Abstract

A manager is a person who controls a business, involving the process of delegating authority to several people in charge with leadership duties, as well as the process of moving, guiding, and controlling all human and material resources to achieve goals. With the challenge of a high workload, a leader must be able to influence his subordinates in order to provide maximum performance. This study aimed to determine the impact of the manager's leadership role in improving employee performance, at the Semen Padang Hospital Outpatient Pharmacy Installation. The design of this study was cross-sectional. This study used a total sampling technique, so that 30 employees who work in the outpatient pharmacy installation were obtained. Data were collected by filling out a questionnaire, then analyzed using Partial Least Square in order to test the relationship between variables. Based on the results of the analysis, the p value was 0.029 and the T-statistic value was 2.188, which means that the role of the manager has a positive and significant effect on employee performance. Thus, it could be concluded that the role of the manager can optimize the performance of employees at the Semen Padang Hospital Pharmacy Installation.Keywords: manager; leadership; employee performance  ABSTRAK Manager adalah orang yang melakukan pengendalian suatu usaha, melibatkan proses delegasi wewenang kepada beberapa penanggung jawab dengan tugas kepemimpinan, serta proses menggerakkan, memberi bimbingan, dan mengendalikan semua sumber daya manusia dan materi untuk mencapai tujuan. Dengan tantangan beban kerja yang tinggi, seorang pimpinan harus mampu memberikan pengaruh terhadap bawahannya agar dapat memberikan performa kinerja yang maksimal. Penelitian ini bertujuan untuk mengetahui dampak peran kepemimpinan manager dalam meningkatkan kinerja pegawai, di Instalasi Farmasi Rawat Jalan Semen Padang Hospital. Rancangan penelitian ini adalah cross-sectional. Penelitian ini menggunakan teknik total sampling, sehingga didapatkan 30 orang karyawan yang bertugas di instalasi farmasi rawat jalan. Data dikumpulkan melalui pengisian kuesioner, lalu dianalisis menggunakan Partial Least Square dalam rangka menguji hubungan  antar variabel. Berdasarkan hasil analisis, didapatkan nilai p adalah 0,029 dan nilai T-statistik adalah 2,188, yang bermakna bahwa peran manager berpengaruh positif dan signifikan terhadap kinerja pegawai. Dengan demikian bisa disimpulkan bahwa peran manager dapat mengoptimalisasi kinerja pegawai Instalasi Farmasi Semen Padang Hospital.Kata kunci: manager; kepemimpinan; kinerja pegawai

Abstract

Protein 16 (p16) merupakan salah satu protein supresor tumor yang memiliki peran dalam patogenesis Karsinoma Sel Skuamosa (KSS) kepala dan leher. Peran protein ini berbeda sesuai dengan faktor risiko paparan karsinogen dan memiliki implikasi terhadap faktor prognostik patologik. Tujuan: Menentukan hubungan protein 16 dengan derajat histopatologik dan Invasi Perineural (IPN) pada KSS kepala dan leher. Metode: Penelitian ini merupakan cross-sectional study dengan sampel sebanyak 60 kasus KSS kepala dan leher yang telah didiagnosis di Laboratorium Patologi Anatomik RSUP Dr. M. Djamil Padang periode Januari 2016 sampai Desember 2018. Sampel diperoleh secara simple random sampling dari blok parafin yang berasal dari jaringan tumor. Dilakukan penilaian ulang pada slaid histopatologik untuk menilai derajat histopatologik dan invasi perineural. Ekspresi p16 pada sel tumor dilihat dengan pulasan imunohistokimia. Korelasi dilakukan analisis statistik bivariat menggunakan uji Chi-square dengan tingkat kemaknaan p<0,05. Hasil: Ekspresi p16 positif didapatkan pada 24 kasus (40%) dengan kasus terbanyak pada KSS kepala dan leher derajat tinggi, sedangkan pada 36 kasus (60%) tidak terdapat ekspresi p16. Ekspresi p16 positif memiliki hubungan yang bermakna dengan derajat histopatologik (p = 0,002) namun tidak terdapat hubungan antara ekspresi p16 dengan IPN (p = 1,000). Simpulan: Ekspresi p16 positif berhubungan dengan derajat histopatologik tumor yang lebih tinggi namun p16 tidak tidak berhubungan dengan kejadian invasi perineural pada KSS kepala dan leher.Kata kunci: derajat histopatologik, ekspresi p16, invasi perineural, KSS kepala dan leher

Abstract

The low achievement of hypertension services in the work area of the Padangsidimpuan City Health Office needs to be analyzed on the level of knowledge and attitude of patients, the availability of facilities and infrastructure, and the support of health workers. System components are also analyzed to find out how input, process and output elements are in hypertension services. The research method combines quantitative and qualitative methods sequentially. A sample of 107 respondents was taken proportionally random sampling in the hypertensive population aged ≥ 15 years. The results of the study revealed that 42.1% of patients had low knowledge, 62.6% of patients had a negative attitude, and 57% of the support of health workers was negative. In the system analysis, (input elements) are known to be inadequate in the availability of human resources and the availability of funds. It was concluded that public attitudes, support of health workers, human resources and inadequate budgets have a relationship with low hypertension service achievement. The author's recommendations include the importance of improving public attitudes through more optimal education. The role of the government must be more decisive on the availability of human resources and consistently provide support for the hypertension program budget.

Abstract

Latar Belakang: Peningkatan mendadak kasus gangguan ginjal akut telah terjadi di Indonesia sejak Juli 2022 lalu. Kasus ini bersifat cepat dan progresif yang tidak dapat dijelaskan penyebabnya sehingga disebut dengan gangguan ginjal akut progresif atipikal (GgGAPA). Objektif: Mengetahui profil klinis dan luaran pasien anak yang dirawat di Pediatric Intensive Care Unit (PICU) RSUP Dr. M. Djamil Padang. Metode: Penelitian ini bersifat analitik retrospektif dengan pengambilan data rekam medik pasien anak yang didiagnosis GgGAPA yang dirawat di PICU RSUP DR. M. Djamil Padang pada tahun 2022 dengan metode total sampling. Hasil: Hasil penelitian ini didapatkan 26 pasien GgGAPA. Pasien GgGAPA sebagian besar perempuan (53,8%), berusia 1–5 tahun (46,2%), dan berstatus gizi baik (73,1%). Pasien GgGAPA diketahui memiliki riwayat demam (84,6%), edema (96,2%), gejala gastrointestinal (69,2%), dan gejala respirasi (65,4%). Penurunan output urin terjadi pada 23 pasien (88,4%) dan 21 pasien memiliki skor PELOD-2 (pediatric logistic organ dysfunction) awal ≤5 (80,8%). Kesan laboratorium ditemukan peningkatan neutrophil to lymphocyte ratio (NLR), kreatinin serum, ureum darah, marker inflamasi, dan penurunan laju filtrasi glomerulus (LFG). Terapi yang diberikan berupa steroid, intravenous immunoglobulin (IVIG), antikoagulan, dialisis, fomepizole, dan ventilator mekanik. Angka kematian pasien adalah 15 dari 26 pasien (57,7%). Kesimpulan: Kesimpulan pada penelitian ini adalah GgGAPA merupakan penyakit dengan mortalitas yang tinggi.

Abstract

Tobacco and cigarette problems in Indonesia are quite problematic. Tobacco industries have significant impact on Indonesian economic through creating employment opportunities informing and manufacturing sectors. The government also gains significant revenue from taxes on tobacco production and trade. Due to these economic benefits. Indonesia does not have adequate regulation for tobacco control. However, the prevalence tobacco consumption is high. This condition causes high prevalence of many severe diseases such as cardiovascular, respiratory and cancer which leads to high morbidity and mortality. Even, if we compare the economic benefits and cost for the health problems, the cost for negative impact is far more than the benefits. Based on these evidences. Indonesia should control tobacco production. Manufacture and trade. Some efforts should be done such as maintaining, taxes rate in current level, advertising regulation. Health warning on cigarette package, education campaign, free smoking in public spaces, create morejob opportunities and law enforcement.

Abstract

Pembangunan merupakan sebuah upaya multidimensi yang meliputi pertumbuhan ekonomi, kesinambungan ketersediaan kebuluhan pokok, lapangan pekerjaan, pendidikan, pelayanan kesehatan dan kesetaraan gender. Berdasarkan sudut pandang ini, pembangunan di Indonesia mengalami peningkatan dalam dekade terakhir. Pertumbuhan ekonomi meningkat sejak tahun 1990 yang ditandai dengan peningkatan GNI perkapita. Rata-rata pencapaian pendidikan juga meningkat, baik pada laki-laki ataupun perempuan. Begitu juga dengan peningkatan kesetaraan gender dan pemberdayaan perempuan. Dengan adanya perbaikan faktor-faktor pembangunan tersebut juga diiringi dengan semakin membaiknya kondisi kesehatan di Indonesia, misalnya peningkatan status kesehatan ibu dan anak dan penurunan berbagai penyakit infeksi. Akan tetapi, kondisi kesehatan di Indonesia masih jauh jika dibandingkan dengan negara-negara maju yang mempunyai infrastruktur, ekonomi dan tingkat pendidikan yang baik. Bahkan, perbaikan masalah kesehatan di Indonesia masih tertinggal jika dibandingkan negara-negara tetangga, misalnya masih tingginya kasus tuberkulosis (TB) paru dan malnutrisi. Oleh karena itu, untuk mcngatasi permasalahan ini maka perlu dilakukan peningkatan pada semua faktor-faktor pembangunan terkait dengan melakukan pendekatan Pelayanan Kesehatan Primer yang komprehensif.Kaia Kunci: pembangunan, pola penyakit, dan masalah kesehatan

Abstract

BACKGROUND: miR-200a is known to alter trophoblast invasion and spiral artery remodeling, leading to defective placentation that causes placental hypoxia, which is the main pathomechanism in early-onset preeclampsia (EOPE). Hypoxic placentas cause systemic endothelial dysfunction that is characterized by low production of endothelial vasodilator, mainly nitric oxide (NO). On the other hand, defective placentation does not cause late-onset preeclampsia (LOPE), making the role of miR-200a expression and NO level as predictors in LOPE questionable. Therefore, this study was conducted to compare miR-200a expressions and NO levels in EOPE and LOPE to clarify their role in pathomechanism of both types of preeclampsia.METHODS: A cross-sectional comparative study was conducted in 62 preeclamptic patients (31 EOPEs and 31 LOPEs). Subjects were classified into EOPE or LOPE groups based on whether the diagnosis of preeclampsia was made at <34 or ≥34 weeks of pregnancy. miR-200a expression was analyzed using real-time polymerase chain reaction technique, and NO level was analyzed using colorimetric assay method.RESULTS: EOPE and LOPE subjects were equivalent in terms of age and parity (p=0.709 and p=0.066), but significantly difference in gestational age (p=0.000). miR-200a were expressed in 74.2% of EOPE and 41.9% of LOPE subjects (p=0.010). Median NO levels were lower in EOPE compared to LOPE subjects (23.75 vs. 106.00 µmol/L) (p=0.027), and lower in subjects with detected miR-200a compared to subjects with undetected miR-200a (62.75 vs. 132.25 µmol/L) (p=0.032).CONCLUSION: miR-200a was more expressed in EOPE compared to LOPE subjects suggesting that it might be a significant in predicting EOPE. While NO level was significantly lower in EOPE whilst higher in LOPE subjects, hence might be potential as a marker to differentiate EOPE and LOPE.KEYWORDS: miR-200a expression, NO level, early-onset preeclampsia, late-onset preeclampsia

Abstract

Background: Pain during labor can be caused by unmanaged anxiety, then causing tension in the lower uterine segment, cervix, and ischemia in the uterine muscles so the sensation and intensity of pain felt during labor is getting stronger. Hypnobirthing is one of the actions that can be done to reduce pain during labor. Purpose: This study aims to determine the effectiveness of hypnobirthing in reducing the scale of pain in mothers giving birth. Methods: This type of research is a narrative literature review using PICO. Article searches are accessed through several databases, including Pubmed, Sciencedirect, Google Scholar, and Portal Garuda. then the articles are selected based on the inclusion and exclusion criteria and then analyzed. Based on 19 journal articles that have been analyzed, the implementation of the hypnobirthing method in mothers in labor has proven effective in reducing pain during the labor process. Conclusion: The benefits of hypnobirthing that directly affect the mother are that it can make the mother feel comfortable and relaxed during the labor process, and reduce the production of catecholamine and adrenaline hormones but increase the endorphin hormone in the mother, proven to be successful in reducing the scale of pain in mothers giving birth. However, there are several factors that affect the effectiveness of hypnobirthing, such as when hypnobirthing is done, how consistently the mother does hypnobirthing and also how focused the mother is on doing deep relaxation as one of the stages of hypnobirthing itself. Further research should be conducted on the combination of the hypnobirthing method with other pain management to determine the effectiveness of this management in reducing pain during the labor process.

Abstract

Breast engorgement is a condition in which the breasts become overly filled with milk, causing swelling, hardness, and pain. The prevalence of breast engorgement Indonesia in 2021 was 37.12%, which was the highest in ASEAN. The condition can be alleviated through non-pharmacological methods, such as warm compresses, which have no side effects. The study aimed to determine the application of warm compresses in managing breast engorgement. This study used a literature review with a scoping review approach. The study was conducted from November 2024 to February 2025. Data were collected from various digital databases, including PubMed Central, ScienceDirect, DOAJ, SINTA, Google Scholar, and Portal Garuda published from 2020 to 2024. Thematic analysis was used to interpret the data. Relevant articles was selected based on inclusion and exclusion criteria, and processed using the PRISMA-ScR flow diagram. 12 articles were identified for analysis.12 articles were analized by thematic analysis. The application of warm compresses 5 between 30 minutes in 3 days significantly reduces breast engorgement. Warm compress therapy offers several benefits, including reducing breast pain and swelling, promoting vasodilation, enhancing milk production, facilitating smoother milk flow, stimulating the let-down reflex, and improving breast emptying.

Abstract

Smoking is still a public health challenge in Indonesia. Nationally, the percentage of the population who smoke has increased from 28.26% in 2022 to 28.99% in 2024. This trend indicates the consistency of smoking habits in society, especially among the productive age group. Men mostly smoke. This can be detrimental to the health of the smoker himself and can also be detrimental to the health of the surrounding environment. There are three important clinical phases in tobacco addiction, namely, trying, sometimes using, and using every day. The purpose of this study was to determine the factors that influence smoking behavior in male adolescents. The method used in this study was a descriptive research type with a population of all productive-age men in Padang City. Sampling used the cluster random sampling technique with a total of 97 respondents. Data were collected through a structured questionnaire that adopted the Global Youth Tobacco Survey (GYTS) instrument. Data were analyzed with a frequency distribution in the form of a percentage. The results showed that 36.1% started smoking at the age of 13 years, the number of cigarettes smoked per day was 1-10 cigarettes (37.1%), the usual place to smoke in public places (30.9%), smoking when gathering with friends (52.6%), Smoking is fun (47.4%), Smoking helps reduce anxiety/anger/restlessness (49.5%), wanting to smoke in the morning immediately after waking up (33%), Getting cigarettes by buying them yourself (55.7%), and the type of cigarette smoked is filtered (39.2%). These findings will inform targeted interventions to reduce tobacco use and promote healthier lifestyles in economically active communities while increasing labor productivity.
