Abstract

Cardiovascular disease is often characterised by symptoms such as chest pain, palpitations, dyspnoea, excessive sweating, trembling, dizziness, and somatic symptoms such as pain and fatigue. In addition to physical problems, cardiovascular patients also often face psychological disorders, especially anxiety. High levels of anxiety in these patients can hinder the treatment process and worsen their condition. One effective nonpharmacological therapy to reduce anxiety in cardiovascular patients is music therapy, which can help calm and relax patients. Aim: to describe whether music therapy intervention is effective in reducing anxiety levels in cardiovascular patients. Methods: the writing of this article uses a systematic review conducted through a search strategy on 4 databases namely Scopus, scince direct, Pubmed, and Clinical key with a total of 3,824 articles obtained. The articles were then filtered based on inclusion and exclusion criteria to leave 5 articles. A total of 5 articles that met the criteria were then subjected to critical appraisal using the JBI Critical Appraisal Checklist guidelines. Results: In general, this systematic review shows that music therapy intervention is effective in reducing the anxiety level of cardiovascular patients.

Abstract

Background: Background: Ventilator-Associated Pneumonia (VAP) is a major complication in mechanically ventilated patients, often resulting from microaspiration due to inadequate endotracheal cuff pressure. Maintaining optimal cuff pressure can be achieved through continuous or intermittent monitoring. Aim : To compare the effectiveness of continuous versus intermittent cuff pressure monitoring in preventing VAP. Methods: A systematic review was conducted across PubMed, Scopus, Taylor dan Francis, and ScienceDirect. Articles were screened based on publication year, title, abstract, and relevance. Results: Of 550 identified studies, 7 met the inclusion criteria. Four studies reported greater VAP prevention efficacy with continuous monitoring (p < 0.05, p = 0.06, p = 0.109, p = 0.02; OR 0.45, 95% CI = 0.22–0.89). Six studies found no significant impact on secondary outcomes, including ICU length of stay, ventilation duration, and mortality. Conclusion: Continuous cuff pressure monitoring ensures more stable pressure regulation, potentially reducing VAP risk. Further studies are needed to assess its impact on clinical outcomes

Abstract

Latar Belakang: Rasa haus merupakan salah satu bentuk komplikasi pada penderita gagal jantung dan gagal ginjal kronis yang menjalankan program pembatasan cairan. Apabila tidak diberikan tatalaksana yang adekuat, rasa haus dapat memperburuk kondisi penyakit akibat kelebihan cairan karena intake yang berlebih. Terapi akupresur merupakan salah satu intervensi nonfarmakologis dapat diterapkan oleh perawat untuk mengatasi haus. Tujuan: Mengeksplorasi efektivitas terapi akupresur dalam manajemen haus pada penderita gagal jantung dan gagal ginjal kronis. Metode: Studi ini menggunakan metode tinjauan sistematis berdasarkan pedoman PRISMA. Artikel diperoleh dari basis data Pubmed, ScienceDirect, ProQuest, Sage Journals, dan Taylor and Franchis, serta sumber lain yang diterbitkan sebelum tahun 2025. Seleksi artikel dilakukan berdasarkan kriteria inklusi dan eksklusi, kemudian dianalisis secara kritis menggunakan pedoman JBI Critical Appraisal Checklist. Hasil: Dari 4.628 artikel, 6 artikel studi RCT dan 1 studi kuasi eksperimen diikutsertakan dalam analisis. Hasil analisis menunjukkan bahwa akupresur terbukti efektif untuk menurunkan intensitas haus, distres haus, dan xerostomia yang dialami pasien gagal jantung dan gagal ginjal kronis. Jenis akupresur dapat berupa akupresur tubuh maupun akupresur telinga. Kesimpulan: Terapi akupresur dapat menjadi alternatif intervensi keperawatan untuk mengatasi rasa haus pada pasien gagal jantung dan gagal ginjal kronis.

Abstract

Background: Thirst is a common complication experienced by patients with heart failure and chronic kidney disease who undergo fluid restriction programs. Without adequate management, thirst can exacerbate the underlying disease due to fluid overload resulting from excessive intake. Acupressure therapy is one of the non-pharmacological interventions that can be implemented by nurses to alleviate thirst. Objective: To explore the effectiveness of acupressure therapy in managing thirst among patients with heart failure and chronic kidney disease. Methods: This study employed a systematic review method based on the PRISMA guidelines. Articles were retrieved from databases including Pubmed, ScienceDirect, ProQuest, Sage Journals, and Taylor and Franchis, as well as other sources published before 2025. Article selection was conducted according to inclusion and exclusion criteria, followed by critical analysis using the JBI Crit ical Appraisal Checklist. Results: Out of 4,628 articles, 6 RCTs and 1 quasi-experimental study were included in the analysis. The results indicated that acupressure, both body and auricular acupressure, was effective in reducing thirst intensity, thirst distress, and xerostomia in patients with heart failure and chronic kidney disease. Conclusion: Acupressure therapy can serve as an alternative nursing intervention to alleviate thirst in patients with heart failure and chronic kidney disease.

Abstract

Intradialytic cycling is a form of aerobic exercise performed during hemodialysis sessions. Several studies have investigated its impact on both physical and psychological functional status, utilizing various parameters such as frequency, duration, exercise intensity, and with differing outcomes. The aim of this systematic review is to examine the effects of intradialytic cycling on physical and psychological functional status in patients undergoing hemodialysis.This systematic review was conducted in accordance with the PRISMA guidelines. Literature was sourced from databases including Scopus, ProQuest, SAGE Journals, SpringerLink, Taylor & Francis, and Cochrane, using keywords based on the PICO framework and combined with Boolean operators “AND” and “OR.” Inclusion criteria for the articles were: publication between 2010 and 2025, studies involving hemodialysis patients aged ≥18 years, research designs including randomized controlled trials (RCTs), quasi-experimental studies, and cohort studies, and publications in English. A total of 10 articles were included in the analysis. Intradialytic cycling demonstrated enhancing physical function and reducing depressive symptoms in hemodialysis patients. This non-pharmacological intervention proved to be effective both as a standalone therapy and in combination with other modalities, such as resistance training, music therapy, and cognitive approaches. However, further research is warranted to develop and adapt intradialytic cycling protocols that align with clinical practice settings in Indonesia

Abstract

Background: Sleep quality is an important yet understudied element influencing postoperative recovery in intensive care units. Considering that there is a lack of consistent knowledge of the quality of sleep, it is difficult to establish appropriate tests and therapies. Objective: To do a conceptual analysis of sleep quality in postoperative intensive care units patients utilizing the Walker and Avant framework, delineating its qualities, antecedents, consequences, and empirical referents. Methods: This study used the Walker and Avant eight-step process for idea analysis, which included a systematic literature evaluation of papers published between 2020 and 2024 in databases such as PubMed, Scopus, ScienceDirect, and ProQuest. Data was evaluated to determine important defining qualities, causes, outcomes, and measurable indicators of sleep quality. Results: The analysis identified four defining attributes of sleep quality: sleep architecture, sleep disturbances, subjective experience, and physiological indicators. Antecedents included environmental noise, lighting, pain, anxiety, and frequent medical interventions. Poor sleep quality was associated with adverse outcomes such as increased delirium risk, delayed wound healing, and prolonged intensive care units      stays. Empirical referents included patient-reported tools like the Pittsburgh Sleep Quality Index, polysomnography, and actigraphy. Conclusion: Sleep quality is a multidimensional concept central to postoperative care in intensive care unit. Addressing environmental and psychological factors through targeted interventions can improve sleep quality and enhance recovery outcomes.

Abstract

Spiritual well-being is a domain affecting the outcome of antiretroviral therapy in PLWH. However, spiritual need recently is still not adequately fulfilled in health service system. Therefore, it is essential to recognize the spiritual experience in PLWH to identify the depiction of their needs. This descriptive phenomenological study aimed to explore the spiritual experience of PLWH in the disease process by involving five participants in in-depth interviews. The thematic content analysis resulted five themes, namely (1) Obtaining guidance, being more religious, being more afraid of sin, more diligent in praying and reading the holy book; (2) Surrender and accept the illness because this is a warning, test and power of Allah; (3) Support from mother by encouraging to recitation and reminding to dhikr; (4) Experiencing sadness, shock, blaming others so that they want to end their life and avoid other people; (5) The participants hope to be healthy in this world and lucky in the hereafter. This study recommended further studies quantitatively and the assessment of PLWH spirituality in the nursing care practice.

Abstract

Background: Patients undergoing chemotherapy often experience persistent physical and psychological symptoms that are difficult to manage independently. Limitations in conventional monitoring have prompted the use of technology-based interventions as a supportive strategy for symptom management. Purpose: To determine the effectiveness of technology-based interventions in reducing symptom burden and improving self-management in chemotherapy patients. Method: A systematic review was conducted using the PRISMA framework. Articles published between 2013-2024 were searched across ProQuest, Scopus, PubMed, Clinical Key for Nursing, and Sage Journals. Inclusion criteria comprised quasi-experimental studies evaluating digital health technologies in chemotherapy patients, with quality assessment performed using the Critical Appraisal Skills Programme (CASP). Results: Ten studies met the inclusion criteria, which included the use of mobile apps, electronic patient-reported outcomes (ePRO) systems, telephone, SMS, and web-based platforms. Overall, digital interventions have the potential to reduce symptom burden and increase patient self-efficacy and active participation. The highest effectiveness was found in interventions that combined daily monitoring with follow-up by healthcare professionals, while adherence rates were found to be very low due to less interactive designs. Conclusion: Technology-based interventions can support symptom management and self-management in chemotherapy patients, provided they are integrated with responsive clinical support. Suggestion: Future development of digital interventions should prioritize the integration of clinical responses, adaptive user-centered design, and long-term evaluation to ensure sustained benefits for patients’ quality of life.   Keywords: Cancer; Chemotherapy; Self-Management; Symptom Burden; Technology-Based Interventions.   Pendahuluan: Pasien yang menjalani kemoterapi sering mengalami berbagai gejala fisik dan psikologis yang berkelanjutan serta sulit dikelola secara mandiri. Keterbatasan pemantauan konvensional mendorong penggunaan intervensi berbasis teknologi sebagai strategi pendukung dalam pengelolaan gejala. Tujuan: Untuk mengetahui efektivitas intervensi berbasis teknologi dalam menurunkan beban gejala dan meningkatkan self-management pada pasien kemoterapi. Metode: Penelitian systematic review ini dilakukan menggunakan kerangka kerja PRISMA. Pencarian artikel publikasi tahun 2014-2024 dillakukan pada database ProQuest, Scopus, PubMed, Clinical Key for Nursing, dan Sage Journals. Kriteria inklusi meliputi studi studi kuasi-eksperimental yang mengevaluasi teknologi kesehatan digital pada pasien kemoterapi, dengan penilaian kualitas menggunakan Critical Appraisal Skills Programme (CASP). Hasil: Sebanyak 10 studi memenuhi kriteria inklusi yang mencakup penggunaan aplikasi mobile, sistem electronic patient-reported outcomes (ePRO), telepon, SMS, dan platform web-based. Secara umum, intervensi digital berpotensi menurunkan beban gejala dan meningkatkan self-efficacy serta partisipasi aktif pasien. Efektivitas tertinggi ditemukan pada intervensi yang menggabungkan pemantauan harian dengan tindak lanjut dari tenaga kesehatan, sementara tingkat kepatuhan ditemukan sangat rendah karena desain yang kurang interaktif. Simpulan: Intervensi berbasis teknologi dapat mendukung pengelolaan gejala dan self-management pasien kemoterapi, terutama jika terintegrasi dengan dukungan klinis yang responsif. Saran: Pengembangan intervensi digital di masa depan perlu memprioritaskan integrasi respons klinis, desain adaptif berbasis kebutuhan pengguna, dan evaluasi jangka panjang untuk memastikan keberlanjutan manfaat bagi kualitas hidup pasien.   Kata Kunci: Beban Gejala; Intervensi Berbasis Teknologi; Kanker; Kemoterapi; Self-Management.

Abstract

Background: Discontinuation of antiretroviral therapy (ART) among people living with HIV (PLHIV) presents significant challenges for HIV care, often leading to physical deterioration, emotional distress, and social isolation. Although prior studies have addressed barriers to adherence, little is known about the experiences of PLHIV following treatment discontinuation in the Indonesian context. Objective: To explore the experiences of Indonesian PLHIV who have discontinued ART. Methods: This qualitative study employed a descriptive phenomenological approach and was conducted in three urban areas of Indonesia (Jakarta, Yogyakarta, and Bandar Lampung) between August and September 2024. Fifteen participants were purposively recruited from community-based organizations. In-depth semi-structured interviews were conducted, transcribed verbatim, and analyzed using Colaizzi’s method to identify themes and meanings in the narratives. Results: Three major themes emerged: (1) Physical and psychological consequences of ARV discontinuation, including fatigue, anxiety, and a sense of losing control; (2) Prolonged social strain and stigma, marked by loss of support and moral judgment from others; and (3) Searching for meaning and coping without treatment, including spiritual resignation and detachment from bodily control. These themes reflect the complex, multidimensional burdens PLHIV face after stopping treatment, particularly in the Indonesian context, where HIV stigma is deeply rooted in sociocultural and religious norms, and access to mental health support remains limited. Conclusion: ART discontinuation is not merely a clinical event but a deeply human experience shaped by physical decline, emotional conflict, social rejection, and existential adaptation. To improve long-term HIV care outcomes, healthcare systems must integrate psychosocial and spiritual support for individuals who disengage from treatment, encouraging re-engagement through nonjudgmental and person-centered care. For nursing practice, this study emphasizes the importance of developing empathetic communication, providing culturally sensitive counseling, and actively addressing stigma during care interactions.

Abstract

Introduction:Sleep disturbance is a frequent problem among critically ill patients and may adversely affect recovery. White noise has been proposed as a non-pharmacological approach to reduce environmental disruption and improve sleep; however, evidence from Indonesian intensive care units (ICUs) is currently lacking. Objectives:This study aimed to evaluate the effect of white noise exposure on sleep quality among critically ill patients treated in ICUs in Indonesia. Methods: A randomized controlled trial was performed in the ICUs of three public hospitals in West Java, Indonesia, involving adult ICU patients. The intervention group received white noise twice daily for three days, while controls received standard care. Sleep quality was measured using the RCSQ and analyzed with repeated-measures ANOVA, Cohen’s d, and GEE models. Results: Final analyses included 25 participants in the intervention group and 25 in the control group. Sleep quality in the intervention group showed a significant improvement at the third measurement point, with a moderate effect size (d = 0.42). No significant changes were observed in the control group. GEE analysis demonstrated a significantly greater improvement in sleep quality in the intervention group compared with the control group after three days (β = 6.43, p 0.001). Conclusions:White noise intervention was associated with improved sleep quality among critically ill ICU patients. These findings support the incorporation of acoustic management strategies into ICU care to enhance patient comfort and recovery.
