Abstract

Breast cancer (Ca Mamae)  is a major health problem in Indonesia and the world. One of the causes of this disease can’t be cured when found in the advanced stage.  The efforts of preventing and controlling breast cancer by the government have been conducted by the clinical Breast Examination (CBE). CBE is done by medical personnel. CBE formed women to be able to perform breast self-examination (CBE). CBE is an early detection intervention that has many advantages including simple, easy and practical. If done correctly and periodically can detect early breast cancer. The purpose of this research is to look at the knowledge and attitude of CBE in women of childbearing age (WUS) in Kalumbuk District.MethodThis type of research uses quantitative descriptive with simple random sampling. Data suspension was conducted on October 2019 – July 2020. The population of this research is 30-50 years old in women of fertile age. The number of samples in this study was 101 respondents. Data processing using   SPSS 25 software.ResultsThe results of the study gained that the knowledge of respondents in the category is sufficient, while the respondents attitude is in the negative attitude category to BSE.ConclusionKnowledge is enough and the negative attitude to BSE this is being done by a number of things such as age, education, employment, and health education BSE.

Abstract

Factors that cause maternal mortality mostly focused on the physical factors like post partum bleeding, eclamsia, sepsys, and abortion complication. However, non physical factor like psychological and emotional factor rarely discussed. In fact, such factor also has important role in determine maternal mortality by causing several complication. The government has created several movement to overcome maternal mortality that caused by psycholgical and emotional factor. The movement are known as “Gerakan Asuhan Sayang Ibu”. The movement has 18 requirement to fulfill in order to prevent maternal mortality. This study aims to analyze the implementation of “Gerakan Asuhan Sayang Ibu” by Independent Midwife Clinic (BPM – Bidan Praktek Mandiri)  in Padang municipality. This study uses qualitative design. Data were collected by deep interview.  The study are is conducted in several BPM in Padang Region on december 2017 until february 2018. There are 3 midwives, 3 patients, and 3 family member of patient were particiapate as informant. The data analysis was performed using Triangulation method. The study shown that more than half midwife do the “Asuhan Sayang Ibu” well. The item that related to physical needs are fulfilled by mostly midwife. But the item that related to communication and emotional needs only fulfilled by several midwife. The deep exploration about management of “Asuhan Sayang Ibu” found the planning, implementation and evaluating phase did not run well properly.

Abstract

Waiting is a waste that is common in outpatient services, especially at Regional General Hospitals, thus reducing the quality of service. The results of the 2018 and 2019 service studies at Rasidin Hospital, the waiting time for outpatient services was 239 and 122 minutes (standard ≤60 minutes). The queuing process started at night and the queue was overflowing in the morning which resulted in late medical record processing, less seating, and low patient satisfaction. The purpose of this service is to design a queue for outpatient services based on Lean Management. The service was carried out in February-July 2020. Lean Management reduces waste on everything involved in the service process, namely Supply, Input, Process, Output and Customer (SIPOC). The service target is the management, head of installation, registration officer, medical record, doctor and nurse and patient. Approaches in the form of surveys, brainstorming, trainning and FGDs for model building. The resulting model is written in a mobile-based application to facilitate queuing patients. The resulting model is an application for booking registration and consultation. Applications can be downloaded on the Googleplay Store. It takes 1 operator to run the application. This application is expected to decipher long queues, provide certainty of time and increase patient satisfaction. It takes commitment and discipline from all parties to carry out services that are on time as promised so that they run on target.

Abstract

The Covid-19 virus is transmitted from human to human through close contact with infected people through their respiratory droplets. A person can also be infected by touching an object contaminated with SARS-CoV-2 and then followed by reaching the eyes, nose, or mouth. Implementing the health protocol is necessary to reduce the number of virus transmissions. In addition, to increase the community's immune against Covid-19, WHO and the government have been developing a vaccination program. However, the recommendation from the latest research stated that people still have to obey the health protocol even after being vaccinated. The purpose of this community service was to help the government to increase people's understanding of the importance of implying health protocols, even after vaccination, to suppress Covid-19 cases. The method used is by holding an online event called a Webinar. It followed by a statement with audiences to commit to implementing the health protocol led by the Rector of Andalas University, then called a SIAGA SAKATO declaration. The activity began with a pre-test questionnaire for initial information collection, followed by education about Covid-19 prevention from the experts to the 512 students. The activity ended with the final data collection using a posttest questionnaire to test the student knowledge after the education. Paired t-test analysis showed a significant increase in student commitment after attending the Webinar (P≤0.05). Based on these results, the Webinar increases students' understanding of the importance of the health protocol in reducing the Covid-19 cases.

Abstract

Background: In providing services, medical professionals have to be aware of social norms, ethics and laws. Behaviors violate those regulations attract public attention and can lead to medical malpractice allegations. This study aimed to explore public opinion on medical malpractice, which was analyzed base on medical ethic principles.Method: To answer the research question, descriptive and qualitative online survey had been conducted from November 2009 to October 2010, which used structured and open-ended questionnaire. The participants were obtained by employing non-probability convenience sampling. Later, the quantitative data was analyzed descriptively, and the qualitative data was analyzed thematically and presented narratively.Results: In general, public opinion on malpractice is similar to the principles of medical ethics. They believe that wrong procedures, diagnosis and treatments are factors of the malpractice. However, the participants adopt more consequentialism philosophy, which emphasize on outcome of patients and lack of consideration what the doctors has done base on medical standards. More over, participants’ explanations on medical malpractice reflect dissatisfaction of medical services which may due to low quality of information and attitude of doctors and other staffs in providing services.Conclusion: To improve public opinion on medical service and prevent medical malpractice allegation, doctors and other staffs have to obey professional standards and ethics, providing adequate information and good communication.

Abstract

Background: Skills lab is a clinical skill training using model and simulation, which is very important in the learning process to develop clinical skill competencies of medical students. Through the process, the next step of learning on real patients will be safer and more effective. The study was conducted to explore learning barriers of medical students in the skill lab training to obtain experiences and competencies.Method: A qualitative study was conducted to explore the problems using in-depth interview as method of data collection. Thematic analysis was used to analyze the data. The respondents of the study were 46 medical students in semester V and VII in the Faculty of Medicine, Andalas University.Results: The main barriers that the students faced were clinical instructor factors, such as late attendance and replacement time with shorter allocation. Due to these factors, the students could not obtain optimal learning process, supervision and required competencies. Other factors such as lack of simulation model and limited time had also significant impact.Conclusion: Learning barriers of skill lab training were caused by multi factors. Therefore, systematic arrangement, facility and assessment, and the quality of instructors’ performance need to be improved.

Abstract

Background: Learning process in medical school is a big stressor for the students. Many researchers have reported higher distress and related psychological health problems in medical students than in general population and non-medical students. This study explored how distress affects Indonesian medical students in FMAU, and its association with academic performance and demographic factors.Method: Cross sectional study was conducted on 185 subjects in the third year medical student of FMAU between April-May 2013. Level of stress was measured using Indonesian version of General Health Questionnaire-12 (GHQ-12), with Liker scale indicator (distress is when the score is >15). Demographic variables such as gender, source of finance, and economic status were assessed in the study. Academic performance was measured by cumulative Grade Point Average (GPA) and last semester GPA. Analyses were done using SPSS.Results: This study identified 35.7% of students were in distress, with average GHQ-12 score 13.42+SD4.97. No association was found between admission pathway, academic performance and distress. However, the study revealed that female students had higher risk of distress (p<0.05).Conclusion: Risk of distress is relatively high in medical students. However, there is no association between demographic factors, academic performance and distress. Further research needs to be conducted to explore risk factors that cause distress of medical students in Indonesian context.

Abstract

AbstrakTujuan akhir dari pendidikan kepaniteraan klinik adalah untuk mencapai kompetensi yang diharapkan, yang mana setelah menyelesaikan kepaniteraan klinik tersebut, mahasiswa harus mencapai level kompetensi sesuai yang ditegaskan dalam kurikukulum. Untuk menilai pencapaian kompetensi mahasiswa setelah menyelesaikan kepaniteraan klinik tersebut, sebuah penelitian evaluatif dilakukan dari Februari hingga September 2009 di Bagian Penyakit Dalam, Bedah dan Kesehatan Anak. Standar Kompetensi Dokter dari Konsil Kedokteran Indonesia (KKI) digunakan sebagai dasar penilaian pencapaian kompetensi. Data-data dianalisa secara deskriptif, dengan fokus pada standar kompetensi level 3 dan 4. Penelitian dilakukan pada dua siklus rotasi kepaniteraan klinik. Dua ratus dua puluh empat mahasiswa dari total 249 (response rate 90%) berpartisipasi pada penelitian ini, dengan jumlah masing -masing bagian secara berurutan 72, 75 dan 77 orang. Didapatkan, mahasiswa mencapai level kompetensi dalam melakukan tindakan pemeriksaan klinik pada semua bagian tersebut. Pencapaian kompetensi dalam melakukan tindakan terapeutik dan invasif hanya tercapai pada Bagian Kesehatan Anak. Akan tetapi, mahasiswa tidak mencapai level kompetensi yang diharapkan dalam melakukan penatalaksanaan kasus pada Bagian Penyakit Dalam, Bedah dan Kesehatan Anak. Mereka mencapai level kompetensi pada level sedang (mencapai rata-rata 3 dari level 4 yang diharapkan) dalam melakukan penatalaksanaan kasus-kasus penyakit infeksi virus pada anak. Hasil penelitian ini mengindikasikan bahwa perlu dilakukan peningkatan penataan dalam proses pendidikan klinik yang mencakup bimbingan yang optimal dan pengalaman penatalaksanaan kasus-kasus yang cukup.Kata Kunci: Kepaniteraan klinik, pengalaman dan kompetensi.ARTIKEL PENELITIAN89AbstractThe goal of clinical clerkship is to achieve professional of medical competencies. The students have to achieve required competencies as stated in the core curriculum after completing the clerkship. An evaluative research was conducted between February and September 2009 to assess clinical performance and competencies of Andalas Medical Students, in Department of Internal Medicine, Surgery and Pediatrics. Standard of clinical competencies from Indonesian Medical Council was used as standard levels the competencies. Later, the data was analyzed descriptively which focus on competencies of level 3 and 4. The study was conducted on two periods of clinical rotation. Two hundreds and twenty four students out of 249 (response rate 90%) participated in this study, which consist of 72, 75 and 77 in each department respectively. The students achieve required level of skill competencies in conducting clinical examination in all departments. However, they achieve required level of competencies in conducting invasive procedure and therapeutic skills are only in Department of Pediatrics. The students do not achieve required level of competencies in management of clinical cases in the Department of Internal Medicine, Surgery and Pediatrics. They achieve competencies only within medium level (level 3 of required level 4) in management of viral infections in pediatrics. The finding of the study implies that the implementation of clinical clerkship should be improved, which includes optimal supervision and exposure to clinical cases.Key words: Clerkship, experiences and clinical competencies.

Abstract

AbstrakDalam menjalankan profesinya, dokter harus taat pada norma-norma sosial, etika profesi dan hukum. Pelanggaran akan nilai-nilai tersebut dapat menjadi sorotan masyarakat, dan bisa berlanjut kepada tuduhan malpraktek kedokteran. Penelitian ini dilakukan untuk menilai bagaimana opini masyarakat tentang malpraktek kedokteran yang dianalisis berdasarkan prinsip-prinsip etika kedokteran. Untuk menjawab tujuan penelitian ini telah dilakukan online survei deskriptif dan kualitatif  pada Bulan November 2009-Oktober 2010, dengan menggunakan kuisioner terstruktur dan pertanyaan terbuka. Respoonden  didapatkan dengan teknik sampling non-probabilitas convenience sampling.  Data dianalisa menggunakan prinsip-prinsip etika kedokteran secara deskriptif. Data kualitatif juga dilakukan analisa tematik serta ditampilkan dalam bentuk narasi. Secara umum opini masyarakat tentang malpraktek kedokteran sesuai dengan pandangan prinsip-prinsip etika kedokteran. Masyarakat berpendapat bahwa terjadinya malpraktek ada unsur kesalahan prosedur atau diagnosis dan penatalaksanaan.  Namun, mereka cendrung pada filosofis konsekuentialis yang lebih menitik beratkan kepada akibat yang dialami oleh pasien yang kurang mempertimbangkan apa yang telah dilakukan dokter berdasarkan standar medis.  Bahkan, ungkapan responden yang dianggapnya sebagai malpraktek lebih disebabkan karena ketidakpuasannya terhadap pelayanan yang diberikan. Ketidakpuasan tersebut timbul akibat rendahnya kualitas informasi dan sikap komunikasi dokter dan petugas dalam melakukan pelayanan. Oleh karena itu, untuk memperbaiki opini masyarakat dan mencegah berbagai tuduhan malpraktek, dokter dan petugas kesehatan harus mentaati etika dan standar profesi dalam melakukan pelayanan, serta memberikan informasi yang cukup dan komunikasi yang baik dengan pasien.Kata Kunci: Malpraktek kedokteran, etika, dan opini masyarakat.  AbstractIn providing services, medical professionals have to aware on social norms, ethics and laws. Abstract

AbstrakRumah Sakit Bersalin (RSB) Budi Kemuliaan Jakarta adalah salah satu RS Sayang Bayi yang menjalankan Inisiasi Menyusu Dini (IMD) sebagai prosedur yang wajib di kerjakan oleh tenaga penolong saat membantu proses persalinan sejak tahun 2008. Tujuan penelitian ini adalah untuk mengetahui bagiamana kebijakan dan implementasi IMD dan advokasi ASI ekslusif di RS Bersalin Budi Kemuliaan. Studi dilakukan secara kualitatif berdasarkan analisis dokumen-dokumen terkait dan observasi langsung ke lapangan pada trimester terakhir tahun 2013 (Oktober-Desember 2013). Hasil kajian ini memperlihatkan pelaksanaan IMD dan pemberian ASI ekslusif di RS Bersalin Budi Kemuliaan belum optimal karena kurangnya fasilitas pendukung, kejelasan implementasi kebijakan dan advokasi petugas kesehatan. Studi ini menunjukkan bahwa IMD dan ASI ekslusif sangat ditentukan oleh advokasi yang baik dan petunjuk teknis kebijakan yang jelas. Studi ini juga memperlihatkan bahwa kesuksesan pencapaian ASI eksklusif terkait dengan keberhasilan pencapaian IMDAbstractMaternity Hospital (RSB) Budi Kemuliaan Jakarta has implemented friendly hospital Early Initiation of Breastfeeding (EIB) approach as standard procedure for asssisting childbirth process since 2008. This study was carried out to examine the policy and the implementation of EIB and exclusive breastfeeding policy in the Maternity Hospital Budi Kemuliaan. To answer the research objectives, the qualitative study was conducted using document analysis and direct observation from Oktober to Desember 2013. The result of the study revealed that the implementation of EIB and the implementation of exclusive breastfeeding did not reach the target due to lack of supporting factors, advocacy, and clear guideline of the policy. The study also indicated that successful implementation exclusive breasfeeding policy related to successful of EIB.
