Abstract

One of the intermediaries for infection transmission between patients is through the hands of health workers. Hand hygiene is an easy, cheap, and effective precaution, but very difficult to implement. This study aims to determine the effect of health beliefs on nurse hand hygiene compliance in the inpatient ward of RSUD dr. H. Kumpulan Pane (HKP). This quantitative analytic study with a cross-sectional design was conducted at the inpatient ward of RSUD HKP. This study involved 210 inpatient ward nurses. Data collection was carried out using survey techniques, valid and reliable questionnaires, and direct observation. The majority of respondents were 36-40 years old (40.5%), were female (91.4%), had an associate degree (74.8%), had worked for 6-10 years (57.1%), and were civil servants (56.7%). All respondents had attended Infection Prevention and Control (IPC) training, experienced sharp object injuries and contact with patient body fluids, and 85.7% of respondents had experienced nosocomial infections. The majority of respondents had good perceptions of susceptibility, severity, benefits, cue to action, and self-efficacy, but the majority of respondents also had high perceptions of barriers. Further observation found that nurses' hand hygiene compliance was only around 56.7%. Data analysis found that nurses’ hand hygiene compliance at RSUD HKP was influenced by perceived susceptibility (p<0.001), severity (p<0.05), cue to action (p<0.001), and age (p<0.005). It can be concluded that although all components of the health belief model were associated with hand hygiene compliance, only perceived susceptibility, severity, cue to action, and age had a significant influence on hand hygiene compliance.

Abstract

Excessive exposure to UV-B rays can increase melanin production, potentially causing skin damage, making it necessary to prevent this through the use of natural-based sunscreens, such as nano gel from methanol extract of andaliman, to protect the skin from the harmful effects of UV-B radiation. This study aims to evaluate the sunscreen effect of andaliman methanol extract nanogel on the amount of melanin in Wistar rats exposed to UV-B light. The method used was a laboratory experimental study with a randomized posttest only with a control group design using mice as the research subjects. The results of the study showed that the SPF values of andaliman extract at concentrations of 2%, 3.5%, and 5% were 2.11, 2.49, and 2.45, respectively, which are considered low for sunscreen protection standards. Additionally, the amount of melanin obtained from each treatment group was also categorized as small, indicating that the andaliman extract in nano gel form did not significantly reduce melanin production in Wistar rats exposed to UV-B rays. In conclusion, there is no sunscreen effect of andaliman methanol extract nanogel on the amount of melanin in Wistar rats exposed to UV-B light. This could be due to factors such as the low SPF value, the lack of stability of the active ingredients in the nano gel, or the need for an increased dosage. The implications of these findings suggest the need for further research to optimize the formulation or explore other more effective ingredients in preventing melanin production caused by UV-B exposure.

Abstract

Abstract. Inpatient medical records play a crucial role as a source of data and health information for inpatients, supporting their treatment and care planning. One of the main factors contributing to is-sues in inpatient medical records management is the lack of discipline among those responsible for handling patients when filling out medical records. This can lead to discrepancies in inpatient medi-cal record management, preventing it from aligning with established medical record service stand-ards. The purpose of this study was to analyze the management of inpatient medical records based on medical record service standards. This research employed a quantitative analytical approach with a cross-sectional design. Data analysis was performed using univariate analysis, bivariate analysis with the chi-square test, and multivariate analysis with multiple logistic regression at a 95% confi-dence level (α=0.05α=0.05). The results indicated that the implementation of inpatient medical rec-ord management based on medical record service standards at Royal Prima Medan Hospital and Royal Prima Marelan Hospital was significantly influenced by work motivation (p=0.001p=0.001) and supervision (p=0.000p=0.000). Meanwhile, the variables that did not have a significant effect were length of service (p=1.000p=1.000), education (p=0.848p=0.848), and teamwork (p=0.241p=0.241). Among these, the supervision variable was the most dominant factor, with an Exp (B) / OR value of 19.542. It is recommended that hospital leadership enforce close supervision (waskat) for all medical record employees and provide motivation through seminars and training pro-grams to enhance the implementation of inpatient medical record management based on medical rec-ord service standards.

Abstract

Abstract. Tuberculosis (TB) remains a major public health issue, and irregular treatment adherence is one of the main challenges in controlling the disease. Irregular TB treatment can lead to drug resistance, prolonged illness, and increased transmission rates. Understanding the determinants affecting irregular TB treatment is crucial to improving patient adherence and treatment outcomes. This paper described aims to identify and analysed the factors influencing irregular pulmonary TB treatment at RSU Royal Prima Medan. A descriptive-analytical study was conducted using a cross-sectional approach. Data were collected through patient interviews and medical record reviews. Factors assessed included patient knowledge, socioeconomic conditions, accessibility to healthcare services, side effects of medication, and healthcare provider support. The findings indicate that irregular TB treatment is influenced by several key factors, including lack of patient awareness about the importance of completing treatment, financial constraints, difficulties in accessing healthcare facilities, adverse drug reactions, and inadequate support from healthcare providers. Among these, socioeconomic factors and side effects of medication were found to be the most significant determinants. Addressing the determinants of irregular TB treatment requires a comprehensive approach, including patient education, financial assistance programs, improved healthcare accessibility, and enhanced healthcare provider engagement. Strengthening these aspects can help improve treatment adherence and overall TB control efforts.

Abstract

Calamus caesius, traditionally used in Asian medicine for managing diabetes mellitus, is a promising source of endophytic bacteria due to its bioactive potential. This study aims to identify the potential of endophytic bacteria isolated from “Pakkat” (Calamus caesius Blume), a traditional food of North Sumatra, as a source of α-glucosidase inhibitors. These inhibitors, such as acarbose, are used to manage type 2 diabetes mellitus but have significant gastrointestinal side effects. This study focuses on isolating and characterizing endophytic bacteria that can produce natural inhibitors, potentially safer alternatives. Twenty endophytic bacterial isolates were successfully isolated, of which five were excluded due to hemolytic activity. Antioxidant activity was determined using the DPPH method, and isolates with the highest activity proceeded to α-glucosidase inhibitor testing. Molecular identification of potential isolates was performed through 16S rRNA gene amplification, and genomic DNA sequencing utilized the Oxford Nanopore Technologies platform. Secondary metabolite analysis was performed using antiSMASH, and gene cluster comparison with CAGECAT. Of 15 isolates, LTUP1.3 (Bacillus cereus) exhibited 86.6% α-glucosidase inhibitor activity, close to the positive control (88%). Although in Bacillus cereus LTUP1.3 no secondary metabolites were found that act as α-glucosidase inhibitors, three enzymes have similarities with enzymes found in Actinoplanes that play a role in acarbose biosynthesis namely dTDP-glucose 4,6-dehydratase RffG, glucose-1-phosphate thymidylyltransferase rmIA and glucose-1-phosphate adenylyltransferase glgC. These results suggest that Bacillus cereus strain LTUP1.3 has a potential biosynthetic pathway similar to the acarbose biosynthetic pathway in Actinoplanes. However, further confirmation is required to establish its overall acarbose production capability.

Abstract

Eggplant skin is the outer part of the eggplant fruit (Solanum Melongena L) which has therapeutic activities such as anti-inflammatory, anti-oxidant, anti-viral, anti-bacterial and anti-cancer. This study aims to evaluate the antioxidant potential of ethanol extract of eggplant skin through phytochemical analysis, antioxidant activity through ABTS and DPPH examination by assessing IC50, and its effect on oxidative stress-related enzymes in aflatoxin B1 (AFB1)-induced rats such as MDA, LPO-4HNE, and evaluating SOD levels in rat serum. The results showed that ethanol extract of eggplant peel has significant antioxidant activity in radical scavenging activity. The antioxidant activity of ethanol extract of eggplant skin in ABTS and DPPH methods showed IC50 values with strong antioxidant potential. In the In-Vivo test, aubergine skin ethanol extract showed a good protective effect against oxidative stress with decreased MDA and LPO-4HNE levels, and increased SOD activity in AFB1-induced rats. The optimal dose of aubergine skin ethanol extract in this study is the treatment with a dose of 600mg/kgBB, which gives the best results in reducing MDA and LPO-4HNE levels, and increasing SOD activity close to the neutral group.
