Abstract

HPV is a sexually transmitted virus, and high-risk HPV DNA was found in 99.7%of cervical cancer specimens. Within 12 to 24 months of exposure to the virus,90% of HPV infections disappear or become inactive. However, infection with high-risk strains of HPV persist which then increases the risk of progression to cervicalcancer. The detection of precancerous lesions consists of various methods,including pap smear (conventional or liquid-base cytology / LBC), visualinspection of acetic acid (IVA), visual inspection of lugoliodine (VILI), and HPV DNAtest (genotyping / hybrid capture).

Abstract

WHO has created a global strategy to accelerate the elimination of cervical canceras a public health problem and a world burden. Cervical cancer is a disease thatcan be prevented and cured, as long as it is detected early and treated effectively.Cervical cancer is also a disease that reflects global injustice. The burden isgreatest in low- and middle-income countries, where access to public healthservices is limited and screening and treatment for the disease has not been widelyapplied. In 2018, nearly 90% of all deaths worldwide occurred in low and middleincome countries. Furthermore, the proportion of women with cervical cancer whodie from the disease is more than 60% in these countries, more than double thenumber in many high-income countries, which is only 30%.

Abstract

Screening is a public health intervention administered to a target population without symptoms. Screening is not performed to diagnose a disease, but to identify individuals with a higher likelihood of developing the disease itself or a precursor to the disease. Not all diseases are suitable for screening programs. The following criteria help determine whether a disease is suitable for a screening program: (1) The disease is bound to have serious consequences. (2) The disease must have a detectable preclinical and asymptomatic stage. (3) Treatment at the preclinical stage should influence the long-term course and prognosis of the disease being screened. (4) Care must be available and accessible to those who have a positive screening test. History, screening tests and treatment options for cervical pre-cancer meet these criteria.

Abstract

Background. Colorectal cancer is the third commonest malignancy and the second leading cause of cancer death in the world. 5-Fluorouracyl-based chemotherapy is the primary treatment modality for colorectal cancer. Cancer stem cells are known to be responsible for chemotherapy resistance. Lgr5 is a colorectal cancer stem cell marker that is the target gene for Wnt signaling. Lgr5 potentiates the Wnt signaling pathway through inhibition of a regulator that inhibits Wnt signaling. Lgr5 overexpression is associated with a worse prognosis and chemotherapy resistance. This study was aimed to investigate the correlation between Lgr5 expression and 5-FU-based chemotherapy response in stage IV colorectal cancer patients at Dr. Mohammad Hoesin Hospital Palembang.Methods. This study used a correlative analysis study with a retrospective design using secondary data from medical records and paraffin blocks of stage IV colorectal cancer patients who received 5-FU-based chemotherapy from September 2018 to September 2020. The number of samples was 30 subjects consisting of 22 cases of negative responses and eight positive responses. All samples were stained with Lgr5 immunohistochemistry. Data analysis used the contingency coefficient correlation test.Results. Of the 30 research subjects, 20 subjects (66.7%) had high Lgr5 expression and ten subjects (33.3%) with low Lgr5 expression. Correlation analysis using the contingency coefficient test showed a weak correlation between Lgr5 expression and 5-FU based chemotherapy response with a positive direction, which means the higher the Lgr5 expression, the less response to chemotherapy.Conclusion. There is a weak correlation between Lgr5 expression and 5-FU based chemotherapy response in stage IV colorectal cancer patients at dr.Mohammad Hoesin Hospital Palembang.

Abstract

Background. Dyspnea could be caused by various reason, one of which is the presence of cardiac abnormality. Physical examination sometimes difficult to distinguish breath caused by heart abnormalities, especially small children, so we need another way to find out.Objective. To evaluate whether the examination of NT-proBNP levels can be used as a screening tool to diagnose cardiac abnormality in children presenting with dyspnea.Methods. A Cross sectional study was conducted from August to October 2015 on pediatric patients aged 1 month to 18 years presenting with dyspnea in pediatric ward Mohammad Hoesin Hospital Palembang. All subjects performed blood sampling for NT-proBNP examination and echocardiography to assess the presence of cardiac abnormalities. The diagnostic value analyzed by ROC curve, and determined the optimal cut-off point, sensitivity and specificity. Result. We obtained 58 subjects with median age 9.5 (1-180) months, consisted of 39 subjects with cardiac abnormalities and 19 subjects without cardiac abnormality. There is a significant difference (p = 0.002) of NT-proBNP levels in both groups with a median  1,775 (189-9,000) pg/ml vs 759 (245-9,000) pg/ml. In ROC curve analysis, AUC value was 0.75, and at the optimal cut-off point 1,235 pg/ml, sensitivity was 74.4% and specificity was 73.7%.Conclusion. The level of NT-proBNP can be used to diagnose cardiac abnormalities in children presenting with dyspnea.

Abstract

Introduction. Sepsis is a syndrome that describes physiological dysfunction, pathological, and biochemistry caused by infection. Fluid balance is an indicator that can monitor input and output. This study was aimed to evaluate the relationship between positive cumulative fluid balance and the mortality rate of sepsis patients treated in the intensive care unit. Method: This study design is a retrospective study. The inclusion criteria consist of patients diagnosed with sepsis written in the medical record, 18-65 years old, and patients admitted in intensive care unit dr Moh. Hoesin General Hospital. Data were analyzed using SPSS 22.0 with the normality of data distribution, independent sample t-test, Mann-Whitney analysis, chi-square, Fisher’s Exact to measure the relationship, and using Medcalc version 14 application to measure cut-off value, ROC curve AUC, cross-sectional point, sensitivity, and specificity. Result. The result showed that characteristic between age (p=0,491), gender (p=0,703) did not differ significantly between survived and non-survived patient. Length of stay (p=0,002), balance (p=0,000), and ward unit (p=0,014) has a significant different between survived and non survived patient. In chi square analysis, p value=0,000with odds ratio 7,083. Cut-off value of ROC curve is -97 mL with AUC 0,844, sensitivity 76,1% and specificity 79,3%. Conclusion. Cumulative positive balance patient in the sepsis patient correlates with increased mortality in a sepsis patient in Dr. Mohammad Hoesin General Hospital Palembang.

Abstract

Introduction. The aerosol box can reduce the risk of droplet and aerosol transmission from the patient to the operator when performing intubation, but in practice, an aerosol box makes the glottis visualization less evident, and the operator moves less space with less space the aerosol box. This study aimed to compare ETT duration using an aerosol box and without an aerosol box using a video laryngoscope. Methods: This study was a clinical trial with a post-test-only control design. The study was carried out from February 2021 to May 2021 at the Central Operating Theater of dr. Mohammad Hoesin General Hospital Palembang. The sample in this study was all patients who underwent elective surgery under general anesthesia using intubation at the Central Surgical Installation of dr. Mohammad Hoesin Palembang. After the data is collected, it is analyzed using the SPSS 22.0 program with the appropriate test. Results. There were no differences in subject characteristics (age, sex, BMI, Mallampati score, TMD, Cormack Lehane, limited mouth opening, short neck, limited mouth movement) between the group using the aerosol box and the group without the aerosol box. The duration of intubation without an aerosol box is 30.67 + 2.63 seconds, and using an aerosol box is 44.53 + 2.89 seconds. There was a significant difference between the two groups in the duration of patient intubation (p < 0.001). However, there was no significant difference in complications in the two groups (p >0.05). Conclusion. The duration of the endotracheal tube insertion using an aerosol box is more extended than without an aerosol box in elective surgery patients.

Abstract

Introduction: Pain is a problem often encountered in postoperative patients. Study has shown after a procedure, 80% patients experience acute pain. This postoperative pain will affect patient’s quality of life therefore necessitating quick and proper treatment. Tissue trauma during surgery will have influence on body system, including endocrine. One of endocrine system response is cortisol secretion. Cortisol levels may be attenuated by bupivacaine and oxycodone. The aim of this study was to determine the efficacy of bupivacaine0,125% and oxycodone 5 mg on pain perception measured by cortisol in patients undergoing orthopedic surgery of the lower limb. Methods: A randomized clinical trial, double-blind study was performed at Mohammad Hoesin General Hospital in Palembang, South Sumatra, from November to May 2021. There were forty samples and divided into two groups (bupivacaine 0,125% and oxycodone 5 mg). Groups were divided by block randomization by computerized random number generator. Blinding were done by making analgesic has the same packages to prevent knowledge of which intervention is being done. Data were analyzed using independent t-test, ANOVA, Mann-Whitney and Chi Square with SPSS version 22.0. Results: There were no statistically significant differences between the two groups on age, gender, body mass index and duration of surgery. In bupivacaine group, cortisol level decreased from 12.94±6.99 µg/dl to 11.32±5.42 µg/dl meanwhile oxycodone group cortisol levels increased from 11.81±8.47 µg/dl to 11.82±7.56 µg/dl. There were no significant difference between two groups relating to cortisol levels. Conclusions: No significant difference was found on administration of bupivacaine 0,125% and oxycodone 5 mg as epidural analgesia on cortisol level in postoperative orthopedic surgery of the lower limb patient.

Abstract

Objective: to analyze the relationship between PE and the newborn hearing loss Method : Analytic observation research withcross sectionaldesign performed inRSMHPalembangsince December 2016 to July 2017, obtained 48 aterm neonates born from mother diagnosed with PE (11 PE and 37 severe PE). Measurements of neonatal hearing loss then performed using emission otoacoustics (OAE) in both ears by ENT division with catagories intepretation of pass and refer. The mothers were physically check and interviewed to obtain demographic data and obstetric history. After the data normality were proved byShapiro Wilktest, we performed bivariate analysis using X2test on demographic and obstetric characteristics of the mother, neonatal demographic characteristics, and determine the relationship of PE with OAE result. The ratio of systolic (SBP) and diastolic (DBP) blood pressure to neonatal hearing loss was determined by independent T test and ROC test. Multivariate analysis was performed to determine the maternal and neonatal risk factors that influenced neonatal hearing loss. Data analysis using SPSS version 18.0. Results : There were no significant differences in maternal and neonatal demographic characteristics in neonatal hearing loss (p> 0.05). No significant relationship was found between PE and OAE of both ear (right, p = 0,437; left, p = 0,368). There was difference of mean of SBP and DBP of mother inneonate OAE of both ears (p <0,05) with cut off point of SBP 160 mmHg and DBP 106 mmHg.There was a significant association between DBP (> 106 mmHg) of the mother and birth weight of the fetus (< 2500 g) with referOAE. Conclusion: There was a significant association between neonatal hearing loss and maternal PE, determined primarily by maternal DBP, and neonatal birth weight. Keywords: maternal blood pressure, OAE,preeclampsia, Abstrak Tujuan: untuk menilai hubungan PE ibu dengan penurunan pendengarah bayyi baru lahir Metode: Penelitian observasi analitik dengan disain cross sectionaldilakukan di RSMH Palembang sejak Desember 2016 sampai Juli 2017, diperoleh 48 neonatus aterm lahir dari ibu PE (11 PER dan 37 PEB). Neonatus dilakukan pengukuran pendengaran menggunakan otoakustik emisi (OAE) pada kedua telinga oleh divisi THT dengan kategori intepretasi refer dan pass. Ibu dilakukan pemeriksaan dan wawancara untuk memperoleh data demografi dan riwayat obstetri. Setelah normalitas data dibuktikan dengan tes Saphiro Wilk, dilakukan analisis bivariat menggunakan tes X2 pada karakteristik demografi dan obstetri Ibu, karakteristik demografi neonatus, dan menentukan hubungan preeklampsia dengan hasil OAE. Perbandingan tekanan darah sistolik (TDS) dan diastolik (TDD) terhadap penurunan pendengaran neonatus ditentukan dengan independent T test dan uji ROC. Analisis multivariat dilakukan untuk menentukan faktor risiko Ibu maupun neonatus yang berpengaruh terhadap penurunan pendengaran neonatus. Analisa data menggunakan SPSS versi 18.0. Hasil: Tidak ditemukan perbedaan signifikan karakteristik demografi Ibu maupun neonatus tehadap penurunan pendengaran neonatus (p>0,05). Tidak ditemukan hubungan signfikan antara kondisi PE dengan OAE kanan (p = 0,437) dan kiri (p = 0,368). Ditemukan perbedaan rerata TDS dan TDD ibu terhadap OAE kedua telinga neonatus (p<0,05) dengan cut off point TDS 160 mmHg dan TDD 106 mmHg. Ditemukan hubungan signifikan antara TDD (>106 mmHg) ibu dan berat lahir janin (>2500 gr) dengan OAE refer. Kesimpulan: Terdapat hubungan bermakna antara penurunan pendengaran neonatus dengan kondisi preeklampsia ibu, yang ditentukan terutama oleh TDD ibu, serta berat lahir bayi. Abstract

Background: Low bone density has been known as one of the factors that increased fracture risk. Aging, estrogen deficiency, low BMI, and inadequate calcium intake are known factors that contribute to decreased bone density. Other than the known factors, some research show that sleep duration can also lower bone density. Sleep affects bone density in a way that it causes increased level of cortisol, proinflammatory cytokine, and decreased in physical activity. Method: This study is an observational analytic study with cross-sectional design. It is conducted on menopausal patients that has been examined with bone mineral densitometry at Poliklinik Reumatologi Penyakit Dalam RSUP Dr. Mohammad Hoesin Palembang in November 2017 to Oktober 2018. Samples are collected using total sampling technique. Results: From 93 subjects that participated in this study, 20,4% of them had osteopenia, and 51,6% had osteoporosis. Patients with low bone density, mainly slept for 6 to 8 hours at night per day, 30 minutes of nap per day, and has a total sleep duration for less than 8 hours each day. This study shows no significant association between night-time sleep, daytime napping, and total sleep duration with bone density (p value = 0,168, p value = 0,831, p value = 0,984). Analysis on other risk factors show significant association between body mass index and low bone density (p value = 0,002). Conclusion: There are no significant association between sleep duration and bone density in menopausal patients at Poliklinik Reumatologi RSUP Dr. Mohammad Hoesin Palembang.
