Abstract

Tooth extraction remains a common dental procedure in Indonesia owing to the increasing prevalence of oral health problems. Despite standard operating procedures and analgesics, pain management following tooth extraction remains a challenge. By analyzing different analgesic options and their associated costs, this study seeks to provide valuable insights for healthcare providers and patients in selecting the most effective and affordable pain management strategies. This study aimed to assess the cost-effectiveness of analgesic drugs for pain management after tooth extraction. This cross-sectional study was conducted on 92 patients who underwent dental extractions in Pabatu Hospital, Serdang Bedagai. Secondary data from the medical records were analyzed to assess patient demographics, diagnoses, analgesics used, pain levels, and treatment costs. Cost minimization analysis (CMA) was employed to compare the direct medical costs associated with different analgesics. The analysis highlighted that the combination of paracetamol and Na-diclofenac had the lowest average cost per patient (IDR 238,150.00), making it a financially viable option for both healthcare providers and patients. In contrast, the average cost of ibuprofen was significantly higher (IDR 433,954.97), suggesting that it may not be the best choice when considering both effectiveness and expense. Approximately 80% of the total direct medical costs for all three analgesics are attributed to procedure and treatment expenses, indicating that the choice of analgesics has a relatively small impact on overall costs. These findings advocate the use of cost-effective analgesic combinations in clinical practice, particularly in settings in which budget constraints are a concern.

Abstract

Ovarian cancer is a serious disease that affects the ovaries, and its early detection is challenging due to vague symptoms often dismissed as minor ailments. Currently, natural sources have gained attention for their potential role in anticancer treatment. This study aimed to utilize network pharmacology to explore the potential targets and mechanisms of Zanthoxylum acanthopodium in the treatment of ovarian cancer. This study utilized the KNApSAcK and Swiss Target Prediction to identify active compounds and target genes. Additionally, ovarian cancer-specific target genes were sourced from the GEO database. To identify possible key target genes, the network interaction between protein-protein using the STRING database and visualized them in Cytoscape. Subsequent analysis using Gene Ontology (GO) and the Kyoto Encyclopedia of Genes and Genomes (KEGG) pathways enabled us to focus on primary therapeutic targets. Our investigation into Zanthoxylum acanthopodium revealed 10 active compounds that pass Lipinski rule of five and oral bioavailability with acceptable pharmacokinetic profiles, 88 therapeutic targets, and identified 5 hub genes: SRC, CCNB2, MMP9, PTGS2, and PTPRC, which are strongly associated with ovarian cancer progression. Pathway enrichment analysis highlighted several pathways significantly related to the pathogenesis of ovarian cancer. This study elucidates the therapeutic potential and mechanisms of action of Z. acanthopodium as a promising candidate for ovarian cancer treatment. However, further research, including both in vitro and in vivo studies, is necessary to understand its molecular mechanisms comprehensively.

Abstract

Background: Type 2 diabetes mellitus (DM) is a chronic disease that continues to increase globally, with a significant health and economic burden. A balanced diet is a key component of type 2 DM management. Purpose: To analyze the factors that influence balanced diet behavior in patients with type 2 DM in Pekanbaru using the Theory of Planned Behavior (TPB) approach. Method: This study used a quantitative method with a cross-sectional design. Data were collected from 117 patients with type 2 diabetes who were registered at the Pekanbaru Health Center. The variables studied included knowledge, attitude, family support, and the role of health workers in balanced diet behavior. Data analysis was performed using chi-square and Odds Ratio (OR) tests to determine the relationship and the chance of influence of independent variables on the dependent variable. Results: The results showed that low knowledge (OR=4.9), negative attitude (OR=2.5), lack of family support (OR=2.8), and non-optimal role of health workers (OR=3.2) had a significant influence on balanced diet behavior in patients with type 2 diabetes (p<0.05). Conclusion: Factors such as knowledge, attitude, family support, and the role of health workers significantly influence the balanced diet behavior of patients with type 2 diabetes. Suggestion: Education-based interventions and strengthening social support are needed to improve patient adherence to a balanced diet.

Abstract

The global rise in diabetes mellitus (DM) among women of reproductive age poses significant challenges to achieving optimal exclusive breastfeeding (EBF) rates, especially in low- and middle-income countries such as Indonesia. This study aimed to examine the relationship between knowledge and experience of mothers with DM and their independence in providing EBF, addressing an urgent need for targeted intervention and policy support for this vulnerable population. An analytic observational study with a cross-sectional design was conducted at community health centers in Pekanbaru from June to November 2024, involving 98 breastfeeding mothers diagnosed with DM, recruited through convenience sampling. Structured questionnaires measured maternal knowledge, prior breastfeeding experience, and independence in EBF. The results showed that 57.1% of mothers had good knowledge and 53.1% had adequate breastfeeding experience, while 69.4% demonstrated independence in providing EBF. Bivariate analysis revealed significant relationships between knowledge and independence (p = 0.026) as well as experience and independence (p = 0.001). These findings underscore the importance of enhancing education and support for mothers with DM to improve EBF practices. It is recommended that healthcare policies and programs prioritize continuous, tailored education and counseling to empower mothers with DM and increase EBF coverage.

Abstract

Hepatic steatosis (HS), characterized by the abnormal accumulation of triglycerides within hepatocytes, is a prevalent pathological condition. However, its detection rate often underestimates its true prevalence, particularly when assessed using abdominal computed tomography (CT) scans. This quantitative cross-sectional study, conducted from September 2024 to February 2025 at Royal Prima Hospital, aimed to investigate the associations of perirenal fat thickness (PrFT), abdominal subcutaneous fat thickness (ASFT), and renal sinus fat diameter (RSFD) with HS. A non-probability sampling method was utilized, and a total of 272 non-contrast abdominal CT scans were analyzed. Statistical analyses were performed using SPSS software. HS was defined as an average hepatic parenchymal Hounsfield Unit (HU) value at least 10 HU lower than that of the spleen, with an absolute hepatic HU attenuation of less than 40. The grading of HS was determined according to the CT liver-spleen (L-S) ratio: mild (0.7 < CT L-S < 1.0), moderate (0.5 < CT L-S < 0.7), and severe (CT L-S < 0.5). The results demonstrated a significant association between the mean right-left PrFT and the presence of HS (p = 0.007), suggesting that perirenal fat may contribute to the development of HS. In contrast, neither the mean right-left RSFD nor the ASFT showed a significant association with HS presence (p = 0.056 and p = 0.904, respectively). Furthermore, none of the fat measurements (PrFT, ASFT, and RSFD) were significantly associated with the grading of hepatic steatosis (p = 0.800, 0.288, and 0.996, respectively). These findings underscore the potential utility of PrFT as a non-invasive indicator for HS diagnosis. The study also highlights the importance of quantitative measurements, such as hepatic and splenic HU values and CT L-S ratios, for the accurate diagnosis of HS, as visual assessment alone may be insufficient.

Abstract

Eye health is often overlooked, leading to many eye diseases that, if not properly treated, can cause visual impairment. Visual impairment remains a significant health issue both globally and in Indonesia, as it can interfere with a person’s daily activities. A tangible impact of impaired visual acuity is a decline in academic performance due to difficulty seeing writing from a distance, ultimately preventing children from fully absorbing the lessons taught. This community service activity aims to increase public knowledge and awareness of eye health through health education and direct eye examinations in Kota Baru Sub-district, Jambi City. The methods used include eye examinations, counseling, and distribution of takjil. The results of this service showed strong public enthusiasm and an improvement in understanding the importance of maintaining eye health. This activity is expected to have a long-term impact on healthy lifestyle behaviors related to vision.
