Abstract

AbstrakPenelitian ini bertujuan mengetahui korelasi indeks massa tubuh (IMT) dengan estimated Glomerular Filtration Rate (eGFR) berdasarkan formula MDRD pada populasi sehat di kota Padang. Penelitian analitik dengan rancangan potong lintang ini mengikutsertakan 121 orang peserta medical check up pada beberapa rumah sakit di Kota Padang pada bulan Agustus 2013-Oktober 2014. Nilai eGFR dihitung menurut persamaan MDRD, kadar kreatinin diperiksa dengan metode enzimatik yang telah distandarisasi. Korelasi IMT dengan eGFR berdasarkan formula MDRD dianalisis menggunakan korelasi Pearson. Kemaknaan secara statistik ditentukan jika nilai p < 0,05. Korelasi IMT dengan eGFR berdasarkan formula MDRD adalah korelasi negatif yang sangat lemah dan tidak bermakna (r = -0,19 ; p > 0,05). Simpulan: tidak didapatkan korelasi antara IMT dengan eGFR berdasarkan formula MDRD.AbstractThe purpose of this study was to evaluate the correlation between BMI and eGFR by MDRD formula among healthy population in Padang. This cross-sectional analytic study enrolled 121 participants taking medical check-up in several hospitals in Padang from August 2013 to October 2014. Value of eGFR was determined by MDRD formula, and a standardized enzimatic method was used for measuring creatinine levels. The correlation between BMI and eGFR by MDRD formula was analyzed using Pearson correlation. Statistical significancy was concluded if the value of p < 0.05. The correlation of BMI and eGFR by MDRD formula was found very weak, negative, and not significant (r = -0.19, p >0.05). Thus, there was no correlation between BMI and eGFR by MDRD formula.

Abstract

The adiponectin and C-reactive protein (CRP) are markers of chronic low-grade inflammation related to obesity. This study aims to determine a correlation between adiponectin level and CRP level in adult obese individuals. This study was analyzed with a cross-sectional design of 55 subjects at Dr. M Jamil Central Hospital Padang from January to September 2019. The subjects included were type 1 obese (25≤BMI≤29.9 kg/m2) and type 2 obese (BMI 30.0 kg/m2). Enzyme-linked immunoassay (ELISA) was used to measure adiponectin and High Sensitivity C-Reactive Protein (Hs-CRP) levels. Univariate statistical analysis is processed and presented in distribution, frequency, mean (standard deviation), median (minimum-maximum value). Bivariate data were analyzed using the Pearson correlation test, significant if p<0.05. The study consists of 16 males and 39 females. The mean age was 35 years old (the range 23 to 57 years old), the mean BMI was 30.8 (4.5) kg/m2. This study consists of 28 subjects, type 1 obese and 27 types II obese. The mean adiponectin level in type 2 obese is lower than in type 2 obese groups. The mean CRP levels are high in all populations. Pearson ln-adiponectin and CRP correlation test in the entire population: r =-0.105 (p=0.444).

Abstract

Aim: This study aims to determine the relationship of HIV-1 genetic diversity with acquired drug resistance mutation in HIV-1 patients who receive first-line antiretroviral drugs at RSUP Dr. M. Djamil Padang. Methods: The study was conducted on 20 people with HIV who were diagnosed based on HIV-1 antibody screening tests using simple / rapid methods at the Central Laboratory of RSUP Dr. _ _ M. Djamil Padang and had received first-line ARV therapy for at least 6 months . Furthermore, proviral HIV-1 DNA extraction was carried out and amplification using a PCR (nested PCR technique) with a specific primer against reverse transcriptase at the Biomolecular Laboratory of the Microbiology Section of the Faculty of Medicine, Andalas University .  Result: The results showed that HIV-1 genetic diversity and changes in ARV regimens with the discovery of drug resistance mutations obtained for first-line ARVs were expected to be the basis for the management of HIV/AIDS patients. Conclusion: The conclusion of this study is that HIV-1 genetic diversity found is a form of recombinant CRF-AE and CRF- BC and subtypes A and B , a acquired drug resistance found is M184L and T215N (types of mutations that are resistant to NRTI- class ARVs)

Abstract

Aktivitas fisik yang kurang merupakan faktor risiko penyakit kardiovaskular (PKV). Indeks aterogenik plasma (IAP) yang diperoleh dari logaritma 10 rasio trigliserida terhadap high density lipoprotein merupakan biomarker yang kuat untuk memperkirakan risiko PKV. Aktivitas fisik yang rendah dan nilai IAP yang tinggi sering ditemukan pada penyandang obes. Tujuan penelitian ini adalah untuk mengetahui hubungan antara tingkat aktivitas fisik dengan IAP pada mahasiswa penyandang obes. Penelitian ini merupakan penelitian analitik observasional dengan desain cross sectional yang dilakukan di Fakultas Kedokteran Universitas Andalas dan Laboratorium Sentral RSUP Dr. M. Djamil Padang dari bulan Desember 2019 hingga November 2020. Subjek penelitian ini adalah 30 mahasiswa penyandang obes di Fakultas Kedokteran Universitas Andalas. Analisis statistik menggunakan uji one-way ANOVA dan Post-Hoc Tukey HSD. Hasil uji bermakna apabila p<0,05. Hasil penelitian didapatkan rerata±SD usia subjek 19,6±0,9 tahun dan rerata±SD IMT 32,5±4,1 kg/m2. Subjek dengan tingkat aktivitas fisik ringan merupakan yang terbanyak (46,7%). Rerata IAP±SD subjek -0,03±0,24. Analisis bivariat didapatkan hubungan bermakna antara tingkat aktivitas fisik dengan IAP (p=0,004). Uji Post-Hoc Tukey HSD menunjukkan perbedaan bermakna nilai IAP antara tingkat aktivitas fisik ringan dengan tingkat aktivitas fisik sedang (p=0,030) dan berat (p=0,008). Simpulan penelitian ini adalah terdapat hubungan terbalik antara tingkat aktivitas fisik dengan indeks aterogenik plasma pada mahasiswa penyandang obes.

Abstract

ABSTRACT THE DIFFERENCE OF NON HIGH DENSITY LIPOPROTEIN CHOLESTEROL LEVELS IN OBESE AND NON OBESE STUDENT By Sri Vanny Suhirman Non High Density Lipoprotein Cholesterol was calculated as total cholesterol levels minus HDL levels. Non HDL-C levels can be used as a predictor of cardiovascular disease risk. Non HDL-C levels tend to increase in obese people. This research aims to determine the differences of non HDL-C levels in obese and non obese students. This research was an observational analytic study with a cross sectional design. This research was conducted at the Central Laboratory of Dr. M. Djamil Hospital Padang from July 2020 to February 2021. The sample was 60 students at Faculty of Medicine, Andalas University, consists of 30 obese students and 30 non obese students. Statistical analysis used the independent T-test. The significant test results if the p value <0.05. The results showed that the mean age of the subjects was 20.32±1.05 years. The mean BMI in obese student was 32.53 ± 4.13 kg/m2 and the mean BMI in non obese student was 21.11 ± 1.85 kg/m2. The mean non HDL-C level in the obese student was 131.43 ± 34.70 mg/dL and the mean non HDL-C level in non obese student was 123.93 ± 35.55 mg/dL. Bivariate analysis found that there was no significant difference between the mean non-HDL C levels in the obese student and non obese student (p = 0.412). The conclusion in this research is the mean non HDL-C levels in the obese and non obese student is not different. Keywords: Body mass index, Non High Density Lipoprotein Cholesterol, Obesity

Abstract

Background: Multiple inflammatory syndromes in children (MIS-C) relate to COVID-19 severity in children. SARS-CoV-2 serology antibody is one of the diagnostic tools of MIS-C. The study aimed to describe the yield of serology antibodies of MIS-C and some characteristics found in hospitalized children with MIS-C suspects. Methods: This research was an analytic observational study. The data were collected retrospectively from some children who were hospitalized in Dr. M. Djamil General Hospital, Padang, West Sumatra, Indonesia, from April - June 2021. The inclusion criteria were children aged 1 month to 18 years, with or without contact history with the person who probable or confirmed COVID-19 and have signs and symptoms as MIS-C base on WHO criteria. Results: About eight out of 44 children showed positive serology antibodies and were diagnosed as MISC (18,2%). Based on demographic characteristics, children aged 11-15 years (27.3%) and boys were more affected (52.3%) as MIS-C suspected. Most of them were referred from a 2nd-level hospital outside Padang City (70.4%), but only 13.6% had a history of contact with COVID-19 confirmation patients. There was a significant difference in cardiovascular signs and symptoms between positive and negative serology antibody SARS-CoV-2 among children with MIS-C suspected (p<0.05), but not in fever, respiratory distress, gastrointestinal, neurology symptoms, either in laboratory results such as leukocytes, CRP and D-dimmer. Intravenous immunoglobulin, steroid, and PICU admitted showed no significant differences between the 2 groups, but more death prevalence in positive than negative. However, no significant differences (12.5% vs. 11.1%, p>0.05). Conclusion: Cardiovascular signs and symptoms could be proposed to be one of the significant differences in clinical conditions to differentiated children with MIS-C suspected and MIS-C due to serology antibody results.

Tujuan: Untuk mengetahui korelasi asupan vitamin C dan vitamin D dengan kadar timbal ibu hamil preeklampsia.Metode: Studi analitik korelatif dengan pendekatan cross sectional. Sampel penelitian ini yaitu 33 orang ibu hamil preeklampsia bertempat tinggal radius ≤10 km, dan 33 orang ibu hamil preeklampsia bertempat tinggal radius >10 km dari PT. Semen Padang. Penelitian dilakukan di RSUD Rasidin, RSPTM Unand, RS. Hermina, RS. Tentara Reksodiwiryo, RS. Ibnu Sina, dan RSUP Dr. M. Djamil Padang pada bulan Mei 2021-Februari 2022. Hasil: Penelitian menunjukkan bahwa asupan vitamin C memiliki korelasi negatif yang sangat kuat dengan kadar timbal ibu hamil preeklamsia yang bertempat tinggal radius ≤10 km dari pabrik PT. Semen Padang (r=-0,872, p=0,000), sedangkan asupan vitamin C tidak berkorelasi dengan kadar timbal ibu hamil preeklamsia yang bertempat tinggal radius >10 km dari pabrik PT. Semen Padang (r=-0,049, p=0,785). Asupan vitamin D dengan kadar timbal ibu hamil preeklamsia yang bertempat tinggal radius ≤10 km dan >10 km dari pabrik PT. Semen Padang masing-masing memiliki korelasi negatif yang kuat dan sedang (r=- 0,696, r=-512, p=0,000, p=0,002).Kesimpulan: Terdapat korelasi asupan vitamin C dan vitamin D dengan kadar timbal ibu hamil preeklampsia setelah dikontrol variabel cofounding (radius tempat tinggal, status merokok, dan lingkungan tempat tinggal).Correlation of Vitamin C and Vitamin D Intake with Lead Levels in Pregnant Women with PreeclampsiaAbstract Objective: This study aims to determine the correlation between the intake of vitamin C and vitamin D with lead levels in  pregnant women with preeclampsia.Method: This research is a correlative analytic study with a cross-sectional. The sample of this study was 33 preeclampsia pregnant women living in a radius of ≤10 km, and 33 preeclampsia pregnant women residing in a radius of >10 km from PT. The research was conducted at Rasidin Hospital, RSPTM Unand, RS. Hermina, RS. Tentara Reksodiwiryo, RS. Ibnu Sina, and RSUP Dr. M. Djamil Padang in May 2021-February 2022. Results: Vitamin C intake has a very strong negative correlation with lead levels of preeclampsia pregnant women who live within a radius of ≤10 km from the Semen Padang Factory (r=-0.872, p=0.000), while vitamin C intake is not correlated with the lead levels of preeclampsia pregnant women who live in a radius of >10 km from the Semen Padang Factory (r=-0.049, p=0.785). Vitamin D intake with lead levels of preeclampsia pregnant women who live in a radius of ≤10 km and >10 km from the Semen Padang Factory each has a strong and moderate negative correlation (r=- 0.696, r=-512, p=0.000, p=0.002).Conclusion: There is a correlation between vitamin C and vitamin D intake and lead levels of preeclampsia in pregnant women after controlling for confounding variables (radius of residence, smoking status, and living environment).Key words : Lead, Vitamin C intake, Vitamin D intake, Preeclampsia.

Abstract

Vitamin D deficiency is associated with various pathological condition like bone diseases, autoimmune diseases, metabolic syndrome, malignancies and infections. Obesity is a risk factor for vitamin D deficiency even though there is no evidence of a suitable causal relationship. This study aimed to compare the levels of 25(OH)D, based on body mass index (BMI). Observational study with cross-sectional design on subjects in Central Laboratory RSUP dr. M. Djamil. Subjects were divided into 2 groups based on BMI for Asia Pacific, namely normal body weight (BMI 18.5-22.9 kg/m2) and excess body weight (BMI ≥23 kg/m2) that met the inclusion and exclusion criteria. Serum 25(OH)D were examined using the immunoassay method. For the hypothesis test a bivariate comparative test is carried out using unpaired T test (numerical data) and Chi-Square test (categorical data). Statistical significancy is if  p < 0.05.  The mean (SD) 25(OH)D levels in normal and overweight subjects were 15.11 (5.46) ng/mL and 15.59 (5.85) ng/mL, p> 0.05, respectively.

Abstract

Latar Belakang: Akumulasi lemak tubuh yang berlebih pada obesitas meningkatkan risiko terjadinya defisiensi vitamin D. Defisiensi vitamin D menyebabkan keadaan hipokalsemia dan terjadi peningkatan masuknya kalsium ke dalam adiposit sehingga meningkatkan adipositas dan terjadi peningkatan kadar trigliserida di sirkulasi. Objektif: Mengetahui korelasi kadar 25-hidroksi vitamin D dengan trigliserida pada penyandang obesitas. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain potong lintang. Penelitian berlangsung dari bulan Mei 2019 hingga Maret 2020. Sampel terdiri dari 22 perempuan obesitas berusia 18-50 tahun. Instrumen penelitian yang digunakan adalah data Laboratorium Sentral RSUP Dr. M. Djamil Padang. Hasil: Rerata usia subjek penelitian 36±6 tahun, rerata IMT 31,5±5,2 kg/m2, rerata kadar 25-hidroksi vitamin D 16,5±6,0 ng/mL, dan trigliserida 138±44 mg/dL. Analisis statistik uji korelasi Pearson menunjukkan tidak terdapat korelasi yang bermakna antara 25-hidroksi vitamin D dan trigliserida (r=-0,009; p=0,968). Simpulan: Pada penelitian ini dapat disimpulkan tidak ditemukan korelasi antara kadar 25-hidroksi vitamin D dengan kadar trigliserida pada penyandang obesitas.

Abstract

Inflammation is caused by infectious diseases, including COVID-19. Severe inflammatory responses contribute to weak adaptive immune response, there by resulting in immune response imbalance. Circulating biomarkers can represent inflammation and immune status are potential predictors for the prognosis of COVID-19 patients. Ferritin and interleukin-6 can serve as significant biomarkers in the detection of cytokine storm, systemic inflammation, and the prognosis of COVID-19. Neutrophil-lymphocyte ratio is an independent prognostic biomarker for COVID-19 patients. The aim of this study to determine differences levels of IL-6, ferritin, and NLR in COVID-19 patients treated ICU and non-ICU. This study was comparative cross-sectional design of 62 COVID-19 patients. Research was conducted at the Central Laboratory and Medical Record of Dr. M. Djamil Padang hospital (May-September 2021). Interleukin-6 levels determined the ECLIA methods, ferritin with the ELFA methods, and NLR with calculation methods. Bivariate data were analyzed with Mann-Whitney test. The characteristics of COVID-19 patients treated in ICU: 64.5% male, average age was 53.52(10.71) years, 48.4% death, and non ICU: female 71.0%, average 42.32 (12.22) years, and recovered 100%. The most common comorbid is hypertension. There were significant differences levels of IL-6, ferritin, and NLR in ICU and non-ICU patients with COVID-19 (p<0.001).
