumur di wilayah penelitian. Penelitian dengan disain crossectional ini dilakukan terhadap masyarakat di wilayah sekitar TPA Rawakucing yang meliputi masyarakat yang bermukim di dalam kawasan (114 orang), dan di luar kawasan TPA (177 orang). Hasil penelitian menunjukkan rata-rata konsentrasi mangan dalam air sumur di wilayah sekitar TPA Rawakucing (4.3 mg/l; SD=2.8873 mg/l). Berbeda sevara bermakna dengan di luar TPA Rawakucing (0.300 mg/l; SD=0.1888 mg/l). (nilai p<0.05). Rata-rata besaran risiko (RQ) gangguan kesehatan akibat mengkonsumsi air yang mengandung mangan pada masyarakat yang tinggal di TPA Rawakucing (0.2347) dan yang tinggal di luar TPARawakucing (0.2955). Masyarakat yang tinggal di TPA berisiko gangguan kesehatan 8,12 kali lebih bedsar daripada yang tinggal di luar TPA(p<0.05) dengan OR=8.109 (95% CI = 2.668-24.650). Manganese that can be found ubiquitously in the air, soil, and water is an essential nutrient for humans and animals. Inadequate intake or over exposure can cause adverse health effects. Although manganese is an essential nutrient at low doses, chronic exposure to high doses may be harmful. Regardless, the nervous system has been determined to be the primary target organ with neurological effects generally observed. The objective of the study is to assess the health risk of manganese exposure from the well drinking water intake. This study was done in Final Disposal Site Rawakucing area and outside of this area. The number of subjects were 114 who lived inside the site and 177 who lived outside the area, respectively. Study design is cross-sectional with hypotheses testing. The result showed that the average manganese concentration was 4.3 mg/l (SD=2.8873), 0.300 mg/l (SD=0.1888) respectively. The result was significant different statistically (p<0.05) for manganese concentration from two study area. The average RQ showed 0.2347 (SD=0.5095) for RQ in Disposal Site Area and 0.2955 (SD=0.2465) for RQ outside Disposal Site Area. The result was statistically significantly different (p<0.05) for RQ value, with OR=8.109 (95% CI = 2.668-24.650).

Abstract

Tempat pembuangan akhir (TPA) sampah domestik dengan sistem penampungan terbuka sangat berisiko mencemari air tanah milik warga yang bermukim di sekitarnya melalui proses perlindian. Untuk mengetahui kandungan logam berat dalam air tanah di sekitar TPA, sebanyak 68 sampel air sumur gali (45 sumur Dusun I dan 23 sumur Dusun IV) dari Desa Namobintang Kecamatan Pancurbatu Kabupaten Deli Serdang, Sumatera Utara, telah dianalisis dengan inductively couple plasma atomic emission spectroscopy. Hubungan jarak sumur dengan konsentrasi kromium, merkuri, dan timbal diuji dengan Mann-Whitney, Spearman’s Correlation dan analisis regresi linier sederhana. Hasil penelitian menunjukkan konsentrasi kromium, timbal, dan merkuri (rerata ± deviasi standar, mg/L) masing-masing 0,036 ± 0,0096; 0,0003 ± 0,00018; dan 0,005 ± 0,0041 (Dusun I); 0,0370 ± 0,0115; 0,00026 ± 0,00013; dan 0,0070 ± 0,0069 (Dusun IV). Dari 68 sumur yang dianalisis, hanya ada 8 sumur yang konsentrasi timbalnya melebihi batas menurut Peraturan Kementerian Kesehatan Republik Indonesia Nomor 492/Per/IV/2010. Sementara itu, tidak ada korelasi jarak sumur gali ke TPA dengan konsentrasi kromium, merkuri, dan timbal dalam air sumur gali tersebut. Disimpulkan bahwa perlindian sampah di Namobintang tidak mencemari air sumur-sumur gali yang berjarak 84 meter atau lebih dari TPA. Dumping site of domestic wastes has potential risk to contaminate groundwater of the surrounding population through leaching process. To determine heavy metals (chromium, lead, and mercury) in groundwater at surrounding dumping site, a total of 68 dig well water samples (45 from Hamlet I and 23 from Hamlet IV) of Namobintang Village, Pancurbatu Sub-District of Deli Serdang Regency, North Sumatra, were analyzed using Inductively Couple Plasma Atomic Emission Spectroscopy. The relationship between the dig well distance and chromium, mercury, and lead content was tested by Mann-Whitney, Spearman’s Correlation and Simple Linier Regression. The result of this study showed that chromium, lead, and mercury concentrations (mean ± SD, mg/L) were 0.036 ± 0.0096; 0.0003 ± 0.00018; and 0.005 ± 0.0041 (Hamlet I); 0.0370 ± 0.0115; 0.00026 ± 0.00013; and 0.0070 ± 0.0069 (Hamlet IV). Of 68 dig well water samples analyzed, there were only 8 water samples that had lead concentrations exceeding the drinking water limit of Peraturan Kementerian Kesehatan Republik Indonesia Nomor 492/Per/IV/2010. Meanwhile, there was no correlation of dig well distance to dumping site and heavy metal concentrations in dig well water. The study concluded that solid waste leachate in Namobintang did not contaminate the dig wells water located at 84 meters or more from the dumping site.
