Abstract

AbstrakTujuan penelitian fenomenologi ini adalah mengidentifikasi persepsi perawat dan keluarga pasien HIV/AIDS tentang pengetahuan yang diperlukan perawat dan keluarga yang merawat untuk merawat pasien HIV/AIDS di rumah sakit dan di rumah. Sejumlah 5 perawat & 5 keluarga pasien berpartisipasi dalam penelitian ini. Dari hasil analisis didapatkan 6 hal yang harus diketahui perawat. Persepsi perawat tentang pengetahuan dan keterampilan yang harus dimiliki perawat saat merawat pasien HIV/AIDS adalah: (1) pengetahuan dan keterampilan tentang konseling, (2) ARV & efek sampingnya, (3) Universal Precaution, (4) symptom management, (5) dukungan mental, dan (6) penanganan jika terpapar HIV. Sedangkan dari keluarga ada 4 hal yang harus diketahui oleh keluarga saat mereka merawat pasien di rumah, yaitu: (1) pengetahuan tentang HIV/AIDS & cara penularannya, (2) pengetahuan tentang tanda & gejala HIV/AIDS, (3) pengetahuan dan keterampilan tentang pencegahan penularan (Universal Precaution) dan (4) symptom management. Rekomendasi penelitian ini adalah perlu ada penelitian lanjut pada jumlah responden yang lebih luas. Hasil penelitian ini dapat digunakan untuk meningkatkan efektifitas asuhan keperawatan pada pasien HIV/AIDS dan juga kualitas hidup pasien saat mereka di rumah. AbstractThe purpose of this phenomenology study was to identify the nurse’s and relative’s perception on knowledge and skill needed by the nurses & relatives in caring patient in the hospital and in their house. Ten respondents (5 nurses and 5 relatives of the patients) participated the study. The findings identified knowledge & skill needed by nurses are: (1) knowledge & skill on counseling, (2) Anti Retro Viral therapy & its side effect, (3) universal precaution, (4) symptom management, (5) mental support, and (6) treatment standard on occupational exposure to HIV. The findings from the relative’s are: (1) knowledge on HIV/AIDS and how it spreads (2) knowledge on signs & symptoms of HIV/AIDS, (3) knowledge & skill on universal precaution and (4) symptom management. Some recommendations of the study were proposed that this study should be continued to have some other respondents. The result of these studies could be beneficial to improve the nursing care to the HIV/AIDS patient and their quality of life during AIDS stage, and it could be beneficial to the improvement the effectively of the nursing care of patient with HIV/AIDS.

Abstract

Men who have sex with men living with HIV (MSM-LWH) experience psychological and social issues, including depression, anxiety, fear of infecting others, frustration, and social isolation. They may also experience problems in their relationships due to a fear of social stigma, such as marital issues, family conflicts, a lack of family support, economic difficulties, and social rejection by the family. This research aimed to assess the relationship between HIV status disclosure and stress in MSM-LWH in Medan, Indonesia. Here, a cross-sectional design and the convenience sampling technique were used. A total of 176 respondents who were MSM, HIV positive, and residents of Medan City were included in this work. Data were collected by means of HIV Status Disclosure questionnaires and a Perceived Stress Scale (PSS). Overall, 70.9% respondents reported disclosing their status to others and approximately half revealed experiencing stress. Moreover, HIV status disclosure was significantly associated with stress (p= 0.025). This study reveals that HIV status disclosure may result in negative effects on MSMLWH, represent a barrier to medical treatment, and increase internal stress. Abstrak Hubungan antara Status Disclosure dengan Stres pada Lelaki yang Berhubungan Seks dengan Lelaki dengan HIV. Lelaki yang berhubungan Seks dengan lelaki (LSL) yang hidup dengan HIV mengalami masalah psikologis dan sosial termasuk depresi, kecemasan, ketakutan menulari orang lain, frustasi dan isolasi sosial. Selain itu juga mengalami masalah dalam hubungan sosial karena takut akan stigma, konflik dalam keluarga, kurangnya dukungan keluarga, kesulitan ekonomi dan penolakan oleh keluarga. Penelitian ini bertujuan untuk mengetahui hubungan antara status disclosure HIV dengan stress pada LSL yang hidup dengan HIV di Medan, Indonesia. Metode yang digunakan adalah cross-sectional dengan menggunakan teknik convenience sampling. Sebanyak 176 responden LSL dengan HIV positif dan tinggal di wilayah kota Medan. Data dikumpulkan dengan menggunakan HIV Status Disclosure Questionare dan Perceived Stress Scale (PSS). Hasil penelitian menemukan bahwa sebanyak 70,9% responden memiliki status disclosure HIV rendah, sementara itu sebanyak 55,1% resonden mengalami stress yang tinggi. Status disclosure HIV secara ber-makna dikaitkan dengan stress (p= 0,025). Penelitian ini mengungkapkan bahwa status disclosure HIV dapat mem-berikan efek negatif pada LSL yang hidup dengan HIV dan menjadi penghalang untuk perawatan medis dan mening-katkan stress internal.Kata Kunci: HIV, lelaki yeng berhubungan seks dengan lelaki, status disclosure, stres

Abstract

Emergency trauma can occur at any time, occur everywhere and can be experienced by everyone. The speed and accuracy of relief in trauma emergency conditions, will determine the outcome of the assistance provided. Help for trauma patients in the Emergency Department begins when the triage officer receives the patient. The triage process sorts patients according to the patient's emergency condition using parameters of level of consciousness, respiratory status and circulation status of the patient. An efficient trauma triage system aims to assist health workers in identifying life-threatening conditions, making timely assessments and management priorities that are appropriate to the severity of the patient. Trauma Code Activation is implemented as a system for providing emergency relief in cases of trauma in the red category. In some countries the application of Trauma Team Activation in cases of trauma in the red category is the main one to optimize the initial management of trauma emergency department. Trauma Code Activation at Cipto Mangunkusumo Hospital Jakarta Emergency Department from September 2018 to June 2019 totaled 362 cases, consisting of 34.1% head injuries (7.6% bleeding), 19.3% fractures, 13% burns, 6, 7% politrauma, and there are 3.8% died.

Abstract

Most cancer patients receive chemotherapy which contributes to prolong life, with the most common side effects of constipation. Constipation causes general symptoms that are bad and affect the quality of life of patients. The purpose of evidance based nursing (EBN) is to identify the effectiveness of Auricular acupressure in overcoming symptoms of constipation in cancer patients receiving chemotherapy. This EBN uses quasi experiments involving 14 participants in the chemotherapy treatment room with pre and post intervention assessments. The results of the analysis of the independent t-test showed that there was a significant difference in the form of faeces on day 6 with p <0.000 (95% CI). Conclusion: Auricular acupressure can be used as a constipation management nursing intervention in cancer patients undergoing chemotherapy.

Abstract

Background: Stroke is a neurological change that occurs quickly caused by disruption of blood supply to parts of the brain. Stroke has a complex impact which includes bio-psycho-social and spiritual. Various complexities of problems encountered in stroke patients so that self-management is needed as a form of adaptation to new conditions after stroke. Self-management interventions are useful for improving self-management skills and behavior in stroke patients. Purpose this study is to analize effectiveness of self-management interventions in stroke patients. Methods : Narrative literature review, research analyzes published in the online databases of ProQuest, Google scholar and Scopus. The study was selected using the criteria and keywords for the 2015-2019 period. Seven studies were analyzed using the literature review process. Results: To improve self-management stroke patients self-management interventions are needed. Self-management interventions can be applied in hospitals when post acute patients are hospitalized, preparation for discharge, return and after discharge home and can be combined with rehabilitation programs in the community. Self-management interventions can improve self-efficacy, self-management behavior, activity daily living (ADL) ability, decreased re-hospitalisation and patient readiness to return to the community. Conclusion: Self-management interventions can be applied in Indonesia, with a note that among others prepared human resources, costs, supporting facilities and policies from the manager.

Abstract

The prevalence of chronic renal failure in Indonesia tends to increase in the lower age group (45–54 years). Chronic renal failure may lead to impaired sexual function. A descriptive phenomenology study with in-depth interviews was carried out with 12 participants, and thematic content analysis was applied. Six themes were revealed, as follows: 1) adaptation process to sexual dysfunction experienced, 2) sexual dysfunction experience, 3) importance of fulfilling sexuality needs, 4) behavior in dealing with sexual dysfunction, 5) perception of the cause of sexual dysfunction, and 6) participants’ expectation of health service related to sexual function. The experience of adapting to sexual dysfunction became a meaningful process through partner involvement. Similar research involving more heterogeneous samples would benefit further discourse. Abstrak Adaptasi Terhadap Disfungsi Seksual Pada Pasien Gagal Ginjal Kronis. Prevalensi gagal ginjal kronis di Indonesia cenderung meningkat pada kelompok usia lebih muda (45-54 tahun). Gagal ginjal kronis sering menyebabkan gangguan fungsi seksualitas (disfungsi seksual). Penelitian ini bertujuan mendapatkan gambaran mendalam tentang pengalaman proses adaptasi pasien gagal ginjal kronis yang mengalami disfungsi seksual. Desain penelitian menggunakan deskriptif fenomenologi dengan wawancara mendalam. Dua belas partisipan diperoleh dengan teknik purposive sampling. Hasil penelitian ini teridentifikasi enam tema yaitu 1) proses adaptasi terhadap disfungsi seksual yang dialami partisipan, 2) disfungsi seksual yang dialami, 3) makna pentingnya pemenuhan kebutuhan seksualitas, 4) perilaku dalam menghadapi disfungsi seksual, 5) persepsi tentang penyebab disfungsi seksual, dan 6) harapan partisipan terhadap pelayanan kesehatan terkait fungsi seksualitas. Proses adaptasi yang dialami partisipan merupakan pengalaman yang sangat bermakna karena melibatkan dirinya sendiri dan hubungan interpersonal dengan pasangannya. Penelitian sejenis dengan sampel lebih heterogen diperlukan untuk memperkaya keilmuan. Kata Kunci: disfungsi seksual, gagal ginjal kronis, pengalaman proses adaptasi pasien

Abstract

Penelitian ini bertujuan untuk menggali secara mendalam persepsi isteri dan persepsi suami tentang pengaruh mastektomi terhadap citra tubuh dan fungsi seksual. Penelitian ini menggunakan rancangan deskriptif eksploratif (descriptive explorative design) dengan metode riset kualitatif. Lima pasien kanker payudara yang menjalani mastektomi dan suaminya diwawancara mulai dari satu minggu sebelum mastektomi hingga sebulan setelah mastektomi. Temuan penelitian ini adalah: 1) persepsi istri dan persepsi suami tentang pengaruh mastektomi terhadap citra tubuh dan fungsi seksual dalam perkawinan memberikan gambaran fenomena yang tidak terpisah tetapi merupakan interaksi dari berbagai faktor; 2) terdapat perbedaan persepsi istri dan persepsi suami terhadap citra tubuh dan fungsi seksual, istri merasa suami tidak tertarik pada penampilannya, sedangkan suami merasa takut menyakiti isterinya jika berhubungan seksual; 3) pengaruh mastektomi terhadap citra tubuh sudah dipersepsikan pasien bahkan sejak sebelum mastektomi dilakukan; 4) upaya mencari dukungan sudah dilakukan sejak sebelum mastektomi, meliputi dukungan spiritual, dukungan social, dan dukungan professional; 5) tem alai yang muncul yaitu beban lain psikologik, stigma social, dan financial yang dipersepsikan oleh pasangan; 6) besarnya pengaruh mastektomi terhadapa citra tubuh dan fungsi seksual dalam perkawinan ditentukan oleh kualitas hubungan perkawinan khususnya komunikasi. Rekomendasi: 1) mengembangkan program pelatihan Bimbingan Antisipatif bagi perawat; 2) memfasilitasi komunikasi yang lebih terbuka antara pasien dan suaminya; 3) mengembangkan program self help group yang terdiri dari pasien dan suaminya; 4) menyediakan informasi tentang sumber bantuan yang tersedia di fasilitas kesehatan maupun masyarakat; 5) mempertahankan kesinambungan asuhan keperawatan; dan 6) melakukan penelitian lanjutan menggunakan pendekatan antropologis dan sosiologis. The purpose of the study was to explore the perception of breast cancer patients and their husband on the influence of mastectomy towards body image and sexual functioning. A descriptive explorative design with qualitative method of research was used. Five selected breast cancer patients and their husbands meeting criteria were interviewed from the first week after diagnose until one month after mastectomy. The findings were: 1) the influence of mastectomy on body image and sexual functioning as perceived by wives and their revealed the contributions   of multi factors and dimensions; 2) there were different perceptions between the patients and their husband on the influence of mastectomy towards body image and sexual functioning, patients felt their husband were not attracted to them anymore while their husbands were afraid to hurt the patients if they had sexual relationship; 3) the influence of the mastectomy on body image had been perceived by the patients even before the mastectomy; 4) both patients and husbands had seeking for spiritual, social and professional supports before mastectomy; 5) other theme revealed were: psychological, social stigma, and financial burdens; 6) the extent of mastectomy had influence body image and sexual functioning was determined by the quality of marriage, particularly the openness of communication. Recommendations were: 1) to develop anticipatory guidance training program for nurse; 2) facilitate open communication between patients and their husband; 3) to develop self group program for patients and their husbands; 4) to provide information on resource available in the community; 5) to sustains continuity of care for patients and family; 6) to conduct further research using anthropological and social approaches.

Abstract

Perawat memiliki peranan yang penting mengatasi konstipasi pada pasien stroke selama perawatan di rumah sakit. Penelitian ini bertujuan untuk mengetahui perbedaan masase abdomen dengan masase abdomen dan minum air putih hangat pada pasien stroke yang mengalami konstipasi terhadap proses defekasi di Kota Medan. Penelitian kuasi eksperimen dengan dua kelompok intervensi dan satu kelompok kontrol menggunakan pendekatan purposive sampling dengan total empat puluh tujuh responden,  masing-masing empat belas responden kelompok masase abdomen, enam belas responden kelompok masase abdomen dan minum air putih hangat, dan tujuh belas responden intervensi standar diobservasi setiap hari selama tujuh hari. Proses defekasi terhadap ketiga kelompok dilihat dari waktu terjadinya defekasi antara kelompok intervensi I dan II dengan nilai p= 0,015, dan dari frekuensi defekasi antara kelompok intervensi II dan kelompok kontrol dengan nilai p= 0,000. Hasil penelitian ini diharapkan dapat menjadi evidence based practice dalam asuhan keperawatan medikal bedah dalam memberikan intervensi keperawatan terhadap pasien stroke yang mengalami konstipasi sehingga perawatan terapi komplementer di bidang keperawatan dapat dikenal dan memberikan manfaat sebagai pencegahan dan pengobatan alami. Abstract Overcoming Constipation on Stroke Patient with Abdominal Massage and Drinking Warm Plain Water. Nurses had an important role to overcome constipation of stroke patient during the treatment in hospital. This study was aimed to find out the difference of abdominal massage and abdominal massage with dringking warm plain water to defecation process of stroke patient in Medan. This quasi experimental study used two intervention groups and one group as a control,  this purposive sampling approach had 47 respondents, they were 14 respondents in the abdominal massage intervention group, 16 respondents in the abdominal massage with drinking warm plain water intervention group and 17 respondents in standard intervention  group. Defecation process was observed everyday for seven days. Defecation process of the three groups were analyzed from the time of significant defecation between the first and the second intervention group (p= 0.015), and the time of significant defecation between the second intervention and control group (p= 0.00). The results of this study are expected to be as evidence-based practice in medical-surgical nursing care in the nursing interventions in stroke patients who experience constipation so that complementary therapies in the field of nursing care can be known and provide to be used as a preventative and natural medicine. Keywords: constipation, abdominal massage, drinking warm plain water, defecation process, stroke

Abstract

Introduction: Indonesia is one of largest contributor countries to pulmonary TB cases in the world. One of the factors that cause it is no adherence in the treatment. It is necessary to develop a model in order to improving compliance in patient. For an initial step, it is necessary to explore patient’s adherence that based on King’s interacting systems theory. The objective of this study was to explore the patients experience that cause them obedient the treatment based on King’s interacting systems theory.Method: This research was a qualitative research with descriptive explorative approach. The samples were selected through purposive sampling method. Participants in this study were 8 patients who met the inclusion criteria. In this study, participants were pulmonary TB patients who have undergone treatment for 5-6 months.Results: There were 12 themes in patients compliance based on King’s interacting systems theory; (1) true perception of pulmonary tuberculosis, (2) self-awareness, (3) optimal growth and development, (4) positive self-image, (5) a healthy environment, (6) discipline to take medication, (7) effective coping, (8) open communication with the patient's health workers and family, (9) to perform its role optimally, (10) balancing stress during illness, (11) knowing the health care bureaucracy and (12) able to take decisions.Discussion: The 12 themes describe that adherent patients have interaction systems that were personal systems, interpersonal systems and social systems.

Introduction: Patients with chronic kidney disease (CKD) undergoing pre-dialysis requires a good self-management to control low protein intake and maintain kidney function. Adaptation to the existing stimulus through coping and adjustment mechanisms is important to maintaining good kidney function. However, few studies applied nursing theory based to guide intervention in helping the adaptation of patient CKD with their condition. The purpose of this study is to evaluate the effectiveness of Roy’s adaptation model towards physiological and psychological adaptation response among patients with CKD undergoing pre-dialysis.Methods: This study was conducted using a quasi-experiment to patients with CKD pre-dialysis, age over 18 years old. We modified Roy’s adaptation model for patients with CKD undergoing pre-dialysis.Results: A total of 70 subjects agreed to join the study, 38 subjects in intervention and 32 subjects in the control group. The mean of eGFR ranged from 26.3 to 26.6 mL/min/1.73 m2. We found that Roy’s adaptation model has significantly improved drinking behavior, reduce protein intake, blood creatinine, and psychosocial adaptation response after the intervention.Conclusion: These study findings suggested that Roy’s adaptation model is effective to help patients with CKD undergoing pre-dialysis improve their behavior and maintain kidneyfunction . Model dissemination, advocacy to related units, and application in nursing care in patients with chronic kidney disease pre-dialysis are necessary.
