Abstract

Acquired Immunodeficiency Syndrome (AIDS) represents the advanced stage of HIV infection and remains a major global public health challenge. Viral load testing is the main predictor of disease progression and a key indicator of antiretroviral (ARV) therapy success. In South Sumatra Province, viral load testing coverage in 2024 reached only 49%, below the national target of 75% and the global target of 95%, hindering progress toward the Ending AIDS 2030 goal. This study aimed to estimate the time to viral load suppression and identify its predictors among people living with HIV (PLHIV), complementing previous studies in areas with lower HIV prevalence. Using a retrospective cohort design and secondary data from the HIV AIDS and STI Information System Application version 2.1 (SIHA 2.1), the study analyzed 1,002 respondents through descriptive statistics and Cox regression survival analysis (Stata 15). Among the participants, 85.63% achieved viral load suppression, while 14.37% did not. Factors significantly associated with viral load suppression time included age (p = 0.016; HR 1.18; 95% CI 1.03–1.35), occupational status (p < 0.001; HR 1.63; 95% CI 1.34–1.99), ARV adherence (p = 0.007; HR 1.22; 95% CI 1.05–1.42), tuberculosis (TB) co-infection (p = 0.003; HR 1.31; 95% CI 1.09–1.57), and loss to follow-up history (p = 0.003; HR 1.41; 95% CI 1.12–1.75), after adjustment for sex and clinical stage. These findings underscore that age, employment, treatment adherence, TB co-infection, and LTFU history are key determinants of viral load suppression, emphasizing the need to strengthen treatment adherence programs to achieve the 95-95-95 targets and End AIDS 2030.

Abstract

The Coronavirus disease (COVID-19) has spread to almost all provinces in Indonesia, including South Sumatra. Epidemiological models are required to provide evidence for public health policymakers to mitigate the virus. The aim of this study is: 1) to create a prediction model for COVID-19 cases in South Sumatra to help inform about public health policy and 2) to reflect on women’s experiences to provide solutions for mitigating the impact of COVID-19. This study uses quantitative and qualitative methods. A quantitative modeling approach called Susceptible–Infected–Recovered (SIR) model is used to predict COVID-19 cases in South Sumatra. The assumption used is that every four days, a doubling of COVID-19 cases is observed, with an average of 15 days for recovery. The sources of data are reports from the South Sumatra Provincial Government and the Ministry of Health of the Republic of Indonesia (MOH RI). Qualitative data are obtained through a feminist participatory action research project, which is focused on children’s experiences of COVID-19. Reflective analysis is conducted to develop insights into how to empower women with respect to mitigating COVID-19. Results show that COVID-19 cases in South Sumatra are still underreported, with only 5%–10% of the total estimated COVID-19 cases being reported. Modeling indicates that over 1,000 people had COVID-19 by the end of April, reaching over 150,000 by the end of May, and over a third of South Sumatra’s population is likely to be infected by the end of June. Multiple interventions are needed to reduce cases and flatten the curve. Women are key to flattening this curve and must be empowered to undertake actions from a familial base.

Abstract

Saat ini Indonesia mengalami masalah “triple burden”, yaitu tingginya kasus stunting dan wasting serta masalah gizi berupa obesitas dan kekurangan zat gizi mikro. Prevalensi stunting di Kota Palembang berada pada angka 14,3%, wasting 7,8%, dan obesitas 5,6%. Secara tidak langsung, kondisi geografis merupakan salah satu faktor yang mempengaruhi status gizi. Tujuan dari penelitian ini adalah untuk mengetahui bagaimana pemetaan kejadian stunting, wasting, dan obesitas di Kota Palembang berdasarkan kondisi geografisnya. Penelitian ini merupakan penelitian kuantitatif dengan desain studi ekologi deskriptif menggunakan data sekunder yang kemudian dianalisis dengan software QGIS. Populasi pada penelitian ini adalah seluruh balita yang ada di Kota Palembang dengan sampel balita stunting, wasting, dan obesitas yang ada di Kota Palembang. Hasil dari penelitian menunjukkan bahwa kasus stunting banyak ditemukan pada wilayah kecamatan padat penduduk di bantaran sungai dan membetuk pola mengelompok, kasus wasting banyak ditemui pada wilayah kecamatan dengan ketersediaan jumlah pasar yang sedikit, dan kasus obesitas banyak ditemui pada wilayah yang memiliki banyak rumah makan/restoran. Dengan dilakukannya pemetaan ini dapat diidentifikasi wilayah-wilayah yang lebih rentan beserta faktor dan karakteristik wilayah tersebut yang mungkin mempengaruhinya sebagai langkah awal dalam menentukan program dan kebijakan penanggulangan yang tepat dan efisien untuk masing-masing wilayah.

Abstract

Tuberkulosis (TB) masih menjadi salah satu permasalahan kesehatan masyarakat yang signifikan di Indonesia dan dunia. Upaya pengendalian penyakit ini sangat bergantung pada kinerja petugas kesehatan di fasilitas pelayanan tingkat pertama. Penelitian ini bertujuan untuk mengevaluasi kinerja petugas pelayanan TBC melalui pendekatan kualitatif dengan desain studi kasus berdasarkan hasil kajian literatur dari berbagai penelitian nasional dan internasional. Data diperoleh dari sepuluh penelitian yang dipublikasikan dalam lima tahun terakhir, terdiri atas lima studi nasional dan lima studi internasional. Hasil analisis menunjukkan bahwa faktor yang paling berpengaruh terhadap kinerja petugas meliputi kompetensi, motivasi kerja, dukungan supervisi, pelatihan, serta ketersediaan sarana dan prasarana. Selain itu, sistem manajemen, koordinasi lintas sektor, dan dukungan kebijakan juga berperan penting dalam peningkatan efektivitas program TBC di tingkat layanan primer. Studi ini menegaskan bahwa peningkatan kinerja petugas TBC tidak hanya memerlukan penguatan kapasitas teknis, tetapi juga dukungan kelembagaan dan motivasi berkelanjutan. Kesimpulan dari penelitian ini menunjukkan bahwa optimalisasi kinerja petugas TBC melalui strategi peningkatan kompetensi dan dukungan organisasi sangat penting untuk mencapai target eliminasi TBC tahun 2030.

Abstract

Background: Indonesia currently faces a triple burden of various disease problems, outbreaks of infectious diseases, and an increase in long-standing infectious diseases, new infectious diseases, and non-communicable diseases (NCDs). A report released in 2020 by the United Nations International Children's Emergency Fund (UNICEF) and the World Health Organization (WHO) indicated that during the Covid-19 pandemic, the postponement of routine vaccinations resulted in approximately 80 million children aged less than or equal to 12 months being susceptible to diphtheria, measles, and polio.Purpose: To review previous research in analyzing the relationship between incomplete basic vaccination and the occurrence of vaccine-preventable disease (VPD) cases.Method: This study employed a literature review design, conducting a systematic review by selecting relevant articles pertaining to the research objective. The literature study selection process was adapted from the Preferred Reporting Items for Systematic Reviews (PRISMA-ScR). Inclusion criteria for this research encompassed originality, usage of both English and Indonesian languages, keywords covering basic vaccination, adverse events following vaccination, Integrated Healthcare Centervisits, education, and the role of Integrated Healthcare Centercadres.Results: Generally, community knowledge, attitudes, family support, and the role of cadres correlated with vaccination coverage achievement. However, some studies indicated no correlation between specific variables and the infants' basic vaccination status. To enhance vaccination coverage and client satisfaction, improving community knowledge, attitudes, family support, the role of healthcare workers and Integrated Healthcare Centercadres, as well as increasing vaccine availability and vaccination guidelines, are necessary.Conclusion: To improve vaccination coverage and client satisfaction, enhancements in community knowledge, attitudes, family support, the role of healthcare workers and Integrated Healthcare Centercadres, alongside increased vaccine availability and vaccination guidelines, are essential.Suggestion: It is advisable for the community to enhance knowledge, attitudes, and support for vaccination, especially among mothers and families. The active involvement of cadres in vaccination programs and public dialogue regarding the moral ethics of vaccination also need improvement.

Abstract

Introduction: HIV incidence among Men who have Sex with Men (MSM) continues to rise and is influenced by behavioral, social, and health-related factors. MSM remain a high-risk group due to risky sexual practices and barriers to accessing healthcare. Objective: This study aims to identify and analyze the key behavioral, demographic, social, and health determinants contributing to HIV infection among MSM, as well as to provide evidence-based recommendations for prevention strategies. Methods: This study employed a systematic literature review of 15 articles that met inclusion criteria from PubMed, PMC, and Google Scholar. Screening followed the PRISMA approach, and findings were synthesized qualitatively based on behavioral, demographic, social, and health characteristics. Results and Discussion: Risky sexual behaviors including unprotected anal/oral intercourse, multiple sexual partners, and receptive sexual positioning were identified as dominant contributors to HIV infection among MSM. Demographic factors (productive age, low education), social-psychological factors (stigma, discrimination, lack of family support), and sexually transmitted infections further increased vulnerability. Poor adherence to antiretroviral therapy resulted in unsuppressed viral load, heightening transmission risk. Conclusion: Comprehensive prevention strategies are essential, including behavioral education, MSM-inclusive healthcare services, stigma reduction, and strengthened ART adherence to reduce HIV transmission among MSM.
