Abstract

Early Warning Scoring (EWS) is a clinical instrument designed to detect early deterioration in a patient's medical condition by monitoring vital signs such as pulse, blood pressure, and respiratory frequency. This early detection allows health workers to carry out medical interventions quickly and precisely, thereby increasing the chances of patient safety. However, the implementation of EWS at Pindad Turen Hospital is still not running optimally. The main problem found was the lack of integration in the assessment and recording of EWS scores by health workers, which resulted in delays in clinical decision-making. This study aims to analyze the level of EWS utilization in the hospital through outreach and training activities. Before the training, a pretest was carried out, and after the training, a posttest was carried out to measure the increase in knowledge of health workers. The results show that there is no uniform standard for writing EWS scores. As a form of intervention, the supervision function is strengthened as cues to action whose role is to encourage and guide health workers in the correct implementation of EWS. Through the active involvement of supervision, it is hoped that there will be increased consistency and accuracy in the assessment and recording of EWS scores by nurses in each service unit.

Abstract

Effective nursing documentation and optimal nurse performance are essential pillars of high-quality healthcare services. Despite standardised guidelines being available, incomplete documentation and low compliance rates persist in various hospital settings. This study investigated the impact of implementing the 3S-based Nursing Care Guidelines, comprising standardised nursing diagnoses (SDKI), interventions (SIKI), and outcomes (SLKI), along with structured supervision by the hospital's nursing committee, on improving documentation quality and nurse performance. Employing a true experimental pre-posttest control group design, the research involved 49 nurses allocated into three groups: control, intervention with 3S, and intervention with 3S combined with the supervision nursing committee. Data were collected through observation sheets, document audits, and performance checklists. Statistical analyses using Wilcoxon, Mann-Whitney U, and Kruskal-Wallis tests revealed significant post-intervention improvements (p = 0.000) in both outcome areas. The results affirm that integrating structured care models and active supervision effectively enhances the quality of nursing care delivery.

Abstract

Validitas data dipastikan melalui triangulasi sumber. Abstract   The quality of public services, especially in primary health facilities such as Community Health Centers, is a vital element in the national health system and is a benchmark for public satisfaction. Regulation of the Minister of State Apparatus Empowerment and Bureaucratic Reform (Permenpan RB) Number 14 of 2017 provides standard guidelines for the implementation of the Community Satisfaction Survey (SKM) to evaluate the quality of these services. The Duduksampeyan Community Health Center, as one of the public service delivery units, needs to analyze the implementation of these guidelines in practice. This study aims to explore and analyze the implementation of Permenpan RB No. 14 of 2017 at the Duduksampeyan Community Health Center, Gresik Regency, with a focus on the nine service indicators contained therein. This study uses a descriptive method with a qualitative approach. Primary data collection was conducted through semi-structured in-depth interviews with 15 health worker informants selected by purposive sampling from various service clusters (Mothers and Children, Adults and Elderly, Cross-Cluster) at the Duduksampeyan Health Center. Data were analyzed inductively based on nine indicators of Permenpan RB No. 14 of 2017, namely Requirements; System, Mechanism, and Procedure; Completion Time; Cost/Tariff; Product Specification Type of Service; Implementer Competence; Implementer Behavior; Handling of Complaints, Suggestions and Input; and Facilities and Infrastructure. Data validity was ensured through source triangulation. The results of the study indicate that the implementation of Permenpan RB No. 14 of 2017 at Duduksampeyan Health Center has generally run well on seven of the nine indicators: Requirements; Systems, Mechanisms, and Procedures; Costs/Tariffs; Product Specification of Service Types; Implementer Competence; Implementer Behavior; and Handling of Complaints, Suggestions, and Input. However, this study identified challenges and less than optimal results in the other two indicators, namely the Service Completion Time which is sometimes still long due to high patient volume or referral process, and the condition of Facilities and Infrastructure which still require continuous improvement and maintenance. It is concluded that Duduksampeyan Health Center has implemented most aspects of the guidelines of Permenpan RB No. 14 of 2017 quite well. However, continuous improvement efforts need to be focused on increasing the efficiency of service time and optimizing the condition of facilities and infrastructure in order to improve the quality of service as a whole and meet the expectations of community satisfaction

Abstract

This study analyzes the influence of knowledge, motivation, and skills of health workers on the reporting of patient safety incidents at the West Kotawaringin Regency Health Center. This study uses an observational design with a cross-sectional approach. The research sample consisted of 116 health workers who were randomly selected in six health centers in West Kotawaringin Regency. The independent variables in this study were knowledge, motivation, and skills, while the dependent variable was patient safety incident reporting. Data were collected using questionnaires that had been tested for validity and reliability, then analyzed using binary logistic regression. The results showed that knowledge (p=0.001), motivation (p=0.002), and skills (p=0.000) significantly affected patient safety incident reporting. Simultaneously, these three variables contributed 56% to patient safety incident reporting. Good knowledge increases understanding of the importance of reporting, high motivation encourages reporting consistency, and adequate skills ensure reporting is done correctly and efficiently. The conclusion of this study confirms the importance of improving the knowledge, motivation, and skills of health workers in supporting patient safety incident reporting systems. As a recommendation, Puskesmas are advised to organize continuous training that includes aspects of knowledge, motivation, and skills, as well as providing incentives to increase the motivation of health workers. The implementation of this strategy is expected to improve the quality of incident reporting and improve patient safety.
