Abstract

Medication adherence plays a crucial role in achieving optimal glycemic control in patients with type 2 diabetes mellitus. However, poor glycemic control remains common despite the availability of antidiabetic therapy through the Chronic Disease Management Program (Prolanis), and evidence regarding this association among program participants is still limited. This study aimed to analyze the association between antidiabetic medication adherence and glycemic control based on HbA1c levels in patients with type 2 diabetes mellitus enrolled in Prolanis. An analytical observational study with a cross-sectional design was conducted among 118 Prolanis participants at primary health care facilities in Lampung Province, Indonesia, in 2025 using consecutive sampling. A total of 118 respondents were included in the final analysis, with a mean age of 54.09 ± 10.66 years. Medication adherence was assessed using the Morisky Medication Adherence Scale-8, while glycemic control was evaluated based on HbA1c categories. Data were analyzed using the Chi-square test with a significance level of p < 0.05. The results showed that most respondents had moderate medication adherence (38.1%) and poor glycemic control (50.8%). A significant association was found between medication adherence and glycemic control (p = 0.037; Cramér's V = 0.208), indicating a small-to-moderate effect size. These findings highlight the need to strengthen adherence-focused interventions, including patient education, routine monitoring, and continuous support, as part of Prolanis implementation to improve long-term glycemic outcomes in primary care settings.

Abstract

Abstract

The increasing resistance of bacteria to antibiotics has encouraged the use of medicinal plants as a natural alternative antibacterial agent. Moringa leaves (Moringa oleifera) contain bioactive compounds, including flavonoids, tannins, saponins, alkaloids, and terpenoids, which are capable of inhibiting bacterial growth. This review article aims to examine the effect of maceration duration using 96% ethanol as a solvent on the antibacterial activity of Moringa oleifera leaf extracts against Escherichia coli. Literature searches were conducted through PubMed, Google Scholar, and Scopus, using a combination of keywords such as “Moringa oleifera,” “leaf extract,” “ethanol,” “maceration time,” and “antibacterial activity,” with a limitation to publications from the last ten years. Relevant articles were selected based on their focus on ethanol extraction of Moringa oleifera leaves and antibacterial testing against E. coli. Recent studies indicate that variations in maceration duration result in differences in inhibition zone diameters ranging from 9.83 to 27.7 mm. Short-term maceration of 24 hours can achieve high antibacterial activity, whereas longer maceration does not necessarily enhance effectiveness. These differences are influenced by extract concentration, bioactive compound stability, and bacterial testing conditions. These findings highlight that the antibacterial effectiveness of Moringa oleifera leaves depends on an optimal combination of maceration duration, extract concentration, and the stability of active compounds. Determining the appropriate maceration time is crucial for achieving maximum antibacterial activity. Furthermore, these results provide a basis for future research on the utilization of Moringa oleifera leaf extracts as a potential natural antibacterial agent.

Abstract

Off-label drug use is still widely found, especially among vulnerable patients. Pediatrics is one of the vulnerable patient groups who still often get off-label drug therapy. This study was conducted with the aim of looking at the description of the use of off-label drugs given to pediatric patients. This research is a type of research with the literature review method, with data obtained from several articles, the search for articles was carried out through the Google Scholar database. The results of this study from 10 articles used as research data, found the use of off-label drugs in pediatric patients with the most results in the off-label dosage category and followed by off-label dosage category, off-label age category, and off-label route of administration category, while the off-label contraindication category was not found. Based on the results obtained, it is concluded that the use of off-label drugs in pediatric patients is quite high, so it is necessary to monitor the use of off-label drugs to reduce the incidence of unwanted side effects

Abstract

Background: Pharmaceutical services at primary health centers (puskesmas) play a vital role in supporting the quality of primary healthcare services. However, patient satisfaction with these services varies, influenced by factors such as service quality and demographic characteristics of the patients. In Lampung Province, the Induk Natar and Rajabasa Indah primary health care centers have different regional characteristics, which may influence patients perceptions and satisfaction with pharmaceutical services. Methods: This study employed an analytical observational method with a cross-sectional design. Samples were selected using systematic accidental sampling, with 100 respondents from each primary health care centers. Quantitative data were collected using the Servqual questionnaire and patient sociodemographic characteristics. Data were analyzed using the Mann-Whitney test and the Chi-Square test. Result: The reliability dimension showed the highest satisfaction level in both Induk Natar primary health care centers (90.10%) and Rajabasa Indah primary health care centers (94.05%). Conversely, the tangible dimension had the lowest satisfaction levels, with 88.60% in Induk Natar primary health care centers and 92.08% in Rajabasa Indah primary health care centers. A significant difference in patient satisfaction levels was found between the two health centers. However, no significant correlation was found between patient characteristics (age, gender, education, and occupation) and satisfaction levels (p-value > 0.05). Conclusion: Rajabasa Indah primary health care centers demonstrated a higher level of patient satisfaction compared to Induk Natar primary health care centers. A significant difference in satisfaction levels between the two centers was confirmed by the Mann-Whitney test.
