Abstract

Chronic Kidney Disease (CKD) is a global health problem that requires patients to undergo long-term hemodialysis therapy. This procedure is frequently associated with various physical and psychological complications, including pain, muscle cramps, anxiety, and decreased quality of life. Pharmacological management of these symptoms has limitations due to potential side effects and the risk of drug dependence, highlighting the need for safe and effective non-pharmacological interventions. This narrative review aims to analyze and synthesize scientific evidence on the effectiveness of acupressure therapy in managing complications experienced by CKD patients undergoing hemodialysis. A total of ten relevant studies were reviewed, focusing on the effects of acupressure on physical symptoms (pain and muscle cramps), psychological symptoms (anxiety), and patients’ quality of life. The findings indicate that acupressure therapy is effective in reducing the intensity and frequency of muscle cramps, alleviating post-hemodialysis headaches, and decreasing anxiety levels. Additionally, acupressure contributes to improvements in overall quality of life among patients receiving regular hemodialysis. In conclusion, acupressure demonstrates significant potential as a complementary non-pharmacological intervention in nursing care for CKD patients undergoing hemodialysis. Further randomized controlled trials with larger sample sizes are recommended to strengthen the existing evidence base.

Abstract

Background: Intravenous fluid management is a critical component of perioperative care in patients with chronic kidney disease (CKD). The type of fluid administered may influence acid–base balance, electrolyte status, and the risk of acute kidney injury (AKI).Purpose: This article aims to review the latest scientific evidence comparing the effects of 0.9% saline and balanced crystalloids in CKD patients in the perioperative setting. Methods: A narrative review was conducted by searching PubMed, Scopus, ScienceDirect, and Google Scholar using keywords related to saline, balanced crystalloids, CKD, perioperative care, and AKI. Eligible articles included randomized controlled trials, meta-analyses, and systematic reviews published between 2015–2024. Results: Evidence from multiple studies indicates that balanced crystalloids are superior in maintaining acid–base stability, reducing chloride levels, and lowering the incidence of AKI compared to saline. Large trials such as SMART, SALT-ED, and BEST-Fluids demonstrated reduced major adverse kidney events and delayed graft function in the balanced group, although mortality outcomes remain inconsistent. Conclusion: Balanced crystalloids provide greater physiological advantages and renal protection compared to saline in CKD patients undergoing surgery. While mortality reduction has not been consistently observed, current evidence supports the use of balanced crystalloids as the preferred perioperative fluid, particularly in patients at high risk of AKI.

Abstract

The use of time in anesthesia services in central surgical installations can be seen in the quality of service provided by health workers to patients. There are many factors that cause anesthesia services to take a long time. This study aims to determine the description of anesthesia ready time (ART) in anesthesia services at the Central Surgery Installation (IBS) of Banjarnegara Islamic Hospital. The research method uses quantitative data with a descriptive observational approach. The sampling technique used was purposive sampling, and the sample size according to the inclusion criteria was 128 respondents from elective surgery patients using spinal anesthesia and general anesthesia and classified as ASA I-IV status. The data collection technique is observation using ART. The results of the study showed that ART was in the good category, resulting in 114 respondents, consisting of 45 respondents with general anesthesia and 69 respondents with spinal anesthesia. Conclusion: Anesthesia services at the central surgical installation of RSI Banjarnegara are categorized as good according to international measurements using ART.

Abstract

ABSTRAK Nephrotomy procedures, whether elective such as Percutaneous Nephrolithotomy (PCNL) or emergency due to renal trauma, carry a significant risk of disrupting fluid and electrolyte homeostasis. Perioperative fluid imbalance increases the risk of complications including hypovolemia, hypervolemia, electrolyte disturbances (hyperkalemia, hyponatremia), metabolic acidosis, and Acute Kidney Injury (AKI), particularly in patients with Chronic Kidney Disease (CKD). This narrative review aims to analyze perioperative fluid management requirements in nephrotomy surgery based on recent scientific literature. Articles were identified through Google Scholar and PubMed, with 15 publications from 2011–2024 selected based on relevance to perioperative fluid therapy and nephrotomy. The synthesis indicates that optimal fluid strategies include preoperative hydration optimization through Enhanced Recovery After Surgery (ERAS) protocols, intraoperative hemodynamic-based fluid therapy such as Perioperative Goal-Directed Therapy (PGDT), preference of balanced crystalloid solutions (Lactated Ringer/Acetated Ringer) to prevent hyperchloremic metabolic acidosis, and strict postoperative fluid monitoring to prevent fluid overload, especially in CKD patients with CRRT indication. In conclusion, perioperative fluid management in nephrotomy must be individualized, carefully monitored, and evidence-based to prevent postoperative renal complications and improve patient outcomes.

Abstract

Introduction: Hypothermia is a common complication in postoperative patients, particularly following spinal anesthesia, which may cause discomfort, increase infection risk, delay wound healing, and prolong recovery time. Preliminary data at RSI Fatimah Cilacap showed that approximately 80% of patients undergoing spinal anesthesia experienced mild to moderate hypothermia, while current management remains largely pharmacological. Objective: The purpose of this service was to implement a rewarming technique using hot packs and aluminum foil blankets to improve body temperature in postoperative hypothermic patients following spinal anesthesia. Method: This public service was conducted on 30 postoperative patients who met hypothermia criteria (body temperature <36°C) in the recovery room of RSI Fatimah Cilacap. The methods included patient education, body temperature monitoring, application of hot packs to the inner arms for 15 minutes, and the use of aluminum foil blankets for 15–30 minutes. Result: The results demonstrated a significant increase in patients’ body temperature, with an average rise of 1.2°C after 15 minutes of combined intervention. There was also a reduction in shivering grade, with patients at grade 0 increasing from 57.7% to 93%. Most patients (83.3%) achieved recovery within 15 minutes, indicating the effectiveness of the rewarming method. Conclusion: The combination of hot packs and aluminum foil blankets is an effective non-pharmacological intervention to increase body temperature and reduce shivering in postoperative hypothermic patients following spinal anesthesia.

Abstract

Introduction: Patients undergoing regional anesthesia are at increased risk of falls during the early postoperative period due to temporary motor and sensory impairment, as well as orthostatic hypotension. Falls may result in serious complications, including spinal cord injury. Therefore, health education on fall risk prevention and safe mobilization is essential to improve patient safety during recovery. Objective: To improve patients' knowledge regarding fall prevention through health education and safe mobilization demonstrations among postoperative patients receiving regional anesthesia. Method: This community service program was conducted in the Recovery Room of RSI Purwokerto involving 30 postoperative patients receiving regional anesthesia. Activities included fall risk screening using the Morse Fall Scale, motor function assessment using the Bromage Score, sensory block assessment, pre-test, health education using leaflets, safe mobilization demonstrations, discussion, and post-test evaluation. The results were analyzed descriptively using frequency and percentage distributions. Result: Most participants were classified as having a moderate fall risk (53.3%). Before mobilization, all participants had a Bromage Score of 3 (100%). After safe mobilization, 66.7% improved to Bromage Score 2. Participants with good knowledge increased from 83.3% before education to 100% after the intervention. Conclusion: Health education combined with fall risk screening, motor function assessment, and safe mobilization demonstrations improved patients' knowledge and supported safer postoperative mobilization. This program can be implemented as part of patient safety improvement efforts in the recovery room.

Abstract

Latar Belakang: Kelengkapan dokumentasi laporan anestesi merupakan indikator penting mutu pelayanan dan keselamatan pasien. Dokumentasi yang tidak lengkap dapat menghambat perencanaan anestesi, evaluasi klinis, serta berpotensi menimbulkan risiko hukum. Tujuan: Penelitian ini bertujuan untuk mengetahui gambaran kelengkapan dokumentasi laporan anestesi pada tahap pra anestesi, intra anestesi, dan pasca anestesi di Rumah Sakit Tk III Wijayakusuma. Metode: Penelitian ini menggunakan desain deskriptif kuantitatif dengan pendekatan retrospektif. Pengumpulan data dilakukan melalui observasi terhadap 270 dokumen laporan anestesi yang memenuhi kriteria inklusi dari total 303 dokumen. Penilaian kelengkapan dilakukan menggunakan checklist berdasarkan format rekam medis anestesi yang berlaku di rumah sakit. Hasil: Hasil penelitian menunjukan bahwa tingkat kelengkapan pengkajian pra anestesi sebesar 89,6%, laporan pra anestesi 91,6%, pengkajian pra induksi 67,3%, laporan intra anestesi 99,8%, monitoring intra anestesi 73,8%, laporan khusus sectio caesarea 3,9%, pengkajian pasca anestesi 74,6%, dan monitoring pasca anestesi 95,4%. Kesimpulan: Temuan ini menunjukan bahwa tidak ada indikator dokumentasi laporan anestesi yang mencapai tingkat kelengkapan 100%. Diperlukan penguatan regulasi, pelatihan berkelanjutan, dan optimalisasi sistem dokumentasi untuk meningkatkan mutu pelayanan dan keselamatan pasien.

Abstract

Perdarahan adalah suatu kondisi dimana darah berlebih keluar dari pembuluh darah akibat kerusakan pembuluh darah. Kabupaten Banyumas dikenal sebagai daerah dengan frekuensi kejadian cedera tertinggi, tercatat sebanyak 4.058 kejadian. Pengabdian masyarakat ini dirancang untuk memperluas pengetahuan dan keterampilan dalam memberikan pertolongan pertama untuk menghentikan perdarahan. Program ini melibatkan 30 peserta dari PMR MAN 1 Banyumas. Metode yang digunakan dalam pengabdian ini meliputi penilaian pengetahuan pre-test dan post-test dan demonstrasi menggunakan lembar ceklis. Metode edukasi yaitu penyuluhan dan ceramah dan metode keterampilan yaitu dilakukan dengan cara mendemosrtrasikan, sedangkan media yang digunakan power point,video dan leaflet. Hasil analisis data dengan distribusi frekuensi SPSS menunjukkan adanya peningkatan pengetahuan peserta. Sebelum edukasi, terdapat 17 peserta (57%) dengan tingkat pengetahuan cukup, dan setelah edukasi, meningkat menjadi 27 peserta (90%) dengan kategori pengetahuan baik, dengan kenaikan pengetahuan sebesar 21,33%. Keterampilan siswa juga menunjukkan hasil yang cukup signifikan hal ini di tandai dengan ketegori tarampil 73,3%. Kegiatan PkM pertolongan pertama tatalaksana menghentikan perdarahan secara signifikan terhadap peningkatan pengetahuan peserta, dengan mayoritas peserta mencapai tingkat yang baik setelahan penyuluhan. Namun untuk keterampilan (skills) mayoritas peserta, yaitu lebih dari dua pertiga, memiliki keterampilan yang memadai atau baik. Hanya sekitar seperempat dari total peserta yang dianggap kurang terampil. Menunjukkan peningkatan berdasarkan hasil dari presentase pre-test, post-test dan keterampilan yang telah dilakukan.   Hemorrhage is a condition where excess blood comes out of the blood vessels due to damage to the blood vessels. Banyumas Regency is known as the area with the highest frequency of injuries, recorded at 4,058 incidents. This community service is designed to expand knowledge and skills in providing first aid to stop bleeding. This program involved 30 participants from PMR MAN 1 Banyumas. The methods used in this community service include pre-test and post-test knowledge assessment and demonstration using a checklist sheet. The educational method is counseling and lectures and the skill method is done by demonstrating, while the media used are power point, video and leaflet. The results of data analysis with SPSS frequency distribution showed an increase in participants' knowledge. Before education, there were 17 participants (57%) with a moderate level of knowledge, and after education, it increased to 27 participants (90%) with a good knowledge category, with an increase in knowledge of 21.33%. Students' skills also showed significant results, marked by 73.3% skilled category. PkM activities for first aid management to stop bleeding significantly increased the knowledge of participants, with the majority of participants reaching a good level after counseling. However, for skills, the majority of participants, more than two-thirds, had adequate or good skills. Only about a quarter of the total participants were considered less skilled. Showing improvement based on the results of the pre-test, post-test and skills percentages that have been conducted.

Abstract

Pemulihan kesadaran pasca general anestesi merupakan indikator penting dalam penilaian efektivitas anestesi dan keselamatan pasien. Faktor-faktor seperti Indeks Massa Tubuh (IMT) dan usia pasien diduga mempengaruhi waktu pemulihan. Penelitian ini bertujuan untuk mengeksplorasi hubungan antara IMT dan usia dengan waktu pemulihan kesadaran setelah general anestesi menggunakan Laryngeal Mask Airway (LMA) di RSUD Prof. Dr. Margono Soekarjo. Studi ini bersifat deskriptif korelasional dengan pendekatan kuantitatif dan desain cross-sectional. Sampel terdiri dari 127 pasien yang menjalani general anestesi dengan LMA selama periode November 2023 hingga Oktober 2023. Data dikumpulkan melalui rekam medis dan observasi langsung di Unit Perawatan Pasca Anestesi (PACU). Analisis statistik menggunakan uji korelasi Spearman untuk menguji hubungan antara IMT dan usia dengan waktu pemulihan. Sebagian besar responden (52,8%) memiliki berat badan normal, dan 23,6% berada pada kategori usia lansia akhir (56-65 tahun). Hasil uji korelasi menunjukkan hubungan signifikan antara IMT dan waktu pemulihan (r = 0,497, p < 0,001), serta antara usia dan waktu pemulihan (r = 0,389, p < 0,001). Analisis regresi multivariat mengonfirmasi bahwa IMT dan usia merupakan prediktor signifikan waktu pemulihan kesadaran. Terdapat hubungan signifikan antara IMT dan usia dengan waktu pemulihan kesadaran pasca general anestesi menggunakan LMA. Temuan ini menyoroti pentingnya mempertimbangkan profil IMT dan usia dalam pengelolaan dosis anestesi dan perencanaan pemulihan pasien.   Recovery of consciousness after general anesthesia is an important indicator in assessing the effectiveness of anesthesia and patient safety. Factors such as Body Mass Index (BMI) and patient age are thought to affect recovery time. This study aims to explore the relationship between BMI and age with recovery time after general anesthesia using Laryngeal Mask Airway (LMA) at Prof. Dr. Margono Soekarjo Hospital. This study is descriptive correlational with a quantitative approach and cross-sectional design. The sample consisted of 127 patients who underwent general anesthesia with LMA during the period from November 2023 to October 2023. Data were collected through medical records and direct observation in the Post Anesthesia Care Unit (PACU). Statistical analysis used the Spearman correlation test to examine the relationship between BMI and age with recovery time. Most respondents (52.8%) had normal body weight, and 23.6% were in the late elderly age category (56-65 years). The results of the correlation test showed a significant relationship between BMI and recovery time (r = 0.497, p < 0.001), and between age and recovery time (r = 0.389, p < 0.001). Multivariate regression analysis confirmed that BMI and age were significant predictors of recovery time. There was a significant relationship between BMI and age with recovery time after general anesthesia using LMA. These findings highlight the importance of considering BMI and age profiles in managing anesthetic doses and patient recovery planning.

Abstract

Nyeri tenggorokan pascaoperasi (POST) merupakan komplikasi umum pada 18-65% pasien pasca-intubasi endotrakeal tube (ETT), dengan faktor risiko kritis berupa ukuran ETT, durasi intubasi, dan jenis kelamin perempuan. Studi cross-sectional ini (n=70) di RSUD Kardinah Tegal (Maret-Mei 2024) bertujuan menganalisis hubungan ETT-POST dan mengidentifikasi prediktor dominan melalui consecutive sampling, pengukuran skala Visual Analog Scale (VAS), dan analisis regresi logistik ordinal. Hasil menunjukkan hubungan signifikan ETT-POST (p=0,003; λ=0,400), dengan 64,3% pasien ETT mengalami POST vs. 0% pada kelompok laryngeal mask airway (LMA). Pasien perempuan dengan ETT ≥7,0mm berisiko 7,2 kali lebih tinggi mengalami POST berat (OR 7,2; 95% CI 2,1-24,3), sementara durasi intubasi <60 menit meningkatkan risiko POST 3,8 kali (OR 3,8; 95% CI 1,5-9,6). Disimpulkan bahwa reduksi ukuran ETT menjadi ≤6,5mm untuk pasien perempuan dan optimalisasi teknik intubasi merupakan strategi pencegahan prioritas di RSUD Kardinah Tegal.   Postoperative sore throat (POST) is a common complication in 18-65% of patients after endotracheal tube (ETT) intubation, with critical risk factors being ETT size, intubation duration, and female gender. This cross-sectional study (n=70) at Kardinah Hospital, Tegal (March-May 2024) aimed to analyze the relationship between ETT and POST and identify dominant predictors through consecutive sampling, Visual Analog Scale (VAS) measurements, and ordinal logistic regression analysis. The results showed a significant association between ETT and POST (p=0.003; λ=0.400), with 64.3% of ETT patients experiencing POST compared to 0% in the laryngeal mask airway (LMA) group. Female patients with an ETT ≥7.0 mm had a 7.2-fold higher risk of severe POST (OR 7.2; 95% CI 2.1-24.3), while intubation duration <60 minutes increased the risk of POST 3.8-fold (OR 3.8; 95% CI 1.5-9.6). It was concluded that reducing the ETT size to ≤6.5 mm for female patients and optimizing intubation technique are priority prevention strategies in regional hospitals.
