Abstract

Abstract

General anesthesia is the standard technique for operation. Due to many complication of general anesthesia, thoracic spinal anesthesia (TSA) is widely used for several operation. Thoracic segmental spinal anesthesia has been proven safe and effective for a variety of laparoscopic surgeries. Thoracal spinal anesthesia is also proven usefully in maintaining hemodynamics and reducing side effects. Laparoscopic surgery is an alternative to the drainage of liver abscess in addition to laparotomy. A 58-year-old man with a diagnosis of posterior segment VI of liver abscess lead a laparoscopic surgery which became a laparotomy under thoracic spinal anaesthesia. Thoracic spinal anesthesia with a median approach at the level of 9-10 intervertebral space (T9-10) single shot using a combination regimen of Isobaric 5 mg and Hyperbaric 2.5 mg, Fentanyl 50 mcg, Dexmetomidine 10 mcg, and Ketamine 20 mg. During the operation there were no hemodynamic fluctuations. Postoperative monitoring found the patient's condition was stable, pain control was good, and there were no postoperative complications such as headache, nausea, and vomiting. Single dose thoracal spinal anesthesia (TSA) is one of the regional anesthetic techniques of choice for abdominal surgery, especially in high-risk patients. In this patient it was found that TSA was effective and safe even though the surgical technique used was different. Monitoring during and postoperatively showed good results without any complications and significant hemodynamic fluctuations.

Abstract

Disasters are a bad condition, especially when these conditions occur in densely populated areas or residential areas, of course this will result in damage to infrastructure, like buildings, houses and other facilities or communities. Of course, this can be of particular concern in the context of the loss or damage to buildings inhabited by the local community. Attention to post-disaster conditions, seen in the context of the availability of houses intended for disaster victims, is of course a major concern. Because the loss or damage to their buildings or homes will have an impact on losing their shelter in this condition. This research tries to develop this problem so that the availability of houses for them can be provided quickly with simple building and construction. The developed building model is very adaptive to the basic needs of the space needed for them to be able to live in the house with the characteristics of a building that is comfortable and safe for them to live in. The development of building field panels that have been developed is very effective and efficient in adapting to the use of lightweight structural systems and materials.

Abstract

This study aims to examine the effect of audit committee characteristics on CSRD in manufacturing and mining sector companies listed on the Indonesia Stock Exchange (IDX) in 2019. In addition, this study also examines whether there is an effect of risk management as a mediating variable in the relationship between audit committee characteristics and CSR disclosure. This research using purposive sampling method, which result in 178 samples. The analytical tool in this research using structural equation modeling (SEM)-PLS (Partial Least Square). The results show that the characteristics of the independent audit committee, number of member meetings, financial expertise and gender diversity have a positive effect on CSRD, while the size of the audit committee has no effect on CSRD. Indirect testing shows that risk management only affects the relationship between the number of member meetings and CSRD. Meanwhile, risk management failed to become a mediator for other variables with CSRD.

Abstract

Introduction: Laparoscopy is mostly performed under general anesthesia (GA) but laparoscopy using anesthesia such as thoracic spinal anesthesia (TSA) is mostly performed by some anesthesiologists and it is very useful when compared to GA. Method: This paper presents a case report of the use of TSA in healthy pediatric patients who administered anesthesia with TSA in the T10-T11 interspace, using 1 ml of hyperbaric Bupivacaine 5 mg/ml mixed with: 1 ml of Levobupivacaine isobaric 5 mg/ml, Fentanyl 50 μg, Ketamine 10 mg and Dexmedetomidine 10 μg mixed in 1 syringe. Results: During procedure, hemodynamically stable, no nausea, vomiting, or discomfort. Postoperative recovery process was very smooth, hemodynamically stable, no pain was reported or PDPH (Post Dural Puncture Headache) even though we used a 26G spinal needle. The use of TSA is considered very practical and more economical even though it is still carried out very carefully. Conclusion: This is only one single case report. TSA can be a better choice compare with general anesthesia. Stable hemodynamic during laparoscope and TSA can avoid systemic effect of general anesthesia like cognitive affect after general anesthesia, longer for recovery from anesthesia, nausea, vomiting, poor control pain and high cost.

Abstract

Cyberbullying is a known risk factor for mental health issues, demanding immediate attention. This study aims to detect cyberbullying on social media in alignment with the third sustainable development goal (SDG) for health and well-being. Many previous studies employ single-level classification, but this research introduces a multi-class multi-level (MCML) algorithm for a more detailed approach. The MCML approach incorporates two levels of classification: level one for cyberbullying or not cyberbullying, and level two for classifying cyberbullying by type. This study used a dataset of 47,000 tweets from Twitter with six class labels and employed an 80:20 training and testing data split. By integrating bidirectional encoder representations from transformers (BERT) and MCML at level two, we achieved a remarkable 99% accuracy, surpassing BERT-based single-level classification at 94%. In conclusion, the combination of MCML and BERT offers enhanced cyberbullying classification accuracy, contributing to the broader goal of promoting mental health and well-being.
