Abstract

Jenis cairan asites transudat atau eksudat perlu dibedakan sebagai tahap awal untuk mengetahui penyebab penyakit yang mendasariasites. Penggabungan beberapa tolok ukur memiliki kepekaan dan kekhasan yang baik dalam membedakan jenis cairan asites. Penelitianini bertujuan untuk mengetahui kesesuaian antara patokan Light dan Serum Ascites Albumin Gradient (SAAG) dalam membedakantransudat dan eksudat pada cairan asites. Penelitian ini merupakan penelitian analitik dengan desain potong lintang terhadap 24 pasienasites di RSUP Dr. M. Djamil Padang, mulai bulan Maret sampai September 2016. Tolok ukur yang diperiksa adalah protein jumlahkeseluruhan (metode kolorimetrik biuret), albumin (metode kolorimetrik bromocresol green), serta laktat dehidrogenase (LDH) (metodeenzimatik). Hasil pemeriksaan setiap tolok ukur dirumuskan kedalam patokan Light dan SAAG. Kesesuaian patokan Light dan SAAGdalam membedakan transudat dan eksudat cairan asites ditentukan dengan uji kappa. Hasil dianggap bermakna secara statistik jikap<0,05. Ciri subjek penelitian ini adalah laki-laki sebanyak 54,2% dan perempuan 45,8% dengan rentang umur 22–76 tahun. PatokanLight dapat menentukan 9 eksudat dan 15 transudat, sedangkan menggunakan SAAG dapat menentukan 2 eksudat dan 22 transudat.Kesesuaian patokan Light dengan SAAG menggunakan uji kappa adalah cukup (nilai kappa=0,26) dan tidak bermakna secara statistik(p>0,05). Hasil penelitian ini menyimpulkan tidak terdapat kesesuaian antara patokan Light dan SAAG dalam membedakan transudatdan eksudat pada cairan asites. Penelitian dalam jumlah besar perlu dilakukan untuk menentukan kepekaan dan kekhasan keduapemeriksaan.

Abstract

Asidosis metabolik adalah kasus yang paling sering ditemukan pada pasien critically ill. Pendekatan Henderson-Hasselbalch gagalmenjelaskan gangguan metabolik yang rumit karena hanya tertuju pada kenasaban pH dengan tekanan parsial karbondioksida dan ionbikarbonat. Pendekatan keseimbangan asam-basa metode Stewart lebih akurat dan dapat menggambarkan gangguan metabolik yangrumit. Modifikasi metode Stewart yang digunakan saat ini adalah metode Figge-Stewart dan Fencl-Stewart yang disederhanakan, dapatdigunakan di tempat dengan sumber daya terbatas. Tujuan penelitian ini adalah mengetahui kesesuaian metode Fencl-Stewart yangdisederhanakan dengan Figge-Stewart dalam mendiagnosis asidosis metabolik di pasien critically ill. Penelitian analitik potong lintangterhadap 40 pasien critically ill yang dirawat di ICU, CVCU dan HCU RSUP Dr. M. Djamil Padang masa waktu Oktober–November 2015.Analisis gas darah diukur dengan potensiometri, elektrolit dengan ion selective electrode dan kadar albumin dengan immunoturbidimetri.Hasil dianalisis dengan program komputer. Kesesuaian metode Fencl-Stewart yang disederhanakan dengan Figge-Stewart dianalisisdengan uji Kappa, bermakna jika nilai p<0,05. Sebanyak 40 orang pasien (18 laki-laki, 22 perempuan) diikutkan dalam penelitian ini.Rerata umur pasien adalah 48,35(18,4) tahun dan diagnosis terbanyak adalah gagal jantung (30%). Hiponatremia, hipernatremia,hipokalemia, hiperkalemia, hipokloremia dan hipoalbuminemia ditemukan masing-masing sebanyak 37,5%, 12,5%, 2,5%, 12,5%,17,5%, 20% dan 87,5% pasien. Terdapat kesesuaian yang baik antara metode Fencl-Stewart yang disederhanakan dengan Figge-Stewartdengan nilai kappa=0,529 dan bermakna secara statistik (p<0,001). Kesesuaian metode Fencl-Stewart yang disederhanakan denganFigge-Stewart dalam mendiagnosis asidosis metabolik di pasien critically ill adalah baik.

Abstract

Metabolic acidosis is the most frequent acid-base disorder in patients of the Intensive Care Unit. By conventional approach based onpH value, [HCO3–], and base deficit (BD) from blood gas analyzer (BGA) measurement are often inappropriate with the clinical stateand inadequate in explaining the mechanism of the metabolic acidosis. The Stewart approach states that the blood pH is determinedby a strong ion difference (SID), the carbon dioxide tension (pCO2), the total concentration of non-volatile weak acid. The Stewartapproach may give a better understanding of the mechanisms that underlie the metabolic acidosis. The purpose of this study is to knowthe correlation of blood pH value measurement from BGA and calculation based on Stewart approach and identifying the mechanismsthat underlie a metabolic acidosis. In this study an analytic observational cross-sectional method was used. The examined subjectsconsisted of 71 patients who were admitted with a metabolic acidosis at the ICU from July up to August 2007. All patients were measuredfor their blood pH, pCO2, [HCO3–], BD, sodium, potassium, calcium, magnesium, chloride, lactate, albumin, and phosphate. The resultwas reported as the mean and standard deviation. The data were analyzed by Pearson’s correlation test and linier multiple regression.Statistical significance was determined at p < 0.05. The mean values of blood pH measurement from BGA and blood pH calculationbased on the Stewart approach were 7.33 (0.11) and 7.49 (0.11) (r = 0.681; p < 0.001). Most patients had two underlying mechanisms ofmetabolic acidosis. Hyperlactatemia was present in 61.8%, hyperchloremia was present in 58.2% of patients. Based on this study so far,by using the Stewart approach there is an excellent and significant correlation between the blood pH measurement from BGA and bloodpH calculation. Hyperlactatemia and hyperchloremia are the main causes of the metabolic acidosis in patients of the ICU ward.

Abstract

HIV diagnosis is confirmed by viral culture, but this process takes a long time. Another method used to detect HIV-specific antigensor antibodies is by immunoassay. Generally, antigen-antibody based methods are used as a screening test. Based on the stability ofthe sequences found in the first (1) study year, the researchers designed this study for the production of p24 recombinant protein.These proteins will be developed as diagnostic markers based on sero-immunology technique. The aim of this study was to know theconstruction and over expression of protein p24gag from local isolates and analysis of the diagnostic potential of doing design specificprimers against p24gag protein, cloning and over expression of the gene, as well as to obtain a p24 protein that has been purified.This research results will be applied later to develop a method based on local isolates of HIV diagnosis. This research was a descriptivestudy, conducted over seven (7) months in the Biomedical Laboratory of the Faculty of Medicine, Andalas University and Departmentof Clinical Pathology, Dr. M. Djamil Hospital, Padang. This study was carried out by using samples of local isolates originating fromthe first year of research. Stages of the research were: 1) the design of primers for cloning, amplification and sequencing, 2) cloninginto pDEST and pENT, 3) transformation of the target gene, 4) detection of fragment insertions, 5) protein expression and proteinanalysis by SDS-PAGE, immunoblotting and 6) purification. The conclusions of this study were: the design of specific primers againstp24gag protein used fragments attb1, attb2, Shine Delgano and Kozac effective for protein expression. The results showed that thepresence of protein 24kDa expression was identical to HIV p24gag protein. Further research needs to be conducted to identify potentialimmunological target protein.

Abstract

Diagnosis dini infeksi Human Immunodeficiency Virus (HIV) dapat mengurangi kebahayaan transmisi. Infeksi akut dapat ditetapkanberdasarkan pemeriksaan antigen atau asam ribonukleat (RNA/proviral DNA) HIV. Enzyme Immunoassay (EIA) generasi keempatadalah immunoassay yang dapat mendeteksi antigen p24 dan antibodi HIV. Tujuan penelitian adalah mengetahui nilai diagnostik ujiHIV generasi keempat di terduga HIV. Penelitian ini merupakan uji diagnostik dengan desain potong lintang. Sampel penelitian adalahsemua pasien terduga HIV yang datang ke poliklinik Volunters Counselling and Testing (VCT) RSUP Dr. M. Djamil Padang masa waktuMaret 2015–Maret 2016. Penelitian ini dilakukan untuk menilai ketepatan diagnostik (kepekaan, kekhasan, nilai peramalan positif,nilai peramalan negatif) uji HIV generasi keempat menggunakan Enzyme Linked Fluorescent Assay (ELFA) terhadap deteksi RNA HIVmenggunakan Reverse Transcriptase Polymerase Chain Reaction (RT-PCR) serta dianalisis menggunakan tabel 2×2. Subjek penelitiansebanyak 70 orang terduga HIV terdiri dari 46 laki-laki (65,7%) dan 24 perempuan (34,3%) dengan rerata umur 27,7 tahun. TransmisiHIV terbanyak adalah perilaku heterokseksual (45,7%). Nilai diagnostik uji HIV generasi keempat terhadap RNA HIV didapatkankepekaan 95%, kekhasan 96%, nilai peramalan positif 97% dan nilai peramalan negatif 92%.

Abstract

Metabolic acidosis is prevalent among critically ill patients and the common cause of metabolic acidosis in ICU is lactic acidosis. However, not all ICUs can provide lactate measurement. The traditional method that uses Henderson-Hasselbach equation (completed with BE and AG) and alternative method consisting of Stewart and its modification (BDEgap and SIG), are acid-base balance parameters commonly used by clinicians to determine metabolic acidosis in critically ill patients. The objective of this study was to discover the association between acid-base parameters (BE, AGobserved, AGcalculated, SIG, BDEgap) with lactate level in critically ill patients with metabolic acidosis. This was an analytical study with a cross-sectional design. Eighty-four critically ill patients hospitalized in the ICU department Dr. M. Djamil Padang Hospital were recruited in this study from January to September 2016. Blood gas analysis and lactate measurement were performed by potentiometric and amperometric method while electrolytes and albumin measurement were done by ISE and colorimetric method (BCG). Linear regression analysis was used to evaluate the association between acid-base parameters with lactate level based on p-value less than 0.05. Fourty five (54%) were females and thirty-nine (46%) were males with participant’s ages ranged from 18 to 81 years old. Postoperative was the most reason for ICU admission (88%). Linear regression analysis showed that p-value for BE, AGobserved, AGcalculated, SIG and BDEgap were 119; 0.967; 0.001; 0.001; 0.689, respectively. Acid-base balance parameters which were mostly associated with lactate level in critically ill patients with metabolic acidosis were AGcalculated and SIG.

Abstract

Human Immunodeficiency Virus-1 (HIV-1) have the high levels of genetic variability (molecular variation) related to the mechanismof deletions, insertions, and especially as a recombination. The recombination between subgroups and their inter subtype HIV-1 producesa new virus strain causing a certain infection. Until now there were about 40 circulating recombinant form (CRF) and 100 uniquerecombinant forms (URF) worldwide. Various HIV-1 molecular variation cause pitfalls HIV detection due to the difficulty test designed todetect all HIV-1 strains. The purpose of this study was to know the determination of the molecular variation that can identify the HIV-1genome sequences which are conserved from local isolates of West Sumatera. The method used is a descriptive study on samples of patientssuspected of being infected by HIV-1 based on the antibody screening results using rapid HIV-1 test in the laboratory of Clinical Pathologyat Dr. M. Djamil Hospital Padang. The RNA isolation and amplification by RT-PCR was performed at the Microbiology Department,Faculty of Medicine of UGM. The amplicons were sequenced using the Sanger method and analyzed by Clustal W with sequence datafrom the HIV its database. The major subtypes of HIV-1 isolates derived from local West Sumatra is AE/B with four (4) isolates (57.1%),followed by subtype AE, 2 isolates (28.6%) and B, 1 isolates (14.3%). Most of the molecular variation in this study related to thesubstitution of one base followed by deletions and insertions. The protease showed a wide variation, which consists of 20–37 substitutionsand 1–3 deletions. p24 is a highly conserved gene, followed by gp120 and vpu. Based on this study, it can be concluded that the isolatesof HIV-1 recombination is the biggest proportion with conserved region found at p24 of protein group of the gag.

Abstract

Atherogenic dyslipidemia is the main risk factor of Acute Coronary Syndrome (ACS), caused atherosclerosis plaque and stenosis of artery coronary. Lipid profile ratio used as a marker of cardiovascular disease severity. Atherogenic Index of Plasma (AIP) calculated as logarithm triglycerides/high-density lipoprotein cholesterol (TG/HDL-C) is a reflection of plasma atherogenicity degree and indicator of small dense low-density lipoprotein (LDL). Small dense LDL is the best predictor for cardiovascular disease, but expensive and difficult to examined. The aim of this study to determine the correlation of AIP with stenosis level of the coronary artery in ACS.  This research was an analytical study with cross-sectional design in 24 ACS patients meet the inclusion and exclusion criteria and conduct blood tests at the Central Laboratory Installation of Dr. M. Djamil Hospital Padang in January 2017- September 2017. Triglycerides and HDL-C performed by an enzymatic colorimetric method and stenosis level determined by coronary angiography. Spearman correlation was used to analyze correlation of atherogenic plasma index with stenosis level of the coronary artery, significant if p<0.05.  The subjects of this study were 20 males (83.3%) and 4 females (16.6%) with mean age 57.6(7.9) years. The mean level of HDL cholesterol and triglycerides in ACS were 34.8 (8.7) mg/dL and 155.8 (51.8)  mg/dL. The mean level of  AIP in ACS was 0.28 (0.18). Median of stenosis level of coronary artery was 80% with range 50% - 95%. Spearman correlation test showed a moderate positive correlation between AIP with stenosis level of the coronary artery in ACS (r= 0.426 ; p<0.05).  There is a moderate positive correlation between AIP with stenosis level of the coronary artery in ACS.

Abstract

AbstrakPenelitian ini bertujuan mengetahui korelasi indeks massa tubuh (IMT) dengan estimated Glomerular Filtration Rate (eGFR) berdasarkan formula MDRD pada populasi sehat di kota Padang. Penelitian analitik dengan rancangan potong lintang ini mengikutsertakan 121 orang peserta medical check up pada beberapa rumah sakit di Kota Padang pada bulan Agustus 2013-Oktober 2014. Nilai eGFR dihitung menurut persamaan MDRD, kadar kreatinin diperiksa dengan metode enzimatik yang telah distandarisasi. Korelasi IMT dengan eGFR berdasarkan formula MDRD dianalisis menggunakan korelasi Pearson. Kemaknaan secara statistik ditentukan jika nilai p < 0,05. Korelasi IMT dengan eGFR berdasarkan formula MDRD adalah korelasi negatif yang sangat lemah dan tidak bermakna (r = -0,19 ; p > 0,05). Simpulan: tidak didapatkan korelasi antara IMT dengan eGFR berdasarkan formula MDRD.AbstractThe purpose of this study was to evaluate the correlation between BMI and eGFR by MDRD formula among healthy population in Padang. This cross-sectional analytic study enrolled 121 participants taking medical check-up in several hospitals in Padang from August 2013 to October 2014. Value of eGFR was determined by MDRD formula, and a standardized enzimatic method was used for measuring creatinine levels. The correlation between BMI and eGFR by MDRD formula was analyzed using Pearson correlation. Statistical significancy was concluded if the value of p < 0.05. The correlation of BMI and eGFR by MDRD formula was found very weak, negative, and not significant (r = -0.19, p >0.05). Thus, there was no correlation between BMI and eGFR by MDRD formula.

Abstract

The adiponectin and C-reactive protein (CRP) are markers of chronic low-grade inflammation related to obesity. This study aims to determine a correlation between adiponectin level and CRP level in adult obese individuals. This study was analyzed with a cross-sectional design of 55 subjects at Dr. M Jamil Central Hospital Padang from January to September 2019. The subjects included were type 1 obese (25≤BMI≤29.9 kg/m2) and type 2 obese (BMI 30.0 kg/m2). Enzyme-linked immunoassay (ELISA) was used to measure adiponectin and High Sensitivity C-Reactive Protein (Hs-CRP) levels. Univariate statistical analysis is processed and presented in distribution, frequency, mean (standard deviation), median (minimum-maximum value). Bivariate data were analyzed using the Pearson correlation test, significant if p<0.05. The study consists of 16 males and 39 females. The mean age was 35 years old (the range 23 to 57 years old), the mean BMI was 30.8 (4.5) kg/m2. This study consists of 28 subjects, type 1 obese and 27 types II obese. The mean adiponectin level in type 2 obese is lower than in type 2 obese groups. The mean CRP levels are high in all populations. Pearson ln-adiponectin and CRP correlation test in the entire population: r =-0.105 (p=0.444).
