Abstract

Abstract Background: Dysphagia is a frequent complication in stroke. Patients with dysphagia have potential risk of pneumonia, malnutrition, dehydration, increased length of stay and even death. One og nursing intervention that is considered important in the management of stroke patients is dysphagia screening because it can reduce aspiration and pneumonia and other complications that can occur in dysphagia. Aim: The aim of this literature review was to comparison between The National Institutes Of Health Stroke Scale, Nursing Admission Screening Tools, and Functional Independence Measure to preventing the occurrence of pneumonia in stroke patients in Indonesia. Design: This paper used narrative review. Data source: A literature search was conducted of articles published from 2010 through October 2018 using the PubMed, Embase, Cochrane Library and Ebscohost databases on compared three of dysphagia screening. Result: This search resulted in a total of 16 articles. Evidence has indicated that The National Institutes Of Health Stroke Scale Screening is considered to be the most effective type of dysphagia screening in preventing pneumonia in stroke patients who experience dysphagia. Conclusion: The NIHSS has a sensitivity value, specificity, positive predictive value, and negative predictive value that is quite good and considered safe to be applied in screening for dysphagia in stroke patients in Indonesia. Abstract

Abstract End stage renal disease (ESRD) is currently a global problem because of its increasing prevalence. Hemodialysis is the main management in ESRD patients. The importance of assessing the adequacy of dialysis greatly influences the quality and continuity of the patient's life. This paper is a literature review to identify the effect of i tai chi exercise on the adequacy of hemodialysis in ESRD patients. The authors used several databases to search for literary sources to be studied, namely Web of Science, EBSCO, PubMed, and Google Scholar. It was found that tai chi is a safe therapy less complications and have beneficial for ESRD patients. Tai chi can suppress bone reabsorption, improve functional balance and strength, increase the rate of blood in the muscles, increase capillary size and surface area which results in increased movement of toxins and urea in cells that move to the vascular eventually towards the hemodialysis machine. In addition, tai chi can be beneficial to make circulation smooth, reduce blood pressure, and facilitate the release of lactic acid so that it can reduce risk factors for cardiovascular disease. Tai chi is recommended as a treatment modality choice to increase the adequacy of hemodialysis in ESRD patients. Keywords: adequate dialysis, ESRD, hemodialysis, intradialysis exercise, tai chi.

Abstract

The current trend of HIV infection shows an increase in cases among men who have sex with men (MSM). Risk behavior in MSM has shown a high prevalence of moderate and worrying risk behaviors for HIV infection, including unprotected anal intercourse (UAI) and drug use. This literature review aims to explore the effectiveness of Personalized Cognitive Counseling (PCC) interventions in reducing risk behaviors for HIV transmission to MSM. Articles discussed in this literature study were obtained from several databases using the keywords “personalized cognitive counseling," “men who have sex with men," and "risk behavior." Cognitive counseling interventions are effective in reducing episodes of anal intercourse. Adapted Personalized Cognitive Counseling (PCC) intervention also has been effective in reducing sexual risk behavior in drug users MSM. This intervention is possible and essential to developing in dealing with risk behavior in MSM that tends to increase in its population. Abstract

Mental health problems such as depression and anxiety are common for people living with Human Immunodeficiency Virus (PLWH). It may have a various negative effect on psychological well-being and the quality of life for PLWH. These adverse effects are in the form of worsening diseases, decreasing the immune system, non-compliance with ARV therapy, and even causing suicide. Various psychosocial interventions were developed to improve the mental health of PLWH. But it often creates confusion and misunderstanding because it seems to overlap each other. The effectiveness of this intervention has been widely investigated, but there is still little confirmatory research on various psychosocial intervention techniques, especially for PLWH. The study aimed at describing the psychosocial interventions for the mental health of PLWH. We conducted literature searches from multiple relevant sources. The four databases we cut included PROQUEST, PubMed, Plos One, and Ebsco. Three types of psychosocial interventions consisted of symptom-oriented interventions, supportive intervention, and meditation. Based on the kind of intervention there are several psychosocial intervention techniques such as cognitive behavior therapy (CBT), stress management, interpersonal therapy (IPT), peer support, psychoeducation, meditation, relaxation, and mindfulness. There is no difference in the effectiveness of all types and intervention techniques in improving the mental health of PLWH. Psychosocial interventions proved to have a positive effect on the mental health of PLWH. Psychosocial interventions can be combined in PLHIW therapy programs with mental health problems Keywords: PLWH, psychosocial interventions, mental health problems.

Abstract

Nursing care services for post-partum patients with medical diagnoses of Miliary Tuberculosis (TB) in the Adult Intensive Care Unit (ICU) require a comprehensive approach. Critical nursing care with ICU settings and the use of portable HEPA filter are employed as alternatives if the ICU does not have an isolation room. Therefore, ICU nurses must concern the patient's needs, both the socio-cultural and cultural bio psycho. Rehabilitating patients in isolation rooms or using a portable HEPA filter that makes the patient's situation isolated results in patients' uncomfortable and restless feeling. In this case, the treated patient hold Middle East culture, and thus, she concerns about covering her head as part of her religious and cultural teachings. However, wearing headgear or hijab is rarely considered in the ICU because ICU nurses prioritize the critical condition of patients as the most vital regard to consider. This case study describes how to provide care to patients in ICU with the use of portable HEPA filter as an alternative TB isolation room and a culturally sensitive approach to meet the socio-cultural needs of the patient. Abstract

Coronary heart disease is a condition of narrowing the coronary artery lumen due to fat accumulation, which causes reduced blood flow. One of the interventions to overcome the blockage is surgical coronary artery bypass graft (CABG). CABG can help patients overcome obstructions in the coronary arteries. However, CABG can cause disturbances in sleep patterns that can reduce the quality of life of patients. The purpose of this paper is to find out what factors influence sleep quality in post CABG patients. This paper is a literature study of several databases in ELSEVIER, PROQUEST, EBSCO, PMC, JMH, JMC. The result found that sleep quality in post CABG patients was influenced by age, sex, emotional distress, the effect of treatment, surgery, and comorbidity, and environmental factors. Nurses need to improve their ability to conduct sleep quality assessments in patients after CABG patients using the international sleep quality assessment format

Abstract

The high degree of HIV transmission in the MSM circles resulted from the risky sexual behaviors. The study aimed to describe the behavioral intervention innovation to reduce sexual risk behavior among HIV-Positive MSM. This literature study was conducted to identify behavioral intervention innovations among HIV-Positive MSM. Articles were retrieved from Wiley Online, ProQuest, and ScienceDirect databases. The keywords used were behavioral Intervention, sexual risk behavior, and HIV-Positif MSM. The articles were limited based on inclusion criteria: English language full-text articles have been published from 2010 to 2019. Research studies also included several study design types, including a quasi-experimental study, a randomized control trial (RCT), and mixed-method design. Based on the research results on the selected journal, it is known that there is a wide range of behavioral therapy Innovations as an intervention carried out to reduce the sexual risk behavior among HIV-Positive MSM. Several interventions enhance the knowledge about HIV among HIV-Positive MSM, including integrated community and clinic-based intervention, Internet application, small group intervention, social network intervention, the treatment advocacy program, and brief Internet-based group intervention. In addition, man to man Sexual Health Seminar, Positive Sexual health  (PoSH),  Men speaking out, and community-based peer education can significantly decrease the sexual risk behavior, increase HIV tests improve attention, and search for health facilities. Behavioral intervention is an intervention that is part of nursing intervention, particularly in the hard-to-reach HIV-Positive MSM, because it is a minority group. With innovations, interventions are made more attractive and more convenient so that they are expected to change behaviors, especially sexual risk behavior that can transmit HIV and sexually transmitted diseases. This behavioral innovation tends to be more effective and can be applied in all countries, including developing countries, to reach all areas like those in Indonesia with extensive regions

Abstract

Spiritual well-being is a domain affecting the outcome of antiretroviral therapy in PLWH. However, spiritual need recently is still not adequately fulfilled in health service system. Therefore, it is essential to recognize the spiritual experience in PLWH to identify the depiction of their needs. This descriptive phenomenological study aimed to explore the spiritual experience of PLWH in the disease process by involving five participants in in-depth interviews. The thematic content analysis resulted five themes, namely (1) Obtaining guidance, being more religious, being more afraid of sin, more diligent in praying and reading the holy book; (2) Surrender and accept the illness because this is a warning, test and power of Allah; (3) Support from mother by encouraging to recitation and reminding to dhikr; (4) Experiencing sadness, shock, blaming others so that they want to end their life and avoid other people; (5) The participants hope to be healthy in this world and lucky in the hereafter. This study recommended further studies quantitatively and the assessment of PLWH spirituality in the nursing care practice.

Abstract

The prevalence of HIV cases increases significantly in the vulnerable population; it is Transvestite. The high prevalence of HIV in transgenders requires treatment and treatment services. However, until now the transgender community is a group that is often not served in various health facilities. Transvestite identity intersections produce unique life experiences, such as multiple identities and spiritual identities. The purpose of the study was to explore the experience of transgender HIV patients when hospitalized in fulfilling spiritual needs. Research method with qualitative design phenomenology study approach. This study involved 12 transvestite participants who had been hospitalized because of HIV positive. Retrieving data using in-depth interviews, data analysis using Colaizzi. The results of the study found four themes namely (1) Spiritual according to HIV patients transvestites means believing in the existence of God; (2) Be grateful for the conditions they have; (3) get closer to God by doing good things; (4) Poor nurse services for transgender HIV patients.

Abstract

The most common symptom in patients with pleural effusion is dyspnea. In addition to pharmacological therapy, acupressure therapy is one of the complementary non-pharmacological treatments in the form of physiotherapy with massage and stimulation of special points on the body. Currently, not much is known about the benefits of acupressure for health. The aim of acupressure is to provide significant benefits, especially in reducing shortness of breath in patients with pleural effusion. This case study was conducted in one of the treatment rooms at the RSUD in Lampung Province. The degree of dyspnea was assessed by the Brog Dyspnea Scale. Cases were patients with the main complaint of shortness of breath, accompanied by cough, chest pain, decreased appetite, nausea and vomiting since 1 week before admission to the hospital. The results of physical examination and investigations showed pleural effusion ec CHF. During hospitalization, acupressure was given to patients who had dyspnea and decreased oxygen saturation for 7 days. The results of this therapy showed a decrease in the Brog's dyspnea scale in these patients. This therapy can be done by the patient independently or with the help of a nurse, and there are very few side effects. In conclusion, acupressure is a complementary therapy that is easy to do, minimal side effects and can be used in patients with respiratory disorders.Keywords: acupressure; pleural effusion; Brog's dyspnea scaleABSTRAK Gejala yang paling sering timbul pada pasien efusi pleura adalah dyspnea. Selain terapi farmakologis, terapi acupressure merupakan salah satu pengobatan non farmakologis komplementer berupa fisioterapi dengan pemijatan dan stimulasi terhadap titik-titik khusus pada tubuh. Saat ini belum banyak diketahui manfaat acupressure untuk kesehatan. Tujuan acupressure adalah memberikan manfaat yang signifikan, terutama dalam menurunkan sesak nafas pada pasien dengan efusi pleura. Studi kasus ini dilakukan di salah satu ruang perawatan di RSUD di Provinsi Lampung. Tingkat dyspnea dinilai dengan Brog Scale dyspnea. Kasus adalah pasien dengan keluhan utama sesak nafas, disertai batuk, nyeri dada, nafsu makan berkurang, mual dan muntah sejak 1 minggu sebelum masuk kerumah sakit. Hasil pemeriksaan fisik dan pemeriksan penunjang menunjukkan efusi pleura ec CHF. Selama dirawat telah diberikan acupressure terhadap pasien yang mengalami dyspnea dan penurunan saturasi oksigen selama 7 hari. Hasil terapi ini menunjukkan penurunan skala dyspnea Brog pada pasien tersebut. Terapi ini dapat dilakukan oleh pasien secara mandiri ataupun dengan bantuan perawat, serta sangat minim akan efek samping. Sebagai kesimpulan, acupressure merupakan terapi komplementer yang mudah dilakukan, minim akan efek samping dan dapat digunakan pada pasien dengan gangguan pernafasan.Kata kunci: acupressure; efusi pleura; skala dyspnea Brog
