Abstract

Delivery using the caesarean section method is one of the JKN-KIS services which has experienced a significant increase in cases from year to year at Graha Sehat Probolinggo Hospital. The general tariff applied by the hospital is IDR 10,800,000, which is 206.15%, far above the INA CBGs tariff of IDR 5,238,800, making the hospital feel the need to carry out descriptive quantitative research using the ABC method and DD method. Then the unit cost results of the ABC method are compared with the INA-CBGs rates. The results of this research show that the unit cost of the ABC method is IDR 5,484,214, which is IDR 245,414 (4.68%) greater than the INA-CBGs tariff. The potential efficiency that can be achieved results in savings of IDR 665,295 so it is hoped that the unit cost of the ABC method will be IDR 4,818,919. IDR 419,881 (8.01%) smaller than the INA-CBGs tariff. The conclusion is that there is a negative difference between the unit costs of the ABC method and the DD method and the INA CBGs rates. Hospitals should submit a tariff review to BPJS, including a review of the provision of separate tariffs for baby handling costs. Apart from that, hospitals also need to make efficiencies by reducing length of stay, reviewing medical services, efficiency in laundry units and CSSD, speeding up RME, as well as efficiency in several other resources while still prioritizing patient safety

Abstract

Kata kunci: panel surya triple junction, iluminasi, body mounted, simple deploy, daya ABSTRACTSatellite design has envolved towards miniaturization to reduce launch costs. Small satellites provide a low-cost platform for space missions. One of the main problems with small satellites is the limited availability of power. Because the availability of power is needed so that the satellite subsystem can work, the satellite design process needs to analyze and estimated power availability as long as the satellite orbits while maintaining the compactness and volume imposed by the standard. This study aims to determine the conditions of solar illumination on solar panels from various alternative design placements in order to obtain an efficient design. Maximum illumination of triple junction solar panel mounted on a small satellite is approximately 60%. Based on the results of this study, the combination of installing 3 body mounted solar panels and 2 simple deploy solar panels produced twice the illumination percentage compared to 5 body mounted solar panels. Keywords: solar panel triple junction, illumination, body mounted, simple deploy, power

Abstract

The transformation of medical services in Indonesia from a disease-centered model to a patient-centered model has created intense competition among medical institutions. At one of the Maternal and Child Hospitals in Jakarta, a decline in visits from Non-BPJS patients indicates the need for a deeper understanding of the factors influencing patients' intentions to revisit. This study aims to analyze the influence of patient involvement and patient experience on the intention to revisit the hospital, with the mediation of the hospital's brand image. A quantitative approach with a cross-sectional design was used in this study. Data were collected through questionnaires from 119 Non-BPJS patients in the Outpatient Unit. Data analysis was conducted using PLS-SEM with the Smart-PLS program. The results showed that patient involvement and patient experience significantly influenced the intention to revisit, with the hospital's brand image serving as a strong mediator. Enhancing patient involvement, positive experiences, and a strong brand image can significantly increase patients' intention to return to the Maternal and Child Hospital in Jakarta.

Abstract

Hospital is one of the health service facilities that tries to overcome the health service balance between investment costs and the availability of applicable unit costs. Calculation of unit costs has an important role, including being used for budget planning, cost control, subsidies and as information in decision making. In the era of National Health Insurance (JKN) in Indonesia, Hospitals that cooperate with the Social Security Administering Agency (BPJS) Health are paid using the Indonesian Case Base Groups (INACBGs) package system. The acquisition of claim payments at Dr. Mintohardjo Naval Hospital according to the INACBGs rate compared to Hospital costs has a negative difference value for Bronchopneumonia cases. The purpose of this study was to analyze the difference in unit costs of Bronchopneumonia services based on Clinical Pathway towards the determination of INACBGs Rates and Hospital Rates at Dr. Mintohardjo Naval Hospital. This type of research is quantitative, comparative analytical observation using a cross-sectional research design according to the Hospital perspective. The subjects of the study were Medical Records and BPJS Kesehatan claim files for Bronchopneumonia services for the period April to September 2023, totaling 61 files. The study was conducted from January 2024 to April 2024. Data analysis used the paired t-test statistical method in the statistical test application (SPSS Version 26). The results of the study on calculating unit costs using the Activity Based Costing (ABC) method are as follows: 1) There is a difference between the calculation of unit costs with INACBGs rates of negative 6.2% and unit costs with Hospital rates of positive 11.2%, 2) There is a difference in the unit cost of Bronchopneumonia services based on Clinical Pathway with the Activity Based Costing method against INA-CBGs Rates, 3) There is a difference in the unit cost of Bronchopneumonia services based on Clinical Pathway with the Activity Based Costing method against Hospital Rates. The suggestions in this study are: 1) For the financial management of RSAL Dr. Mintohardjo to improve competence in calculating unit cost rates for services based on clinical pathways with the Activity Based Costing method, one of which is through training, 2) Updating and conducting clinical pathway audits periodically, 3) Re-evaluating hospital rates to increase the margin of each service so that the margin obtained is in accordance with what has been set, 4) Digitizing patient usage data that was previously still written manually to avoid errors in entering details of service costs.

Abstract

The implementation of patient safety standards is a major challenge in healthcare services, particularly in hospitals such as RSAU Soemitro. Data indicates that there were 25 patient safety incidents in the Inpatient Installation in 2023, with falls being the most common case (9), followed by nosocomial infections, medication errors (4), and the absence of patient identification bracelets during surgery (7). This study aims to analyze the influence of clinical skills and supervisory techniques on the implementation of patient safety standards at RSAU Soemitro, with caring behavior as an intervening variable. This quantitative research employs a cross-sectional study design, using a sample of 110 nurses in the inpatient installation. Data collection was conducted using questionnaires, and data analysis was performed using SEM-PLS techniques. The results show that the implementation of patient safety standards is simultaneously influenced by clinical skills, supervisory techniques, and caring behavior. Caring behavior can be significantly influenced by both clinical skills and supervisory techniques. Additionally, the implementation of patient safety standards is influenced by clinical skills and supervisory techniques both directly and indirectly through caring behavior. Furthermore, caring behavior has been proven to be the most significant factor, contributing 53.1%. These findings suggest that hospital management should focus on interpersonal skills training to support better implementation of patient safety standards.
