Abstract

Penurunanangka cakupan peserta KB aktif IUD di Surabaya dalam 3 tahun terakhir dari 12,27%menjadi 6,1% menjadi perhatian khusus Dinas Kesehatan Kota Surabaya, hal ini terjadi akibatbelum optimalnya kinerja bidan dalam pelayanan kontrasepsi IUD di Puskesmas wilayahSurabaya. Tujuan penelitian menganalisis faktor-faktor yang berhubungan dengan kinerja bidandalam pelayanan kontrasepsi IUD di Puskesmas Wilayah Kota Surabaya.Penelitian analitikobservasional ini dilakukan dengan pendekatan cross sectional. Populasi penelitian adalahseluruh bidan di Puskesmas kota Surabaya yang memberikan pelayanan KB dan telah mengikutipelatihan CTU (362 bidan). Subjek penelitian 78 responden dipilih secara proporsional stratifiedrandom sampling. Pengumpulan data dilakukan dengan wawancara terstruktur dan observasi.Analisis data menggunakan uji regresi linear ganda.Hasil penelitian menunjukkan kinerja bidandalam pelayanan kontrasepsi IUD di wilayah Kota Surabaya 55,1 % dalam kategori baik.Pengetahuan bidandalam pelayanan kontrasepsi IUD (82,1%) kurang baik khususnya dalam halpenapisan dan konseling pra pemasangan. Motivasi bidan (55,1%) baik, sikap bidan (51,3%)kurang mendukung dalam pelayanan kontrasepsi IUD.Persepsi bidan terhadap kepemimpinankepala puskesmas (52,6%) baik. Persepsi bidan terhadap imbalan yang didapatkan 53,8% baik.Sarana prasarana (53,8%) sudah tersedia dalam menunjang pelayanan kontrasepsi IUD.Motivasi, sikap dan persepsi kepemimpinan berhubungan dengan kinerja bidan sedangkanpengetahuan, persepsi terhadap imbalan dan sarana prasarana tidak berhubungan dengan kinerjabidan dalam pelayanan kontrasepsi IUD. Secara bersama-sama motivasi dan persepsikepemimpinan berhubungan dengan kinerja bidan dalam pelayanan kontrasepsi IUD.DisarankanDKKuntuk melaksanakan supervisi secara langsung dan berkala untuk mengevaluasi danmonitoring pelaksanaan program di setiap Puskesmas serta memberikan motivasi dalamkemudahaan izin belajar bagi bidan yang ingin melanjutkan pendidikan.  The decrease of IUD active family planning participants’ coverage in Surabaya in the last three years, from 12.27% to 6.1%, became a special attention for Surabaya district health office. The decrease was caused by inadequate work performance of midwives in implementing IUD contraception service in the primary healthcare centers in Surabaya area. Objective of the study was to analyze factors related to the work performance of midwives in the IUD contraception service in the primary healthcare centers of Surabaya city area. This was an analytical- observational study using cross sectional approach. Study population was all midwives in the primary healthcare centers in Surabaya city who provide family planning service, and who had attended in CTU training (362 midwives). Study subjects were 78 respondents, and they were selected using proportional stratified random sampling method. Data collection was conducted through structured interview and observation. Multiple linear regression tests were applied in the data analysis. Results of the study showed that work performance of midwives in the IUD contraception service in Surabaya city area was in a good category (55.1%). Midwives’ knowledge on IUD contraception service specifically on screening and counseling before insertion was insufficient (82.1%). Motivation of midwives was good (55.1%). Midwives attitude regarding IUD contraception service was not supportive (51.3%). Perception of midwives towards head of primary healthcare center leadership was good (52.6%). Midwives perception on remuneration they received was good (53.8%). Facilities to support IUD contraception service were available (53.8%).Motivation, attitude, and perception on the leadership were associated with midwives’ work performance. However, knowledge, perception of remuneration, and facilities were not associated with midwives’ work performance in the IUD contraception service. Motivation and perception on the leadership, in common, were associated with midwives’ work performance in the IUD contraception service.Suggestions for district health office was to perform supervision directly and periodically, to evaluate and monitor the implementation of program in every primary healthcare center, and to provide permission for midwives who want to take further study.

Abstract

Laju pertumbuhan penduduk Indonesia cukup tinggi, sehingga penggunaan MKJP sangat diperlukan. Penggunaan MKJP di Kabupaten Sidoarjo cenderung menurun, dan lebih rendah dibanding penggunaan MKJP di tingkat Provinsi. Penelitian ini bertujuan untuk menganalisis hubungan pengetahuan, sikap akseptor terhadap MKJP, dan akses pelayanan KB, serta sikappetugas dalam pelayanan KB dengan penggunaan MKJP di Kabupaten Sidoarjo Provinsi Jawa Timur. Jenis penelitian adalah observasional analitik dengan disain Cross Sectional. Populasi penelitian adalah akseptor aktif di Kabupaten Sidoarjo. Subyek sejumlah 100 orang akseptor MKJP dan non MKJP, dipilih secara purposif. Pengumpulan data menggunakan data primer dan sekunder, dan dianalisis secara deskriptif menggunakan distribusi frekuensi, Analisis bivariat dilakukan dengan uji Chi-Square, dan analisis multivariat dengan Regresi Logistik ganda. Hasil penelitian menunjukkan 63% responden berumur lebih dari 30 tahun, 54% berpendidikan Dasar, 65% bekerja sebagai ibu rumah tangga, 52% berpenghasilan kurang dari UMR, 79% memiliki paritas rendah (<3), dan 81% umur anak terkecil ≥ 3 tahun. Analisis bivariat didapatkan ada hubungan pengetahuan (p=0,0001), sikap akseptor terhadap MKJP (p=0,001), sikap akseptor terhadap akses pelayanan KB dengan penggunaan MKJP (p=0,019). Sikap petugas dalam pelayanan KB (p=0,715) tidak berhubungan dengan penggunaan MKJP. Variabel yang bersamasama berhubungan dengan penggunaan MKJP adalah pengetahuan, sikap akseptor tehadap MKJP, dan akses pelayanan KB. Faktor yang berhubungan paling kuat dengan penggunaan MKJP adalah pengetahuan akseptor tentang MKJP. Growth rate of Indonesian population was high, and long term contraception method (MKJP) was needed. The use of MKJP in Sidoarjo district tended to decrease; it was lower than the use of MKJP in province level. Objective of this study was to analyze association between knowledge, attitude of acceptors towards MKJP, family planning access, workers attitude in the family panning (KB) service and the use of MKJP in Sidoarjo district, East Java province. This was an observational-analytical and quantitative study using cross sectional approach. Study population was active acceptors in Sidoarjo district.Study subjects were 100 MKJP and non MKJP acceptors who were purposively selected. Data collection was done by collecting primary and secondary data. Data was analyzed by applying descriptive analysis, Chi-Square test, and multiple logistic regressions. Results of the study showed that 63% of respondents were more than 30 years old, 54% of respondents’ level of education were elementary school, 65% of respondents worked as housewives, 52% of respondents had salary below the minimum regional pay (UMR), 79% of respondents had low parity (< 3), and 81% of respondents had the last child with age 3 years old or higher. Results of bivariate analysis showed association between the use of MKJP and knowledge (p= 0.0001), acceptor attitude towards MKJP (p= 0.019), acceptor attitude towards AKB service accessibility (p= 0.019). Workers attitude in the KB service (p= 0.715) did not associate with the use of MKJP. Variables that jointly rellated to the use of MKJP were knowledge, attitude of acceptor towards MKJP, and accessibility to KB service. The strongest factor that associated with the use of MKJP was knowledge of acceptor about MKJP.

Abstract

Kementerian  Kesehatan  melaksanakan  program Mobile  Clinic,  untuk  lebih  mendekatkan  akses  pelayanan  kesehatan  pada  masyarakat di  Tanah  Papua.  Target  cakupan  mobile  clinic masih  belum tercapai,  walaupun  biaya  yang  dikeluarkan  sangat  besar.  Tujuan penelitian  adalah  melakukan  evaluasi  pelaksanaan  program mobile  clinic  dalam  pelayanan  kesehatan  ibu  di  Kota  Sorong  Propinsi  Papua  Barat.  Jenis  penelitian  ini  adalah  deskriptif  kualitatif, Informan  utama  adalah  seluruh  tim  mobile  clinic Kota  Sorong.  Informan  triangulasi  adalah  kader  mobile  clinic dan pengelola program mobile clinic. Pengumpulan data dilakukan melalui indepth interview dan focus group discussion. Pengolahan data dilakukan dengan metode analsis isi (content analysis). Hasil penelitian menunjukkan bahwa perencanaan sudah dilakukan oleh semua tim mobile clinic. Ada kendala keterbatasan tenaga pada 2 puskesmas sehingga perlu penunjukan tenaga pelaksana dari unit kerja lain. Pengorganisasian dengan pemberdayaan kader sudah terlaksana dengan baik. Pada  penggerakkan  dan  pelaksanaan  terdapat  dua    kebijakan  yang  tidak sesuai  Juknis  yakni  penambahan  tenaga  dan  pergantian  personil  TMC  (Tim  Mobile  Clinic).  Penambahan  tenaga  meningkatkan  pencapaian   cakupan   dan   penambahan   jenis   pelayanan   yaitu   ANC terpadu.  Pergantian  personil  TMC  pada  tenaga  yang  tidak  dilatih berdampak  tidak tercapainya  target  cakupan. Kerjasama tim, lintas program dan lintas sektoral terjalin baik.Pelaporan tidak lengkap dan  supervise  yang  dilakukan  tidak  memberikan masukan.  Sulitnya  jangkauan  dan  besarnya  biaya  transportasi  tidak  menghambat  pelayanan karena  didukung  alokasi  dana  yang  memadai.  Direkomendasikan agar program dilakukan kembali dengan perbaikan petunjuk teknis dan biaya yang disesuaikan dengan Perda Ministry of Health implemented mobile clinic program to bring health service access closer to the community in Papua land. Target coverage of mobile clinic program was not attained, although the cost spent was very high. Objective of the study was to evaluate the implementation of mobile clinic program in maternal health service in Sorong city of West Papua province. This was a descriptive-qualitative study. Main informants were all team of mobile clinic of Sorong city. Triangulation informants were mobile clinic cadres and mobile clinic managers. Content analysis was applied in the data management. Results of the study showed that planning had been done by all mobile clinic teams. There were limited staffs in two primary healthcarecenters(puskesmas), and this situation allowed choosing executor workers from other work units. Organizational activities with empowerment of cadres had been done properly. There were two policies that did not agree with technical guidelines in the actuating and implementing of the program namely addition of staffs and changing of mobile clinic team (TMC) personnel.Addition of the staffs increased coverage and added type of service. The added type of service was an integrated antenatal care (ANC). The change of TMC personnel with untrained personnel caused a fail to attain the target coverage. Collaboration among teams, cross programs and sectors had been performed well. Reporting was not good, and supervision done did not provide inputs. Difficulty in reaching location and expensive transportation costs did not hamper services due to sufficient funding allocation support. Reimplementation of the program with technical guidance improvement and budget aligned with Perda is recommended.

Abstract

AKI dan AKB di Kabupaten Semarang masih tinggi yaitu 146,24/100.000 KH dan 13,40/1000 KH pada tahun 2011. Pertolongan persalinan oleh tenaga kesehatan menurun dan masih terdapat 410 persalinan oleh dukun dengan 0,24 % kematian ibu pada tahun 2011. Hal ini terjadi meskipun sudah dilakukan kemitraan bidan dukun dan jampersal. Tujuan penelitian ini untuk menganalisis alasan pemilihan penolong persalinan oleh ibu di Kabupaten Semarang. Penelitian dilakukan dengan metode kualitatif. Pengumpulan data dengan wawancara mendalam dan observasi. Subyek penelitian terdiri dari informan utama sejumlah 8 ibu bersalin (4 ibu bersalin oleh dukun dan 4 ibu bersalin oleh bidan) dan 17 informan triangulasi (4 suami/keluarga dari ibu bersalin dengan dukun, 4 suami/keluarga dari ibu bersalin dengan bidan, 4 tokoh masyarakat/ tokoh agama, 4 bidan koordinator dan Kepala Seksi Kesehatan Keluarga Dinas Kesehatan. Metode analisis data dengan content analysis. Hasil penelitian menunjukkan tingkat pengetahuan ibu tentang persalinan sehat dan aman masih kurang. Ibu yang memilih dukun sebagai penolong persalinan bersikap negatif terhadap bidan karena bidan merobek jalan lahir. Ibu yang memilih bidan berpersepsi baik karena bidan ramah, trampil dan cekatan, ibu yang memilih dukun berpersepsi baik dalam hal perawatan post partum. Tidak adabudaya khusus dalam pertolongan persalinan baik oleh bidan maupun dukun. Kemudahan akses pada saat melahirkan merupakan faktor penentu pada pemilihan penolong persalinan. Biaya persalinan dukun terjangkau karena bersifat suka rela dan dapat berbentuk barang. Dukungan suami dan keluarga cukup kuat dalam pemilihan penolong persalinan. Disimpulkan bahwa pemilihan dukun sebagai penolong persalinan berkaitan dengan pengetahuan, sikap, persepsi mutu pelayanan, biaya dan kemudahan akses bagi ibu bersalin.AKI and AKB in Semarang district were still high, 146.24/100 000 live-birth and 13.40/ 1000 live-birth in 2011. Delivery assistance by health workers deceased, and 140 deliveries were assisted by dukun (traditional delivery assistance) with 0.24% maternal mortality in 2011. Although collaboration between dukun and jampersal (maternity insurance), yet this situation still happened. Objective of this study was to analyze reasons for selection of delivery assistance by maternities in Semarang district. Qualitative method was applied in this study. Data were collected through in-depth interview and observation. Study subjects were main informants and triangulation informants. The main informants were eight maternities (four maternities were assisted by dukun, and 4 maternities were assisted by midwives); 17 triangulation informants (four husbands/ family of maternity assisted by dukun, four husbands/ family of maternity assisted by midwives, four coordinator midwives, and a head of family health section of Semarang district health office. Content analysis was applied in the data analysis. Results of the study showed that knowledge level of mothers about healthy and safe labour was deficient. Mothers who chose dukun as their delivery assistance had negative attitude towards midwives because midwives torn the birth canal. Mothers who chose midwives had good perception towards midwives; they said that midwives were kind and skillful. Mothers who chose dukun had good perception on the post-partum management. No special culture in the delivery assistance by midwives or dukun. Accessibility to the delivery assistance when mothers were in labour was a determinant factor in the selection of the delivery assistance. Fee of delivery by dukun was reachable because it was voluntary and could be substituted by goods. Support from husband and family in the selection of delivery assistance was strong enough. In conclusion, selection of dukun as delivery assistance was related to knowledge, attitude, perception on the quality of service, fee, and accessibility for in labour mother.

Abstract

Penyebab  utama  kematian  ibu di  Kabupaten Indramayu  dari tahun 2012 - 2016 adalah preeklampsia yaitu 40 %.  Penelitian ini bertujuan untuk menganalisis faktor-faktor risiko kejadian preeklampsia di Puskesmas Kabupaten Indramayu.Jenis penelitian observasional dengan rancangan kasus kontrol. Subyek sejumlah 55 kasus ibu preeklampsia dan 55 kasus ibu tidak preeklampsia dan dipilih secara purposive sampling. Pengumpulan data dilaksanakan dengan wawancara menggunakan kuesioner terstruktur, pengukuran antropometri dan metode food recall 2 x 24 jam. Analisis data dilakukan dengan chi-square dan regresi logistik ganda.Hasil penelitian menunjukkan faktor risiko terjadinya preeklampsia adalah umur kehamilan berisiko (<20 atau >35 tahun) (OR=5,4, p=0,001), riwayat preeklampsia sebelumnya (OR=7,1, p=0,001), riwayat preeklampsia dalam keluarga (OR=7,2, p=0,001), obesitas (OR=6,5, p=0,001), tingkat kecukupan protein kurang (OR=2,7, p=0,020), tingkat kecukupan vitamin D kurang (OR=3,7, p=0,013), stress (OR=2,6, p=0,013) dan paparan asap rokok (OR=2,3, p=0,030), secara multivariat umur kehamilan berisiko (<20 atau >35 tahun) (OR=2,9, p=0,047), riwayat preeklampsia (OR=4,3, p=0,024), riwayat preeklampsia dalam keluarga (OR=9,1, p=0,001), obesitas (OR=7,1, p=0,001) dan tingkat kecukupan protein (OR=4,1, p=0,026) secara bersama-sama merupakan faktor risiko kejadian preeklampsia. Disimpulkan bahwa faktor risiko yang paling kuat adalah riwayat preeklampsia dalam keluarga dan obesitas.  Disarankan agar memasukkan riwayat preeklampsia dalam keluarga pada daftar data perawatan antenatal, mengintensifkan kunjungan nifas dan melakukan pengawasan bagi mereka yang telah terindentifikasi mempunyai risiko terjadinya preeklampsia.

Abstract

Internal marketing and service quality BPJS are things that influence the level of satisfaction of doctors that becomes the deciding factor of doctor performance. The aim of this study is to find the influence of internal marketing and service quality performance of BPJS Kesehatan with doctor performance through doctor satisfaction at RSUD Ulin Banjarmasin. This type of research is analytic with cross sectional design. The research method was developed using a questionnaire based on the literature. The number of respondents who came from the science of internal medicine, pediatric, obstetrical and gynaecology, and surgery and met the inclusion criteria as many as 52 people were surveyed. Data was analyzed using Pearson correlation and multiple linear regression. The results showed correlation between internal marketing to the satisfaction of doctors (r = 0.584, p = 0.000), between service quality BPJS and satisfaction of doctors (r = 0.499, p = 0.001), between the satisfaction of doctors and the performance of doctors (r = 0.583, p = 0.0001). Results of research by multiple linear regression analysis showed that the internal marketing and service quality BPJS have contributed influence on physician satisfaction of 44.6% while 55.4% described other factors. Physician satisfaction have contributed influence on physician performance by 32.7% while 67.3% described other variables outside the model. It was concluded that internal marketing and service quality effect on doctors satisfaction which in turn affect the performance of doctors. Suggested to BPJS Health to improve the financing system INA-CBG’s and disseminate laws and regulations better.

The prevalence of under-five children with severe malnutrition in Sorong city West Papua in 2008, 2009 and 2010 consecutively was 3.59%, 1.1% and 1.9%. Implementation of a program could be influenced by many factors such as communication, resources, disposition and bureaucracy factors. The objective of this study was to know the implementation of severe malnutrition control program at primary healthcare centers (puskesmas) in the working area of Sorong city health office West Papua in 2010. This was a qualitative study with cross sectional approach. Data was collected through in-depth interview using interview guideline. There were 5 primary healthcare centers studied with program executors as main informants. Triangulation informants were family or parents of under-five children with severe malnutrition, head of puskesmas and head of nutrition section of Sorong city health office West Papua province. Results of the study showed implementation of the program had not run according to the standard of implementation from the Ministry of Health. It was caused by not optimal communication that was done by Sorong city health office through program socialization. The availability of resources such as human resource to implement the program was insufficient. Only those who had nutrition educational background implemented the program and other workers were not involved in the program. There was no nutrition care team in all puskesmas. Additionally, the majority of the workers had not received training yet. Management of program funding was not according to the unit cost. Facilities were inappropriate and there was still improper puskesmas. The executor workers were needed for implementing the program; unfortunately due to less transparency and socialization, commitment was also low. No authority or standard operating procedure (SOP) given by Sorong city health office to puskesmas. In addition there was no supervision to the nutrition program workers. It was concluded that the implementation of severe malnutrition control program in Puskesmas in the working area of Sorong city health office West Papua province was not optimal. It was suggested to Sorong city health office to improve socialization about severe malnutrition control program to all program executors including heads of puskesmas in the working area of Sorong city health office; to create nutrition care and training teams. Funding allocation should be given to the right target and facilities should be completed.   Supervision, monitoring and evaluation were done continuously.

Abstract

 AbstrakPenyebab  utama  kematian  ibu di Kabupaten Pati dalam tiga tahun terakhir  adalah preeklampsia. Pada tahun 2016, 35% kasus kematian ibu disebabkan oleh preeklampsia. Penelitian ini bertujuan untuk menganalisis faktor-faktor risiko kejadian preeklampsia di Wilayah Kabupaten Pati. Jenis penelitian observasional analitik dengan rancangan kasus kontrol. Subjek sejumlah 40 kasus preeklampsia dan 120 kasus non preeklampsia  dipilih secara purposive. Pengumpulan data dilakukan dengan wawancara terstruktur menggunakan kuesioner. Data asupan makanan diperoleh dari  FFQ (Food Frequency Questionnaire). Analisis data dilakukan dengan chi-square dan regresi logistik ganda. Hasil penelitian menunjukkan faktor risiko terjadinya preeklampsia adalah faktor risiko internal : riwayat keluarga preeklampsia (OR=6,74; 95%CI:2,54-17,89, obesitas (OR=2,68; 95%CI:1,29-5,60), riwayat hipertensi (OR=7,4; 95%CI:2,91-18,82), stress  (OR=2,64; 95%CI:1,26-5,51), dan faktor risiko eksternal: asupan anti oksidan yang  rendah (OR=3,71; 95%CI:1,23-11,19), asupan natrium berlebih (OR=2,54; 95%CI:1,14-5.67), paparan asap rokok (OR=2,43; 95%CI:1,14-5,17).  Analisis multivariat menunjukkan riwayat  keluarga preeklampsia  (OR=5,32; 95%CI:1,63-17,40), obesitas (OR=4,01; 95%CI:1,60-10,09, riwayat hipertensi (OR=7,38; 95%CI:2,32-23,45), stress atau kecemasan (OR=3,30; 95%CI:1,31-8,33), asupan anti oksidan yang rendah (OR=4,22; 95%CI:1,13-15,74) secara bersama-sama merupakan faktor risiko kejadian preeklampsia. Variabel yang bukan merupakan faktor risiko preeklampsia adalah usia ibu, paritas, dan penambahan berat badan berlebihan selama kehamilan. Disimpulkan bahwa faktor risiko yang paling kuat adalah riwayat hipertensi ibu sebelum kehamilan. Disarankan agar memasukkan riwayat hipertensi, riwayat keluarga preeklampsia, obesitas, dan stres dalam anamnese  pelayanan antenatal untuk mendeteksi dini preeklampsia.

Abstract

Tingginya capaian cakupanprogram pelayanan kesehatan ibu dan anak (KIA) belum berpengaruh terhadap penurunan jumlah kasus kematian ibu di Kabupaten Temanggung. Penyebab kematian ibu terbanyak adalah karena penyakit penyerta yang seharusnya dapat terdeteksi pada pemeriksaan antenatal trimester pertama yang dilakukan oleh bidan desa. Penelitian ini bertujuan untuk menganalisis kualitas pelayanan antenatal trimester 1 kehamilan oleh bidan desa di Kabupaten Temanggung. Metode penelitian adalah survey analitik dengan design cross sectional. Subjek penelitian adalah 60 orang bidan desa yang dipilih secara systematic random sampling dari 14 Puskesmas terpilih kasus kematian ibu. Pengumpulan data dilakukan dengan angket dan observasi langsung menggunakan kuesioner terstruktur . Analisis data menggunakan uji chi square, kualitas pelayanan ANC dinilai dengan ketepatan pelayanan bidan dalam pemberian pelayanan ANC sesuai standar asuhan kebidanan dan standar operasional 10 T ANC.Hasil penelitian menunjukkan faktor yang berhubungan dengan kualitas pelayanan antenatal trimester I kehamilan oleh bidan desa adalah beban kerja(p = 0,002), pengetahuan(p = 0,006) dan supervisi(p = 0,002). Faktor yang tidak berhubungan adalah dana (p = 0,570), ketersediaan dan pelaksanaan SOP(p = 0,164), dan ketersediaan peralatan(p = 0,251). Ada hubungan bersama – sama variabel beban kerja(p = 0,002) pengetahuan responden(p = 0,006) dan supervisi(p = 0,002)dengan kualitas pelayanan Antenatal Trimester I Kehamilan.Disarankan kepada Dinas Kesehatandan Puskesmas di Kabupaten Temanggung untuk lebih memfokuskan bidan desa pada tugas pokok utamanya sebagai pemberi pelayanan KIA disamping tugas tambahan bidan desa lainnya. Mengadakan peningkatan pengetahuan dan ketrampilan tentang asuhan pelayanan antenatal secara merata untuk seluruh bidan yang kegiatannya dilaksanakan secara berkesinambungan, dan meningkatkan kualitas pelaksanaan kegiatan supervisi fasilitatif sesuai petunjuk tekhnis pelaksanaan yang telah ditentukan. Kepada Bidan Koordinator dan bidan desa diharapkan bersama – sama menguatkan kembali komitmen untuk meningkatkan dan menjaga kualitas pelayanan antenatal pada setiap ibu hamil yang dilayaninya.

Abstract

The availability of blood in Blood Donor Unit of Indonesian Red Cross (BDU-IRC) cannot fulfil the necessity of blood in Pontianak City for every year. Until the year 2014, there was only 37.3% of the total blood demand that could be provided. The aim of this study was to analyse problems of resource, attitude and commitment, a bureaucracy structure, communication, and social condition in implementing the blood donor recruitment program in BDU-IRC in Pontianak City.This was an observational study using a descriptive-qualitative design. Data were collected using methods of indepth interview, Focus Group Discussion, and observation. Main informants consisted of five P2D2S officers (a head of department and four staffs of P2D2S). Informants for triangulation purpose were 22 persons consisted of two decision makers, (a head of BDU and a deputy head of IRC), five coordinators of KDD, five persons of DDS, five persons of DDP, five citizen of Pontianak City. Data were analysed using content analysis.The results of this research showed that resource was quantitatively sufficient but it was qualitatively insufficient, particularly willingness of the officers in implementing their duties. Amount of budget was limited. There were a poor attitude and lack of commitment of the officers due to lack of willingness and low motivation to implement their duties in accordance with available regulations and goals. Not all the implementation was referred to Standard Operating Procedure released by the main office of IRC because the officers assumed that they had understood the ways to recruit blood donor. Communication was unclear and inconsistent. Support from blood donor groups was not optimal due to lack of attention and support from the program implementer such as supervising, providing rewards, and providing a supporting facility. In conclusion, the implementation of the blood donor recruitment program have not been well implemented. BDU-IRC needs to improve a competency of the officers by conducting training for untrained officers. The unit also needs to conduct a meeting to arrange a work program and a budget allocation annually. In addition, a community needs to be proactively asked to provide their blood to the IRC.
