Abstract

Keberhasilan pemberian ASI eksklusif masih belum mencapai target global dan nasional, terutama pada ibu yang bekerja. Lingkungan kerja memiliki pengaruh signifikan terhadap keberhasilan ASI eksklusif melalui penyediaan fasilitas fisik, kebijakan pendukung, dan dukungan sosial. Namun, implementasi kebijakan tempat kerja ramah ASI masih rendah penelitian ini bertujuan untuk menyintesis bukti ilmiah terkait kebijakan tempat kerja ramah ASI terhadap keberhasilan ASI eksklusif. Penelitian ini menggunakan desain Systematic Literature Review. Sebanyak 14 artikel yang memenuhi kriteria inklusi dianalisis secara tematik untuk mengidentifikasi pola, determinan, dan pengaruh kebijakan tempat kerja terhadap praktik ASI eksklusif. Sintesis menunjukkan bahwa keberhasilan ASI eksklusif dipengaruhi kuat oleh beberapa faktor utama, yaitu: ketersediaan fasilitas laktasi yang memadai; durasi cuti melahirkan; pumping break dan fleksibilitas kerja; dukungan sosial dari supervisor dan rekan kerja;  lemahnya implementasi kebijakan meskipun regulasi tersedia; pengaruh budaya kerja dan norma social; efektivitas intervensi komprehensif workplace breastfeeding support. Kebijakan tempat kerja ramah ASI memiliki peran penting dalam meningkatkan keberhasilan ASI eksklusif pada ibu bekerja. Implementasi yang kuat, fasilitas yang memadai, dukungan sosial yang positif, serta intervensi komprehensif diperlukan agar ibu bekerja dapat mempertahankan praktik ASI eksklusif. Reformasi kebijakan, pengawasan implementasi, dan kolaborasi lintas sektor menjadi kunci keberhasilan.Kata kunci: ASI eksklusif, tempat kerja ramah ASI, kebijakan laktasi, workplace breastfeeding support, ibu bekerja.

Abstract

Integrasi tiga kebijakan utama dalam Jaminan Kesehatan Nasional Indonesia, Kelas Rawat Inap Standar (KRIS), sistem rujukan berbasis kompetensi, dan sistem pembayaran iDRG, menjadi strategi penting untuk meningkatkan kualitas, keadilan, dan efisiensi layanan kesehatan. Penelitian ini bertujuan untuk menelaah bagaimana ketiga kebijakan tersebut saling terintegrasi menggunakan kerangka Walt & Gilson serta mengevaluasi dampaknya terhadap manajemen rumah sakit, kepuasan peserta, dan pembiayaan nasional. Metode yang digunakan adalah tinjauan dokumen kebijakan dan analisis literatur resmi dari Kemenkes, BPJS Kesehatan, DJSN, serta fasilitas layanan kesehatan. Analisis menyoroti tujuan dan isi kebijakan, latar belakang dan dinamika eksternal, proses perumusan dan implementasi, serta aktor kunci yang terlibat. Hasil kajian menunjukkan setiap kebijakan mendukung peningkatan ekuitas, mutu layanan, dan efisiensi klaim, namun harmonisasi dan koordinasi antar-pemangku kepentingan tetap diperlukan. Rekomendasi terkait strategi kolaborasi, peningkatan kapasitas SDM, dan pemantauan implementasi diharapkan dapat memperkuat efektivitas kebijakan serta keberlanjutan JKN di Indonesia.Kata Kunci: Kelas Rawat Inap Standar; sistem rujukan; iDRG; JKN; kebijakan Kesehatan

Abstract

Indonesia continues to face nutritional challenges, with a high prevalence of stunting. incidence Padang Lawas Regency is one of the regions in North Sumatra Province with a relatively high of stunting. Stunting is influenced by several factors, one of which is food security. Food security depends on family income; inadequate family income leads to poor food security, making families unable to meet children's primary and secondary needs. This study aims to analyze differences in food security based on family income and its impact on stunting incidence among children under five in Padang Lawas Regency North Sumatera Province. This research is an analytical survey using a cross-sectional study design. The study was conducted in Padang Lawas Regency, specifically in six sub-districts with the highest number of children under five: Sosa, Hutaraja Tinggi, Barumun, South Barumun, Batang Lubu Sutam, and Sosa Julu. The research was carried out from October 2024 to February 2025. The study population consisted of all families with children aged 6-59 months across the six sub-districts, totaling 9,181 families. Using Slovin's formula, the sample size was determined to be 384 families. A multistage cluster sampling technique was employed for sample selection. Data collection was conducted through interviews and observations. Data analysis included univariate analysis and bivariate analysis using an independent t-test and simple logistic regression with a significance level of 95% (α = 0.05). The study results indicate a significant difference in food security based on family income (P-value = 0.001). Moreover, there is a significant impact of family food security on stunting incidence among children under five in Padang Lawas Regency (P-value = 0.001; Exp.B = 13.908). It is recommended that the Padang Lawas Regency Government improve the local economic system by facilitating job opportunities for the community. Keywords: Family income, food security, stunting

Abstract

Stigma against People with Mental Disorders (ODGJ) remains a major challenge in Indonesia's mental health system. This stigma affects various aspects of their lives, from social discrimination to limited access to mental health services. This study aims to identify the factors influencing stigma against ODGJ in Indonesia through a systematic review of literature published in the last ten years. The analysis focuses on individual, familial, socio-cultural, and structural factors contributing to stigma. The findings indicate that stigma is influenced by the public's lack of understanding of mental health, social pressure experienced by ODGJ families, cultural norms linking mental disorders to mystical beliefs, and limited government policies supporting mental health services. A comprehensive approach, including mental health education, family empowerment, community-based interventions, and improved mental health policies, is needed to reduce stigma and enhance the quality of life for individuals with mental disorders.

Abstract

Dysmenorrhea is menstrual pain that can interfere with activities. Primary dysmenorrhea is a condition that occurs due to changes in prostaglandin hormone levels. In Indonesia, the prevalence of primary dysmenorrhea in 2019 was 54.89%. Dysmenorrhea can be alleviated through non-pharmacological methods, one of which is the progressive muscle relaxation technique, which has no side effects. This study aim to examine the effect of progressive muscle relaxation techniques in reducing primary dysmenorrhea. This study was a literature review using the scoping review method, conducted from April 2024 to February 2025. Data was collected from databases (PubMed, ScienceDirect, Google Scholar, and GARUDA) with publications from 2019 to 2024. The data analysis was carried out using thematic analysis. The selected articles were organized into a PRISMA-ScR (Preferred Reporting Items for Systematic Review and Meta-Analyses extension for Scoping reviews) flow diagram, resulting in 15 analyzed articles. The research results indicate that progressive muscle relaxation techniques have an effect on reducing primary dysmenorrhea. However, factors such as stress experienced by women during relaxation exercises and an unconducive environment may affect the success of the technique

Abstract

Background: Pregnancy anemia is condition when the hemoglobin (Hb) less than 11 g/dL. Based on the Indonesian Healthy Survey in 2023, the prevalance of pregnancy anemia was 27,7%. One of the risk factors was excessive consumption of tea, because it contained tannins that inhibited iron absorption.Purpose: The purpose of this study was to determine the relationship between tea drinking habits and the incidence of anemia in pregnant woman.Methods: This type of research was a literature review with a scoping review method, conducted from March 2024 to March 2025. Data were collected through digital database sources, namely: Pubmed Central, Sciencedirect, Google Scholar, DOAJ, and Garuda, published from 2019-2024. Data were analyzed using thematic analysis.Results: Relevant articles was selected based on inclusion and exclusion criteria, and processed usinh the PRISMA-ScR flow diagram. 21 articles were identified for analysis. 21 articles were analizied by thematic analysis.Conclusion: The study show, that the habit of drinking tea is associated with the incidence of anemia during pregnancy. The frequency of daily tea consumption with an amount of 1, 2, or 3 cups causes anemia and the timing of tea consumption before, after or with meals within a time span of up to 1 or 2 hours is also associated with pregnancy anemia

Abstract

Menarche is the first menstrual period that indicates that a woman has reached biological maturity. The age  of menarche continues to decline over time and is influenced by various factors such as nutrition, genetics, and socioeconomics. The intake of macronutrients such as carbohydrates, proteins, and fats plays a role in the maturation process of reproductive and hormonal organs that affect the age of menarche. Changes in nutrient consumption patterns among adolescents today can have an impact on  the earlier or delayed age shift of menarche which can be a sign of developmental or hormonal problems. This study used an associative analytical design with a cross sectional approach and purposive sampling techniques for 55 respondents. The instrument used was the SQ-FFQ questionnaire to measure macronutrient intake with the results of measuring total energy and menarche  age data obtained through interviews. The results of the study showed that 67.3% of respondents had no more energy intake, 32.7% had more energy intake, and  85.5% had normal menarche age, 14.5% abnormalized menarche age. The results of the analysis of Macronutrient energy intake with menarche age  with the Yates Correction or Continuity Correction test were obtained p = 0.125. These results show that there is no relationship between macronutrient energy intake and menarche age  in adolescent girls at SMPN 13 Padang.Based on the results of the study, it can be concluded that there is no statistically significant relationship between macronutrient energy intake and menarche age. It is hoped that the age  of menarche is influenced by many factors apart from macronutrient intake, namely nutritional quality, hormones, and socio-economic factors.

Abstract

Introduction: COVID-19 vaccination represents a crucial public health strategy for controlling viral transmission by inducing protective immunity and reducing the severity of symptoms. Knowledge levels and social media exposure are among the factors that may influence individuals’ willingness to participate in vaccination programs. This study aimed to analyze the relationships between knowledge level, social media use, and community participation in the COVID-19 vaccination program in Pangkalan Kerinci Kota Urban Village. Methods: An analytic, cross-sectional design was employed involving 106 residents aged 17–65 years selected through proportionate stratified random sampling and consecutive recruitment. Data were analyzed using the Chi-square test. Results:Most respondents demonstrated good knowledge regarding COVID-19 vaccination and reported frequent use of social media platforms. Statistical analysis revealed no significant relationship between knowledge level and participation in the vaccination program (p = 0.610) or between social media use and vaccination participation (p = 0.302). Discussion:The absence of significant associations suggests that vaccination uptake in this population may be influenced more strongly by structural or contextual factors, such as access to health services, trust in local authorities, and availability of vaccination sites, rather than by cognitive understanding or social media exposure alone. High knowledge levels and active social media use do not necessarily translate into health-seeking behavior, indicating a possible gap between awareness and action. Future studies should explore additional determinants, including perception of risk, cultural influences, and policy-driven motivators. Conclusion: Knowledge level and social media use were not significantly associated with participation in the COVID-19 vaccination program in Pangkalan Kerinci Kota Urban Village.

Abstract

Hypertension is a degenerative condition that significantly contributes to global morbidity and mortality, making medication adherence a critical factor in achieving long-term therapeutic success. This study investigates the relationship between patient-related factors and adherence to antihypertensive medication. A cross-sectional analytical survey was conducted among 195 hypertensive patients in Samarinda, East Kalimantan. Data were collected using a structured questionnaire incorporating the MMAS-8 scale, which categorizes adherence into high (score = 8), moderate (score = 6-7), and low (score < 6), alongside a validated instrument assessing sociodemographic, clinical, and behavioral determinants. Statistical analysis employed chi-square tests and odds ratios (OR) with 95% confidence intervals. The majority of patients demonstrated low adherence (65.6%), followed by moderate (28.7%) and high adherence (5.6%). Significant associations were found between adherence levels and gender (P = 0.033, OR = 0.513, 95% CI: 0.276-0.951), knowledge level (P = 0.013, OR = 2.190, 95% CI: 1.176-4.079), duration of hypertension (P = 0.017, OR = 0.483, 95% CI: 0.265-0.881), and insurance status (P = 0.004, OR = 0.080, 95% CI: 0.009-0.680). Knowledge level emerged as the most influential factor in promoting adherence. These findings highlight the importance of targeted educational interventions and sustained support for patients at risk of poor adherence, aiming to optimize antihypertensive therapy and prevent long-term complications. The study advocates for strengthened evidence-based clinical practices and underscores the need for health policy reforms particularly in patient education and insurance accessibility to improve hypertension management in primary care settings. Abstract

The rise in non-communicable diseases (NCDs) is driven by risk factors such as smoking, physical inactivity, low consumption of fruits and vegetables, and alcohol use. Community-based surveillance of these risk factors is essential for NCD prevention; however, the availability and use of accurate data remain limited in many regions. Therefore, research is needed to enhance both the quality and utilization of surveillance data in NCD prevention efforts. This study aims to examine community-based risk factor surveillance activities and their role in preventing NCDs in Padang. A qualitative approach using a phenomenological method was employed. Research informants included individuals responsible for NCD programs at community health centers, health cadres, and members of the productive-age population in Padang in 2024. Informants were selected through purposive sampling. Data were primarily collected via in-depth interviews guided by a structured interview protocol. Interview transcripts were analyzed using an interactive analysis model, which included data reduction, data presentation, and conclusion drawing/verification. Findings indicate that risk factor data collection at integrated health posts was conducted both manually and electronically, with data entered into the Ministry of Health's Asik application. Data processing and analysis were performed using the Asik application, with descriptive interpretations presented in tables and diagrams. Dissemination of risk factor information occurred through mini-workshops, although not all stakeholders were involved. Surveillance results were used to inform NCD prevention programs through counseling and community outreach. Overall, community-based risk factor surveillance data were collected manually and electronically via the Asik application, analyzed descriptively, and disseminated through workshops and follow-up activities supporting NCD prevention. The study highlights the need to improve the quality and use of community-based surveillance data, increase stakeholder engagement, evaluate intervention effectiveness, and assess the impact of surveillance applications on data processing and ongoing monitoring of NCD risk factors.
