Abstract

Workload, burnout and distress are closely related to nurse performance, because increasing workload, burnout and distress can reduce nurse performance. The purpose of this study was to determine the effect of workload, burn out, distress on the performance of nurses after the Covid-19 pandemic. Method: This study is quantitative with a causal associative design. The population in this study were nurses at the Prof. Dr. M. Ildrem, Metal Hospital, totaling 184 people, with a sample of 65 people in the study. Data were collected using questionnaire. Data were analyzed using multiple linear regression. The study shows that workload and distress influence the performance of nurses after the Covid-19 pandemic at the Prof. Dr. M. Ildrem, Mental Hospital, Medan, while burn out has no effect on the performance of nurses after the Covid-19 pandemic at the Prof. Dr. M. Ildrem, Mental Hospotal, Medan. Workload, burn out and distress together influence the performance of nurses after the Covid-19 pandemic at the Prof. Dr. M. Ildrem, Mental Hospital, Medan. Hospitals need to pay attention to workload elements that can affect nurses' performance, as well as exploring the individual abilities of nurses, providing opportunities for nurses to be creative and self-actualizing to reduce burn out as well as screening nurses for psychological problems regularly to support nurses who work in the Mental Hospital.

Abstract

Manganese (Mn) is a heavy metal that naturally exists in the environment and can contaminate drinking water sources through anthropogenic activities, particularly in agricultural areas. Long-term exposure to Mn levels above safe limits can lead to neurological disorders in humans. This study aims to analyze the risk management of manganese contamination in drinking water sources in Langkat District, North Sumatra. A cross-sectional study was conducted on 8 samples of well water and 209 respondents. Data collection included water sampling and neurological symptom assessment using the Q18 questionnaire. Risk analysis was carried out based on concentration exposure, body weight, and daily water intake to calculate the Risk Quotien (RQ). Manganese concentrations in water ranged from 0.991 mg/L–2.809 mg/L, exceeding the Indonesian Ministry of Health threshold of 0.1 mg/L. Six respondents experienced numbness, and four experienced palpitations. Risk management analysis showed a safe body weight at 36.26–141.81 kg, the safe length of residence ranged at 23.01–58.72 years, and safe Mn concentration at 0.62–2.78 mg/L. This study provides important scientific evidence for developing risk management strategies for manganese contamination in drinking water to prevent neurological issues in Langkat Regency. Recommendations were given to improve water quality monitoring and community health education.

Abstract

ABSTRACT Diabetes mellitus is one of the non-communicable diseases that ranks 4th most common cause of death in the world. Labuhanbatu Regency is one of the districts in the main Sumatra province that must improve the control of the number of Type 2 diabetes because Type 2 diabetes cases fluctuate every year with the percentage of patients who get health services is only 90.2 percent. Therefore, the purpose of this study was to analyze the influence of internal and external factors on the incidence of Type 2 diabetes mellitus at the age of 40 years in Labuhan Batu Regency. This type of research is quantitative research with case control design. The study was conducted in 5 health centers that have the highest number of patients with Type 2 diabetes mellitus in Labuhanbatu Regency with a large sample of 43 cases and 43 controls so that the total sample size is 86 people. Based on the results of the study, the influence of internal factors include age (p-value=0.026), sex (p-value=0.017) and family history of diabetes mellitus (p-value=0.000) on the incidence of Type 2 Diabetes mellitus at the age of 40 years in Labuhanbatu Regency. The results also showed the influence of external factors including dietary habits (p-value=0.028), physical activity (p-value=0.006) and obesity (p-value=0.018) on the incidence of Type 2 Diabetes mellitus at the age of 40 years in Labuhanbatu District. The dominant factor influencing the incidence of Type 2 Diabetes mellitus at the age of 40 years in Labuhanbatu District is a family history of DM with Exp (B) value of 38.395. It is expected to the Government of Labuhanbatu Regency to be able to carry out supervision of Type 2 diabetes mellitus in individuals who have not been able to carry out supervision of the patient's familytype 2 and type 2 diabetes mellitus supervision education in families, as well as supervision of individuals who have been able to carry out supervision of families with minimal medication and education to prevent eating patterns and physical activities carried out to control stable blood sugar. Keywords: Diabetes Mellitus, Family History, Prevention  ABSTRAK Diabetes melitus menjadi salah satu penyakit tidak menular yang menempati urutan ke-4 penyebab kematian terbanyak di dunia. Kabupaten Labuhanbatu merupakan salah satu kabupaten di Provinsi Sumatera utama yang harus meningkatkan pengendalian jumlah DM tipe 2 karena kasus DM tipe 2 yang fluktuatif setiap tahunnya dengan persentase penderita yang mendapatkan pelayanan kesehatan hanya sebesar 90,2 persen. Maka dari itu, tujuan penelitian ini untuk menganalisis pengaruh faktor internal dan eksternal terhadap kejadian DM tipe 2 pada usia ≥ 40 tahun di Kabupaten Labuhan Batu. Jenis penelitian ini adalah penelitian kuantitatif dengan desain case control. Penelitian dilakukan di 5 puskesmas yang memiliki jumlah penderita DM tipe 2 tertinggi di Kabupaten Labuhanbatu dengan besar sampel 43 orang kasus dan 43 orang kontrol sehingga total besar sampel yaitu 86 orang. Berdasarkan hasil penelitian didapatkan adanya pengaruh faktor internal meliputi usia (p-value=0,026), jenis kelamin (p-value=0,017) dan riwayat keluarga DM (p-value=<0,001) terhadap kejadian Diabetes Melitus Tipe 2 pada usia ≥ 40 tahun di Kabupaten Labuhanbatu. Hasil penelitian juga menunjukkan adanya pengaruh faktor eksternal meliputi kebiasaan diet (p-value=0,028), aktivitas fisik (p-value=0,006) dan obesitas (p-value=0,018) terhadap kejadian Diabetes Melitus Tipe 2 pada usia ≥ 40 tahun di Kabupaten Labuhanbatu. Faktor dominan yang mempengaruhi kejadian Diabetes Melitus Tipe 2 pada usia ≥ 40 tahun di Kabupaten Labuhanbatu yaitu riwayat keluarga DM dengan nilai Exp(B) sebesar 38,395. Diharapkan kepada pemerintah Kabupaten Labuhanbatu untuk dapat melakukan pengendalian penyakit DM tipe 2 pada individu yang belum terpapar dengan melakukan pendataan keluarga pasien DM tipe 2 dan edukasi pencegahan DM tipe 2 pada keluarga, serta pengendalian pada individu yang sudah terpapar dengan melakukan pemeriksaan kepatuhan minum obat dan edukasi mengenai pola makan serta aktivitas fisik yang dianjurkan untuk mengontrol kadar gula darah stabil. Kata Kunci: Diabetes Melitus, Riwayat Keluarga, Pencegahan

Abstract

One of the factors that influence the incidence of hypertension is obesity. The purpose of this study was to analyze the effect of obesity on the incidence of hypertension at the age of 25-45 years in Padang Lawas Regency in 2024. The research method used a case control design with a large sample size of hypertension and non-hypertension of 73 people each. Data analysis using logistic regression test. The results showed that obesity is a factor influencing the incidence of hypertension (OR = 6,056). It is necessary to conduct a public education campaign targeting young adults in Padang Lawas Regency, focusing on the importance of controlling the frequency of fat consumption, healthy types of fat, and associated obesity risks, as well as utilizing social media and holding special counseling sessions for food businesses.

Abstract

Introduction: Pleural effusion is associated with a high mortality rate. Tube thoracostomy remains the standard treatment. Despite this, double-lumen catheters, typically used for central venous access, are infrequently employed for pleural drainage. This study compared the characteristics of patients undergoing double-lumen catheter versus pigtail catheter placements for pleural effusion. Methods: A retrospective cross-sectional study was conducted on adult patients at Prof. Dr. Chairuddin Panusunan Lubis Universitas Sumatera Utara Hospital, Haji Adam Malik General Hospital, and St. Elisabeth Hospital, Medan, from September 2022 to April 2024. This study analyzed patient demographics and clinical presentations for those receiving either catheter type. Results: The mean age of patients with double-lumen catheters was 58.21 years old, while those with pigtail catheters averaged 54.9 years old. Patients with double-lumen catheters frequently presented with a combination of shortness of breath, cough, and chest pain, while those with pigtail catheters primarily reported shortness of breath. Both groups predominantly exhibited exudative pleural effusions. Radiological evaluations indicated moderate pleural effusion was most common in both groups, with thoracic ultrasound revealing fluid volumes between 500 and 2,000 cc. Conclusion: Double-lumen catheters are more frequently utilized for moderate to massive pleural effusion, whereas pigtail catheters are typically reserved for moderate cases. The choice of catheter depends on the patient's condition, the underlying cause of the effusion, and radiological findings.

Abstract

Introduction: Lung cancer is a malignancy of the lung parenchyma or bronchi. Molecular targeted therapy directed at oncogene driver mutations, such as anaplastic lymphoma kinase (ALK), improves the prognosis of patients with non-small cell lung carcinoma (NSCLC). This study aimed to determine the characteristics of lung adenocarcinoma patients and the prevalence of ALK rearrangement among the North Sumatra population. Methods: This descriptive study used data from patients' formalin-fixed paraffin-embedded (FFPE) ALK examination results and medical records. The laboratory analyzed the patient’s FFPE for ALK fusion protein expression using the VENTANA anti-ALK (D5F3) procedure to determine the prevalence of ALK rearrangement. Results: Of the 34 subjects, it was revealed that the characteristics of lung adenocarcinoma patients were 18 patients aged >60 years old (52.9%), 26 male patients (76.4%), and 24 heavy smoker patients (70.6%). Based on the pathological tumor-node-metastasis (pTNM) stage, most samples were classified as stage IVA, with 24 cases (70.6%) showing the highest metastases to the pleura. There were 2 cases of ALK mutations obtained through immunohistochemical examination with a percentage of 5.8%. Conclusion: There are relatively few ALK rearrangement mutations in lung adenocarcinoma patients without screening. Additional research is needed to ascertain the distribution of lung adenocarcinoma patient characteristics associated with a higher prevalence of ALK rearrangement mutations.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a significant health issue with high morbidity and mortality rates. However, risk factors for COPD, particularly in cases with type II respiratory failure, are not fully understood. This study analyzes factors influencing the mortality rate of COPD patients with type II respiratory failure. Methods: This observational study used a cross-sectional design, analyzing medical records of COPD patients with type II respiratory failure treated at H. Adam Malik General Hospital, Medan, from November 2021 to September 2022. A total of 42 patients met the inclusion criteria. Data analysis included descriptive statistics and inferential tests using the Chi-Square test and multiple logistic regression to identify the most dominant factors. Results: Among the 42 patients, the mortality rate was 50%. Variables such as gender, age, number of comorbidities, smoking habits, and history of exacerbations were not significantly associated with mortality. Patients who did not regularly use inhalers had a 19-fold higher mortality risk than those who did (P=0.004; OR=19.79; 95% CI=2.67-146.99). Length of stay was inversely associated with mortality, with each additional day reducing the risk by 0.77 times (P=0.011; OR=0.77; 95% CI=0.63-0.94). Conclusion: Routine inhaler use and length of stay are significant factors influencing the mortality rate in COPD patients with type II respiratory failure.

Abstract

Introduction & ObjectivesThere are studies that state that estrogen deficiency can exacerbate visual disturbances in glaucomapatients. To determine the relationship between estrogen levels and intraocular pressure (IOP) inpatients with POAG. MethodsThis study used a case-control design in 30 POAG patients who came to the OphthalmologyPolyclinic at the University of North Sumatra Hospital and Network Hospital. The data collectionmethod in this study used a consecutive sampling technique starting from February 2022 until thesample was filled. The inclusion criteria in this study were all women with POAG who were ?40years old and willing to be the research sample. The exclusion criteria for this study were patientswith a history of secondary glaucoma or a history of optic nerve disease, a history of ocular surgery(except cataracts), a history of orbital trauma, menstrual disorders, or currently on hormonaltherapy, a history of systemic or autoimmune disease. Data is then collected and analyzed. ResultsOf the 30 patients with POAG, the majority were aged 45-65 years (46.7%) with an average age of55.87(±12.91) years and had suffered from POAG for an average of 1.62(±2.08) years. The averagepatient had an intraocular pressure of 25.07(±6.12) mmHg, MD -12.99(±8.28) dB, VCDR0.78(±0.13) mm, and estrogen levels 2.47(±2.11) pg/dL. There was no significant relationshipbetween estrogen levels and intraocular pressure (p=0.851). ConclusionThere was no significant association between estrogen levels and intraocular pressure in patientswith POAG.

Abstract

Ground water could be contaminated by heavy metal Mercury (Hg), which could come from the activity of gold mining at the side of river flow. Waste with mercury content will flow to the river so could pollute wellwater along the watershed. The contamination of wellwater is affected by some factors.The objective of the research was to analyze some factors which influenced mercury (Hg) content in people’s wellwater at Tamiang Village, Kotanopan Subdistrict, Mandailing Natal District. The research used an analytic survey with cross sectional study. The samples of the research were the wellwater used as drinking water and the soil around these sites. The data were analyzed by using Mann-Whitney test and multiple linear regression tests.The result of the research showed that the highest mercury measured was 0,00207mg/l, Variables of the distance of polluting source from the wells (p = 0.000), and soil porosity (p = 0.000) influenced the mercury (Hg) content in wellwater, while the age of the wells (p = 0.183), well construction/physic (p >0.25), wellwater temperature (p = 0.760), and well depth (p = 0.594)did not have any influence on the mercury (Hg) content in wellwater. The multiple linear regression tests showed that the mercury content in wellwater was affected by the variable of the distance of the polluting source from the wells with coefficient value Exp (β) of 5.882E6 and the soil porosity with coefficient value Exp (β) of 5.317E-5.

Abstract

Background: Patients with chronic suppurative otitis media (CSOM) often experience hearing loss from mild to very severe degrees. In addition, patients with CSOM often have intraoperative pathological findings. Aims of this study is to determine the correlation between intraoperative tympanomastoidectomy pathological findings with the type and degree of hearing loss using pure tone audiogram images in CSOM patients. Methods: This study is an analytic type study that uses a cross-sectional design. This research was conducted in three hospitals during October 2021 to December 2021. The sample size in this study was 37 patients who would undergo tympanomastodectomy surgery. Results: The most common pathological finding was granulation in 36 patients (97.3%) and followed by sclerotic in 31 patients (83.8%). The average patient had more than 2 pathological findings. Most of patients (78.4%) had conductive hearing loss and most of patients had moderate degree of hearing loss in 25 patients (67.6%). Based on type of hearing loss, there were significant associations with several pathological findings, namely cholesteatoma (p=0.003), defect ossicles (p=0.009), ossicular defects (p=0.001), facial nerve defects (p=0.009). In terms of the number of pathological findings, there was a significant correlation with hearing loss (p=0.004, r=0.467) and the degree of hearing loss (p=0.029, r=0.360). Conclusion: Among the pathological findings by type of hearing loss, a significant correlation was found.
