Abstract

Background: Thymic epithelial tumors (TETs) are uncommon malignancies originating in the mediastinum, characterized by considerable histopathological diversity and variable clinical trajectories. Programmed Death-Ligand 1 (PD-L1), an immune checkpoint protein, is implicated in mechanisms of tumor immune evasion. This study aimed to investigate the correlation between PD-L1 immunoexpression and distinct histopathological types, as well as the Masaoka-Koga stage, in TETs. Methods: This cross-sectional investigation analyzed 29 archival cases of TETs diagnosed between January 2019 and December 2024 at the Anatomical Pathology Laboratory of Dr. M. Djamil General Hospital Padang. Samples were procured via consecutive sampling from formalin-fixed paraffin-embedded (FFPE) tumor tissues. Histopathological classification was reassessed according to the WHO 2021 criteria. PD-L1 expression was evaluated immunohistochemically and quantified using the Tumor Proportion Score (TPS). Masaoka-Koga staging was determined from clinical records. Statistical analysis of correlations was performed using the Chi-square test. Results: PD-L1 immunoexpression was detected in the preponderance of cases. Low positive PD-L1 expression (TPS 1-49%) was observed in 82.8% of TETs, while high positive expression (TPS ≥50%) was noted in 10.3%. Thymic carcinoma constituted the most prevalent histopathological category (51.7%), and the majority of patients (91.7%) presented at an advanced Masaoka-Koga stage. Statistical analysis did not demonstrate a significant correlation between PD-L1 expression levels and histopathological type (p=0.195). Furthermore, no significant association was identified between PD-L1 expression and Masaoka-Koga stage (p=0.800). Conclusion: This study indicated that while PD-L1 is frequently expressed in TETs within this cohort, its expression level did not exhibit a significant correlation with specific histopathological subtypes or the Masaoka-Koga clinical stage. Further investigations incorporating larger sample sizes are warranted to delineate the precise role of PD-L1 within the complex biological spectrum of thymic neoplasms.

Abstract

Background: Endometrioid endometrial carcinoma (EEC) is a prevalent gynecological malignancy whose prognosis is influenced by factors including histopathological grade and lymphovascular invasion (LVI). Angiogenesis, crucial for tumor growth and metastasis, is significantly mediated by vascular endothelial growth factor (VEGF). This study aimed to investigate the expression of VEGF in EEC and its correlation with histopathological differentiation and LVI. Methods: This observational analytical study employed a cross-sectional design using 36 archival paraffin block samples of EEC diagnosed between January 2022 and December 2024 at the Anatomical Pathology Laboratory of Dr. M. Djamil General Hospital Padang. Cases were selected via simple random sampling from a population of 59. Histopathological grade (Grade 1, 2, or 3 based on FIGO architectural and nuclear criteria) and LVI (negative, focal, or substantial) were re-evaluated from Hematoxylin-Eosin (H&E) stained slides. VEGF expression was assessed by immunohistochemistry, scored semiquantitatively based on the percentage of positive tumor cells and staining intensity, and categorized as low or high. Data were analyzed using Chi-square tests, with p<0.05 considered statistically significant. Results: The mean age of patients was 54.36 years, with the highest prevalence in the 51-60 age group (41.7%). Grade 3 tumors were most common (38.9%), followed by Grade 2 (33.3%) and Grade 1 (27.8%). LVI was present in 47.2% of cases, predominantly focal (38.9%). High VEGF expression was observed in 58.3% of EEC cases. A statistically significant association was found between high VEGF expression and higher histopathological grade (p=0.000), with 66.7% of Grade 3 tumors showing high VEGF expression. No significant association was found between VEGF expression and LVI (p=0.080). Conclusion: High VEGF expression significantly correlated with higher histopathological grades in EEC, suggesting its role in tumor aggressiveness and dedifferentiation. However, a significant association with LVI was not established in this cohort. VEGF expression warrants further investigation as a potential prognostic biomarker and therapeutic target in EEC.

Abstract

Antenatal care (ANC) merupakan pelayanan untuk ibu hamil dalam upaya menjaga kesehatan ibu dan bayi dan mengurangi risiko komplikasi saat kehamilan dan persalinan dengan mendeteksi dini masalah dan risiko tinggi. Pada tahun 2022, cakupan K1 dan K4 ANC Puskesmas Ambacang menempati posisi yang cukup tinggi, namun terdapat tiga kematian ibu di wilayah kerja Puskesmas Ambacang. Efektivitas dan kualitas ANC di tidak hanya berdasarkan jumlah kunjungan, tetapi juga berbagai aspek, yaitu input, proses, dan output, serta kepuasan ibu hamil. Penelitian ini bertujuan untuk melihat gambaran pelaksanaan pelayanan ANC di Puskesmas Ambacang dengan meninjau aspek input, proses, output, dan kepuasan. Penelitian ini menggunakan metode kualitatif dengan perspektif studi kasus dan menghasilkan data deskriptif berupa gambaran dan kata-kata tertulis dari wawancara mendalam terhadap informan serta perilaku yang diobservasi, juga telaah dokumen. Informan yang diwawancara pada penelitian ini sebanyak 12 orang. Komponen input sudah sesuai standar dengan sumber daya manusia yang memadai, baik dari kualitas dan kuantitasnya. Dana pelayanan dari BOK, APBD, dan BLUD. Sarana dan prasarana sudah layak, namun terdapat kendala pada luas ruangannya. Terdapat SOP pelayanan yang mengacu pada kebijakan dan standar pelayanan minimal. Komponen proses sudah berjalan dengan baik dan melaksanakan pelayanan 10T, namun pencatatan kunjungan pelayanan belum maksimal. Komponen output berupa angka cakupan K1–K4 yang mengalami penurunan dan berada dibawah target nasional. Secara garis besar, ibu hamil sudah merasa sangat puas dengan aspek SERVQUAL pelayanan yang diberikan. Secara keseluruhan, pelayanan ANC di Puskesmas Ambacang berjalan dengan baik, namun perlu mengoptimalkan proses pencatatan angka kunjungan untuk meningkatkan kualitas pelayanan. --- Antenatal care (ANC) is a service provided to pregnant women to maintain the health of both the mother and the baby, and to reduce the risk of complications during pregnancy and childbirth by early detection of problems and high-risk factors. In 2022, the coverage of K1 and K4 ANC at Puskesmas Ambacang was relatively high, although there were three maternal deaths within its service area. The effectiveness and quality of ANC are not solely measured by the number of visits but also by various aspects, including input, process, output, and maternal satisfaction. This study aims to assess the implementation of ANC services at Puskesmas Ambacang by reviewing these aspects. The research adopts a qualitative approach with a case study perspective, producing descriptive data from in-depth interviews with 12 informants, observations, and document reviews. The input components meet the required standards, with adequate human resources in terms of both quality and quantity. Service funding is sourced from BOK, APBD, and BLUD. The facilities and infrastructure are adequate, though there are challenges related to the space available. Standard Operating Procedures (SOPs) are in place, aligned with policies and minimum service standards. The process component is functioning well, with the implementation of 10T services, although documentation of service visits has not been fully optimized. The output component, which refers to K1-K4 coverage, has declined and is below the national target. Overall, pregnant women express a high level of satisfaction with the SERVQUAL aspects of the services provided. In conclusion, ANC services at Puskesmas Ambacang are generally functioning well, but improvements are needed in optimizing the documentation and reporting of visit data to enhance service quality.

Abstract

Non-compliance with taking medication in DM patients can reduce a person's quality of life and become the main cause of a number of microvascular problems. Therefore, complex problems related to diabetes are one of the global health problems that must be addressed immediately. The purpose of this study was to determine the factors related to medication compliance in type 2 DM patients. This study used a mixed method study approach or a combination of two types of research, namely quantitative research and qualitative research. The type of mixed method research design used is explanatory sequential design. The sampling method in this study was by proportional sampling. Furthermore, to determine the target sample, a simple random block sampling technique was used, namely 347 DM patients. The results of the analysis showed no significant relationship between gender p-value = 1,000 (> 0.05), education p-value = 0.232 (>, 0.05), knowledge p-value = 0.076 (> 0.05), dxuration of suffering p-value = 0.162 (> 0.05), belief p-value = 0.680 (> 0.05), access to health services p-value = 0.602 (> 0.05), and there was a significant relationship between family support and DM treatment compliance with p-value = 0.020 (<0.05). The conclusion of this study is that more than half of DM patients have low treatment compliance, are female, have low education, have a duration of <5 years, have access to health that takes a long time, and lack of family support. More than half of DM patients have high knowledge and positive beliefs.

Abstract

Introduction : Preeclampsia is a hypertensive disorder of pregnancy that contributes to high maternal mortality rates. Kidney Injury Molecule-1 (KIM-1) has been identified as a potential biomarker for detecting kidney injury and is suspected to play a role in the pathogenesis of preeclampsia. Objective : This study to determine the differences in urinary KIM-1 levels among normal pregnancies, early-onset preeclampsia, and late-onset preeclampsia at Dr. M. Djamil General Hospital, Padang. Methods : Cross-sectional comparative study included 72 pregnant women, consisting of 24 with normal pregnancies, 24 with early-onset preeclampsia, and 24 with late-onset preeclampsia. Urinary KIM-1 levels were measured using the ELISA method at the Biomedical Laboratory, Faculty of Medicine, Andalas University. Results : The mean urinary KIM-1 levels in the normal pregnancy group were 2.834 ± 0.957 ng/mL, in the early-onset preeclampsia group were 2.659 ± 1.175 ng/mL, and in the late-onset preeclampsia group were 2.694 ± 0.978 ng/mL. Statistical analysis showed no significant difference in urinary KIM-1 levels among the three groups (p > 0.05). Conclusion : There was no significant difference in urinary KIM-1 levels between normal pregnancies, early-onset preeclampsia, and late-onset preeclampsia. Further studies with larger sample sizes and more specific methods are needed to explore the potential of KIM-1 as a preeclampsia biomarker.

Abstract

Background: Overactive bladder (OAB) is a syndrome characterized by unstable bladder contractions and significantly impacts the quality of life. Risk factors such as age, pregnancy, and childbirth are often associated with lower urinary tract symptoms. While national epidemiological data on OAB in Indonesia is unavailable, a small study at a public health center in Padang revealed a high prevalence of OAB among pregnant women (97.14%). This study aims to analyze the association between mode of delivery and the incidence of OAB. Method: This study employed a cross-sectional design conducted at community health centers in Padang. Data were collected through structured interviews and medical records. The inclusion criteria were women aged 20–45 years who had experienced at least one delivery. OAB was assessed using the Overactive Bladder Symptom Score (OABSS). Statistical analysis was performed to determine the association between delivery methods (vaginal or cesarean) and the incidence of OAB. Result: Out of the total participants, 65.4% reported OAB symptoms. The incidence was significantly higher in women with a history of vaginal delivery compared to those with cesarean delivery (p < 0.05). Furthermore, age and parity were found to be contributing factors to OAB symptoms. Discussion: The findings suggest that the mode of delivery is associated with the incidence of OAB, with vaginal delivery posing a higher risk. These results underscore the importance of counseling pregnant women about potential long-term urinary health outcomes associated with childbirth techniques. Further studies with larger samples and more diverse populations are recommended to validate these findings. Keywords: overactive bladder, vaginal delivery, caesarean section

Abstract

Latar Belakang: Anosmia adalah gejala khas pada rinitis akibat infeksi virus dan sering dilaporkan menonjol pada rinitis terkait COVID-19. Memahami perbedaan angka kejadiannya penting untuk penapisan klinis, keputusan isolasi, dan komunikasi risiko pada pasien. Tujuan: untuk melihat bagaimana anosmia pada rinitis virus dan rinitis COVID-19. Metode: penelitian ini merupakan suatu tinjauan naratif yang menggunakan metode PRISMA berdasarkan kriteria inklusi dan eksklusi. Hasil: anosmia yang disebabkan oleh rinitis COVID-19 memiliki angka kejadian yang lebih tinggi dibandingkan rinitis yang disebabkan oleh virus lain. Kesimpulan: angka kejadian anosmia lebih tinggi terjadi pada rinitis COVID-19 daripada rinitis virus. Anosmia dapat menjadi gejala alarm untuk mencurigai seseorang mengalami rinitis COVID-19.

Abstract

Background Adenoid cystic carcinoma is a malignant salivary gland tumor with unique features, including slow growth, progressive, poor prognosis, recurrence propensity, and perineural invasion tendency. SOX10 is a transcription factor expressed in the majority of tumors. SOX10 overexpression was hypothesized to play an important role in tumor-stroma interactions, especially perineural invasion and histopathological patterns.   Method A cross-sectional study was performed using 30 blocks of formalin-fixed embedded specimens previously diagnosed as salivary gland adenoid cystic carcinomas. Perineural invasion and histopathological patterns were evaluated followed by immunohistochemical staining to evaluate SOX10 expression. The staining intensity and proportion of positively stained cells in both tumor and stroma cells were grouped into high and low expression levels. Chi-square tests were used for statistical analysis.   Result The cribriform pattern was the most common histopathological pattern in both high and low SOX10 expression. The majority of tumors with high SOX10 expression (66.67% in stroma cells and 73.33% in tumor cells) were found to have more perineural invasion.   Conclusion There was a tendency for perineural invasion in tumors with high SOX10 expression, although this was not statistically significant. There was no significant association between SOX10 expression and histopathological pattern.

Abstract

Health education within schools plays a crucial role in enhancing children's health and establishing a safe and healthy learning environment. Despite this, the risk of illness in schools remains a concern, particularly for elementary school-aged children who are susceptible to sanitation-related health issues. The “Dokter Kecil” (or School Health Ambassador) program is designed to engage students in promoting the health of individuals, families, and their surroundings. This research aims to reintroduce the program in the Puskesmas Seberang Padang area, focusing on a promotive and preventive approach known as DINOS. The program's effectiveness was evaluated using pre-test and post-test questionnaires to assess the knowledge of the student participants. Based on the normality test of pretest and post-test score data using statistical tests, it is found that the Sig value (2-tailed) is 0.002 < 0.05. Therefore, it can be concluded that there is a significant difference between the results of the DINOS material exposure and simulation on pre-test and post-test data. Data analysis revealed a significant difference between the pre-test and post-test results, indicating the effectiveness of the material presentation and DINOS simulation. These results are expected to help better school health program implementation and more successfully address health issues for Indonesian school-age children, especially in the Puskesmas Seberang Padang area.
