Abstract

Abstract Background: Surgery and anesthesia can affect the immune and neuroendocrine systems which can cause the release of hormones and cytokines as a result of changes in the body's physiological responses. Neuroendocrine pathways to the immune system related to stress responses due to surgery. surgery and a decrease in the number of T lymphocytes causes a shift in the balance between immunosuppressive regulatory T lymphocytes and immune-triggering T helper and cytotoxic T cells in a predominance of regulatory T cells. Apart from causing a decrease in the number of T lymphocytes, it also causes suppression of NK cells and an increase in the number of neutrophils. Research purposes determine the effect of various anesthetic techniques on changes in leukocyte count levels. Method: This research is a type of experimental research with a pre-pos test group design approach. The samples for this study were taken from patients undergoing surgery and anesthesia with inclusion criteria: age 17-65 years, ASA 1-2, no infectious diseases. The sample size is the sampling technique is Consecutive sampling. Sample measurements were taken from venous blood before and after surgery and anesthesia. The parameters measured are leukocyte counts which include leukocytes, basophils, neutrophils, monocytes and lymphocytes. Blood samples were taken before surgery and anesthesia, and 2 hours after surgery and anesthesia using a hematology analyzer. The analytical tests used are the parametric dependent test (paired T-test) or the non-parametric Wilcoxon test and the independent test. Results: There is a significant difference in the count of leukocyte types between general anesthesia and regional anesthesia Conclusion: General anesthesia can increase leukocyte counts significantly compared to regional anesthesia, but eosinophils are not significant in both groups

Abstract

Background :Smoking is one of the habits that is inherent in humans because it is addictive. Active smokers in Indonesia have increased significantly in 10 years. Smoking has a negative impact on health, one of which is changes in salivary flow rate. Changes in salivary flow rate in smokers can cause complications in the airway when performing surgery using inhalation general anaesthesia. The purpose of this study was to determine the relationship between smoking status and salivary flow rate in patients with inhalation general anaesthesia. Method : This type of research is observational analytic with a cross sectional approach. The sample in the study was 35 respondents with 4 inclusion criteria. The sample in this study was taken with purporsive sampling technique. Data collection stimulated saliva by suction method. Data analysis using univariate and bivariate analysis, namely the Chi-Square test. Results : The results showed that inhalation general anaesthesia patients were more smokers with 24 respondents (68.6%) and respondents who experienced changes in salivary flow rate obtained the most respondents who experienced hypersaliva as many as 15 respondents (42.9%). The results of bivariate analysis using the Chi-Square test showed that 11 respondents (31.4%) who smoked experienced hypersaliva with a p-value of 0.000 <0.05. Conclusion : There is a relationship between smoking status and salivary flow rate in inhalation anaesthesia patients, namely the increase in salivary flow rate in smoking respondents with inhalation general anaesthesia.

Abstract

Nyeri tenggorokan pascaoperasi (POST) merupakan komplikasi umum pada 18-65% pasien pasca-intubasi endotrakeal tube (ETT), dengan faktor risiko kritis berupa ukuran ETT, durasi intubasi, dan jenis kelamin perempuan. Studi cross-sectional ini (n=70) di RSUD Kardinah Tegal (Maret-Mei 2024) bertujuan menganalisis hubungan ETT-POST dan mengidentifikasi prediktor dominan melalui consecutive sampling, pengukuran skala Visual Analog Scale (VAS), dan analisis regresi logistik ordinal. Hasil menunjukkan hubungan signifikan ETT-POST (p=0,003; λ=0,400), dengan 64,3% pasien ETT mengalami POST vs. 0% pada kelompok laryngeal mask airway (LMA). Pasien perempuan dengan ETT ≥7,0mm berisiko 7,2 kali lebih tinggi mengalami POST berat (OR 7,2; 95% CI 2,1-24,3), sementara durasi intubasi <60 menit meningkatkan risiko POST 3,8 kali (OR 3,8; 95% CI 1,5-9,6). Disimpulkan bahwa reduksi ukuran ETT menjadi ≤6,5mm untuk pasien perempuan dan optimalisasi teknik intubasi merupakan strategi pencegahan prioritas di RSUD Kardinah Tegal.   Postoperative sore throat (POST) is a common complication in 18-65% of patients after endotracheal tube (ETT) intubation, with critical risk factors being ETT size, intubation duration, and female gender. This cross-sectional study (n=70) at Kardinah Hospital, Tegal (March-May 2024) aimed to analyze the relationship between ETT and POST and identify dominant predictors through consecutive sampling, Visual Analog Scale (VAS) measurements, and ordinal logistic regression analysis. The results showed a significant association between ETT and POST (p=0.003; λ=0.400), with 64.3% of ETT patients experiencing POST compared to 0% in the laryngeal mask airway (LMA) group. Female patients with an ETT ≥7.0 mm had a 7.2-fold higher risk of severe POST (OR 7.2; 95% CI 2.1-24.3), while intubation duration <60 minutes increased the risk of POST 3.8-fold (OR 3.8; 95% CI 1.5-9.6). It was concluded that reducing the ETT size to ≤6.5 mm for female patients and optimizing intubation technique are priority prevention strategies in regional hospitals.

Abstract

Latar belakang: Pasien lanjut usia (lansia) paska operasi patah tulang pinggul atau Total Hip Arthroplasty (THA) memiliki permasalahan yang menyebabkan proses penyembuhannya menjadi terganggu, seperti dislokasi pinggul yang pada umumnya terjadi dalam kurun tiga bulan pertama setelah operasi THA. Tujuan: untuk mengetahui kualitas hidup lansia yang mengalami dislokasi paska operasi patah tulang pinggul. Metode: scoping review menggunakan Preferred Reportung Items for Systematic review and Meta-Analyses (PRISMA Flowchart), dengan kriteria inclusi (1) Artikel dari tahun 2017-2024, (2) metode Randomize control trial (RCT), (3) Artikel tentang dislokasi dan kualitas hidup lansia operasi patah tulang pinggul. Hasil: Pencarian literatur telah mengidentifikasi 1534 artikel, 198 duplikat artikel yang dikeluarkan dan didapatkan 10 artikel RCT yang memenuhi kriteria untuk direview. Ditemukan dua tema utama yaitu; (1) Risiko dislokasi setelah operasi patah tulang pinggul Lansia; a) penyebab terjadinya dislokasi patah tulang pinggul setelah operasi; b) dislokasi berulang; 3) kualitas hidup pasien pasca operasi patah tulang pinggul. (2) Faktor yang terkait dengan pemulihan fungsional dislokasi patah tulang pinggul lansia; a) meningkatkan mobilitas; b) Terapi hemiarthoplasty; c) Pendekatan anterior mengurangi dislokasi dibandingkan pendekatan posterior. Kesimpulan: Dislokasi paska operasi patah tulang pinggul menyebabkan perburukan qualitas hidup lansia. Program pencegahan dislokasi paska operasi patah tulang pinggul menjadi hal yang penting dalam proses rehabilitasi pasien.

Abstract

Postoperative complications following general anesthesia, especially those affecting the respiratory system, present a significant challenge in healthcare services, as they can lead to oxygen desaturation and hypoxemia, ultimately impairing vital organ function. One intervention to improve oxygen saturation is the balloon blowing technique. This community service activity aimed to provide education on balloon blowing breathing relaxation techniques to enhance oxygen saturation in postoperative patients under general anesthesia. The method included a 5-minute educational session and a 3–5 minute demonstration of the balloon blowing technique. Measurements of knowledge levels and oxygen saturation were conducted before and after the intervention. The results showed a significant improvement: prior to the education, 70% of participants (21 individuals) experienced decreased oxygen saturation, while after the intervention, all participants (100%) showed increased oxygen saturation. Knowledge levels also improved, with 70% of participants initially in the low category, increasing to 100% in the good category after the education.

Abstract

Choking is a medical emergency caused by a foreign object obstructing the airway, potentially leading to hypoxia or even death if not treated promptly. One recommended emergency intervention is the back blow technique, which involves applying forceful blows to the victim’s back to dislodge the obstruction. This community service activity aimed to increase public knowledge and skills in handling choking emergencies using the back blow technique. The method consisted of health education through lectures and live demonstrations, followed by pre- and post-tests to assess participants' knowledge and skills using structured questionnaires and skill checklists. The results indicated significant improvement. Prior to the intervention, 43% of participants had low knowledge and 33.3% had moderate knowledge. Post-intervention, all participants (100%) showed improved knowledge. In terms of skills, 73.3% were categorized as having low skills before training, while after demonstration, all participants (100%) reached a good skill level. The outputs of this program included an educational pocketbook and a journal publication.

Abstract

ABSTRACT Consciousness is the awakening from the effects of anesthetic drugs after the anesthesia process and completion of surgery. Several factors that affect conscious recovery are the effects of anesthetic drugs, age, weight, duration of surgery, physical status and acid-base or electrolyte disorders. This study aims to determine the factors that affect conscious recovery after inhalation anesthesia desflurane and sevoflurane at Majenang Hospital. This study uses a quantitative method that is descriptive analysis with a Cross-sectional approach and two tests, namely the chi-square and mann-whitney tests. The sample in this study were all adult patients who underwent general anesthesia at IBS Majenang Hospital during the study period. The research data showed that the characteristics of the respondents were mostly adults 26-35 years old and the elderly aged 56-66 years old as many as 14.67% for desflurane, the largest female gender with desflurane use as much as 36%, BMI based on ideal body weight 18.5-24.9 kg / m2 with sevoflurane use as much as 38.67%. Furthermore, with a fast operation duration of <1 hour in the use of desflurane as much as 49.33% and a fast operation duration of <1 hour in the use of sevoflurane as much as 36%, the type of general anesthesia in respondents who recovered consciousness after desflurane inhalation anesthesia was 53.33% and sevoflurane as much as 46.67%, the Recovery Time of Consciousness was a fast recovery time of <15 minutes as much as 40% and in the use of sevoflurane with a fast recovery time of <15 minutes as much as 28%, The dose of inhalation anesthetic drugs used was 22 for desflurane and 36 for sevoflurane. After that, there was no effect of age, BMI, gender, type of inhalation agent and dose of anesthetic drugs on recovery of consciousness. The next result showed that there was an effect of the duration of surgery on recovery of consciousness after desflurane and sevoflurane inhalation anesthesia.  Keywords: General Anesthesia, Sevoflurane and Desflurane, Recovery of Consciousness  ABSTRAK  Pulih sadar merupakan bangun dari efek obat anestesi setelah dilakukan proses pembiusan dan selesai tindakan pembedahan. Beberapa faktor yang mempengaruhi pulih sadar adalah efek obat anestesi, usia, berat badan, durasi operasi, status fisik dan gangguan asam basa atau elektrolit. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang memengaruhi pulih sadar pasca anestesi inhalasi desfluran dan sevofluran di RSUD Majenang. Penelitian ini menggunakan metode kuantitatif yang bersifat deskripsi analisis dengan pendekatan Cross sectional dan dua uji yaitu uji chi square dan mann whitney. Sampel dalam penelitian ini adalah seluruh pasien dewasa yang menjalani tindakan general anestesi di IBS RSUD Majenang selama periode penelitian. Data hasil penelitian menunjukan bahwa karakteristik responden mayoritas dewasa 26-35 tahun dan lansia akhir 56-66 tahun sebanyak 14,67%untuk desfluran, jenis kelamin perempuan terbanyak dengan pemakaian desfluran sebanyak 36%, IMT berdasarkan berat badan ideal 18,5-24,9 kg/m2 dengan penggunaan sevofluran sebanyak 38,67%. Selanjutnya dengan durasi operasi cepat <1 jam pada penggunaan desfluran sebanyak 49,33% dan durasi operasi cepat <1 jam pada penggunaan sevofluran sebanyak 36%, jenis general anestesi pada responden pulih sadar pasca anestesi inhalasi desfluran sebanyak 53,33% dan sevofluran sebanyak 46,67%, waktu Pulih Sadar waktu pulih sadar cepat <15 menit sebanyak 40% dan pada penggunaan sevofluran dengan waktu pulih sadar cepat <15 menit sebanyak 28%, penggunaan dosis obat anestesi inhalasi sebanyak 22 untuk desfluran dan 36 untuk sevofluran. Setelah itu tidak adanya pengaruh usia, IMT, jenis kelamin, jenis agen inhalasi dan dosis obat anestesi terhadap pulih sadar. Hasil selanjutnya menunjukan bahwa ada pengaruh durasi operasi terhadap pulih sadar pasca anestesi inhalasi desfluran dan sevofluran.  Kata Kunci: General Anestesi, Sevofluran dan Desfluran, Pulih Sadar

Abstract

Introduction: Hypertension is a noncommunicable disease with a high prevalence among the elderly and can increase the risk of morbidity and mortality. This condition is a major concern because it can seriously affect the quality of life of the elderly and increase the risk of cardiovascular complications. Nonpharmacological therapy with slow deep breathing can improve blood flow in hypertensive patients with impaired blood flow, thereby lowering blood pressure. Objective: This study aims to improve the knowledge and skills of older adults in performing SDB therapy to control blood pressure. Method: The method used in this activity was education through lectures, demonstrations, and SDB training with a one group pretest-posttest design. A total of 31 elderly people participated in this activity. Result: The results showed a decrease in average systolic blood pressure before and after the intervention from 150.74 mmHg to 147.45 mmHg and diastolic blood pressure from 89.23 mmHg to 84.81 mmHg. In addition, most participants showed an improvement in their ability to perform slow deep breathing correctly. Conclusion: Education and training in slow deep breathing have been shown to have a positive impact on lowering blood pressure and increasing the independence of older adults in managing hypertension.

Abstract

Introduction: Asthma is a chronic respiratory disease characterized by airway inflammation and narrowing, which can lead to symptoms such as shortness of breath, coughing, and wheezing. Many people still lack adequate knowledge about asthma management and prevention. Method: This community service program was conducted at GOR Satria Purwokerto and involved elderly community members. The activity included free health checks (BMI, blood pressure, SpO₂, blood glucose), educational sessions on asthma (causes, symptoms, prevention, and proper inhaler technique), and a Q&A discussion. Result: The counseling activity positively contributed to improving community knowledge and awareness regarding asthma, especially among elderly participants. The interactive education session enhanced understanding through direct communication and participation. Conclusion: This activity demonstrated the importance of health education and preventive efforts for chronic diseases like asthma. Challenges such as limited health literacy were noted, but overall, the activity strengthened promotive and preventive roles of health students in the community.

Abstract

Introduction: Uric acid is a metabolic byproduct in the human body that must remain within normal limits to prevent adverse health effects. Elevated uric acid levels, known as hyperuricemia, are a major risk factor for gout or gouty arthritis, a form of rheumatic disease affecting the joints. This condition can impact not only the elderly but also individuals of productive age. Objective : To increase public awareness about the dangers of elevated uric acid levels and promote healthy lifestyle practices, including dietary management and physical activity, as preventive strategies against gout and related complications. Method : This awareness program involved nutrition counseling sessions focusing on purine-restricted diets, risk factor education, and lifestyle modification. Counseling materials were supported by evidence from medical literature and adapted for community understanding. Physical activity promotion included introducing regular exercise and aerobic routines as part of the daily schedule. The target audience was the general adult population, including those at risk for hyperuricemia. Result : Participants demonstrated increased knowledge regarding uric acid management, dietary risk factors, and the importance of regular exercise. Many reported positive changes in food choices and incorporation of physical activity into daily routines. Anecdotal feedback indicated greater motivation to maintain healthy habits and seek further medical advice when needed. Conclusion: Hyperuricemia poses significant health risks, including gout and cardiovascular complications. Nutrition counseling, combined with promotion of regular physical activity, is an effective strategy to improve community awareness and encourage sustainable lifestyle changes.
