Abstract

Objective: To compare the behavior of Andalas University medical and law students towards COVID-19. Methods: This is an observational analytic study with a cross-sectional design. The sampling technique used proportional stratified random sampling with a sample size of 204 students of medical and law at Andalas University. The Data obtained are primary data collected using a questionnaire via google form which were then analyzed by bivariate analysis with the Mann-Whitney. Results: The results showed a statistically significant difference (p<0,05) between knowledge (p=0,00), attitude (p=0,045), and practices towards COVID-19 (p=0,038). Conclusion: This study shows that there are differences in the knowledge, attitudes, and practices towards COVID-19 between medical and law students.

Abstract

Abstract Introduction: Cesarean deliveries are increasing worldwide and now account for one fifth (21%) of all deliveries. This has exceeded the standard for caesarean section operations by WHO by around 5-15%. In Indonesia cesarean delivery rate was 17.6% percent with the highest proportion in DKI Jakarta (31.1%), the lowest in Papua (3.3%). West Sumatra is ranked second highest with a caesarean section rate of 23.8%. Objective: To determine the relationship between age, education, employment, nutritional status, number of ANCs, underlying diseases, and the severity of caesarean section. Method: This research is an observational analytical design with a croseksio area approach. It aims to relate the relationship between age, education, employment, nutritional status, number of ANCs, and underlying diseases, and the severity of caesarean section. Results: This study had 400 research subjects who underwent caesarean section operations, with 16 research subjects experiencing caesarean sections with severe severity, 276 research subjects experiencing moderate severity, and 108 research subjects experiencing mild severity. Using the Chi-Square correlation test, there was significant relationship between a history of ANC and the history of underlying disease on the severity of caesarean section. There was no significant relationship between age, education, employment, or nutritional status on the severity of caesarean section. Conclusion: There was significant relationship between history of ANC and history of underlying disease on the severity of caesarean section. Abstract

Food poisoning outbreaks are a serious problem that can have a significant impact on public health. Food poisoning occurs when food consumed is contaminated with bacteria, viruses, parasites, or hazardous chemicals. Medan City experiences fluctuating food poisoning incidents, so continuous monitoring is needed to identify and address the causes. The purpose of this study was to determine early warning by using a module to prevent food poisoning outbreaks. This study was conducted quantitatively using an experimental design with the one group pretest-posttest design (before and after) approach. The location of the study was carried out at the Medan City Health Office and a population of 20 respondents, while the sample, namely the total population, was the sample. The data obtained were analyzed univariately, bivariately. The results showed that knowledge in catering owners with n = 20, paired sample T-test there was a significant difference between the level of knowledge of respondents about food poisoning before and after the intervention using the module p value = 0.0001 (<0.05). The module that has been developed and validated by language experts with results of 94.3, material experts with 97.3 and media experts with 91.3 can effectively prevent poisoning. It is recommended to the local Health Service and Health Center to be able to use the module as an early warning model for food poisoning outbreaks, especially for catering business owners, so that they can increase knowledge, especially in carrying out prevention. Abstract

Drug instances have increased in Solok Regency, with Kubung District seeing the largest increase in cases. Middle school students are a vital population because, during this formative period of personality formation, they experiment and seek validation from others, which makes more supervision necessary. The purpose of this study is to ascertain how psychoeducation about self-control can help junior high school pupils in Kubung District, Solok Regency, avoid drug misuse in 2023. With a one group pretest and posttest design and proportionate random sample of 110 respondents, This research uses the Quasi Experiment method. The analysis's findings indicated that 110 students' mean self-control scores increased from 42.51 to 48.28 after receiving psychoeducation, with a p-value of 0.025 (p-value <0.05) for the Wilcoxon test. These results show an increase in the average student pretest and post-test scores, indicating a significant influence of psychoeducation on increasing self-control in preventing drug abuse, which leads one to the conclusion that psychoeducation influences junior high school students' self-control in preventing drug abuse.

Abstract

Background: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the digestive tract, primarily driven by mutations in KIT or PDGFRA genes. CD117 (c-KIT) expression is a key diagnostic marker, while the Ki67 labeling index reflects cellular proliferation. Risk stratification, often using modified NIH criteria based on tumor size, mitotic rate, and location, guides prognosis and treatment. This study investigated the distinct roles of CD117 and Ki67 expression in relation to risk stratification in GIST patients. Methods: This cross-sectional analytical study examined 27 GIST cases diagnosed between January 2021 and December 2024 from three Indonesian hospitals. Formalin-fixed paraffin-embedded tissues were analyzed using immunohistochemistry for CD117 (clone YR145) and Ki67 (clone K2). CD117 positivity was defined as ≥5% tumor cell staining, and high Ki67 expression as >10% nuclear staining. Risk stratification utilized the modified NIH criteria. The Chi-square test assessed correlations (p<0.05 significance). Results: The cohort predominantly comprised patients >50 years (66.7%), males (59.3%), with gastric tumors (51.9%), large tumor size (>5cm in 96.3%), spindle cell morphology (77.8%), and high mitotic rates (74.1%). Most cases (85.2%) were classified as high-risk. CD117 was positive in 81.5% (22/27) of cases but showed no significant correlation with risk stratification (p=0.561). High Ki67 expression was found in 74.1% (20/27) of cases and demonstrated a significant positive correlation with high-risk stratification (p=0.002). The combination of CD117 and Ki67 status also showed a significant association with risk stratification (p=0.001). Conclusion: While CD117 expression remains a cornerstone for GIST diagnosis and targeted therapy selection, it did not correlate significantly with risk stratification in this cohort. Conversely, a high Ki67 labeling index was significantly associated with high-risk GIST, underscoring its potential as a valuable prognostic marker alongside established risk stratification parameters.

Abstract

Background: Thymic epithelial tumors (TETs) are uncommon malignancies originating in the mediastinum, characterized by considerable histopathological diversity and variable clinical trajectories. Programmed Death-Ligand 1 (PD-L1), an immune checkpoint protein, is implicated in mechanisms of tumor immune evasion. This study aimed to investigate the correlation between PD-L1 immunoexpression and distinct histopathological types, as well as the Masaoka-Koga stage, in TETs. Methods: This cross-sectional investigation analyzed 29 archival cases of TETs diagnosed between January 2019 and December 2024 at the Anatomical Pathology Laboratory of Dr. M. Djamil General Hospital Padang. Samples were procured via consecutive sampling from formalin-fixed paraffin-embedded (FFPE) tumor tissues. Histopathological classification was reassessed according to the WHO 2021 criteria. PD-L1 expression was evaluated immunohistochemically and quantified using the Tumor Proportion Score (TPS). Masaoka-Koga staging was determined from clinical records. Statistical analysis of correlations was performed using the Chi-square test. Results: PD-L1 immunoexpression was detected in the preponderance of cases. Low positive PD-L1 expression (TPS 1-49%) was observed in 82.8% of TETs, while high positive expression (TPS ≥50%) was noted in 10.3%. Thymic carcinoma constituted the most prevalent histopathological category (51.7%), and the majority of patients (91.7%) presented at an advanced Masaoka-Koga stage. Statistical analysis did not demonstrate a significant correlation between PD-L1 expression levels and histopathological type (p=0.195). Furthermore, no significant association was identified between PD-L1 expression and Masaoka-Koga stage (p=0.800). Conclusion: This study indicated that while PD-L1 is frequently expressed in TETs within this cohort, its expression level did not exhibit a significant correlation with specific histopathological subtypes or the Masaoka-Koga clinical stage. Further investigations incorporating larger sample sizes are warranted to delineate the precise role of PD-L1 within the complex biological spectrum of thymic neoplasms.

Background: Endometrioid endometrial carcinoma (EEC) is a prevalent gynecological malignancy whose prognosis is influenced by factors including histopathological grade and lymphovascular invasion (LVI). Angiogenesis, crucial for tumor growth and metastasis, is significantly mediated by vascular endothelial growth factor (VEGF). This study aimed to investigate the expression of VEGF in EEC and its correlation with histopathological differentiation and LVI. Methods: This observational analytical study employed a cross-sectional design using 36 archival paraffin block samples of EEC diagnosed between January 2022 and December 2024 at the Anatomical Pathology Laboratory of Dr. M. Djamil General Hospital Padang. Cases were selected via simple random sampling from a population of 59. Histopathological grade (Grade 1, 2, or 3 based on FIGO architectural and nuclear criteria) and LVI (negative, focal, or substantial) were re-evaluated from Hematoxylin-Eosin (H&E) stained slides. VEGF expression was assessed by immunohistochemistry, scored semiquantitatively based on the percentage of positive tumor cells and staining intensity, and categorized as low or high. Data were analyzed using Chi-square tests, with p<0.05 considered statistically significant. Results: The mean age of patients was 54.36 years, with the highest prevalence in the 51-60 age group (41.7%). Grade 3 tumors were most common (38.9%), followed by Grade 2 (33.3%) and Grade 1 (27.8%). LVI was present in 47.2% of cases, predominantly focal (38.9%). High VEGF expression was observed in 58.3% of EEC cases. A statistically significant association was found between high VEGF expression and higher histopathological grade (p=0.000), with 66.7% of Grade 3 tumors showing high VEGF expression. No significant association was found between VEGF expression and LVI (p=0.080). Conclusion: High VEGF expression significantly correlated with higher histopathological grades in EEC, suggesting its role in tumor aggressiveness and dedifferentiation. However, a significant association with LVI was not established in this cohort. VEGF expression warrants further investigation as a potential prognostic biomarker and therapeutic target in EEC.

Abstract

Antenatal care (ANC) merupakan pelayanan untuk ibu hamil dalam upaya menjaga kesehatan ibu dan bayi dan mengurangi risiko komplikasi saat kehamilan dan persalinan dengan mendeteksi dini masalah dan risiko tinggi. Pada tahun 2022, cakupan K1 dan K4 ANC Puskesmas Ambacang menempati posisi yang cukup tinggi, namun terdapat tiga kematian ibu di wilayah kerja Puskesmas Ambacang. Efektivitas dan kualitas ANC di tidak hanya berdasarkan jumlah kunjungan, tetapi juga berbagai aspek, yaitu input, proses, dan output, serta kepuasan ibu hamil. Penelitian ini bertujuan untuk melihat gambaran pelaksanaan pelayanan ANC di Puskesmas Ambacang dengan meninjau aspek input, proses, output, dan kepuasan. Penelitian ini menggunakan metode kualitatif dengan perspektif studi kasus dan menghasilkan data deskriptif berupa gambaran dan kata-kata tertulis dari wawancara mendalam terhadap informan serta perilaku yang diobservasi, juga telaah dokumen. Informan yang diwawancara pada penelitian ini sebanyak 12 orang. Komponen input sudah sesuai standar dengan sumber daya manusia yang memadai, baik dari kualitas dan kuantitasnya. Dana pelayanan dari BOK, APBD, dan BLUD. Sarana dan prasarana sudah layak, namun terdapat kendala pada luas ruangannya. Terdapat SOP pelayanan yang mengacu pada kebijakan dan standar pelayanan minimal. Komponen proses sudah berjalan dengan baik dan melaksanakan pelayanan 10T, namun pencatatan kunjungan pelayanan belum maksimal. Komponen output berupa angka cakupan K1–K4 yang mengalami penurunan dan berada dibawah target nasional. Secara garis besar, ibu hamil sudah merasa sangat puas dengan aspek SERVQUAL pelayanan yang diberikan. Secara keseluruhan, pelayanan ANC di Puskesmas Ambacang berjalan dengan baik, namun perlu mengoptimalkan proses pencatatan angka kunjungan untuk meningkatkan kualitas pelayanan. --- Antenatal care (ANC) is a service provided to pregnant women to maintain the health of both the mother and the baby, and to reduce the risk of complications during pregnancy and childbirth by early detection of problems and high-risk factors. In 2022, the coverage of K1 and K4 ANC at Puskesmas Ambacang was relatively high, although there were three maternal deaths within its service area. The effectiveness and quality of ANC are not solely measured by the number of visits but also by various aspects, including input, process, output, and maternal satisfaction. This study aims to assess the implementation of ANC services at Puskesmas Ambacang by reviewing these aspects. The research adopts a qualitative approach with a case study perspective, producing descriptive data from in-depth interviews with 12 informants, observations, and document reviews. The input components meet the required standards, with adequate human resources in terms of both quality and quantity. Service funding is sourced from BOK, APBD, and BLUD. The facilities and infrastructure are adequate, though there are challenges related to the space available. Standard Operating Procedures (SOPs) are in place, aligned with policies and minimum service standards. The process component is functioning well, with the implementation of 10T services, although documentation of service visits has not been fully optimized. The output component, which refers to K1-K4 coverage, has declined and is below the national target. Overall, pregnant women express a high level of satisfaction with the SERVQUAL aspects of the services provided. In conclusion, ANC services at Puskesmas Ambacang are generally functioning well, but improvements are needed in optimizing the documentation and reporting of visit data to enhance service quality.

Abstract

Non-compliance with taking medication in DM patients can reduce a person's quality of life and become the main cause of a number of microvascular problems. Therefore, complex problems related to diabetes are one of the global health problems that must be addressed immediately. The purpose of this study was to determine the factors related to medication compliance in type 2 DM patients. This study used a mixed method study approach or a combination of two types of research, namely quantitative research and qualitative research. The type of mixed method research design used is explanatory sequential design. The sampling method in this study was by proportional sampling. Furthermore, to determine the target sample, a simple random block sampling technique was used, namely 347 DM patients. The results of the analysis showed no significant relationship between gender p-value = 1,000 (> 0.05), education p-value = 0.232 (>, 0.05), knowledge p-value = 0.076 (> 0.05), dxuration of suffering p-value = 0.162 (> 0.05), belief p-value = 0.680 (> 0.05), access to health services p-value = 0.602 (> 0.05), and there was a significant relationship between family support and DM treatment compliance with p-value = 0.020 (<0.05). The conclusion of this study is that more than half of DM patients have low treatment compliance, are female, have low education, have a duration of <5 years, have access to health that takes a long time, and lack of family support. More than half of DM patients have high knowledge and positive beliefs.
