Abstract

Approximately 70% of acute ischemic stroke patients suffered from cognitive impairment that can be a direct insult of the ischemic stroke itself or as a consequence of the interaction between ischemic stroke event and its associated risk factors. Hyperhomocysteinemia is one of the vascular risk factors which can contribute to the pathophysiology of cognitive impairment after ischemic stroke. This condition can be due to an increase in the SAM / SAH ratio associated with a high methionine-containing diet, genetic predisposition, and deficiency of vitamin B6 and folic acid. The role of homocysteine ??in the pathogenesis of post-stroke cognitive impairment has not yet been studied extensively. Homocysteine ??is toxic and can induce systemic inflammatory responses, disruption of the blood-brain barrier, neuroinflamation, and neuronal death that lead to neurodegenerative processes. Neurodegenerative processes in particular brain structures that carry out certain function of cognitive domains will disrupt the function of those cognitive domains. Understanding the immunological aspects of the role of homocysteine ??in the occurrence of cognitive impairment among ischemic stroke patients is beneficial in providing an opportunity for development of interventions against homocysteine ??and its immunological responses consequences as the part of optimal management of post-ischemic cognitive impairment.

Abstract

Cognitive impairment is one of the important complications of type 2 diabetes mellitus. Type 2 diabetes mellitus-associated cognitive impairment has wide spectrum, ranging from mild cognitive impairment to dementia. The susceptibility of patients with type 2 diabetes mellitus to experiencing cognitive impairment is also determined by their previous cognitive reserve status. A good cognitive reserve of a patient with type 2 diabetes mellitus will protect him from cognitive impairment. The components of cognitive reserve, including education, occupation, hobbies, and social and physical activities provide a protective effect through neural reserve and neural compensation mechanisms that lead to a serial processes of neurogenesis, production of neurotropic factors, and neurotransmitter regulation. An understanding of the neuroprotective effects of the components of cognitive reserve is very important in determining optimal cognitive impairment management strategies in type 2 diabetes mellitus patients, in such a way that they are still able to carry out daily functional activities properly.

Abstract

Background: Diabetes Melitus (DM) is a degenerative diseases characterized by hyperglycemia condition. Basic Health Research data for 2018, shows an increase in prevalence of DM in Indonesia over last 5 years with 6,9% in 2013 to 8,5% in 2018. Pancreatic insulin receptor expression can be a marker for assessing presence of ? cells, production and secretion of insulin. Insulin could cause GLUT4 to be translocated to cell membrane of hippocampal neurons. In DM conditions, there may be changes in expression of pancreatic insulin receptors accompanied by changes in expression of GLUT4 hippocampal neurons. Methods: This experimental research used 16 white rats (Rattus Novergicus) which divided randomly into 4 groups, namely 2 control grups (A0 and A1) and 2 treatment groups (B0 and B1). Treatment group were induced by diabetes with a single dose of nicotinamide 110 mg/kgBB and streptozotocin 70 mg/kgBB intraperitoneally. The research data were obtained from immunohistochemical analysis of pancreatic and hippocampal tissue. Result: The mean expression of pancreatic insulin receptor in diabetic rat group was higher but not significant on the 14th day observation compared to the 0th day (p>0,05), while the mean GLUT4 expression of hippocampal neurons was significantly higher on 14th day of observation compared to the 0th day (p<0,05). The result of correlation test did not show a significant results in each group both at the 0th and 14th day observation (p>0,05). Conclusion: There was no significant effect of pancreatic insulin receptor expression on hippocampal neuron GLUT4 expression in diabetic model rats.

Abstract

Background: Epilepsy is the highest prevalence diseases in neurology. The decreased of cognitive function is one of the complications which is important. The purpose of this study is to determine the correlation between Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment in Indonesian Version (MoCA-Ina) score on patient with epilepsy in Mataram. Method: This is an observational cross-sectional study with 56 subjects with epilepsy from West Nusa Tenggara 2 center hospital. Data that collected from the samples were MMSE and MoCA-Ina score. Beside of that patients’ characteristics data were also collected which is age, gender, education, seizure control, etiology of epilepsy, seizure type, antiepileptic drugs treatment, and smoking status from the subjects. Both instrument’s mean scores were analyzed for the correlation using Spearman correlation test. Result: This study showing that the subjects average age were 32.9 years old, female (55.4%), and low educated (62.5%). According to the clinical characteristics most subjects were not-depressed (67%), poor quality-controlled seizure (55.4%), idiopathic etiology (66.9%), generalized seizure (71.4%), and having monotherapy (85.7%). Most of the subjects were not a smoker (76.8%). In this study both the mean score of MMSE and MoCA-Ina were 25.3 and 21.3. MMSE and MoCA-Ina score mean have a strong correlation (r=0.766; p<0.001). Conclusion: MMSE and MoCA-Ina score mean on patients with epilepsy have a strong correlation. Therefore, both instruments could be used to evaluate cognitive function on patients with epilepsy

Abstract

Background: Post-ischemic stroke cognitive decline is significantly affecting the quality of life of its survivors. Its prevalence was about 7.5-72% which was mostly determined by the existing of vascular risk factors and cognitive reserve of the subjects. Level of education is one of determinants of cognitive reserve, a factor that affect the susceptibility of subjects to cognitive decline after experiencing ischemic stroke-related neuronal damage. Since level of education is protective for cognitive function, the intervention on it can reduce the occurrence of cognitive decline.Objective: To investigate the relationship between level of education and cognitive status among hospital-based ischemic stroke survivors.Methods: This cross-sectional study involved post-ischemic stroke outpatients in two hospitals. The data collected in this study were demographic data, including level of education, and clinical data as well. The level of education was categorized into ≥12 years and <12 years groups. Cognitive status was assessed using Montreal Cognitive Assessment in Indonesia version (MoCA-Ina) and subjects with score of 26-30 were normal. The relationship between level of education as well as clinical data and cognitive status were analyzed using chi-square test.Results: There were 166 subjects eligible for this study (n=166). The mean age of subjects was 58 years and 68.67% of them were male. Cognitive decline were found 80.12% of subjects (n=133). The level of education was significantly associated with cognitive status of the subjects and hypertension as well.Conclusion: The level of education had significant relationship with cognitive decline in the hospital-based population of ischemic stroke survivors.

Abstract

Background: Obstructive Sleep Apnea (OSA) is one of the most common respiratory disorders found in society. This condition is often found in men with obesity, men with hypertension history, and elderly. In relation to this, it is found that the researches on the effect of OSA risk level on language functions is rarely done. Therefore, this study is aimed at investigating the effect of OSA risk levels on Boston Naming Test score to the population in Mataram. Objective: to investigate the effect of OSA risk levels on language functions used the Boston Naming Test to the population in Mataram.Methods: This study belongs to analytic descriptive study with cross sectional  approach. Held in Udayana Park, Dasan Sari, Mataram in May 2017 and September 2018. This study involved 132 respondents by ages among 40-74 years old. They are Chosen based on inclusion criteria. The OSA risk level was measured using The STOP-Bang questionnaire while language functions was measured using the Boston Naming Test. After That, the Mann-whitney comparative test was employed to analyzed the data.Results: The result of this study showed that, from 132 respondents, it was found that there were 66 (50%) respondents had a high risk level of OSA, and 82 (62%) respondents had language function disorders. Besides, the Mann-Whitney comparative analysis showed no effect on the risk level of OSA on Boston Naming Test score (p > 0.05).Conclusion: The risk level of OSA has no effect on Boston Naming Test score to the population aged 40-74 years in Mataram.

Abstract

Background: Identification of appropriate biomarker involved in the pathophysiology of diabetes-associated cognitive decline in animal model is necessary. Glucose transporter 4 (GLUT4) is the component of insulin-dependent neuronal signaling pathway needed for maintaining cognitive function. It is highly expressed in hippocampal neurons.Objective: To investigate the effect of GLUT4 expression in hippocampal neurons on the spatial memory of diabetes-induced Rattus novergicus.Methods: This experimental study used 24 male Rattus novergicus divided into control groups terminated on day-0 (C0), 14 (C1) and 28 (C2) and diabetic groups terminated on day-0 (D0), 14 (D1) and 28 (D2). Diabetes was induced using streptozotocin injections intraperitoneally. Spatial memory (travel time) of rats was assessed on day-14 and 28. Brain tissue sampling was performed for assessment of GLUT4 expression using immunohistochemical methods. Significant differences in means of GLUT4 expression and travel time between groups on day-0, 14 and 28 as well as correlation between means of GLUT4 expression and travel time in D1 and D2 were analyzed statistically.Results: The mean of GLUT4 expression in diabetic group was significantly higher compared to control on day-14, but not on day-0 and 28. The immunohistochemical examination confirmed this results. There were no significant differences in mean of travel time between control and diabetic groups on day-14 and 28. There were no significant correlation between means of GLUT4 expression and travel time in diabetic groups on day-14 and 28.Conclusion: The GLUT4 expression of hippocampal neurons did not significantly affect spatial memory of diabetes-induced Rattus novergicus.

Abstract

Executive dysfunction is a consequence of type 2 diabetes mellitus decreasing the quality of life of the patients. Currently, its prevalence is still scarce. Early detection of this executive dysfunction is beneficial in improving the clinical outcomes of the patients. This study aims to investigate the prevalence of executive dysfunction among type 2 diabetes mellitus patients in Mataram. This case-control study involving 106 subjects divided into two groups, type 2 diabetes subjects (n=53) and healthy subjects (n=53) in outpatient setting. The data collected from both groups were age, gender, years of education, TMT-B score, and executive function status. The data collected only from type 2 diabetes mellitus group were age, gender, years of education, duration of diabetes, treatments, smoking, hypertension, coronary heart disease, body mass index, and dyslipidemia. There was significant difference in the frequency of executive dysfunction between type 2 diabetes mellitus and healthy subject groups (p<0.05). The prevalence of type 2 diabetes mellitus-associated executive dysfunction was 49.1%. Gender was the only characteristics associated with the prevalence of type 2 diabetes mellitus-associated executive dysfunction (OR=0.25; 95%CI=0.07-0.88, p<0.05). There was a high prevalence of type 2 diabetes mellitus-associated cognitive impairment and it was associated with female gender.

Abstract

Latar belakang: Stroke merupakan salah satu penyebab utama kematian yang dapat dicegah. Probabilitas stroke iskemik dapat diukur menggunakan skor risiko Framingham. Korelasi kadar asam urat serum dengan probabilitas stroke iskemik masih kontroversial. Tujuan: Mengetahui hubungan antara kadar asam urat serum dan probabilitas stroke iskemik pada penduduk di Kecamatan Sekarbela Mataram. Metode: Penelitian deskriptif analitik dengan desain potong lintang pada 59 orang penduduk di Kecamatan Sekarbela Mataram. Data meliputi usia, kadar asam urat serum, penyakit kardiovaskular, diabetes melitus, terapi anti hipertensi, fibrilasi atrium, hipertrofi ventrikel kiri, tekanan darah sistolik dan merokok. Hasil: Uji korelasi menunjukkan tidak terdapat hubungan signifikan antara kadar asam urat serum dan probabilitas stroke iskemik (p=0,167). Simpulan: Tidak terdapat hubungan signifikan antara kadar asam urat serum dan probabilitas stroke iskemik pada penduduk di Kecamatan Sekarbela Mataram.Background: Stroke is one of the major preventable cause of mortality. Objective: To determine the correlation between serum uric acid level and probability of ischemic stroke in Sekarbela District population, Mataram. Methods: A descriptive analytic study using cross sectional design involving 59 residents in Sekarbela District, Mataram. Age, serum uric acid level, cardiovascular disease, diabetes mellitus, antihypertensive therapy, atrial fibrillation, left ventricular hypertrophy, systolic blood pressure, and smoking were measured. Results: No significant correlation between serum uric acid level and the probability of ischemic stroke (p=0,167). Conclusion: No significant correlation between serum uric acid level and the probability of ischemic stroke in Sekarbela District population, Mataram.

Abstract

Stroke merupakan penyebab kecacatan dan kematian utama di dunia. Faktor risiko stroke iskemik antara lain penurunan kadar kolesterol HDL serum. Penelitian deskriptif cross sectional ini dilakukan untuk mengetahui hubungan antara kadar kolesterol HDL serum dan Framingham stroke risk score pada 55 orang penduduk Kecamatan Sekarbela Mataram. Kolesterol HDL serum rendah didapatkan pada 56,36% subjek, lebih banyak pada perempuan. Rerata tekanan darah sistolik subjek penelitian 143,47 ± 25,54 mmHg. Proporsi subjek dengan penyakit diabetes melitus, penyakit kardiovaskular, hipertrofi ventrikel kiri, dan fibrilasi atrium tergolong rendah, sehingga rerata probabilitas risiko stroke iskemik rendah yaitu 10,415%. Tidak didapatkan hubungan bermakna antara kadar kolesterol HDL serum dan Framingham stroke risk score (p>0,05).Stroke was a major cause of disability and death in the world. Decreased serum high density lipoprotein (HDL) cholesterol was the risk factor for ischemic stroke. A cross sectional descriptive study was done on 55 subjects in Sekarbela Subdistrict Mataram to determine the relationship between HDL cholesterol level with Framingham stroke risk score. Low HDL cholesterol serum was found in 56.36% subjects, mostly women. The mean systolic blood pressure of subjects was 143.47 ± 25.54 mmHg. Diabetes mellitus, cardiovascular disease, left ventricular hypertrophy and atrial fibrillation prevalences are low. The community in Sekarbela Subdistrict had low stroke risk (10,415%). No significant relationship between high density lipoprotein cholesterol level with Framingham stroke risk score (p>0,05).
