Abstract

The industrial sector, especially tea plants, is a sector that has a positive impact on the development of the State. However, this development is also closely related to risk factors that can lead to occupational accidents and occupational diseases that require further treatment such as Carpal Tunnel Syndrome (CTS). This study was conducted to analyze work factors moderated individual factors on CTS in tea harvesters using machines at PTPN IV Sidamanik. This study used quantitative methods of cross sectional design with Thinel test and Boston Carpal Tunnel Syndrome Questionnaire (BCTQ) on 98 workers using correlation test and Moderated Regression Analysis (MRA). The results showed a positive relationship between work and individual factors with CTS (p value <0.05). There was an increase in R square on the interaction of work factors with individual factors by 16.3% on the age variable, 2.4% on the smoking variable and 5.7% on the Body Mass Index (BMI) variable. Individual factors (age, smoking and BMI) strengthened the influence of work factors on CTS with significance <0.05. This study is expected to provide information, input and consideration to companies in making policies regarding occupational illness, especially CTS. And can increase workers' awareness to use personal protective equipment when working and conduct an examination if they find symptoms of CTS. Keywords: Age, Body Mass Index, Carpal Tunnel Syndrome, Posture, Smoking

Abstract

Abstract Introduction & Objectives : Conjunctivitis is an eye infection accounted as the third leading ophthalmology cases after cataract and glaucoma globally, with Streptococcus pneumoniae as the main causes in children. Several studies had indicated that Piper betle L. extract inhibited Streptococcus pneumoniae growth. To determine and evaluation of pharmacological activities of Piper Betle L. extract on bacterial conjunctivitis model of wistar rat caused by Streptococcus Pneumoniae Methods : This study was an analytical study with true experimental design and pre-test-post-test control group. Streptococcus pneumoniae colony growth media was analyzed using Mueller Hinton agar. Streptococcus pneumoniae colony was calculated using Total Plate Count methods. Results : Sample of this study involved 30 male wistar rat aged 4 – 9 minggu. Kruskal Wallis test reveald a significant differences of Streptococcus pneumoniae colony based on the treatment group (p=0,023). Subsequent test using Posthoc revealed that there was a significant differences in Streptococcus pneumoniae colony between placebo group and group using 25% (p = 0,039) and 30% (p=0,006) piper betle. Test results also revealed that no significant difference was found between placebo group and groups using 20% piper betle (p=0,159) and ofloxacin (p=1,000). Conclusion : There was a significant difference in Streptococcus pneumoniae colony among placebo group and piper betle eyedrops 25 % and 30 % group. However, significant result among placebo group and piper betle L. 20 % and ofloxacin group were absent.

Abstract

Stunting adalah ketidakseimbangan gizi yang merupakan penurunan kecepatan pertumbuhan dan gangguan pertumbuhan fisik. Masalah gizi terkait stunting masih menjadi permasalahan yang terjadi di Indonesia. Dampak jangka panjang akibat stunting yang masih terjadi adalah gangguan perkembangan fisik, mental, intelektual, serta kognitif. Kejadian stunting yang tinggi pada anak akan mengakibatkan permasalahan jangka panjang dan sulit untuk dikendalikan sehingga akan menyebabkan risiko pada keturunan yang akan rentan dengan berat badan lahir yang rendah. Dukungan dari tenaga kesehatan menjadi hal yang sangat penting dalam pencegahan stunting. Tujuan dari penelitian ini adalah untuk menganalisis dukungan tenaga kesehatan dalam pencegahan stunting di Desa Tuntungan 2 Kecamatan Pancur Batu Kabupaten Deli Serdang dengan menggunakan disain cross sectional. Penelitian ini dilakukan pada 135 orang ibu yang memiliki balita usia 6-59 bulan. Hasil penelitian menunjukan bahwa dukungan tenaga kesehatan kurang baik dalam pencegahan stunting yaitu sebesar 27,4% dan kejadian stunting sebesar 41,5%. Untuk meningkatkan upaya pencegahan stunting, diharapkan terjadi peningkatan peran dan dukungan dari pemerintah agar pencegahan stunting menjadi prioritas.

Abstract

This research aimed to analyze the psychological relationship with the performance of executive nurses at the Prof. Mental Hospital. Dr. Muhammad Ildrem Medan in 2023. Quantitative type of research with a cross-sectional design. The population in this study were all executive nurses who worked in the mental inpatient ward at the Prof. Mental Hospital. Dr. Muhammad Ildrem Medan as many as 90 people. The sampling technique used simple random sampling so that 48 people were obtained. Data collection techniques used the Psychological Capital Questionnaire (PCQ) questionnaire with 24 items to measure psychological capital and the nurse performance questionnaire with 25 items. The data that has been obtained will be analyzed univariate and bivariate using the pearson correlation test. The research results showed that the significance value of the psychological capital variable and the performance variable of the implementing nurse was 0.000 (p<0.05). In conclusion, there is a significant positive relationship between psychological capital and the performance of implementing nurses at the Prof. Mental Hospital. Dr. Muhammad Ildrem Medan, where the more positive psychological capital one has, the higher the performance of implementing nurses. Keywords: Executive Nurse, Performance, Psychological.

Abstract

Introduction: Chronic obstructive pulmonary disease is associated with acute exacerbations that can lead to respiratory failure and as major cause of morbidity and mortality worldwide. The Aim of study was to explore survival rate of COPD patients with type II respiratory failure Methods: Observational research with retrospective cohort design using data from medical records of chronic obstructive pulmonary disease patients with type II respiratory failure treated at H. Adam Malik General Hospital Medan for 10 months from November 2021 to September 2022. Analyzed of independent and dependent variables used Chi Square test. To obtain most dominant independent variable that influence dependent variable, a multiple logistic regression test is used. Results: There were 21 patients (50%) who alived until end of research but also 21 deaths occured. Survival rate was 50% with a median survival time 14 days (CI 95%: 7.178-20.822 days). Survival rate of COPD patients with type II respiratory failure who do not routinely received therapy was 16.7% with a median survival time 4 days (CI 95%: 42.962-5.039). Survival rate of COPD patients with type II respiratory failure who routinely received therapy was 75%. Based on log rank test, there was a significant difference median length of life between patients who did not and who routinely received therapy (p <0.001). Conclusion: The survival rate in COPD patients who regularly use inhaler therapy is significantly higher than patients who do not routinely use inhaler therapy.

Abstract

Patients with chronic obstructive pulmonary disease (COPD) commonly exhibit muscle atrophy and dysfunction due to a reduction in muscle mass; and protein supplements such as chicken egg whites have been reported to improve muscle mass. The aim of this study was to evaluate the impact of physical exercise and egg white supplementation on the muscle mass of COPD patients. An experimental study was conducted among stable COPD patients at Universitas Sumatra Utara Hospital Medan, Indonesia, between August and October 2022. The patients were divided into two groups, control and interventional groups, with each patient subjected to a pre- and post-muscle mass assessment. All the patients performed respiratory endurance and upper extremity muscle strength training three times/week for a total of 12 weeks. In addition, the patients in the intervention group were also given egg white supplementation (10 eggs/day) during the period of intervention in addition to the physical training. The Wilcoxon and Mann-Whitney tests were performed to identify the significance of the difference between pre- and post-intervention and between the control and intervention groups, respectively. A total of 38 COPD patients were included in the study, 19 from each group. Our data suggested no significant difference in muscle mass of the patients in the control group before and after 12 weeks of physical exercise (pre-intervention 27.37±4.54% and post-intervention 27.68±4.5% with p=0.174). However, there was a significant muscle mass increment of patients in the intervention group upon 12 weeks of physical training and egg white supplementation (pre-intervention 27.18±4.15%, post-intervention 29.95±3.76%, p<0.001). A significant difference in muscle mass was observed between patients in the control and the intervention groups (p=0.046) after the intervention. The study highlights that physical exercise in combination with egg white supplementation may serve as potential and effective non-pharmacological treatment for muscle mass restoration in COPD patients as compared to physical exercise alone.

Abstract

The low treatment rate, low treatment success rate and high mortality rate of patients with pre-extensively drug-resistant tuberculosis (pre-XDR-TB) and extensively drug-resistant TB (XDR-TB) need serious attention. The aim of this study was to describe the profiles of patients with pre-XDR-TB and XDR-TB cases and to determine associated risk factors of their incidence in Indonesia. A retrospective case-control study was conducted at H. Adam Malik General Hospital in Medan, North Sumatra of which all sensitive-drug TB (SD-TB), pre-XDR-TB, and XDR-TB patients aged 18 years or older treated between October 2019 to June 2022 were included. Chi-squared test or Kruskal Wallis test and multiple logistic regression were used to determine the risk factors associated with pre-XDR-TB and XDR-TB incidence. A total 16 patients of case group (15 pre-XDR-TB and one XDR-TB) and 116 SD-TB patients (control group) were included in the final analysis. Out of total patients within case group, 62.5% were male, 43.8% aged between 56–65 years, 62.5% graduated from high school or equivalent, and 25% were unemployed. The majority of patients had no comorbid (62.5%), had history of anti-TB treatment (93.8%), and had secondary resistance (93.8%). Multivariate analysis indicated that age (OR: 10.01; 95%CI: 1.49–66.91, p=0.018) and previous history of anti-TB treatment (OR: 216.25; 95%CI: 18.62–2511.60, p<0.001) were significantly associated with incidence of pre-XDR-TB and XDR-TB. This study highlights that having previous history of anti-TB treatment and older age are the predictors of the incidence of pre-XDR-TB and XDR-TB.

Abstract

Chronic obstructive pulmonary disease (COPD) is among top ten of the world's causes of death. The development of the “Paru Sehat” smartphone application is a positive initiative and might have the potential to improve the disease management of COPD, improve patient’s quality of life, and reduce complications associated with COPD. However, its effectiveness in improving the knowledge of the COPD patients is unknown. The aim of this study was to determine the effectiveness of “Paru Sehat” in increasing knowledge on COPD and its non-pharmacological management in COPD patients. A quasi-experimental study with a one-group pretest-posttest was conducted among stable COPD patients at Prof. Chairuddin Panusunan Lubis Hospital, Medan, Indonesia. Bristol COPD knowledge questionnaire (BCKQ) was used to assess the knowledge scores of the patients before and after exposure to the “Paru Sehat” twice a week for four weeks. A dependent Student t-test was used to compare the knowledge scores between pre- and post-intervention for both knowledge domains (i.e., knowledge on the disease and non-pharmacological management). Student t-test or one-way ANOVA were used to determine the association between patients’ characteristics and the knowledge scores within pre- and post-treatment. Our data indicated a significant improvement of the knowledge scores on disease between pre- and post-treatment (15.92±3.79 vs 19.56±3.68, p<0.001). The knowledge score on non-pharmacological management also increased significantly post-treatment (7.52±2.02) compared to pre-treatment (10.08±2.379), p<0.001. In addition, this study found that educational attainment was associated with the scores of both knowledge domains of which individuals with senior high school or higher education level had significantly improvement of knowledge scores. This study highlights that although “Paru Sehat” application could improve the knowledge on COPD and its non-pharmacological management, its effects are less effective among individuals with low educational attainment.

Abstract

Coronavirus disease 2019 (COVID-19) has been identified for more than two years, yet studies assessing post-infection lung function are limited. Reports on lung function in COVID-19 patients indicate that patients have restrictive defects and small airway dysfunction that can persist and are not necessarily related to the severity of the disease. The aim of this study was to assess the incidence of small airway obstruction and its incidence-associated factors among COVID-19 survivors to better describe the long-term effects of COVID-19. A cross-sectional study was conducted among COVID-19 survivors who less than 50 years at Medan Adventist Hospital between 2020–2022. The data were collected through interview, direct assessment and respiratory examination. A total of 89 COVID-19 survivors were recruited of which the majority of them were female with a mean age of 32.6-year-old with the largest group was 19–30 years. The comorbidities found among the survivors were heart and thyroid disorders, with the most common symptom of post-COVID-19 was fatigue. Most of them had mild COVID-19. The mean forced mid-expiratory flow (FEF25–75%) was 96.3±20.22, with an incidence rate of small airway obstruction was 19.1%. Univariate and multivariate analyses indicated no significant association between age, gender, comorbidities, history of oxygenation during COVID-19 treatment, COVID-19 severity and the type of post COVID-19 syndrome symptoms with the incidence of small airway obstruction. In conclusion, among COVID-19 survivors who were less than 50 years old, those studied variables seems have less association with the incidence of small airway obstruction. Nevertheless, a further study with a bigger sample size is important to be conducted.

Abstract

Background: Patients with chronic suppurative otitis media (CSOM) often experience hearing loss from mild to very severe degrees. In addition, patients with CSOM often have intraoperative pathological findings. Aims of this study is to determine the correlation between intraoperative tympanomastoidectomy pathological findings with the type and degree of hearing loss using pure tone audiogram images in CSOM patients. Methods: This study is an analytic type study that uses a cross-sectional design. This research was conducted in three hospitals during October 2021 to December 2021. The sample size in this study was 37 patients who would undergo tympanomastodectomy surgery. Results: The most common pathological finding was granulation in 36 patients (97.3%) and followed by sclerotic in 31 patients (83.8%). The average patient had more than 2 pathological findings. Most of patients (78.4%) had conductive hearing loss and most of patients had moderate degree of hearing loss in 25 patients (67.6%). Based on type of hearing loss, there were significant associations with several pathological findings, namely cholesteatoma (p=0.003), defect ossicles (p=0.009), ossicular defects (p=0.001), facial nerve defects (p=0.009). In terms of the number of pathological findings, there was a significant correlation with hearing loss (p=0.004, r=0.467) and the degree of hearing loss (p=0.029, r=0.360). Conclusion: Among the pathological findings by type of hearing loss, a significant correlation was found.
