Abstract

Depresi merupakan salah satu komplikasi penting dari stroke iskemik. Prevalensi depresi pada pasien stroke secara global cukup tinggi, yaitu sekitar 55%. Pengabaian kondisi depresi pada pasien stroke akibat rendahnya pemahaman mereka mengenai pentingnya penanganan depresi dan faktor-faktor risikonya akan menyebabkan terjadinya penurunan kualitas hidup pasiennya. Oleh karena itu, pengabdian masyarakat yang ditujukan untuk deteksi dini depresi pasca stroke dan edukasi terkait pentingnya penatalaksanaan depresi dan faktor-faktor risiko penyertanya pada pasien stroke iskemik penting untuk dilakukan. Sebanyak 22 pasien stroke iskemik dengan waktu awitan stroke 3 bulan pertama yang datang ke Poli Saraf Rumah Sakit Umum Daerah Provinsi Nusa Tenggara Barat selama Bulan Agustus 2019 sampai dengan Maret 2020 turut berpartisipasi sebagai subjek dalam kegiatan ini. Frekuensi depresi pada subjek dengan stroke iskemik yang berpartisiapasi dalam kegiatan pengabdian ini cukup tinggi (45%). Sebagian besar subjek memiliki rentang usia produktif, berjenis kelamin laki-laki, memiliki tingkat pendidikan rendah, memiliki lokasi infark pada hemisfer kiri, dan memiliki hipertensi. Seluruh karakteristik demografik dan klinik yang teridentifikasi tidak berhubungan secara signifikan dengan frekuensi kejadian depresi pasca stroke (p<0.05). Seluruh subjek dan anggota keluarga yang mendampinginya menunjukkan antusiasme yang tinggi dalam kegiatan ini. Kegiatan deteksi dini depresi ini sangat bermanfaat bagi subjek dan edukasi terkait pentingnya penatalaksanaan depresi dan faktor-faktor risikonya ini bermanfaat dalam meningkatkan pengetahuan mereka terkait tindak lanjut yang harus dilakukan untuk mencegah atau mengatasi depresi pasca stroke.

Abstract

Myasthenia gravis is one of the most common autoimmune diseases at the neuromuscular junction. Although this disease is rare, the incidence of this disease is believed to have increased in the last seven decades. This community service activity aims to provide education to the public regarding myasthenia gravis. This community service activity was carried out in the form of counseling at the Neurology Polyclinic of UNRAM Hospital on Friday, January 20 2023. All participants were asked to take part in all counseling activities which consisted of pre-test activities, delivery of material on the topic of myasthenia gravis, and post-test. The enthusiasm of the participants in this activity was presented in the form of photo documentation. Statistical analysis using paired t-test was carried out to analyze the difference between the pre-test and post-test mean values ​​and the results were considered significant if a p value <0.05 was obtained. As many as 23 participants who attended the Neurology Polyclinic at UNRAM Hospital enthusiastically participated in the whole series of activities. The average participant post-test score was significantly higher than the pre-test means score (p = 0.025). This activity was able to increase the participants' knowledge about myasthenia gravis.

Abstract

Cognitive impairment is one of the important complications of stroke with a high prevalence. If not detected and managed properly, this cognitive disorder can worsen into post-stroke severe cognitive impairment (vascular dementia) with social and economic impacts on the family. This community service activity aims to increase public understanding of the importance of recognizing memory loss in stroke patients. This activity is carried out in the form of counseling at the Neurology Polyclinic at UNRAM Hospital once a week for a period of 3 weeks involving visitors to the Neurology Polyclinic at UNRAM Hospital, both outpatients and their caregivers. The participants were asked to take part in a whole series of activities which included pre-test, counseling, discussion, and post-test. The number of participants who took part in this counseling activity were 36 subjects. The participants' relatively high interest in participating in this activity can be seen from the documentation of the entire series of activities and the significant increase in the post-test mean score compared to the pre-test average score at the statistical analysis stage (p <0.05). This activity showed high effectiveness in increasing participants' understanding of memory loss in stroke patients.

Abstract

This community service activity was organized by the Neurology Department of Brawijaya University in collaboration with the Neurology Department of Mataram University. The event included a health education session on cognitive disorders, delivered both offline at the Neurology Polyclinic of RSUD dr. Saiful Anwar Malang and online via Zoom and YouTube. Participants were introduced to dementia screening using the SINARSIS web-based application. Through this socialization, we hope that the community will find it easier to identify cognitive disorders by using the SINARSIS application for screening. The activity consisted of pre- and post-tests, educational material delivery, discussions, and the administration of the Positive Aspects of Caregiving (PAC) questionnaire. A total of 98 participants from Malang and Mataram attended the Zoom session, while 45 attended the offline session. Among the participants, 74 were caregivers of dementia patients. The high level of engagement was evident from the documentation and the improvement in the post-test scores compared to the pre-test. The activity demonstrated its effectiveness in enhancing participants’ understanding of memory loss in dementia patients. Additionally, the results from the PAC questionnaire indicated a positive shift in caregivers' attitudes and behaviors toward caring for individuals with cognitive impairments. Abstract

The exploration of potential blood-based biomarkers that could be useful in the early detection of cognitive impairment associated with ischemic stroke is still being studied. The objective of this study was to examine the correlation between serum clusterin levels and the prevalence of cognitive impairment in individuals with ischemic stroke. A total of 86 outpatients with mild ischemic stroke within the first three months of its onset were recruited from three primary hospitals in Mataram, Indonesia. ELISA was used to measure the serum clusterin level. Patients’ sociodemographic and clinical data were also collected as covariates. The Montreal Cognitive Assessment-Indonesian version (MoCA-INA) instrument was used to evaluate cognitive status. The study used multivariate logistic regression analysis to investigate the effect of clusterin on the occurrence of cognitive impairment associated with ischemic stroke while controlling for other variables. Results: The multivariate logistic regression analysis revealed a significant correlation between elevated serum clusterin levels and a higher prevalence of cognitive impairment in ischemic stroke patients (odds ratio [OR] 3.56, 95% confidence interval [CI] 1.04-12.16, p = 0.043). Conclusion: Elevated serum clusterin levels have been associated with a higher occurrence of cognitive impairment in ischemic stroke patients.

Abstract

Cognitive impairment can occur due to factors that affect the structural and functional integrity of brain areas that specifically play a role in each cognitive domain. Severe cognitive impairment causes morbidity and mortality. One condition that can lead to complications such as a cognitive impairment is Decompression Sickness (DCS). DCS is an important risk for divers and manifestations of cognitive impairment need to be recognized earlier to prevent progression to a more severe stage. The literature used was collected from the Google Scholar, ScienceDirect, and PubMed databases with restrictions: publication year 2015-2025, Indonesian or English languages, and research with human subjects. The results of the literature review indicate that the pathophysiology of cognitive impairment after onset of DCS is still not clearly understood. The principles that are understood so far include the impact of nitrogen gas bubble circulation in cerebral arteries, extensive brain ischemia, white matter damage, and hyperbaric pressure administration can simultaneously improve cognitive function in patients. It can be concluded that neurological symptoms in DCS can develop into severe cognitive impairment if not recognized and treated immediately. The underlying relationship is mainly related to the impact of nitrogen gas embolism, cerebral ischemia, and extensive damage to cognitive areas of the brain.

Abstract

Mucuna pruriens also known as velvet bean (koro benguk) is a plant commonly found in tropical regions, including Indonesia. Mucuna pruriens is known to contain high levels of Levodopa, making it a potential natural treatment for Parkinson's disease. Parkinson's disease is a neurodegenerative disorder characterized by tremors, rigidity, bradykinesia, and postural instability. The estimated global incidence of Parkinson's disease in 2024 is expected to reach 12 to 17 million patients. Synthetic Levodopa has been the standard and proven effective therapy for Parkinson's disease. The method used in the preparation of this article is a literature review by collecting relevant and focused articles. The literature sources were accessed through comprehensive searches in three databases (Pubmed, Google Scholar, and Researchgate) from 2014 to 2024, using keywords such as “Mucuna pruriens,” “Levodopa,” and “Parkinson's disease.” The result of the literature review indicates that Levodopa is the dominant active component found in Mucuna pruriens compared to other compounds. The naturally occurring Levodopa functions as a precursor to dopamine, which plays a role in the treatment of Parkinson's disease, thus it can be developed into a complementary therapy in regions with abundant resources.

Abstract

Cognitive decline is a significant complication that affects most stroke survivors. Early detection of cognitive decline in ischemic stroke patients and identification of risk factors improves their clinical outcomes. This study aimed to determine the characteristics of cognitive status in the sub-acute phase of ischemic stroke. A cross-sectional study was conducted on 89 sub-acute ischemic stroke patients in three hospitals in West Nusa Tenggara recruited consecutively from August 2019 to April 2020. The data collected were demographic and clinical characteristics, cognitive status, and functional outcome. The association between clinical and demographic characteristics and cognitive decline was analyzed using logistic regression. In addition, the relationship between cognitive status and functional outcomes of these patients was examined using the chi-square test. This study revealed that the prevalence of cognitive decline in these subjects was 71.9%. Multiple logistic regression showed that age was the only characteristic associated with cognitive decline in the subjects (OR = 5.12,95% CI = 1.08-24.28). Furthermore, the frequency of cognitive decline in these subjects was significantly associated with functional outcomes (p-value =0.014). Thus, there was a high prevalence of cognitive decline in sub-acute ischemic stroke patients associated with increasing age and poor functional outcomes.

Abstract

Cognitive dysfunction is an important consequence of ischemic stroke, which can progress in the first few years and is primarily determined by clinical factors. This study aimed to investigate the clinical determinants of cognitive dysfunction in stroke survivors in West Nusa Tenggara Province, Indonesia. This cross-sectional study assessed 255 ischemic stroke survivors with a mean age of 57.1±9.3 years old and 29–79 years old, recruited consecutively in three main hospitals in West Nusa Tenggara Province between March 2019 and October 2021. Categorical data collected included age, sex, education level, clinical determinants of ischemic stroke, and cognitive status of the patients. The association between the clinical determinants of ischemic stroke and the risk of cognitive dysfunction in patients was analyzed using logistic regression after adjusting for age, sex, and level of education. The final multiple logistic regression analysis models revealed infarct diameter as the only clinical determinant significantly associated with an increased risk of cognitive dysfunction (OR = 3.14;95% CI = 1.20–8.23). Thus, a larger infarct diameter is the only clinical determinant of cognitive dysfunction in ischemic stroke survivors in West Nusa Tenggara Province, Indonesia.

Abstract

Cognitive impairment is a medical condition commonly found in elderly populations, which can be due to vascular risk factors in patients. There remains limited data on risk factors for cognitive impairment among coastal region populations. This study aimed to investigate risk factors for cognitive impairment in the adult population of Maringkik Island, West Nusa Tenggara Province, Indonesia. Data collected were age, sex, education level, hypertension, antihypertensive treatment, diabetes mellitus, cigarette smoking, and body mass index status. A total of 114 participants were recruited using a consecutive sampling method. The participants’ cognitive function assessment used the Mini-Cog instrument. The cognitive impairment frequency in the island’s adult population was approximately 48.2%. The final model of multiple regression analysis showed that hypertension (OR: 2.3; 95%CI: 1.0 – 5.0; p-value: 0.045) was a characteristic associated with the cognitive impairment frequency. Thus, the high frequency of cognitive impairment and hypertension frequency as primary risk factors for cognitive impairment in the island’s adult population implies the need to develop strategies for detecting and managing hypertension and hypertension-related cognitive impairment in the population by local health authorities.
