Abstract

The prevalence of chronic energy deficiency in pregnant women in Indonesia is still high. This condition is one of the unresolved nutritional problems such as stunting. This study aims to develop the Angraini Model, as an effort to early detection of chronic energy deficiency in pregnancy. This research is a quantitative study with a case-control design on 190 CED and non-CED pregnant women in the city of Bandar Lampung. The research took time from October 2018 to July 2021. The data used in this study are 18 indicators and 7 latent variables. Latent variables consist of socioeconomic (education, employment, income, knowledge), culture (age, parity, food taboo), BMI (prepregnancy BMI), laboratory (anemia, iron status, protein status), food intake (energy, protein, fat carbohydrates, iron), weight gain during pregnancy (pregnancy weight gain), and CED (chronic energy deficiency). Data were analyzed using a structural equation model (SEM) with Lisrel software and then built into a web-based expert system. The results of the SEM analysis stated that food intake, laboratory values, and weight gain during pregnancy directly affect the incidence of CED. socioeconomic variables (knowledge, education, employment, and income), culture (age, parity, and food taboo), and prepregnancy BMI indirectly affect the incidence of CED through food intake variables. The model obtained based on SEM analysis is then built in a web-based expert system with the address modelangraini.com. The Angraini model is a web-based expert system that can be used to detect early CED in pregnant women for health workers in primary healthcare facilities.

Abstract

The immunization program is a global program to prevent child deaths and provide immunity against infectious diseases. In Padang City, complete basic immunization coverage (IDL) in 2021 is still low (76.5%) compared to the target of 95%. This research aims to analyze the achievements of the IDL program in Padang City during the adaptation period to the COVID-19 pandemic. This research method is mixed methods research with a stratified random sampling technique. The results showed that the age frequency distribution of the baby's mothers was mostly 26-40 years old (83.2%), with a high level of education (78.4%) and more people working (52%). The frequency distribution of IDL status in Padang City shows an incomplete category of 67.2%. There is a significant relationship between the level of education, level of knowledge, attitudes and roles of health workers, and IDL status. The most dominant factor is the level of knowledge. IDL coverage is still low due to mothers' lack of understanding about the immunization schedule due to mothers' busy lives, family restrictions, and socio-cultural factors.

Abstract

Adolescent pregnancy, especially in low- and middle-income countries, is a global concern with significant health implications for both mothers and infants. This research compares the effectiveness of two interventions, the Continuity of Midwifery Care (CoMC) model and the Maternal and Child Health (MCH) Book, in enhancing family and spousal support for adolescent pregnant mothers. Using a quantitative approach with a quasi-experimental design, the study is conducted in Padang City over four months, involving 20 Independent Midwife Practices (IMP). Purposive sampling is employed based on specific criteria. Data collection utilizes questionnaires measuring family and spousal support in emotional, appreciation, instrumental, and informational dimensions, which underwent validation and reliability testing. Preliminary results indicate that both interventions improve family and spousal support, with the CoMC model showing greater efficacy. Appreciation support, emotional support, instrumental support, and informational support play crucial roles in positively impacting adolescent pregnant mothers' well-being. The CoMC model excels in delivering personalized care, emphasizing emotional support, providing practical assistance, and offering timely and relevant information. In conclusion, this study underscores the importance of family and spousal support in adolescent pregnancies and highlights the effectiveness of both interventions. The CoMC model stands out as a more comprehensive and personalized approach. Implementing this model in healthcare policies can significantly enhance care quality for pregnant adolescents, fostering a supportive environment for both mothers and infants.

Abstract

Background: Hypertension in pregnancy needs to beware out because it can progress to more severe conditions, namely preeclampsia and eclampsia, thereby endangering the mother and the unborn baby. Risk factors in Hypertension in pregnancy are as age, education, occupation, parity, gestational age, economic factors, personal and family history of hypertension, obesity and activity/exercise.Objective: This study aims to determine the risk factors for Hypertension in pregnancy in Deli Serdang Regency, North Sumatra.Method: This study method used analytic observational with a cross sectional design. The study subjects were pregnant women at Public Health Center of Deli Serdang Regency who were considered the inclusion criteria and were selected as study subjects. The sampling technique by means of random sampling amounted to 164. Univariate analysis used to get an overview of the frequency distribution and percentage of the variables to be studied. Bivariate analysis used to determine the relationship between the independent variable and the dependent variable. The statistical test was Chi-Square with CI 95%.  If p < 0.05. Multivariate analysis with logistic regression statistical tests to determine the influence of the independent variables on the dependent variable. Independent variables with p value <0.25 were included in the multiple logistic regression test model and selected using the enter method. The strength of the influence of the independent variable on the dependent variable is the Exp value (ß).Result: The results of the study proved the results of Multivariate analysis, can be synthesized that the risk factors for pregnancy hypertension were influenced by factors: education (p=0.045), parity (p=0.047), gestational age (0p=0.040), personal history of hypertension (p=0.013), history family (p=0.049), and obesity (p=0.016). While the factors of age, occupation, economy and activity/exercise have no effect on risk factors for hypertension in pregnancy. Based on Exp value (B), the most influential factor was personal history of hypertension (p=0.013). This is indicated by the Exp value (B) = 2.710 which is in the interval 1.234 – 5.953, followed by obesity (p=0.016) with the Exp value (= 2.612) which is in the interval 1.192 – 5.725.
