Abstract

Stunted is a condition of malnutrition caused by inadequate nutrient intake, especially in protein and zinc. Zinc deficiency can lead to impaired growth and increased risk of infectious disease due to inadequate zinc intake and low bioavailability of zinc in food. The purpose of this research was to analyze the nutrient intake and phytate-to-zinc molar ratio in children. This research was an analytical observational study with cross-sectional design. Children aged 6-59 months with stunted (n=61) and non-stunted (n=65) nutritional status were chosen by stratified random sampling in Ciptomulyo Health Center, Malang city. Nutrient and phytate intake were taken using 2x24 hour food recall method, while zinc bioavailability was calculated using phytate-to-zinc molar ratio. The difference of independent variables were analyzed using Independent t-test and Mann Whitney test. The result of this study showed that there were significantly different energy and protein adequacy levels (p=0.018; p=0.001) and phytate intake (p=0.016) among stunted and non-stunted children. On the other hand, fat, carbohydrate, and zinc adequacy level did not show any significant differences. Furthermore, there were no significant difference in phytate-to-zinc molar ratio in the two group (p=0.158) with more than half children had high phytate-to-zinc molar ratio category. In conclusion, the stunted and non-stunted children showed significant differences in energy, protein, and phytate intake, while their zinc intake was not significantly different resulting in similar phytate-to-zinc molar ratio between both groups. This study show that sufficient intake of zinc can help to lower phytate-to-zinc molar ratio regardless of nutritional status of children.

Abstract

The high prevalence of stunting is a major nutritional problem that must be resolved because it is a threat to the future of Indonesian children. One of the efforts to prevent stunting needs to be done by searching for solutions that can be practiced by the general public by applying positive deviance to improve behavior in fulfilling nutritional content in pregnant women and children. The purpose of this research is to identify and study positive deviance behavior to prevent stunting. The research method used is the qualitative method. The research was conducted for 28 months (February 2020-June 2022) in Gunung Maddah Village, Sampang Regency, Madura Island, East Java, Indonesia. Data collection techniques included the results of Focus Group Discussions (FGDs), in-depth interviews, and observations obtained from 55 informants including mothers of toddlers, parents/parents-in-law, community members, cadres, and health workers. Data analysis techniques included content analysis consisting of data preparation, data editing, and data cleaning. The results showed that positive deviance behavior to prevent stunting in Gunung Maddah Sampang Village included the consumption of milk by pregnant women, the provision of milk to children (breast milk and/or cow's milk), mothers' efforts to overcome eating difficulties, limiting and regulating children's snacking habits, and the consumption of rice with vegetables and side dishes.

Abstract

Severe acute malnutrition (SAM) is a global health concern that affects children and leads to delayed growth. The aim of this study was to compare the impact of F100 milk and Bregas Nutriroll, a local ready-to-use therapeutic food (RUTF), on SAM children. An unmasked, non-blinded, two-group, and simple randomized controlled trial was conducted. Indonesian children aged 12–59 months with SAM were randomly assigned to receive either F100 milk or Bregas Nutriroll. After eight weeks, the increase in the mean weight gain in both groups was assessed. The Bregas Nutriroll group (n=19) had a mean weight gain of 1.07±0.09 kg, while the F100 group (n=17) had a mean weight gain of 1.05±0.11 kg. The Bregas Nutriroll group had a slightly higher gain of mid-upper arm circumference (MUAC) than the F100 group (0.62±0.34 cm vs 0.50±0.37 cm). The gain of children’s height of the Bregas Nutriroll group was 0.96±0.42 cm, while the F100 group was shorter at a mean of 0.81±0.44 cm. Statistically significant differences (p<0.001) in nutritional status were observed based on weight, MUAC, and height/length after F100 and Bregas Nutriroll interventions. The intervention with F100 increased hemoglobin (Hb) levels of 0.71±1.25 mg/dL, while the Bregas Nutriroll intervention led to an increase of 0.11±1.39 mg/dL. In conclusion, our study showed that community-based treatment with F100 milk or Bregas Nutriroll resulted in nutritional status in children with SAM. These findings suggested that both treatments could be effective in treating SAM in improving nutritional status and child health outcomes.

Abstract

The role of women in food consumption decision-making is closely related to cultural norms. The primary objective of this study was to describe food habits based on gender perspective in West Java. This study used a cross-sectional design. The study was conducted for 12 months in Cianjur Regency (representing rural area) and City of Sukabumi (representing urban area), West Java, Indonesia. The samples were selected using clustered purposive sampling. The total subjects were 150 households. This study showed that in term of food allocation, fathers have privilege, such as fathers eat before mothers and children, setting aside foods for fathers first before sharing the rest with family members, and food menu that the father received was more complete or better. The wives have greater control than husband in aspects of household food management. The wives were also dominant in deciding food expenses in the family rather than their husbands. There were significant differences in food allocation between rural and urban areas. The differences including: fathers eat before mothers and children (p<0.001), menu differences between fathers and other family members (p=0.002), and menu differences among family members (p=0.008). There was a significant difference in decisions making to determine the daily food menu between rural and urban areas (p=0.004). Since mothers play an important role in food purchasing, improving nutrition knowledge is necessary for better quality food at the household level. This will have an impact on the nutritional status of all household members.

Abstract

The prevalence of anaemia among adolescent girls in Indonesia is 18.4%. One of the main indicator targets is reducing the prevalence of anaemia is in the form of iron-folic acid (IFA) supplementation for adolescent girls. The research aims to analyze knowledge of female youth on anaemia and management of iron-folic acid supplementation. This study used a cross-sectional design, and it was conducted in 24 senior high schools in Cianjur Regency (high stunting area) and Depok City (low stunting area). The total sample of this study was 243 female youth. The study began from September 2019 to February 2020 for the 1st term and continued from October 2022 to March 2023 for the 2nd term. The quantitative result showed that there was a significant difference in knowledge about the frequency of providing the iron-folic acid supplement (IFAS) at school and the duration in a year that students must consume the IFAS between high stunting and low stunting (p=0.000). A significant proportion of female youth, 67.5% in high stunting areas and 71.7% in low stunting areas, exhibit insufficient nutrition knowledge, underscoring the necessity for a nutrition education initiative targeting high school students. The qualitative results showed the input-process-output of the IFA program have been administered for the last few years, providing several constraints in order to improve the IFA program management. Based on those findings, the IFA supplementation program provides a potential chance for addressing anaemia in female youth.
