Abstract

ABSTRAKLatar Belakang: Peningkatan infeksi HIV anak di Indonesia searah dengan peningkatan presentase penularan AIDS dari ibu ke anaknya dari 3% (2013) menjadi 4,6% (2015). HIV anak menjalani terapi antiretroviral (ARV) untuk meningkatkan jumlah sel T-CD4+. Stadium klinis berat pada HIV anak menurunkan jumlah CD4+. Pemberian suplementasi zat gizi makro dan mikro dapat meningkatkan status gizi HIV anak yang menjalani ARV.Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan asupan energi, protein, dan lemak terhadap status gizi berdasarkan berat badan dan jumlah CD4+ pada HIV anak di Kota dan Kabupaten Semarang. Metode: Penelitian ini menggunakan desain cross-sectional. Subjek yaitu anak usia 1-14 tahun sebanyak 31 subjek. Data dikumpulkan meliputi tinggi badan (TB), berat badan (BB), asupan zat gizi diperoleh dengan metode food recall 2x24 jam. Jumlah CD4+ melalui pemeriksaan darah subjek. Data dianalisis menggunakan uji Chi-square dan Regresi Logistik untuk menghitung Prevalence Rasio (PR). Hasil: Asupan protein memberikan risiko bermakna terhadap rendahnya jumlah CD4+ (PR=2,8; p=0,018; CI=1,331-5,891). Kesimpulan: Asupan gizi (energi, protein, lemak) tidak terkait dengan berat badan rendah (BB/U) dan stunting (TB/U). Asupan zat gizi yang berhubungan bermakna dengan jumlah CD4+ yang rendah (<500 sel/mm3) hanya asupan protein.  Asupan protein yang kurang pada HIV anak berisiko mengalami suppresi berat (jumlah CD4+ <500 sel/mm3) sebesar 3,036 kali KATA KUNCI: asupan gizi, HIV anak , jumlah CD4 +, stunting ABSTRACTBackground: The increase in HIV-infected children in Indonesia in line with the increase percentage of HIV positive children from mother to child transmission from 3% (2013) to 4.6% (2015). HIV-infected children using antiretroviral therapy (ARV) to increase the T-cells CD4+ count in HIV-infected children patients. Clinical stage heavily on lowering the CD4+ count for HIV-infected children. Supplementation of macro and micronutrients can improve the nutritional status of children using antiretroviral HIV. Objectives. The study aimed to analyze the relationship of intake of energy, protein, fat and clinical stage of  nutritional status and CD4 counts for HIV-infected children in the regional district and the city of Semarang.Methods: A cross-sectional study in The Regional District and the City of  Semarang. The subject of 31 HIV-infected children aged 1-14 years. Data collected included height, body weight, nutrient intake obtained by the method of Food Recall 2x24 hours. The number of CD4+ through blood test subjects. Data were analyzed using the Chi-Square test. Results: The intake of protein significantly increase the of low CD4+ count (PR = 3.036; p = 0.021; CI = 1.211 to 7.608 and PR = 2.8; p = 0.018; CI = 1.331 to 5.891). Conclusions: Nutrient intake (energy, protein, fat) is not associated with low body weight (WAZ), stunting (HAZ) and nutrient intake (energy, protein, fat) is not associated with low CD4 + incidence (<500 cells/mm3). KEYWORDS: CD4+ count, HIV-infected children, nutrient intake, stunting

Abstract

Background: Low independent nurse actions leads to poor health care quality. Therefore, understanding the factors affecting independent nursing action is necessity. Objectives: This study aims to analysis the independent nurse actions and its related and predicting factors. Methods: This study used a cross-sectional design with observational technique for data collection. There were 165 nurses recruited using simple random sampling in 14 inpatient wards of the General Hospital of dr. M. Haulussy Ambon, Indonesia. Spearman Rank Correlation and multiple regressions were used for data analysis. Results: Results of this study showed that there were significant relationships of age, gender, education, employment time, family burden, working experience, knowledge, workload, and nurse ability with independent nursing action with p-value <0.05. Nurse knowledge was the most dominant factor predicting independent nursing action with R-value 0.450. Conclusions: Independent nurse actions in daily living activities, caring & support, and rehabilitation were influenced by multiple factors such as nurse knowledge, ability, education and gender. Nurse knowledge is the most predicting factors affecting independent nursing action. Therefore, hospital and nurse managers need to well manage these predicting variables in order to improve independent nurse action that leads to the better quality of health service in the hospital.

Abstract

Background: Depression is one of the most common psychosocial problems in the elderly. The incidence of depression in the elderly is greater in the elderly living in the social service unit than the elderly in the community. Management of depression in the elderly should be more emphasized on interventions that focus on the individual needs of the biopsychosocial and spiritual aspects. Brain gym therapy and Benson relaxation are considered appropriate in decreasing depression. Objective: This study was to determine the effect of brain exercise and Benson relaxation on the level of depression in elderly in the elderly social service unit. Methods: This was a quasi-experimental study with pretest posttest control group design. Fifty-six elderly included in this study, which 28 assigned in the experiment and control group. The depression level was assessed using Geriatric Depression Scale (GDS). Wilcoxon and Mann-Whitney test were used for data analyses. Results: The study showed that the mean GDS score in the experiment group before the intervention was 7.21, while the mean value of GDS in the control group was 6.64. The mean value of GDS in the experiment group after the intervention was 5.04 and the mean in control group was 6.18. There was a significant difference of GDS score between experiment and control group with p-value of 0.021 (<0.05). Conclusion: There was a significant effect of brain gymnastics and Benson relaxation therapy in reducing depression in elderly. brain gymnastic therapy and Benson relaxation can be one of independent nursing interventions as an effort to improve care for elderly groups.

Abstract

Background: The COVID-19 pandemic has led to the enforcement of distance learning. This may cause negative impacts on adolescents' eating habits and physical activity.Objectives: This study aimed to analyse the differences in eating habits and physical activity before and during distance learning in adolescents.Materials and Methods: A cross-sectional study was conducted on a sample of 95 adolescents aged between 15-18 years. Subjects were selected using a simple random sampling method. Eating habits and physical activity variables were measured using modified Eating Habit and Lifestyle Changes in COVID-19 and Eating Habits Questionnaire. Wilcoxon Signed-Rank and McNemar tests were used to analyse the data.Results: There was an increase in frequency of meals two times/day (9.5%) and snacking three times/day (4.2%) reported during distance learning. There were significant differences in number of meals (p=0.014) and snacking (p=0.034), carbohydrates sources intake (p=0.046), sweet food (p=0.014), snack (p=0.016), exercise (p=0.035), exercise duration (p=0.004), and exercise frequency (p=0.030) before and during distance learning. There were no significant differences in protein-sources intake, vegetable, fruit, sweetened beverages, fried food, processed food, junk food, emotional eating, physiological eating, and ways of obtaining food before and during distance learning (p >0.05).Conclusion: Significant differences were found in eating habits comprised of the number of main meals and snacking, intake of carbohydrates sources, sweet food, snack, and physical activity before and during distance learning

Abstract

Background: A Kader refers to a Village Health Worker (VHW), a volunteer, which becomes one of the sources of community reference. Commonly, they sustain a mother's knowledge regarding adequate complementary feeding. However, there are still some VHW who have not possessed a health education background nor been able to be independent.Objectives: This study aimed to analyze the impact of collaborative models on VHW's competence as in knowledge, self-efficacy, attitudes, and counseling skill on the topic of complementary feeding.Materials and Methods: The design of this research is a quasi-experiment control group pretest-postest, with the retrieval of subjects using purposive sampling. Subjects were selected based on the location of the posyandu. The population in this research was VHWs in the Lembang district were 40 VHWs in each group. The treatment group was given training intervention for 1 month with a collaborative model, combining several methods into a series. The main topic was counseling and complementary feeding. The control group was given booklets and leaflets. VHW's competence was measured using questionnaires. This research was conducted in January-April 2020.Results: Statistical test results before the treatment of both groups showed no difference (p>0,05) in each variable. Two months after the intervention, there were significant differensces in the mean score of knowledge (p=0,001), attitude (p=0,001), and VHWs self-efficacy (p=0,000) in both groups. VHW counseling skills (p=0,149) until the first month there was not a significant difference. Unexpectedly, in the second month, the VHW counseling skills could not be observed due to the global COVID-19 pandemic. The mean score of each group increased significantly, but the score of the treatment group was higher than the control group.Conclusions: A collaborative model is effective when compared to only providing booklets and leaflets in increasing VHW's knowledge, attitudes, and self-efficacy but not effective yet for VHW counseling skills.

Abstract

Background: Stunting in children aged 36-59 months is caused by the intake of energy and protein below the average RDA recommendation. This low consumption has an impact on different growth and development disorders.Objectives: This study aimed to determine the differences in the development and diet of stunted and non-stunted children aged 36-59 months.Materials and Methods: This was an observational study with a cross-sectional approach, which was carried out at the work area of the Rowosari Health Center. The sample population consisted of 67 children aged 36-59 months, which were selected using the simple random sampling technique. The characteristics of the subjects and mothers were then collected using a questionnaire. Meanwhile, data on the diet and child development were obtained with the 24-hour recall method and Developmental Pre-Screening Questionnaire (KPSP), respectively.Results: The results showed that there were differences in the development as well as energy and protein adequacy level of stunted and non-stunted children with a p-value <0.05.Conclusion: There were several deviant developments in non-stunted toddlers due to the lack of nutritional intake, stimulation, interaction with the environment as well as the low knowledge of mothers about child care patterns.

Abstract

Background: Anemia is a globally public health problem, including in Indonesia (22.2%) and it has negative health impacts. Adolescent girls have high risk of anemia. Previous studies reported that adolescent girls at islamic boarding school had low food intake and poor knowledge about nutrition in preventing anemia. Objectives: To assess the prevalence of anemia and to analyze association between knowledge, nutritional status, and dietary habits and anemia in adolescent girls. Materials and Methods: A cross-sectional study was conducted in November 2020 among adolescent girls at islamic boarding school in Semarang. A total of 162 respondents were selected by cluster sampling. Anemia was determined by measuring the hemoglobin level in the blood by Cyanmethemoglobin method. Knowledge and dietary habits were collected through questionnaires. Nutritional status was assessed by measuring weight and height, then classified by body mass index for age using WHO Anthro. Bivariate and multivariate logistic analysis were used to test hypothesis and it was significant if p<0.05. Results: The prevalence of anemia was found to be 17.3%. In the bivariate analysis, overweight was more likely to have anemia (p=0.044). There was no association between father’s education, mother’s education, knowledge, frequency of staple food, breakfast habits, consumption of animal side dishes, consumption of vegetable side dishes, consumption of sweet tea and anemia. Mulitvariate model showed that overweight (OR=3.658; 95%CI=1.224-10.932; p=0.020) and good knowledge (OR=3.652; 95%CI=1.221-10.922; p=0.020) were significant associated with the anemia.Conclusion: Nutritional status and knowledge were significantly associated with anemia among adolescent girls.

Abstract

Background: Health status can affect the Human Development Index (HDI) of a country. To improve the quality of human resources, the Indonesian Government has applied various policies, such as the nutrition improvement program, since there are still lots of toddlers suffering from underweight, wasting dan stunting. Even one out of three toddlers in Indonesia were detected stunting.Objectives: This study described the implementation of a nutrition improvement program for toddlers in the Central Buton District Health Office.Materials and Method: This was a qualitative study that involved ten informants. Three of those informants are nutritionists in the District Health Office, Head of Public Health Department, Head of NutritionDepartement. Also, two nutrition workers in primary healthcare centers and two mothers of malnourished toddlers.Results: These findings showed that in terms of input, trained human resources on nutrition were lacking, and the budget for the nutrition improvement program was inadequate. While, in the process, all implementors had done very well, although they still had no collaboration across sectors. In the output aspect, the health status of underweight, wasting, and stunting toddlers improved. Also, monitoring and evaluation were conducted on toddlers registered at the integrated service posts in 2018.Conclusions: There were still constraints on the input, process, and output aspects, even though there was an increase in the nutritional status of children under five, but nutrition problems for children under five in Central Buton Regency were still high.

Abstract

Margorejo District is the sub-district with the lowest exclusive breastfeeding coverage in Pati District in 2020 at 10.4%. One of the reasons for the failure of exclusive breastfeeding in the Margorejo Subdistrict is the advertisement of formula milk to provide formula milk, the mother's belief that her baby will remain healthy even though she is not given exclusive breastfeeding, and the mother's assumption that exclusive breastfeeding is not a natural family planning method. This study aims to analyze the factors that influence exclusive breastfeeding behavior based on the Health Belief Model. Analytical design with a cross-sectional study design with a quantitative approach accompanied by a qualitative approach as support (mixed method). The sample set was 102 mothers using systematic random sampling. Quantitative data were analyzed using the chi-square test and logistic regression. Qualitative data are presented in the form of a narrative. The variables in this study related to exclusive breastfeeding behavior in the Margorejo sub-district were perceptions of vulnerability p-value 0.008, perceived seriousness p-value 0.000, perceived benefits p-value 0.001, perceived barriers p-value 0.000, and cues to action p-value 0.000 (p less than 0.05). Variables that have a joint relationship with exclusive breastfeeding behavior are perceived seriousness p-value 0.004, perceived barriers p-value 0.006, and cues to action p-value 0.024. Abstrak: Kecamatan Margorejo merupakan kecamatan dengan cakupan ASI Eksklusif terendah di Kabupaten Pati tahun 2020 sebesar 10.4%. Kegagalan ASI Eksklusif di Kecamatan Margorejo salah satunya yaitu disebabkan karena iklan susu formula untuk memberikan susu formula, keyakinan ibu bahwa bayinya akan tetap sehat walaupun tidak diberikan ASI Eksklusif, dan anggapan ibu mengenai bahwa ASI Eksklusif bukan merupakan KB alami. Penelitian ini bertujuan untuk menganalisis faktor yang berpengaruh terhadap perilaku pemberian ASI Eksklusif berdasarkan Health Belief Model. Desain analitik dengan rancangan cross–sectional study dengan pendekatan kuantitatif yang disertai dengan pendekatan kualitatif sebagai pendukung (mixed method). Sampel yang ditetapkan sebanyak 102 ibu menggunakan systematic random sampling. Data kuantitatif dianalisis menggunakan uji chi–square dan regresi logistik. Data kualitatif disajikan dalam bentuk narasi. Variabel dalam penelitian ini yang berhubungan dengan perilaku pemberian ASI eksklusif di Kecamatan Margorejo adalah persepsi kerentanan p-value 0.008, persepsi keseriusan p-value 0.000, persepsi manfaat p-value 0.001, persepsi hambatan p-value 0.000, dan cues to action p-value 0.000 (p kurang dari 0.05 ). Variabel yang memiliki hubungan bersama dengan perilaku pemberian ASI eksklusif adalah persepsi keseriusan p-value 0.004, hambatan persepsi p-value 0.006, dan cues to action nilai p-value 0.024.

Abstract

ENGLISHSamin Society is located in the area of Baturejo Village, Sukolilo District, Pati Regency. The initial data of 79 children consist of 2 to 5 years old that have status immunization 1 child completely immunized and 78 children uncompletely immunized. The purpose of this study is to determine the factors that affect the immunization rejection of Samin community in Baturejo Village, Sukolillo District, Pati Regency. This study uses a qualitative approach with a cross sectional observation. Techniques of collecting data included indepth interviews, field-event observation and document-observation. The technique for determining informants used purposive sampling. The results of this study were: 1) perception that it is not susceptible to the disease due to not immunized, 2) the perception that the disease due to not immunized is not harmful, 3) perception that immunization is no benefit, 4) the perception that the loss or greater obstacles in immunizing their children and 5) cues to act is not conducive to behave immunizing their children.  INDONESIAMasyarakat Samin berada di wilayah Desa Baturejo Kecamatan Sukolilo Kabupaten Pati. Data awal menunjukkan sebanyak 79 anak umur 2 sampai dengan 5 tahun. 1 anak berstatus imunisasi dasar lengkap, sedangkan 78 anak berstatus tidak imunisasi dasar lengkap. Tujuan penelitian ini untuk mengetahui faktor-faktor yang memengaruhi penolakan imunisasi di masyarakat Samin Desa Baturejo Kecamatan Sukolillo Kabupaten Pati. Penelitian ini menggunakan metode kualitatif dengan pendekatan crosssectional dilakukan dengan observasional. Teknik pengumpulan  data melalui wawancara mendalam, observasi lapangan, dan observasi dokumen. Teknik penentuan informan dengan teknik purposive sampling. Hasil penelitian ini adalah: 1)  persepsi bahwa tidak rentan terhadap penyakit akibat tidak imunisasi, 2) persepsi bahwa penyakit akibat tidak imunisasi tidak membahayakan, 3) persepsi bahwa imunisasi tidak ada manfaatnya, 4) persepsi bahwa kerugian atau hambatan lebih besar dalam mengimunisasi anaknya, dan 5) isyarat untuk bertindak tidak mendukung untuk berperilaku mengimunisasi anaknya.
