Abstract

Latar Belakang: Penyakit Arteri Perifer (PAP) adalah tersumbatnya pembuluh darah perifer sebagian atau total akibat oklusi aterosklerosis terutama pada ekstremitas bawah. Hiperhomosisteinemia merupakan salah satu tanda penyakit kardiovaskular (PKV) yakni menunjukkan proses terbentuknya aterosklerosis. Penelitian sebelumnya telah melaporkan bahwa terdapat peningkatan level homosistein pada pasien dengan Diabetes Mellitus (DM). Namun, data mengenai level homosistein pada penderita PAP dengan DM tipe 2 terbatas. Oleh karena itu, penelitian ini bertujuan untuk mencari hubungan antara DM dengan level homosistein pada pasien PAP di Kota Mataram. Metode: Penelitian ini merupakan penelitian potong lintang yang dilakukan di Rumah Sakit Islam Siti Hajar Kota Mataram. Sampel penelitian adalah subjek dengan PAP dengan nilai Ankle Brachial Index (ABI) <0,90 dan DM tipe 2. Hasil penelitian ini adalah level homosistein pada pasien DM dengan PAP dan non DM dengan PAP. Analisis data menggunakan uji T tidak berpasangan. Hasil: Lima puluh dua pasien PAP berpatisipasi dalam penelitian ini, dengan 39 sampel (75%) berjenis kelamin wanita dan rata-rata usia 57,4 tahun. Rerata level homosistein pada pasien DM tipe 2 dengan PAP (5,7±0,9 µmol/L) lebih tinggi secara signifikan dibandingkan level homosistein pada pasien non DM tipe 2 dengan PAP (4,8±1,0 µmol/L) (p=0,001). Kami menemukan bahwa riwayat hipertensi (p=0,012) dan nilai ankle brachial index (p=0,0049) berhubungan dengan DM dan PAP. Jenis kelamin, usia dan merokok tidak berhubungan dengann DM dan PAP. Kesimpulan: Terdapat perbedaan yang signifikan antara rerata level homosistein pada kelompok subjek PAP dengan DM dan non DM. Kata Kunci: Penyakit Arteri Perifer, Diabetes Mellitus Tipe 2, Level Homosistein.

Abstract

Abstrak: Potensi Momordica Charantia Sebagai Antidiabetik : Studi Literatur. Momordica charantia (MC) atau yang dikenal dengan buah pare merupakan tanaman tradisional yang memiliki senyawa aktif seperti charantin polypeptide-P insulin untuk menurunkan kadar glukosa. Diabetes melitus adalah penyakit yang tidak menular dengan angka prevalensi penderita diabetes pada usia 20-79 tahun sekitar 11,3% yang menempatkannya di posisi ke-3 dengan wilayah arab-afrika dan pasifik barat sebagai posisi ke-1 dan ke-2. Dan Indonesia menjadi satu-satunya negara Asia Tenggara yang masuk kedalam 10 besar negara yang diproyeksikan IDF dengan angka diabetes ke-7 tertinggi. Metode yang digunakan yaitu dengan cara pencarian secara komprehensif pada 2 database (Pubmed dan Google Scholar) dari tahun 2015-2023, serta menggunakan kata kunci “Momordica charantia”, “Antidiabetes”, “Diabetes mellitus”. Data yang ditelaah dari setiap artikel penelitian yaitu meliputi identitas artikel, definisi, senyawa aktif yang berperan, patofisiologi, mekanisme kerja senyawa serta hasil dan kesimpulan penelitian. Hasil yang didapatkan menunjukkan bahwa Momordica charantia berguna untuk menurunkan glukosa darah. Ekstrak air yang terkandung dalam pare dapat mempengaruhi penggunaan glukosa dengan mengaktifkan Adenosine-monophosphate-activated protein kinase (AMPK) di jaringan otot.

Abstract

Diabetes mellitus is a metabolic disease characterized by hyperglycemia due to abnormalities in insulin secretion. The prevalence of people with diabetes mellitus in Indonesia reached 6.2% per year in 2020 and ranks 7th in the world with 8.5 million people. Based on the results of observations at the Praya Hospital in 2021, type-II diabetes mellitus is the 4th highest-order disease suffered by the community, especially Central Lombok Regency, with a total incidence of 131 patients. This study aims to determine the pattern of drug use and HbA1c profile in patients with type-II diabetes mellitus in the inpatient installation of Praya Hospital in 2021. This study was retrospective, using the Nonprobability Sampling method, namely the type of purposive sampling so that a sample of 96 medical records of diabetes mellitus patients was obtained type-II. The data is presented as pie charts and tables in percentages, which were analyzed using Microsoft Excel and SPSS version 25. From the results of this research, it was found that the pattern of drug use at Praya Hospital in 2021 was given the highest from each group namely the oral antidiabetic drug group (OAD) was in the Biguanid class with 61 drugs (8,13%), while insulin drugs were in the Rapid-Acting class with 343 drugs (45,73%), and the OAD + Insulin combination with 119 drugs (15,87%). For the HbA1c profile, high HbA1c levels were found in 76 patients (79%). This shows that blood glucose control is not reasonable. Based on the study's results, the recommended recommendation is that patients routinely control blood sugar and be educated on the accuracy of drug use to control blood sugar.

Abstract

Indonesia, consisting of tens of thousands of islands, has a rather long coastline with relatively high percentage of it is population that lives in the coastal area. High consumption of saltwater fish in the coastal population is an inevitability. Several studies have observed a relation between saltwater fish consumption and the risk of stroke with contradicting results, protective effect on one side and harmful on the other. The protective effect is mainly mediated by the poly-unsaturated fatty acid (PUFA) content of the saltwater fish while the harmful effect is mediated by the sodium content. PUFA exerts its protective effect through it is antioxidant effect on the endothelial wall, anti-inflammatory effect and the activation of anti-apoptotic pathways in the state of ischemic stroke while sodium increases the risk of hypertension which is the main predisposing factor for stroke and induces anatomical remodeling of the endothelial wall as well as functional abnormalities. Several public health interventions have been carried out to mitigate the harmful effect of the sodium content of saltwater fish yet there hasn’t been a specific intervention for coastal population.

Abstract

Background: Cognitive dysfunction is one of the main impacts of hypertension in the elderly population. Early detection and adequate management of early-stage cognitive dysfunction in hypertensive elderly is expected to improve their cognitive status and quality of life. Objective: This study aimed to determine the association between hypertension and cognitive dysfunction in a sub-population of the elderly in Mataram, Indonesia. Methods: This cross-sectional study involved elderly sub-population recruited consecutively in three public health centers in Mataram, Indonesia. Data included in this study were age, gender, occupation, educational level, hypertension, diabetes mellitus, and cognitive status. Cognitive status was assessed using the Indonesia version of Montreal Cognitive Assessment instrument. Multiple logistic regression analysis was performed to test whether hypertension was a determinant of cognitive dysfunction in participants taking into account the presence of socio-demographic status and diabetes mellitus as another vascular risk factor. Results: This study included 88 elderly as eligible participants. The frequency of cognitive dysfunction among participants was 61.4%. Multiple logistic regression analysis revealed that hypertension was the single variable significantly associated with a high frequency of cognitive dysfunction in elderly sub-population (odds ratio = 3.7; 95% confidence intervals = 1.3 – 10.4; p = 0.014). Conclusion: The frequency of cognitive dysfunction in the elderly sub-population in Mataram was high, amounting to 61.4%. Hypertension was the determinant of this high frequency of cognitive dysfunction in the sub-population studied.

Abstract

Background: A decline in cognitive function is often considered normal in the elderly, but the advanced stages of cognitive decline can decrease the ability to perform functional activities. Brain gym is one of the physical activity therapies that is proven to improve cognitive function. The purpose of this study is to know the effect of brain gym on cognitive function. Objective: This study aims to determine the correlation of cognitive function to brain gym in elderly. Methods: This is a queasy experimental study in 70 respondent with a pre and post-test group design. The subjects were elderly who were examined with Montreal Cognitive Assessment Indonesian (MoCA-Ina) and Visual Reproduction Test (VRT) on the first day and given brain gym 13 times, which was done 2 times in 1 week and re-examined MoCA-Ina and VRT after the intervention. Results: The research results show improvement in the MoCA-Ina and VRT pre and post-examinations after being given intervention. In the MoCA-INA examination, the mean score of the pretest was 23.30 and the mean score of the post-test was 24.50, while in the VRT examination, the mean score of the pretest was 10.08, and the post-test mean score was 10.83. However, there was no significant difference between MoCA-Ina or VRT in pre- and post-interventional brain gym (p>0.05). Conclusion: There is a relationship between cognitive function which is marked by an increase in the results of Moca INA and VRT with brain gym in the elderly at Balearjosari Malang

Abstract

Stroke is a focal or global clinical sign that occurs suddenly disrupts cerebral function, lasting more than 24 hours or causes death for no other reason than vascular. Stroke is the second leading cause of death and dementia and long-term disability worldwide. Approximately 85% of strokes are ischemic and 15% of strokes are intracerebral hemorrhage. Post-stroke disability can worsen the patient’s quality of life due to difficulty carrying out daily activities. Spontaneous recovery of maximum function after stroke occurs within 3 months. Neurorehabilitation with specific exercise techniques (task specificity) triggers disability improvement by activating neuroplasticity mechanisms. Post-stroke patients are limited in carrying out rehabilitation therapy in developing countries because there are various obstacles such as high transportation costs to health facilities and the need for family assistance while undergoing therapy, so one effort to overcome this problem is by providing training in basic rehabilitation techniques that can be carried out by the patient and family themselves at home to achieve optimal recovery of disability in post-stroke patients. This program’s activity used the PowerPoint and liquid crystal display media matriculation method, attended by 16 family participants and post-stroke patients at the West Lombok Regional Hospital's Neurology Clinic. Data analysis on changes in participants' pre-test and post-test knowledge showed that there was a significant increase in participants' knowledge regarding stroke and neurorehabilitation therapy exercise techniques with p<0.05. This result shows that this program’s activity can increase participants' knowledge about stroke and neurorehabilitation therapy exercise techniques at home.

Abstract

Otosclerosis is a hearing loss disease with abnormal bone growth in the middle ear. The purpose of this study was to determine the relationship between gender and the incidence of otosclerosis based on ear location at the Medismart Mataram Clinic in 2012-2020. This cross-sectional study was conducted on 63 otosclerosis patients at the Medismart Mataram Clinic. Sampling using the total sampling technique and data were analyzed using the chi square test. The results of the statistical test showed a p-value = 0.108 in other words the p-value is greater than α = 0.05, so it can be concluded that there is no significant relationship between gender and the incidence of otosclerosis based on ear location at the Medismart Mataram Clinic. Based on the results of this study, it shows that the incidence of otosclerosis in male and female patients is not much different, the number, although women are slightly higher. So it is recommended that there is a follow-up to provide the best solution for managing otosclerosis at the Medismart Mataram Clinic.

Abstract

Hormonal contraception is the most widely used contraceptive method in Indonesia, this is one of the risk factors for meningioma. Meningioma is the most common intracranial tumor. Risk factors for meningioma include a history of trauma, genetic history, hormonal exposure, history of menarche. in this study, the focus is on discussing literature on hormonal contraceptive exposure. This paper is a literature review. The method used in this literature review is to search and review various literature sourced from online databases such as PubMed, Google Scholar, and Proquest.  The keywords used in the literature search process include “hormonal”, “contraception”, “progesterone” “meningioma”. The use of hormonal contraceptives has been shown to increase the risk of meningioma, which is attributed to the progesterone receptors present in the normal meningens and the presence of the same progesterone receptors in meningiomas. The results of older studies have different results from recent studies, these inconsistencies may be associated with other variables such as duration, dose, and consistency in use, differences in the type of contraceptive use, current or previous use and other risk factors, further research needs to examine these variables and the underlying hormonal mechanisms to obtain more accurate results.

Abstract

Alzheimer's disease (AD) is a severe and high costs health problem all over the world. To date, there is no therapy capable of curing AD, making drug discovery a challenging and intriguing field to explore. Targeting marine natural products (MNPs) as a source of drug leads is a suitable choice considering the content of bioactive compounds and promising pharmacological benefits. This article aims to look at MNPs with prospects in several prime targets in AD pathology to cure AD. Marine vertebrates (fishes), marine invertebrates (sponges, tunicates, ascidian, clams, scallops, sand dollars, crabs, cockle, sea cucumber, shrimp, bryozoa, marine worm), algae/seaweed, and marine microorganisms (marine fungi, bacterium, dinoflagellata, microalgae) can be potentially used as disease-modifying treatments (DMTs) for AD. By targeting multiple aspects of AD pathology, these MNPs offer a multifaceted approach to treating and potentially modifying the disease course. This result is an intriguing gap for researchers in the discovery and development of new drugs that can improve AD pathology.
