Abstract

Tingkat kepuasan pasien terhadap pelayanan keperawatan di Indonesia mayoritas masih kurang puas. Penyebab ketidakpuasan pasien diantaranya faktor kesalahan identifikasi, komunikasi, pemberian obat, dan risiko jatuh. Penelitian bertujuan untuk mengidentifikasi hubungan penerapan keselamatan pasien dengan kepuasan pasien di Rumah Sakit X. Desain penelitian menggunakan pendekatan cross sectional dengan menyebarkan kuesioner kepada 143 pasien. Pengambilan sampel menggunakan cluster random sampling dengan cara menetapkan jumlah sampel yang memenuhi kriteria inklusi, kemudian diberikan kuesioner hingga terpenuhi jumlah sample, dan melakukan penelitian pada setiap sampel yang terpilih. Data dianalisis menggunakan independent t-test dan uji chi-square. Hasil penelitian didapatkan ada hubungan penerapan keselamatan pasien dengan kepuasan pasien (p= 0,001; OR=1,216; α= 0,05). Karakteristik pasien berupa umur, jenis kelamin, pendidikan, pekerjaan, dan kelas rawat tidak berhubungan dengan kepuasan pasien (p= 0,331; 0,818; 0,949; 1,000; dan 0,382; α= 0,05). Hasil penelitian juga didapatkan bahwa penerapan aspek keselamatan pasien berupa reassessment pasien risiko jatuh dan dimensi kehandalan (memberi petunjuk, memberi penjelasan) ketika akan melakukan tindakan keperawatan masih belum optimal sehingga menjadi saran untuk ditingkatkan agar kepuasan pasien di rumah sakit X semakin meningkat. Abstract Patient Satisfaction Against Patient Safety Application at Hospital. The level of patient satisfaction with nursing services in Indonesia is mostly unsatisfied. The cause of patient dissatisfaction is due to misidentification, miscommunication, medication errors, and inappropriate risk management of falls. This study aimed to identify the relationship between the application of patient safety and patient satisfaction at Hospital X. The study design used a cross-sectional approach by distributing questionnaires to 143 patients. Sampling using cluster random sampling by determining the number of samples that meet the inclusion criteria, then given a questionnaire until the number of samples is met, and conduct research on selected samples. Data were analyzed using independent t-test and chi-square test. The results showed that there was a correlation between the application of patient safety and patient satisfaction (p= 0.001, OR= 1.216; α= 0.05). Patient characteristics in the form of age, gender, education level, occupation, and nursing class were not related to patient satisfaction (p= 0.331; 0.818; 0.949; 1,000; and 0.382; α= 0.05). The results also found that the application of patient safety aspects in the form of reassessing patients to falling risks and dimensions of reliability (giving instructions, giving explanations) when going into nursing actions was not optimal so that suggestions were increased so that patient satisfaction at hospitals X increased. Abstract

This study aims to optimize the nurse manager's planning function based on identifying needs or solving problems in the service unit. The method used is a case report by conducting interviews, observing using observation sheets, and distributing questionnaires. The study results show optimizing the planning function of nurses as managers of nursing services. Preparation of draft Guidelines and SPO for home health care services is a solution. Implementation is carried out through Lewin's three phases process planned change approach. In conclusion, nurses need to show that nurses have a responsibility to ensure patient safety with adequate competence, including by maximizing the function of nurses as managers of nursing services by optimizing the planning function of nurse managers. Abstract

This research aims to conduct resocialization by providing repeated understanding and service guidelines as a reference in services that staff can follow and implement. The method used is a pilot study using observation techniques, interviews, and questionnaires with the head of the room managerial function approach. The study results showed that the resocialization of cardiovascular palliative service guidelines with audio-visual methods in the form of videos made staff more familiar with and understanding cardiovascular palliative care. Resocializing service guidelines in cardiovascular palliative unit services show increased staff knowledge about cardiovascular palliative services. Abstract

Introduction: Clinical nurse leaders (CNL) play a significant role in improving the quality of nursing services. Such a role includes satisfying clients during the care process by referring to the nursing goal. In meeting the standard, CNL should enhance their knowledge, attitude, and soft and hard skills. Purpose: Identifying the competences of CNL through ADDIE model-based learning. Method: The present study relied on a pre-experimental design with a one-group pre-test and post-test.This study used quantitative research with the pre experiment method, and pre and post test.As many as 20 clinical nurse leaders from inpatient units of accredited public and private hospitals in Singkawang were involved as the sample. Further, the data analysis in this study used Paired T test. The result of this study found a meaningfulimprovement of competences among CNL through ADDIE model-based learning (p=.006). Recommendation: ADDIE model-based learning attempts to enhance the competences of CNL. This research recommends the addition of the ADDIE model to advance the competence as the clinical nurse leaders.  Moreover, this strategy can be applied to develop CNL’s ability in other managerial fields.

Abstract

This research aims to describe the planned change process using Kurt Lewin's theory to optimize effective communication during ward handovers by improving the nursing manager's function. The method used is a case study including assessment activities, problem analysis, preparation of planning of action (POA), implementation, and evaluation. The research results show that in the unfreezing phase, problems were found that the performance of effective communication during the handover was not optimal, so the priority strategy for solving the problem was optimizing the planning function for handover activities. In the movement phase, a POA was prepared with indicators that were successfully achieved, namely the preparation of a revised handover SPO draft and socialization of the revised SPO draft with the nursing sector, Head Nurse, and Clinical Care Manager. In contrast, this could not be done in the refreezing phase due to time constraints, so a follow-up plan was prepared and agreed with the PIC of the activity. In conclusion, the planned change process towards optimizing effective communication during handover requires an increase in the function of nursing managers, especially in the planning function. Managers must provide policies, one of which is SOP handover, and must review and revise policies to remain new and can be applied. Abstract

This study report aims to identify planned changes in the initiation of optimization of the planning function in Linen Management. This method uses a case study approach using interviews, document review and observation, and Ishikawa's analysis. the results of the study shared that the application of the planning function in linen handling had not been optimal, as a result it had an impact on nursing services in the room. The management of linen in the room does not yet have standard guidelines and regulations for managing linen in each room. In conclusion, optimizing the planning function of the Head Nurse in linen management through drafting guidelines for linen management, the leadership's efforts to strengthen and sustain changes can be made by ratifying linen management regulations.Keywords: planning function, service quality, linen management, planned change.

Abstract

 AbstrakSupervisi merupakan fungsi manajemen pengarahan yang perlu dilakukan oleh perawat manajer secara terstruktur dan sistematis. Supervisi adalah aktivitas dari fungsi pengarahan yang memerlukan perencanaan seperti pembuatan jadwal yang tersusun secara terstruktur, agar dapat diimplementasikan dengan baik dan dapat meningkatkan pelayanan kesehatan secara tepat serta profesional. Tujuan penelitian ini untuk mendorong optimalisasi kegiatan supervisi berjenjang secara lebih sistematis dan terstruktur. Metode yang digunakan adalah pilot study dengan pendekatan program inovasi dan problem solving. Pengumpulan data dilakukan dari bagian Substansi Pelayanan Keperawatan, kepala ruangan, Clinical Instructor (CI), Primary Nurse (PN), dan Perawat Asosiet (PA) dengan teknik  dengan wawancara, observasi, dan penyebaran kuesioner. Hasil penelitian menunjukkan bahwa: 1) tahap identifikasi masalah, perlu peningkatan dalam pelaksanaan supervisi keperawatan berjenjang; 2) tahap pelaksanaan, perlu adanya pengembangan panduan, Standar Prosedur Operasional (SPO), dan format penilaian, pembuatan jadwal, sosialisasi, dan uji coba pelaksanaan supervisi keperawatan berjenjang; 3) tahap evaluasi, perlu pendampingan, monitoring evaluasi, dan penyebaran kuesioner terkait pelaksanaan uji coba supervisi keperawatan berjenjang. Simpulan, optimalisasi pelaksanaan supervisi keperawatan berjenjang perlu adanya kebijakan dan dukungan dari pihak manajemen rumah sakit dan manajer keperawatan yang ada di RS X. Diharapkan agar hasil dari kegiatan supervisi keperawatan berjenjang yang belum optimal dapat ditindak lanjuti dengan program Pengembangan Profesional Berkelanjutan. Kata kunci: Supervisi Berjenjang, Sistematis, Terstruktur, Perawat

Abstract

 Anxiety is an emotion that is often experienced by patients who admitted to the ICU. Anxiety occurs due to the ofenvironmental stressors, multiple invasive actions, feelings of loneliness and fear of death. There are severalcomplementary therapies that are feasible to be administered to reduce anxiety levels, such as music therapy and murotaltherapy. The purpose of this study was to identify the application of music therapy and murotal therapy to reduce anxietyin patients treated in the intensive care. The study design used a case study on 1 respondent with the criteria of patientsbeing treated in the ICU > 48 hours on a ventilator, with compos mentis awareness. This study was conducted for 5 days.The results showed that there was a decrease in the level of anxiety of patients who were treated before and after givenmusic therapy and murotal therapy . Before the intervention was applied, Mrs. I anxiety score is 86 (moderate) and afterbeing administered for 5 days, the anxiety of the respondent decreased with score of 62 (mild). The pain level in Mrs. Ibased on Behavior Pain Scale (BPS) has decreased from scale of 6 (moderate) to scale od 3 (mild). Based on these twomeasuring instruments, the vital signs which previously rose within the upper normal limit decreased to within the normalrange. Based on the data this intervention is considered effective in relieving anxiety, pain, and making vital signs stablewithin the normal range experienced by critical care patient.

Abstract

Healthy behavior carried out by nurses often does not produce good results due to several factors. The risks that arise due to not behaving healthy nurses can reduce the health condition of nurses and the quality of service. The purpose of this study was to identify the factors that can influence the behavior of healthy nurses. The research method used a cross-sectional design to 356 nurses at the General Hospital in Jakarta. The results of the analysis showed that there was a significant relationship between age 35 years (p = 0.038) and education D3 Nursing/Midwifery (p = 0.034) with healthy nurse behavior. In multivariate analysis, the most influential variables on the behavior of healthy nurses were physical activity (p<0.05; OR = 4.760, 95%CI), spiritual (p=0.211; OR = 1.456, 95%CI) and stress management (p< 0.05; OR = 4.549, 95%CI). Recommendations to improve the behavior of healthy nurses by making healthy nurse programs on a regular basis.

Abstract

This study aims to improve the implementation of the handover between shifts through the direction of the head of the room such as motivation, communication, delegation and supervision in two maternity wards consisting of one room head and fifteen nursing staff. The study was conducted for five weeks at X Hospital with data obtained through interviews, observations and surveys. The method used is a case study through problem analysis using a fishbone diagram and then making innovative changes using Lewin's 3-step model approach. Implementation is carried out for one week at the time of handing over the morning shift. The results showed that the program was still in the movement stage, the handover time was adequate according to Standard Operating Procedures, the head of the room supervised two wards alternately, surveys of nursing staff related to the function of directing the head of the room increased by 6.7%. In conclusion, the implementation of the briefing function carried out by the head of the room in the handover process between shifts in the two maternity rooms of X Hospital has increased based on the results of the questionnaire distributed to the nursing staff. The results of the implementation of supervision show that the time required in the handover process is more adequate and in accordance with the existing SPO. The recommendation is that the hospital needs to continue this program to the next stage, the nursing sub-services need to make regulations on nursing handover guidelines and carry out tiered supervision in the wards.
