Abstract

This study aimed to identify the potential of natural compounds in tomatoes for diabetic complications intervention using amino acid profile, HP-TLC, antioxidant assay, enzymatic inhibitor assay, and in silico approach. Fresh air-dried tomatoes were analyzed for several screening assays including amino acid determination, HP-TLC, antioxidant activity using FRAP, α-amylase, and α-glucosidase enzyme inhibition. Virtual screening, molecular docking and molecular dynamics were performed using Molinspiration, pKcSM, AutoDock Vina, Discovery Studio, PyMOL, and Yasara software. Tomato bioactive compounds showed promising drug-likeness, antioxidant and α-amylase/glucosidase inhibitory activities, and potential for AGE-RAGE interaction. Out of 19 compounds from whole tomatoes complying with Lipinski’s rule of five, genistein, apigenin, and naringenin exhibited high oral absorption potential. Tomato contains genistein compound based on HP-TLC and the compound has high antioxidant and antidiabetic activities. Genistein has a stronger binding affinity with RAGE compared to AGE, indicating its potential as a competitive inhibitor. Additionally, genistein displayed stable ligand movements and higher binding energy values in MD simulations compared to the control. These findings suggest the potential of tomato bioactive compounds for further development as antidiabetic agents targeting AGE-RAGE interaction. In conclusion, genistein in tomatoes is indicated as a candidate for anti-complications of diabetes mellitus.

Abstract

Non-communicable diseases (NCDs) such as hypertension are a major public health burden, particularly in rural areas with limited healthcare access. This community engagement program aimed to reduce the burden of NCDs through a comprehensive community-based intervention model involving the revitalization of inclusive Posbindu (integrated health posts) and the establishment of a Hypertension School. The intervention included cadre training, early risk screening, support group formation, and the application of a positive deviance approach. The results showed improved capacity of health cadres in hypertension detection and education, increased early screening in the productive age group, and enhanced self-management among patients. This model contributed to improved quality of life for hypertensive patients and better achievement of Minimum Service Standards (SPM) at the local health center. The integrative approach demonstrates significant potential in strengthening community-based health systems for sustainable NCD management.

Abstract

Abstract

Fatigue is a major problem commonly experienced by chronic kidney disease patients undergoing hemodialysis. However, currently, the factors that correlate with fatigue in chronic kidney diseases patients undergoing hemodialysis are not comprehensive studies and there are still contradictory results from previous studies in Indonesia. This study aims to investigate the correlational analysis of physiological and psychological factors with fatigue on chronic kidney disease patients undergoing hemodialysis. This studi use a cross-sectional study with a sample count of 80 patients undergoing hemodialysis in Yarsi Public Hospital Pontianak, Indonesia. The sampling was carried out using the total sampling approach. A FACIT fatigue scale tool and depression and anxiety stress scale (DASS) 42 were employed to measure fatigue and depression, while hemoglobin and urea levels were taken based on medical history. The Spearman’s Rho test shows the physiological factor from the urea level had an insignificant correlation with fatigue (p-value 0.585). The physiological factor of hemoglobin level had a weak positive correlation (p-value 0.001 and r= 0.349) with fatigue. The psychological factor of depression had a strong negative correlation (p-value 0.000 and r = -0.812) with fatigue. The psychological factor of depression was the dominant factor causing fatigue than other factors based on the standardized coefficient beta value of -0.717. Factors that correlate with fatigue on chronic kidney disease patients undergoing hemodialysis are hemoglobin levels and depression. The most dominant factor influencing fatigue in chronic kidney disease patients undergoing hemodialysis is depression.Abstrak: Kelelahan merupakan masalah utama yang sering dialami oleh pasien gagal ginjal kronik yang menjalani hemodialisis. Abstract

Background: The pro-inflammatory cytokine interleukin-6 (IL-6) is thought to be the catalyst for endothelial dysfunction caused by smoking. Flow-mediated dilatation (FMD) is a non-invasive method to identify the development of blood vessel endothelial dysfunction at an early stage. However, several studies have not sufficiently explained how smoking and IL-6 interact. Objective: This study focuses on the role of Interleukin-6 (IL-6) in the process of endothelial dysfunction confirmed using non-invasive modalities such as Flow Mediated Dilatation (FMD) in smokers related to the risk of cardiovascular disease in the future with the hope of providing insight in education to smokers to quit smoking.Material and Methods: In this study, we gathered 116 male participants, of which 56 were nonsmokers, and 60 were smokers, and we used IL-6 and FMD testing. SPSS for Windows version 23 will be used to analyze the data.Result: According to the analysis of laboratory tests, the average IL-6 level in light smokers was 67.7±3.2 and in moderate smokers it was 95.1±11.0; with a p-value of 0.009, indicating a significant difference among the groups. The average FMD in the smoking group was 5.4±0.4% (p-value 0.000), whereas the average FMD in the non-smoker samples was 10.9±0.9%. We also looked at the average FMD among light and moderate smokers based on Brinkman Index, which was 5.5±0.4% and 5.0±0.4%, with a p-value of 0.780, indicating no significant difference between the groupsConclusion: Compared to non-smokers, this study demonstrates a correlation between IL-6 and smoking activity. However, there was no statistically significant difference between the prevalence of endothelial dysfunction and the degree of smoking dependence in this investigation.

Abstract

Background : Tobacco smoke exposure induces intima-media thickness by reducing nitric oxide and increasing adhesive molecule activity, with circulating cotinine serving as a marker; we hypothesize a correlation between monocyte chemoattractant protein-1 (MCP-1) and carotid intima-media thickness (cIMT) in active male smokers. Method : We conducted an observational cross-sectional analytic study involving 125 male participants, with 62 being active tobacco smokers and 63 non-smokers. Data were presented as mean ± SD, and the correlation between variables was analyzed using Pearson correlation. Result : Cotinine and MCP-1 levels were significantly higher in the smoker population (p 0.000) compared to non-smokers. The incidence of positive cIMT findings was higher in the smoker group (5%) than in the non-smoker group (2%). In the active smoker population, cotinine (r 0.21; p 0.11) showed a positive but non-significant correlation with positive cIMT findings, while MCP-1 showed a negative correlation (r -0.19, p 0.14) with positive cIMT findings. Smoking duration (r 0.162; p 0.223) and the amount of tobacco smoke (r 0.003; p 0.982) demonstrated a positive correlation with cotinine. MCP-1 exhibited a non-significant positive correlation with smoking duration (r 0.122; p 0.345) and a non-significant negative correlation with the amount of tobacco smoke (r -0.002; p 0.989). Conclusion : Among active tobacco smokers, cotinine showed a positive but non-significant correlation with positive cIMT findings, while MCP-1 exhibited a non-significant negative correlation with positive cIMT findings.

Abstract

Background: Burnout among hemodialysis nurses has reached critical levels globally. Grounded in Watson's Theory of Human Caring, this study examined the relationship between Watson's ninth carative factor (assisting with human needs gratification while preserving dignity and wholeness) and burnout levels among hemodialysis nurses. Methods: A cross-sectional correlational study was conducted among 113 hemodialysis nurses from the Indonesian Dialysis Nurses Association in Malang (April-May 2025). Stratified random sampling used Slovin's formula with 5% margin of error. Inclusion criteria: actively working hemodialysis nurses providing informed consent. Exclusion criteria: nurses on leave, retired, experiencing health issues, or in training programs. Burnout (independent variable) was measured using Professional Quality of Life Scale version V (ProQOL-V) burnout subscale (10 items, 5-point Likert, scores 10-55). Human needs fulfillment (dependent variable) used 10 items from Caring Nurse-Patient Interactions Scale (5-point Likert, scores 10-50). Both instruments underwent pilot testing (n=10), demonstrating validity (r>0.632) and reliability (Cronbach's α>0.6). Data collection utilized online surveys. Statistical analysis employed Spearman's correlation (α=0.05). . Results: Participants were predominantly female (59.3%), aged 36-45 years (48.7%), with bachelor's degrees plus nursing credentials (53.1%), and >5 years experience (65.5%). Burnout distribution: 56.6% low, 43.4% moderate, 0% high. Human needs fulfillment: 90.3% high, 9.7% moderate, 0% low. Spearman's analysis revealed significant negative correlation between burnout and human needs fulfillment (ρ=-0.446, p<0.001, moderate effect size). Conclusion: Higher implementation of Watson's ninth carative factor significantly reduces burnout among hemodialysis nurses. The theoretical model demonstrates that addressing patients' holistic needs while preserving dignity creates meaningful work experiences protecting nurses from burnout. Healthcare organizations should integrate Watson's Theory into professional development programs. Future longitudinal studies across diverse contexts are recommended to establish causal relationships.
