Abstract

INTRODUCTION: Dementia is a leading cause of disability, dependency and poor quality of life. Its prevalence has increased consistently in recent years. Coastal communities who have limited access to health services have a higher risk of developing degenerative diseases including dementia. METHOD: A total of 154 middle-age peoples from four coastal area were recruited in this study. Participants were interviewed to obtain data on sociodemographic characteristics, lifestyle and medical history, then completed neuropsychological tests. Cognitive function was assesed by Mini-cog instruments. RESULT: The number of participants with cognitive decline was 87 people (56%) out of a total of 154 participants. 66% of all participants with cognitive decline were female. History of hypertension (67%), DM (16%), 40% with a BMI ≥22.5, 23% with a history of smoking, and 45% of participants with a low activity level. Multiple linear regression shows that Age and history of Hypertension is associated with cognitive decline (Sig.0.005 and 0.037). DM, BMI, smoking history and activity level is not associated significantly. CONCLUSION: More than half of middle age people on the coast of Lombok Island have cognitive decline. Age and history of hypertension are influencing factors.

Abstract

Latar Belakang: Penyakit dekompresi merupakan sekumpulan gejala akibat terbentuknya gelembung udara dalam aliran darah atau jaringan selepas elevasi mendadak pada penyelam. Gelembung udara yang terbentuk seringkali mengakibatkan gejala-gejala obstruktif pada jaringan yang terdampak. Pada jaringan otak, emboli gelembung gas yang masuk ke dalam sirkulasi menyebabkan manifestasi stroke akibat hipoperfusi jaringan parenkim otak. Insidensi dan derajat keparahan yang diakibatkan oleh penyakit dekompresi ini dipengaruhi oleh beberapa faktor yang terkait dengan kedalaman dan penyelam itu sendiri. Tatalaksana yang diberikan pada pasien stroke terkait dengan penyakit dekompresi memiliki pendekatan yang melingkupi rekompresi dan terapi stroke. Metode: Literatur-literatur yang dipilih merupakan studi yang relevan dengan topik dari tinjauan pustaka ini. Semua sumber penelitian yang dicakupkan dalam penelitian ini telah diajukan kepada dokter pembimbing untuk diteliti lebih lanjut. Pencarian literatur dilakukan dari database secara online melalui Google Scholar dan PubMed. Beberapa literatur yang relevan akhirnya ditemukan melalui kata-kata kunci yang disaring dari literatur yang telah ditemukan sebelumnya. Kesimpulan : Penyakit dekompresi terjadi obstruksi akibat emboli gas di bagian pembuluh darah arteri yang memperfusi otak sehingga terjadi stroke iskemik, perlu mempertahankan jaringan yang mengalami penurunan perfusi dan mengembalikan aliran darah ke jaringan yang terdampak.

Abstract

This community service activity was organized by the Neurology Department of Brawijaya University in collaboration with the Neurology Department of Mataram University. The event included a health education session on cognitive disorders, delivered both offline at the Neurology Polyclinic of RSUD dr. Saiful Anwar Malang and online via Zoom and YouTube. Participants were introduced to dementia screening using the SINARSIS web-based application. Through this socialization, we hope that the community will find it easier to identify cognitive disorders by using the SINARSIS application for screening. The activity consisted of pre- and post-tests, educational material delivery, discussions, and the administration of the Positive Aspects of Caregiving (PAC) questionnaire. A total of 98 participants from Malang and Mataram attended the Zoom session, while 45 attended the offline session. Among the participants, 74 were caregivers of dementia patients. The high level of engagement was evident from the documentation and the improvement in the post-test scores compared to the pre-test. The activity demonstrated its effectiveness in enhancing participants’ understanding of memory loss in dementia patients. Additionally, the results from the PAC questionnaire indicated a positive shift in caregivers' attitudes and behaviors toward caring for individuals with cognitive impairments. Abstract

Parkinson's disease is a neurological disorder that can be a major source of disability throughout the world. Changes in conditions in the body, such as α-Syn mutations can increase the risk of developing Parkinson's disease. α-Syn mutations will cause the loss of large and selective dopaminergic neurons in the substantia nigra pars compacta (SNpc) thereby triggering the early symptoms of Parkinson's disease. The high risk of side effects in the use of modern pharmacological therapy, indicates that a new breakthrough in the form of herbal-based ethnomedicine therapy for patients with Parkinson's disease is urgently needed. Ginkgo leaf, from the Ginkgo biloba plant, has long been known as a popular herbal supplement in Europe, Asia and America. The content of active compounds in ginkgo leaves was found to have the potential to provide therapeutic effects in patients with Parkinson's disease. This article will discuss the potency, mechanism of action, extraction process, and comparison of the potency of the two compounds with previous therapies. The development of the potential of ginkgo leaves is expected to reduce the increasing number of cases and provide a better quality of life in patients with Parkinson's disease.

Abstract

Decompression sickness is considered as an important complication of deep diving. Since the trend of diving activities is increasing throughout the world and is dominated by novice divers, the incidence of decompression sickness should be high. Severe form of decompression sickness can cause damage to the central nervous system with varying manifestations of mild to severe neurological deficits. Recent studies show that sleep disorders need to be considered as an important consequence of decompression sickness. However, the pathophysiology of decompression sickness-associated sleep disorders is currently not well understood. Since sleep disorders are associated with dysfunction of anatomical structures and the neurotransmitter systems in the brain that regulate the sleep-wakefulness cycle, decompression sickness-associated sleep disorders can be postulated to be related to dysfunction of these two regulators of sleep-wakefulness cycle. Sleep disorders found in patients with decompression sickness are more likely to occur in type 2 decompression sickness and may be found in the form of insomnia, hypersomnia, circadian rhythm disorders, obstructive sleep apnea, and narcolepsy. This review aims to discuss the pathophysiology of sleep disorders and decompression sickness as well as possible mechanisms underlying sleep disorders in decompression sickness through theoretical approaches and the results of available studies. In conclusion, decompression sickness-related sleep disorders are thought to involve two pathological conditions in the brain, namely structural lesions and dysfunction of the neurotransmitter system in the brain.

Abstract

Alzheimer's Disease (AD) is the most common form of dementia, characterized by progressive neurodegeneration leading to cognitive decline and memory impairment, affecting various cognitive functions. AD pathology involves the accumulation of neurotoxic substances such as amyloid beta (Aβ) and tau proteins, mitochondrial dysfunction, oxidative stress, and chronic neuroinflammation, resulting in synaptic loss and neuronal apoptosis. The transcription factor nuclear factor erythroid 2-related factor 2 (NRF2) has emerged as a pivotal regulator of cellular defense mechanisms, including antioxidant response, autophagy, and inflammation. NRF2 not only inhibits Aβ synthesis and enhances tau protein clearance via autophagy but also reduces neuroinflammation by modulating microglial activation and cytokine production. Additionally, NRF2 activation preserves mitochondrial function by attenuating reactive oxygen species (ROS) production and sustaining mitochondrial biogenesis. Recent studies highlight the therapeutic potential of NRF2 activators, such as sulforaphane and echinacoside, in preclinical AD models, demonstrating reductions in Aβ and tau pathologies and improved cognitive outcomes. These findings underscore the multifaceted role of NRF2 in mitigating AD progression and its promise as a target for developing novel therapeutic interventions. By integrating NRF2-based strategies, future treatments may offer more effective approaches to combat the complex pathophysiology of AD.

Abstract

Childhood is a golden period for brain development. Musical activities such as listening, singing, and playing instruments involve various sensory and motor systems and can enhance brain function. These activities stimulate different areas of the brain and are believed to support cognitive functions, including concentration during learning. This study used a literature review method. Sources were collected from academic databases such as PubMed, Google Scholar, and ProQuest. The review found that musical activities enhanced the structure and function of several brain regions. Early musical training increased grey matter connectivity in hippocampus, primary motor cortex, supplementary motor area, and broca’s area. White matter development also occurred in motor cortex, occipital lobe, arcuate fasciculus, and corpus callosum. In addition, musical engagement activated brain's reward system, particularly involving dopamine, which supported persistence and focus in learning. Musical hobbies contributed positively to improvement of children's learning concentration. This occurred through mechanisms of brain neuroplasticity that were influenced by regular engagement with musical activities. It was recommended that musical activities be introduced from an early age, especially during golden period, with selection of appropriate types of music, such as classical music that promotes calmness and supports attention.

Abstract

Indonesia memiliki ribuan pulau. Hal ini berdampak pada meningkatnya kegiatan penyelaman bawah laut. Penyelaman yang tidak sesuai prosedur dapat menimbulkan gangguan kesehatan berupa penyakit dekompresi yang salah satunya mengakibatkan neuropati. Emboli gas nitrogen menimbulkan respons inflamasi sistemik yang dapat memicu kerusakan pada saraf perifer. Faktor risiko gangguan dekompresi antara lain kurang pengalaman dalam menyelam, melakukan penyelaman berulang kali dalam waktu yang sama, tidak mematuhi prinsip-prinsip penyelaman dan sering kali melakukan yo-yo diving. Gangguan saraf perifer yang dapat terjadi berupa sindroma terowongan carpal, neuropati nervus peroneus, neuropati nervus ulnaris, mononeuritis multiplex. Pemeriksaan elektrodiagnostik menunjukkan adanya demielinisasi yang ditandai dengan peningkatan kecepatan hantar saraf. Pemberian terapi hiperbarik oksigen sesuai standar Angkatan Laut Amerika (United State Navy Treatment) menjadi standar dalam pengobatan penyakit dekompresi disertai dengan pemberian terapi simptomatik dapat memperbaiki gejala gangguan saraf tepi. Abstract

Epileptic seizures have a significant negative impact on the lives of patients, especially those who frequently experience relapses due to low medication adherence. Epilepsy patients often face stigma and social discrimination. Misunderstandings and negative societal attitudes, influenced by myths surrounding epilepsy, can prevent patients from seeking treatment and living confidently. This community service activity aimed to provide information and education about epilepsy, covering topics from its definition to an in-depth discussion of myths and facts about the disease. The ultimate goal was to enhance the knowledge of patients, their families, and visitors at the neurology outpatient clinic of Mataram University Hospital. The activity was conducted through interactive counseling sessions using PowerPoint presentations. Participants completed a pre-test before the counseling session, followed by a question-and-answer discussion after the material was delivered, and concluded with a post-test. A total of 56 participants attended. Both pre-test and post-test consisted of five questions addressing basic knowledge about the definition of epilepsy, myths and facts, and appropriate actions when encountering an epileptic seizure. The average pre-test score was 68.57, while the average post-test score increased to 76.44 after the educational session and discussion. Effective communication, education, and information dissemination proved to be key solutions for improving participants' knowledge. These efforts are expected to help epilepsy patients avoid stigma, gain full support from family and their surroundings, ensure optimal treatment outcomes, and enable patients to live confidently. Effective education and communication are critical in improving understanding of epilepsy, reducing stigma, and fostering social support for epilepsy patients.

Abstract

Stroke merupakan penyakit neurologis akibat kekurangan suplai darah parsial atau total di bagian otak akibat obstruksi aliran darah (stroke iskemik) atau pecah pembuluh darah otak (stroke hemoragik) menyebabkan suplai darah yang membawa oksigen dan nutrisi ke otak berkurang sehingga menimbulkan nekrosis jaringan otak. Stroke menimbulkan masalah global dari segi ekonomi dan fasilitas kesehatan dimana sepertiga pasien mengalami disabililtas dan menjadi ketergantungan (parsial atau total) terhadap caregiver. Sebesar 85% stroke jenis iskemik dan 15% stroke merupakan perdarahan intraserebral. Tujuan kegiatan pengabdian masyarakat ini untuk meningkatkan pengetahuan masyarakat mengenai jenis stroke, tanda dan gejala, faktor risiko dan usaha-usaha pencegahan terhadap stroke serta segera ke rumah sakit jika mengalami gejala stroke. Kegiatan pengabdian ini menggunakan metode matrikulasi media powerpoint (ppt) dan LCD (liquid crystal display) diberikan materi mengenai stroke seperti gejala dan tanda stroke, jenis stroke, faktor risiko dan pencegahan stroke serta tatalaksana rujukan ke rumah sakit. Kegiatan ini diikuti oleh 20 partisipan keluarga dan pasien yang berkunjung di Poliklinik Neurologi RSUD Lombok Barat. Seluruh partisipan menjawab soal pretes dan post tes dengan antusias. Hasil jawaban pre tes dan post tes partisipan menunjukan terjadi perubahan berupa peni ngkatan pengetahuan mengenai stroke. Kemudian semua data dianalisis menggunakan SPSS versi 20. Hasil analisis data secara statistik menunjukkan adanya peningkatan pengetahuan secara signifikan peserta mengenai penyakit stroke dan cara pencegahannya (p<0,001). Hasil kegiatan pengabdian masyarakat ini menunjukkan bahwa terjadi peningkatan pengetahuan partisipan mengenai stroke, faktor risiko dan tindakan pencegahannya pada para peserta setelah penyulihan dibandingkan sebelumnya (p<0,05).
