Abstract

Background: Lung adenocarcinoma patients receiving EGFR-TKI may develop acquired resistance within 7-16 months of treatment initiation, which is characterized by the presence of exon 20 T790M mutations in treatment response patients and can be assessed objectively by CECT and then evaluated by RECIST 1.1. The purpose of this study is to look into the association between acquired resistance EGFR Exon 20 T790M mutation and treatment response in lung adenocarcinoma patients receiving EGFR-TKI. Method: This research is an analytic study with a retrospective cohort design carried out at the Oncology Polyclinic at Haji Adam Malik Hospital from October 2020 to January 2021 in all patients with adenocarcinoma lung cancer who were treated with EGFR-TKI for more than 6 months. After that, an evaluation was carried out based on RECIST 1.1 and then examined for EGFR mutations from liquid biopsy specimens in the form of circulating tumor plasma DNA (ct-DNA) with the droplet digital Polymerase Chain Reaction (ddPCR) method to detect EGFR exon 20 T790M mutations as a marker of acquired resistance. Results: It was found that the majority of subjects were female (64.5%), aged 20-69 years (58%), and non-smokers (67.7%). The most common EGFR mutation was exon 19 deletion (58.1%). The incidence of acquired resistance was found in 10 subjects (32.3%). The distribution of RECIST 1.1 results on positive acquired resistance includes progressive diseases of 35.2%; stable disease of 11.1%; partial response of 33.4%; and 100% complete response. Negative acquired resistance includes 64.8% progressive disease, 88.9% stable disease, 66.6% partial response, and 0% complete response (P=0.93). Conclusion: There is no significant association between the incidence of acquired resistance mutations EGFR exon 20 T790M and treatment response in patients with lung adenocarcinoma who received EGFR-TKI therapy.

Abstract

Noise is one of the factors that can affect worker productivity. High noise intensity in the workplace can affect blood pressure. Every workplace always contains various hazard factors that can affect the health of workers. The purpose of this study was to determine the relationship between noise levels and blood pressure in workers at PT. Pertamina RU II. This research is an analytic study using a study design. The population sampled in this study is 130 people. The results showed that the noise intensity in the refinery area exceeded the NAB by 87 dbA, and in the office by 30-40 dbA. The workers' systolic blood pressure increased by 75%, diastolic blood pressure increased by 70%, and pulse rate increased by 89%. . The results of blood pressure in the study in two groups, namely exposed and unexposed, there was no significant difference between systolic and diastolic pressure in the exposed and unexposed groups of noise, but there was a significant difference between the difference in pulse rates in the exposed and unexposed groups. noise in the refinery area of ​​PT. Abstract

ABSTRAKLatar Belakang: Gastroenteritis pada bayi adalah peradangan selaput mukosa lambung dan usus kecil yang ditandai dengan gejala diare, mualmuntah dan demam ringan disertai dengan hilangnya nafsu makan dan rasa tidak nyaman di perut. Pada tahun 2010 gastroenteritis adalah penyebab utama kematian di Indonesia dan berada di urutan pertama sebagai penyebab pasien dirawat di rumah sakit. Tujuan: Menganalisis hubungan praktik pemberian makan bayi dengan kejadian gastroenteritis. Metode: Penelitian ini menggunakan metode case control yang menganalisis hubungan antara pola makan dan kejadian gastroenteritis pada bayi di wilayah kerja Puskesmas Silangit Kabupaten Tapanuli Utara. Sampel dalam penelitian ini adalah bayi berusia 7 hingga 12 bulan sebanyak 94 orang, 47 kasus dan 47 kontrol. Data dianalisis menggunakan uji Chi Square dan Regresi Logistik Berganda. Hasil: Hasil penelitian menunjukkan distribusi pola makan bayi sebagian besar tidak baik sebanyak 78,7%. Hasil analisis penelitian menunjukkan bahwa ada hubungan yang signifikan antara pola makan dengan kejadian gastroenteritis, dengan nilai p <0,001 dan nilai OR sebesar 8,202. Simpulan: Ibu yang memiliki bayi diberi dorongan untuk memberikan pola makan yang baik kepada bayi dengan cara menyusui secara eksklusif, menerapkan perilaku hidup bersih dan sehat untuk meningkatkan kesehatan bayi.Kata kunci: praktik pemberian makan, gastroenteritis, bayiThe Relationship Between Feeding Practice and the Incidence of GastroenteritisABSTRACTBackground: Gastroenteritis in infants is an inflammation of the mucous membrane of the stomach and small intestine characterized by symptoms of diarrhea, nausea, vomiting and mild fever accompanied by loss of appetite and discomfort in the stomach. In 2010 gastroenteritis is the leading cause of death in Indonesia and is the first order that causes patients hospitalized in the hospital. Objective: to analyze the relationship between feeding practice and the incidence of gastroenteritis. Method: This study used a case control method that analyzed the relationship between feeding practice and the incidence of gastroenteritis in infants in the work area of Silangit Public Health Center of North Tapanuli Regency. The sample in this study was 7 to 12 months old infants as many as 94 people, 47 cases and 47 controls. Data were analyzed using Chi Square test and Multiple Logistic Regression. Results: The results showed the distribution of infant feeding practice is mostly not good as much as 78.7%. The result of research analysis showed that there was a significant correlation between feeding pattern with the incidence of gastroenteritis with p value <0.001 and OR value of 8,202. Conclusion: Mothers with babies are encouraged to provide good feeding practice to infants by exclusive breastfeeding, applying clean and healthy living behaviors to improve infant health.Keywords: Feeding practice, gastroenteritis, infants

Abstract

ABSTRACT  Waste is the outcome and activities of human beings. Its existence cannot be avoided so that it has to be managed well because unhygienic waste will cause environmental pollution. There are 126 cubic meters of waste per day transported to the garbage dump in Subulussalam with 42% of services of the total of 542 cubic meters per day. The objective of the research was to analyze Waste Management in Subulussalam, in 2017.The research used descriptive method by conducting in-depth interviews with 10 respondents. The data were obtained from the Sanitation Agency and gathered by using questionnaires, observation, and documentary study.The result of the research showed that organic waste (70%) was more than the waste from batteries (0.02%). The income and retribution obtained from waste management in Subulussalam increased to 25.2% each year. Facility and infrastructure indicated that waste transportation from the people’s homes used pushcarts (69.2%) since their houses were far from the main street and served by workers manually, while 30.7% were transported by garbage trucks.It is recommended that the City Administration promote the program which encourages waste recycling in order to decrease the waste volumes to the garbage dump and encourage compost making program, the stem of discriminating, collecting, transporting, and dumping waste by adding garbage trucks and workers.   Keywords: Analysis, Waste Management, Landfill

Abstract

ISPA (Upper Respiratory Tract Infection) is an environment-based disease and one of the major causes of the morbidity and mortality of communicable diseases in the world. WHO states that almost four million people die from ISPA every year; 98% of them were caused by lower respiratory tract infection. The rate of mortality is high in babies, children and adults. The objective of the research was to discover the influence of home physical environment, pollutant source and family characteristics on ISPA in balita. This is an analytical survey research with cross sectional research design. The population was all balita i.e. 200 children and 65 of them were taken as the samples non-randomly. Chi-square testing and multiple logistic regression testing were employed for the data analysis. The results of the research demonstrated that ventilation, lighting and smoking history had significant influence on the prevalence of ISPA in balita. Meanwhile, occupancy density did not have any influence on the prevalence of ISPA. The results of the multiple logistic regression testing showed that smoking history was the variable with the most dominant influence on the prevalence of ISPA with p value 0,003, PR=11,517;95% CI=2,360-56,198, which indicated that the respondents with smoking history had 11,517 times higher risk of ISPA in balita compared to the respondents without smoking history. It is suggested that the Puskesmas (Public Health Center for sub district level) improve the counseling about health especially information related to ISPA, so that it can reduce the prevalence of ISPA or other disease in balita. Keywords:    ISPA, Home Environment, Under Five Year Old Children, Smoking History

Abstract

Diabetes mellitus merupakan penyakit tidak menular (Non Comunicable Disease) yang cukup banyak diderita oleh masyarakat di dunia dan Indonesia. Penderita kasus baru diabetes mellitus di Desa Juhar Ginting Sadanioga pada bulan Februari 2018 adalah 195 kasus. Penggunaan pestisida dengan dosis tinggi dan dalam jangka panjang dapat menimbulkan dampak negatif bagi masyarakat. Penelitian ini bertujuan untuk mengetahui hubungan indeks massa tubuh dan lama penyemprotan dengan kejadian diabetes mellitus pada petani. Penelitian ini menggunakan desain cross sectional dengan jumlah sampel 60 orang petani penyemprot di Desa Juhar Ginting Sadanioga dengan teknik simple random sampling. Data dikumpulkan dengan pemeriksaan kadar gula darah, pengukuran berat badan, dan wawancara menggunakan kuesioner. Hasil menunjukkan bahwa ada hubungan IMT dengan Diabetes Mellitus (p=0,024) dan ada hubungan lama penyemprotan dengan diabetes mellitus (p=0,023). Dari dua variabel tersebut, variabel lama penyemprotan merupakan variabel yang paling berhubungan dengan diabetes mellitus yaitu nilai RP sebesar 8,800. Kepada petani diharapkan untuk mengontrol berat badan dan melakukan penyemprotan tidak lebih dari 3 jam per hari untuk meminimalisir risiko diabetes mellitus.

Abstract

Toluene exposure and urine hippuric acid level in printing workers in Medan in 2018Purpose: The printing industry has been one of the growing industries. The printing industry in Indonesia in 2011 is estimated to reach 4.7 percent or higher than the average world growth, which is only 1.6 percent. The production and printing services needs in Indonesia are significantly increasing. Industrial processes that use labor, especially those related to using chemicals in the production process, will be at high risk of potential hazards. According to the Ministry of Industry and Trade, 45 industries use hazardous materials, one of which is the printing industry. This study aims to analyze exposure to toluene through urine hippuric acid levels in the printing industry workers. Methods: This research is a quantitative observational analytic study using a cross-sectional design conducted from March to July 2018. The sample of this study is 50 respondents located in 6 printing industries in Medan City. Most of the respondents are males, which is 38 people (76%). Results: The average value of ambient air toluene exposure in 6 printing companies was 6.97 ppm, much lower than the threshold value of 50 ppm, and the average hippuric acid level of printing employees is 143.5 mg/g creatinine, which is far from the threshold value of 1600 mg/g creatinine. There is a correlation between sex with hippuric acid levels (p <0.05). Conclusions: Printing business owners should monitor their employees' health periodically. The Health Ministry also needs to regulate the protection of workers from toluene. To reduce toluene exposure, printing industry owners should equip their employees with personal protective equipment and manage better office hours, a day off, and furlough.

Abstract

Background. Majority of population in Tanah Karo District, Indonesia are farmers (88.54%), which is always use pesticides to their planting. The use of pesticides with high intensity and continuously will cause a negative impact on health.Purpose. The purpose of this research was to analyze the relationship between pesticide exposure, including the length of work, length of spraying, spraying techniques and Personal Protection Equipment (PPE) with farmers' cholinesterase levels.Method. This research was an observational analytic with a cross sectional approach. Samples were 40 farmers. Data collection through questionnaires and health checks. Data analysis was performed using univariate and bivariate with chi-square test.Result. The result showed that from 40 farmers, 13 farmers had below normal cholinesterase levels, there was relationship between the length of spraying, spraying techniques, and the use of PPE with cholinesterase levels with p<0,05 and there was no relationship between length of work with  level of cholinesterase p>0,05.Conclusion. Farmers who have long been exposed to pesticides can reduce levels of cholinesterase, especially if they don't pay attention to spraying techniques and use PPE, it can cause poisoning. It is recommended to the Department of Agriculture Karo District to monitor and supervise the use of pesticides on farmers, and for farmers to pay attention to the use of PPE, spraying techniques, and monitor their health status regularly at the health center.

Abstract

The purpose of this study is to analyze of the relationship of income and knowledge to habits of open defecation in Langkat Regency in 2022. The type of research used in this study is a quantitative study with a cross sectional approach. The population of this study is all heads of families in Langkat Regency, totaling 256,025 Families. The sample in this study is the subject taken from the population of the community residing in 2 Sub-Districts from 23 Sub-Districts in Langkat Regency. The total of each sample is Tanjung Pura and Stabat Districts 4 and 2. Data analysis used bivariate analysis. The results show that there is a significant relationship between income and knowledge to habits of open defecation in Langkat Regency in 2022.
