Abstract

Introduction: Uric acid is a metabolic byproduct in the human body that must remain within normal limits to prevent adverse health effects. Elevated uric acid levels, known as hyperuricemia, are a major risk factor for gout or gouty arthritis, a form of rheumatic disease affecting the joints. This condition can impact not only the elderly but also individuals of productive age. Objective : To increase public awareness about the dangers of elevated uric acid levels and promote healthy lifestyle practices, including dietary management and physical activity, as preventive strategies against gout and related complications. Method : This awareness program involved nutrition counseling sessions focusing on purine-restricted diets, risk factor education, and lifestyle modification. Counseling materials were supported by evidence from medical literature and adapted for community understanding. Physical activity promotion included introducing regular exercise and aerobic routines as part of the daily schedule. The target audience was the general adult population, including those at risk for hyperuricemia. Result : Participants demonstrated increased knowledge regarding uric acid management, dietary risk factors, and the importance of regular exercise. Many reported positive changes in food choices and incorporation of physical activity into daily routines. Anecdotal feedback indicated greater motivation to maintain healthy habits and seek further medical advice when needed. Conclusion: Hyperuricemia poses significant health risks, including gout and cardiovascular complications. Nutrition counseling, combined with promotion of regular physical activity, is an effective strategy to improve community awareness and encourage sustainable lifestyle changes.

Abstract

Postoperative cognitive dysfunction (POCD) is a clinical concern that can affect patients undergoing craniotomy, often impairing memory, attention, and executive function. Despite advances in neurosurgical and anesthetic techniques, POCD remains prevalent and under-recognized. This review aims to explore the underlying mechanisms and contributing factors of POCD in the context of craniotomy under general anesthesia. A narrative literature review method was employed to examine publications retrieved from major databases including PubMed, Scopus, and ScienceDirect between 2015 and 2025. Selected studies focused on adult patients and provided insights into both clinical manifestations and molecular pathways linked to POCD. The analysis revealed that factors such as advanced age, duration of anesthesia, and neuroinflammatory responses significantly influence the development of POCD. Mitochondrial dysfunction and oxidative stress were frequently cited as key contributors to neuronal damage following surgery. In terms of clinical management, the literature suggests that early cognitive assessment and tailored anesthetic strategies may reduce the risk of long-term impairment. Although definitive treatment remains elusive, this review highlights the importance of early identification and multidisciplinary approaches to mitigate POCD in post-craniotomy patients. Future research should prioritize standardized diagnostic criteria and explore neuroprotective interventions to improve cognitive outcomes following neurosurgical procedures.

Abstract

Spinal anesthesia is a method of anesthesia by injecting local anesthetic drugs into the subarachnoid space in the lumbar region. The Bromage Score is used to assess the recovery of motor function in the lower extremities and to determine the patient's readiness to be transferred from the recovery room to the inpatient ward. The recovery level of motor function is influenced by various factors, including the patient's physical status assessed through the American Society of Anesthesiologists (ASA) classification. This study aims to determine the relationship between ASA physical status and the time to achieve the Bromage Score in post-spinal anesthesia patients at RSUD dr. R. Goeteng Taroenadibrata Purbalingga. This study used a survey research design with a cross-sectional approach, where each subject was observed once, and measurements were conducted simultaneously. The study results showed that the patient age distribution was 22 people (38.5%), gender distribution was 39 people (68.4%), Body Mass Index (BMI) was 31 people (54.4%), and drug type usage was 57 people (100%). Patients with ASA II physical status numbered 35 people (61.4%), and the time to achieve a Bromage Score < 4 hours was found in 32 people (56.1%). The conclusion is that there is a significant relationship between the physical status of the American Society of Anesthesiologists (ASA) and the time to achieve the Bromage Score in post-spinal anesthesia patients at RSUD dr. R. Goeteng Taroenadibrata Purbalingga, with a p-value of 0.001 (p < 0.05).

Abstract

Post-mastectomy pain in breast cancer patients is a clinical problem that often has long-term effects, including the development of post-mastectomy pain syndrome (PMPS). The use of single opioids has limitations due to the risk of side effects and dependence, so intravenous multimodal analgesia approaches with various combinations of analgesic agents are increasingly being studied. This study aims to evaluate the effectiveness of multimodal intravenous analgesic combinations in controlling postoperative pain in breast cancer patients undergoing mastectomy with general anesthesia. A systematic review was conducted according to the PRISMA 2020 guidelines through searches in PubMed, Cochrane Library, ScienceDirect, and EBSCO for publications from 2005 to 2025. Articles that met the inclusion criteria were randomized controlled trials (RCTs) and cohort studies that assessed the effectiveness of multimodal intravenous analgesic combinations in mastectomy patients. The methodological quality was assessed using the Joanna Briggs Institute (JBI) instrument. A total of 11 studies involving more than 800 mastectomy patients were analyzed. The combination of pregabalin and esketamine was shown to significantly reduce the incidence of chronic pain up to 6 months post-surgery (14.3% and 7.1% vs. 46.3% and 31.7%; p<0.01), as well as reduce opioid consumption. Remifentanil–dexmedetomidine provided better analgesia while improving the patient's immunological profile. Ketamine-based anesthesia was more effective than opioids in reducing acute pain, while the combination of ketamine–magnesium sulfate was proven to be safe and opioid-sparing. The use of diclofenac reduced opioid consumption but increased the risk of bleeding, while intravenous lidocaine showed inconsistent results. The combination of multimodal intravenous analgesics is effective in optimizing pain control after mastectomy, reducing opioid consumption, and potentially preventing chronic pain. Adjusting the multimodal protocol according to the patient's profile is necessary to maximize benefits while minimizing the risk of side effects.

Abstract

Latar belakang: Tersedak atau choking merupakan situasi darurat yang perlu segera ditangani, penundaan pada penanganan ini dapat berdampak terhadap kematian karena tersedak mengakibatkan penyumbatan yang dapat menghalangi pernafasan di paru-paru. Seorang remaja diharapkan dapat melakukan pertolongan pertama khususnya pada seseorang yang mengalami tersedak dikarenakan penanganan tersedak merupakan tindakan yang harus segera dilakukan. Tujuan: mengetahui tingkat pengetahuan dan keterampilan sebelum dan setelah edukasi penanganan kegawatdarutan tersedak di SMK Kesatrian Purwokerto. Metode: metode yang digunakan dalam pengabdian kepada masyarakat yaitu dengan studi deskriptif pada siswa SMK Kesehatan untuk mengetahui tingkat pengetahuan dan keterampilan penanganan tersedak sebelum dan sesudah edukasi. Responden dalam pengabdian kepada masyarakat sebanyak 30 pasien. Alar ukur yang digunakan dengan menggunakan kuesioner pengetahuan dan keterampilan penanganan tersedak. Hasil: Karakteristik peserta berdasarkan usia yang dominan pada umur 17 tahun sebanyak 14 siswa (46,7%). Jenis kelamin yang paling dominan adalah perempuan sebanyak 21 siswa (70%). Tingkat pengetahuan pretest dan posttest pelatihan penanganan kegawatdarutan tersedak di SMK Kesatrian Purwokerto diperoleh hasil rata-rata skor pretest 7,3 dengan nilai minimal 5 dan nilai maksimal 9. Hasil post test mengalami peningkatan menjadi rata-rata 8,93 dengan skor minimal 7 dan skor maksimal 11. Keterampilan penanganan tersedak setalah dilakukan edukasi paling dominan dengan kategori baik (70%).

Abstract

ABSTRACT Background: Guava (Psidium guajava Linn) is a medicinal plant that is easy to find and can be grown in almost all areas. The content of active compounds in guava leaves, among others, are flavonoids and tannins that can reduce the incidence of infection in both acute and chronic wounds. Objective: This study aims to determine the effectiveness of P'Zalm Care Wound Cleanser Spray on decreasing the population of Gram Positive and Negative Bacteria. Methods: This study uses an experimental method with a pre-post test design approach with control. The sample was divided into an intervention group and a control group. The intervention group received P'zalm Care Wound Cleanser Spray without dilution and the control group, namely P'zalm Care Wound Cleanser Spray at a dilution of 1: 10, with an evaluation looking at the initial bacterial population and 30 seconds after being given the intervention then comparing the 3 types of Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli. Data collection is carried out after passing the ethical test. Results: The results showed that there was a significant effect of giving P'zalm Care Wound Cleanser Spray in the experimental group and the control group on gram-positive and gram-negative bacteria with a decrease in bacterial colonies of Log 6 log redox 99.9%. Conclusion: This research is guava leaf infusion has an antibacterial effect against Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli in Vitro with an effective concentration close to 100%. The increasing concentration of guava leaf infusion showed the larger the diameter of the growth inhibition zone of Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli. Abstract

Indonesian Workers were undergoing 18% of hemodyalysis from the number of hemodyalysis patients. They did not know about CRF. The study aimed to analyze the behavior of migrant workers who were suffering hemodyalysis. The study used a qualitative design with a phenomenological approach, that is carried out on former migrant workers who suffer from chronic kidney failure. Indepth interview used as data collection. While data validation used triangulation and confirmability. Data analysis techniques was done by reducing and interpreting the data. The study show migrant workers behavior before becoming migrant workers were very pathetic. They often drunk kind of energy drinks. Further, following the lifestyles of migrant countries such as drinking alcohol, soft drinks, fast food becoming their routine activities as lack of drinking water, vegetables, fruit, and also rarely did exercises. Migran workers activities influenced by the work system and individual interests to get excess income, then they chose to work overtime as their priority. In conclusion, the problem of migrant workers' behavior and hypertensive hereditary disease has an impact on decreasing kidney function. Migrant workers have no knowledge of healthy lifestyles and CRF. Wrong perceptions and attitudes of migrant workers to support culture in migrant countries.

Abstract

Introduction: Surgery and anesthesia are often factors that trigger anxiety in preoperative patients. Pre-anesthesia anxiety can affect blood pressure, heart rate, and anesthesia stability during surgery. This condition affects physiological responses through activation of the amygdala and sympathetic nervous system, which reduces patient comfort. Anxiety can be managed through non-pharmacological therapies such as Surah Ar-Rahman recitation therapy, which can be used to reduce patient anxiety levels. Objective: This activity aims to apply Surah Ar-Rahman recitation therapy as a non-pharmacological intervention to reduce preoperative anxiety and increase patient comfort. Method: The implementation method involved 35 preoperative patients aged 18–75 years who were Muslim and experienced mild to moderate anxiety. Surah Ar-Rahman verses 1–78 were recited for 15 minutes using a smartphone and headphones in a quiet room. Anxiety levels were measured using the Amsterdam Preoperative Anxiety and Information Scale (APAIS) before and after therapy, and reevaluated 15 minutes before surgery. Data were analyzed descriptively by comparing the percentage change in anxiety levels before and after the intervention. Result: The results showed an increase in the proportion of patients with mild anxiety from 40% to 57.1% and a decrease in moderate anxiety from 60% to 42.9% after therapy. The average anxiety score also decreased from 15.2 to 11.8 points. Murottal therapy proved to be effective in providing relaxation, safe without causing side effects, and supporting the spiritual and emotional comfort of patients. Conclusion: In conclusion, education and murottal therapy of Surah Ar-Rahman can be a beneficial complementary intervention in preoperative anxiety management, while strengthening holistic and humanistic nursing care.

Abstract

Hypotension is one of the most common complications following spinal anesthesia, particularly in patients undergoing cesarean section. If not properly managed, it can lead to serious consequences such as nausea, vomiting, loss of consciousness, cardiovascular collapse, and decreased uteroplacental blood flow, which may endanger both the mother and fetus. Therefore, preventive measures are essential to ensure patient safety during surgery. This study aimed to compare the effectiveness of colloid fluid administration and intravenous ephedrine therapy in preventing hypotension after spinal anesthesia in cesarean section patients. This was a quantitative comparative study using a two-group pretest-posttest design, involving 80 respondents at RSUD Ajibarang. The subjects were divided into two intervention groups: one receiving colloid fluids and the other receiving intravenous ephedrine. Blood pressure measurements were taken before and after the interventions, and data were analyzed using paired sample t-tests and independent t-tests to assess intra- and inter-group differences.The results showed that both interventions were able to increase blood pressure; however, intravenous ephedrine administration was more effective in maintaining hemodynamic stability. A statistically significant difference was found in post-intervention measurements between the two groups (p = 0.046), with the ephedrine group demonstrating a higher mean arterial pressure compared to the colloid fluid group. These findings suggest that intravenous ephedrine is superior to colloid fluid administration in preventing post-spinal anesthesia hypotension during cesarean section. Ephedrine can therefore be considered a preferred intervention for improving maternal safety and reducing the risk of complications during cesarean procedures.

Abstract

Background: Spinal anesthesia is a commonly used technique for cesarean section procedures. However, this technique carries a risk of complications, one of which is post-dural puncture headache (PDPH). PDPH can be caused by several factors, including multiple attempts at spinal needle insertion. Objective: To determine the relationship between the number of spinal anesthesia punctures greater than once and the incidence of PDPH in cesarean section patients at RSUD Ajibarang. Methods: This study used a quantitative design with a cross-sectional approach. A total of 80 cesarean section patients who underwent spinal anesthesia were selected using purposive sampling. Data were collected through observation and interviews and analyzed using the Spearman correlation test. Results: A total of 96.3% of patients experienced two punctures, while 3.8% experienced three punctures. Most patients did not experience PDPH (90.0%), while 8.8% reported moderate headache and 1.3% reported severe headache. The analysis showed a significant relationship between the number of spinal punctures and the incidence of PDPH (p=0.001; r=0.367). Conclusion: There is a significant correlation between the number of spinal anesthesia attempts and the incidence of PDPH among cesarean section patients at RSUD Ajibarang. The greater the number of spinal punctures, the higher the risk of developing PDPH.
