Abstract

Introduction: Lung cancer is a malignancy of the lung parenchyma or bronchi. Molecular targeted therapy directed at oncogene driver mutations, such as anaplastic lymphoma kinase (ALK), improves the prognosis of patients with non-small cell lung carcinoma (NSCLC). This study aimed to determine the characteristics of lung adenocarcinoma patients and the prevalence of ALK rearrangement among the North Sumatra population. Methods: This descriptive study used data from patients' formalin-fixed paraffin-embedded (FFPE) ALK examination results and medical records. The laboratory analyzed the patient’s FFPE for ALK fusion protein expression using the VENTANA anti-ALK (D5F3) procedure to determine the prevalence of ALK rearrangement. Results: Of the 34 subjects, it was revealed that the characteristics of lung adenocarcinoma patients were 18 patients aged >60 years old (52.9%), 26 male patients (76.4%), and 24 heavy smoker patients (70.6%). Based on the pathological tumor-node-metastasis (pTNM) stage, most samples were classified as stage IVA, with 24 cases (70.6%) showing the highest metastases to the pleura. There were 2 cases of ALK mutations obtained through immunohistochemical examination with a percentage of 5.8%. Conclusion: There are relatively few ALK rearrangement mutations in lung adenocarcinoma patients without screening. Additional research is needed to ascertain the distribution of lung adenocarcinoma patient characteristics associated with a higher prevalence of ALK rearrangement mutations.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a significant health issue with high morbidity and mortality rates. However, risk factors for COPD, particularly in cases with type II respiratory failure, are not fully understood. This study analyzes factors influencing the mortality rate of COPD patients with type II respiratory failure. Methods: This observational study used a cross-sectional design, analyzing medical records of COPD patients with type II respiratory failure treated at H. Adam Malik General Hospital, Medan, from November 2021 to September 2022. A total of 42 patients met the inclusion criteria. Data analysis included descriptive statistics and inferential tests using the Chi-Square test and multiple logistic regression to identify the most dominant factors. Results: Among the 42 patients, the mortality rate was 50%. Variables such as gender, age, number of comorbidities, smoking habits, and history of exacerbations were not significantly associated with mortality. Patients who did not regularly use inhalers had a 19-fold higher mortality risk than those who did (P=0.004; OR=19.79; 95% CI=2.67-146.99). Length of stay was inversely associated with mortality, with each additional day reducing the risk by 0.77 times (P=0.011; OR=0.77; 95% CI=0.63-0.94). Conclusion: Routine inhaler use and length of stay are significant factors influencing the mortality rate in COPD patients with type II respiratory failure.

Abstract

Introduction & ObjectivesThere are studies that state that estrogen deficiency can exacerbate visual disturbances in glaucomapatients. To determine the relationship between estrogen levels and intraocular pressure (IOP) inpatients with POAG. MethodsThis study used a case-control design in 30 POAG patients who came to the OphthalmologyPolyclinic at the University of North Sumatra Hospital and Network Hospital. The data collectionmethod in this study used a consecutive sampling technique starting from February 2022 until thesample was filled. The inclusion criteria in this study were all women with POAG who were ?40years old and willing to be the research sample. The exclusion criteria for this study were patientswith a history of secondary glaucoma or a history of optic nerve disease, a history of ocular surgery(except cataracts), a history of orbital trauma, menstrual disorders, or currently on hormonaltherapy, a history of systemic or autoimmune disease. Data is then collected and analyzed. ResultsOf the 30 patients with POAG, the majority were aged 45-65 years (46.7%) with an average age of55.87(±12.91) years and had suffered from POAG for an average of 1.62(±2.08) years. The averagepatient had an intraocular pressure of 25.07(±6.12) mmHg, MD -12.99(±8.28) dB, VCDR0.78(±0.13) mm, and estrogen levels 2.47(±2.11) pg/dL. There was no significant relationshipbetween estrogen levels and intraocular pressure (p=0.851). ConclusionThere was no significant association between estrogen levels and intraocular pressure in patientswith POAG.

Abstract

Ground water could be contaminated by heavy metal Mercury (Hg), which could come from the activity of gold mining at the side of river flow. Waste with mercury content will flow to the river so could pollute wellwater along the watershed. The contamination of wellwater is affected by some factors.The objective of the research was to analyze some factors which influenced mercury (Hg) content in people’s wellwater at Tamiang Village, Kotanopan Subdistrict, Mandailing Natal District. The research used an analytic survey with cross sectional study. The samples of the research were the wellwater used as drinking water and the soil around these sites. The data were analyzed by using Mann-Whitney test and multiple linear regression tests.The result of the research showed that the highest mercury measured was 0,00207mg/l, Variables of the distance of polluting source from the wells (p = 0.000), and soil porosity (p = 0.000) influenced the mercury (Hg) content in wellwater, while the age of the wells (p = 0.183), well construction/physic (p >0.25), wellwater temperature (p = 0.760), and well depth (p = 0.594)did not have any influence on the mercury (Hg) content in wellwater. The multiple linear regression tests showed that the mercury content in wellwater was affected by the variable of the distance of the polluting source from the wells with coefficient value Exp (β) of 5.882E6 and the soil porosity with coefficient value Exp (β) of 5.317E-5.

Abstract

Background: Patients with chronic suppurative otitis media (CSOM) often experience hearing loss from mild to very severe degrees. In addition, patients with CSOM often have intraoperative pathological findings. Aims of this study is to determine the correlation between intraoperative tympanomastoidectomy pathological findings with the type and degree of hearing loss using pure tone audiogram images in CSOM patients. Methods: This study is an analytic type study that uses a cross-sectional design. This research was conducted in three hospitals during October 2021 to December 2021. The sample size in this study was 37 patients who would undergo tympanomastodectomy surgery. Results: The most common pathological finding was granulation in 36 patients (97.3%) and followed by sclerotic in 31 patients (83.8%). The average patient had more than 2 pathological findings. Most of patients (78.4%) had conductive hearing loss and most of patients had moderate degree of hearing loss in 25 patients (67.6%). Based on type of hearing loss, there were significant associations with several pathological findings, namely cholesteatoma (p=0.003), defect ossicles (p=0.009), ossicular defects (p=0.001), facial nerve defects (p=0.009). In terms of the number of pathological findings, there was a significant correlation with hearing loss (p=0.004, r=0.467) and the degree of hearing loss (p=0.029, r=0.360). Conclusion: Among the pathological findings by type of hearing loss, a significant correlation was found.

Abstract

Acute Respiratory Infections (ARI) in the working area of Mulyorejo Public Health Center, where an aluminum factory is located, rank first among the ten most common diseases. This study aims to analyze the influence of sociodemographic characteristics and PM2.5 exposure on ARI complaints in the aluminum factory area of Payageli Village, Deli Serdang Regency. The cross-sectional observational study involved 160 housewives living within a radius of 10–1500 meters from the factory’s chimney. The analysis included univariate, bivariate, and multivariate methods. Results showed that 58.1% of respondents experienced ARI symptoms in the past month. The average ambient (outdoor) PM2.5 concentration measured from three sampling points was 12.3 µg/m³, which did not exceed the WHO standard (<15 µg/m³). The highest concentration was recorded in the southern direction (15.5 µg/m³). Bivariate analysis indicated that education (p = 0.010), direction of house location (p = 0.001), and indoor PM2.5 concentration (p = 0.006) were significantly associated with ARI. Multivariate logistic regression analysis showed that the most dominant factor influencing ARI was indoor PM2.5 concentration (p = 0.039), followed by education (p = 0.013) and direction of house location (p = 0.001). This study provides a scientific basis for air quality control and housing environment improvement to reduce the risk of environmentally related diseases in industrial zones.

Abstract

Smoking can cause health problems for both active and passive smokers because the toxic substances contained in it cause health problems, especially in the larynx, one of which is laryngeal cancer. Animal studies have shown histopathological changes in mice exposed to cigarette smoke, including increased immune cells and focal inflammation in these mice. The study aims to determine the relationship between cigarette smoke and the number of inflammatory cells, edema, hyperplasia, and dysplasia after exposure to cigarette smoke for 120 minutes in histopathological sampling at observations of 0, 10, 20, 30 and 40 days. This study used a true experimental design model with a post-test only group laboratory experimental design. The results of the study found a significant difference in the number of lymphocytes before and after exposure to cigarette smoke with p = 0.043. There was no significant change in the number of macrophages with p = 0.406 and eosinophils with p = 0.119. There were no histopathological changes, namely edema (p = 0.06), dysplasia (p = 0.126) and hyperplasia (p = 0.960). In conclusion, there is a relationship between cigarette smoke exposure and laryngeal histopathology in Rattus novergicus Rats.

Abstract

Diabetes mellitus and hypertension can be prevented through the consistent adoption of a healthy lifestyle. Women participating in religious study groups are an important segment of the community, as they play a significant role in shaping family lifestyles. A total of 30 Muslim women from the Perwiritan Muslimat Al-Jamiatul Amaliyah in Kampung Lalang, Sunggal, Deli Serdang, participated in the community service activities consisting of education and training on the prevention and management of diabetes mellitus and hypertension. The activities began with socialization of the community service program, followed by preparation by the team, including the development of educational materials and the design of pre- and post-test instruments. Based on the Wilcoxon test, the educational intervention was found to have a significant effect. This was reflected in an increase in participants’ knowledge regarding diabetes mellitus and hypertension after the intervention. Among the 30 participants, 43.3% were classified as pre-hypertensive, 40% had stage 1 hypertension, 10% experienced a hypertensive crisis, 3.3% had stage 2 hypertension, and only one participant had normal blood pressure. Of the 30 women whose fasting blood glucose levels were measured, 43.3% were classified as pre-diabetic, 23.3% had diabetes mellitus, and 33.3% had normal glucose levels. The participants expressed their hope that future activities would include cholesterol level examinations.

Abstract

Komunikasi terapeutik medik adalah proses interaksi tenaga medik dan pasien secara individual dengan tujuan membantu pasien merasa nyaman dan tidak takut untuk mengungkapkan perasaan dan pikiran mereka. Hanya saja, ada indikasi komunikasi terapeutik medik di RSUD Dr. Pirngadi Kota Medan belum efektif. Salah satu penyebabnya adalah tingkat pengetahuan dan keterampilan yang rendah dari dokter mengenai cara menyamankan dan meyakinkan pasien. Agar dokter dapat meningkatkan keterampilannya mengenai bagaimana memprioritaskan kenyamanan dan keyakinan pasien saat berkomunikasi dengan pasien, maka diperlukan pelatihan mendengar aktif, mendengar dengan empati, komunikasi persuasif, dan komunikasi asertif. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan dan keterampilan dokter mengenai cara menyamankan dan meyakinkan pasien berobat di rumah sakit. Pelatihan ini menggunakan metode ceramah, games, dan role play. Peserta pelatihan yang mengikuti sampai akhir hanya 12 orang. Berdasarkan evaluasi pelatihan dengan pre-test dan post-test, pengetahuan peserta tentang cara menyamankan dan meyakinkan pasien berobat di rumah sakit RSUD Dr. Pirngadi Kota Medan meningkat dari 0% menjadi 83,3%. Berdasarkan hasil evaluasi pelatihan dengan simulasi, seluruh peserta mendapatkan nilai 70 ke atas. Disimpulkan bahwa pelatihan komunikasi terapeutik medik yang telah dilakukan dapat meningkatkan pengetahuan dan keterampilan dokter mengenai cara menyamankan dan meyakinkan pasien berobat di rumah sakit RSUD Dr. Pirngadi Kota Medan. Diharapkan pelatihan ini dapat dilanjutkan secara mandiri oleh mitra dan didukung oleh pihak terkait, serta ke depan pelatihan serupa dapat diperluas ke rumah sakit lain dengan pemantauan berkelanjutan.

Abstract

Diabetes mellitus (DM) and tuberculosis (TB) have a synergistic effect in worsening the disease severity and treatment outcomes. Chest radiography is the standard imaging modality for TB diagnosis, so identifying characteristic chest radiograph findings has the potential to accelerate early diagnosis and anticipate management in TB patients with and without DM. This study aimed to compare chest x-ray findings between tuberculosis patients with and without DM. This was a cross-sectional study of pulmonary TB patients who underwent posteroanterior (PA) chest x-ray at the Pulmonology Clinic of Haji Adam Malik General Hospital from January 2023 to June 2025. Data were obtained from patient medical records. Bivariate analysis used the chi-square test if the data were normally distributed, or the Fisher exact test if the data were not normally distributed. A p-value <0.05 was considered statistically significant. In TB patients with DM, 81.3% of patients had lesions in the upper lung fields, 65.3% had lesions in the middle lung fields, and 50.7% had lesions in the lower lung fields. The most dominant lesion characteristics were infiltrates (73.3%), fibrosis (42.7%), and consolidation (26.7%). In TB patients without DM, 36% of patients had lesions in the upper lung fields, 29.3% of patients had lesions in the middle lung fields, and 21.3% of patients had lesions in the lower lung fields. The most dominant lesion characteristics were infiltrates (44%), consolidation (25.3%), and fibrosis (18.7%). In TB patients with and without diabetes, the majority of patients had lesions in the upper lung fields, and infiltrates were the most dominant lesion.
