Abstract

Intradialytic cycling is a form of aerobic exercise performed during hemodialysis sessions. Several studies have investigated its impact on both physical and psychological functional status, utilizing various parameters such as frequency, duration, exercise intensity, and with differing outcomes. The aim of this systematic review is to examine the effects of intradialytic cycling on physical and psychological functional status in patients undergoing hemodialysis.This systematic review was conducted in accordance with the PRISMA guidelines. Literature was sourced from databases including Scopus, ProQuest, SAGE Journals, SpringerLink, Taylor & Francis, and Cochrane, using keywords based on the PICO framework and combined with Boolean operators “AND” and “OR.” Inclusion criteria for the articles were: publication between 2010 and 2025, studies involving hemodialysis patients aged ≥18 years, research designs including randomized controlled trials (RCTs), quasi-experimental studies, and cohort studies, and publications in English. A total of 10 articles were included in the analysis. Intradialytic cycling demonstrated enhancing physical function and reducing depressive symptoms in hemodialysis patients. This non-pharmacological intervention proved to be effective both as a standalone therapy and in combination with other modalities, such as resistance training, music therapy, and cognitive approaches. However, further research is warranted to develop and adapt intradialytic cycling protocols that align with clinical practice settings in Indonesia

Abstract

Lost to follow-up (LTFU) in the treatment process for HIV AIDS patients suffering from Tuberculosis is a serious problem that is a challenge for health services. This condition needs to be managed well so it must be treated immediately because the problem of Tuberculosis will have an impact not only on the sufferer himself but also cause an increase in the spread and transmission to other people. Handling the problems of HIV AIDS with tuberculosis infection needs to be managed well to ensure the implementation of the treatment process, to minimize the risk of dropping out. It is hoped that the use of digital applications in the form of telehealth will be able to have an effective function in controlling and integrating the treatment process being undertaken by HIV AIDS sufferers with tuberculosis infection. This systematic review aims to identify and provide a critical assessment. It is also to find evidence based on published articles, and carry out a synthesis of the articles obtained. This method uses a search process carried out through 5 databases (Ebsco Host, Proquest, Medline, PubMed, Cochrane), articles in English, published in 2014-2024 which were assessed by using JBI (Joanna Briggs Institute) as a guide in synthesizing the results searched and reviewed using PRISMA For Systematic reviews. The results obtained were 10 studies that described the use of digital or telehealth applications to combine adherence and loss to follow-up cases in tuberculosis treatment.  The use of telehealth can contribute to overcoming the problem of patients who are lost to follow-up and increasing adherence to HIV AIDS treatment with tuberculosis infection.

Abstract

HIV/AIDS remains a global health challenge, with adherence to antiretroviral therapy (ART) being crucial for its management. This comprehensive concept analysis explores adherence to ART in people living with HIV/AIDS, considering its multifaceted nature and the diverse factors influencing it. The purpose of this study was to explore the concept of adherence and define the operational definition of adherence to antiretroviral therapy in PLWH. Walker and Avant’s eight-step concept analysis was used in this study. The terms "adherence", "compliance", "concordance", and "definition" were used in a literature search that was done using the EBSCOhost, Proquest, Science Direct, and Google Scholar databases. The inclusion criteria were full text, written in English or Indonesian, and open access. This study identified four attributes that constitute the concept of adherence: the ability to follow recommended treatment recommendations, collaborative/cooperative whether forced or not, obedience/loyalty/discipline, adaptability and flexibility. The antecedents of adherence to ARV are patient-related factors, therapy-related factors, environment-related factors and health care system-related factors. The consequences of adherence are increasing health outcomes, reducing the transmission of HIV and health care costs. This concept analysis provides a comprehensive understanding of adherence, emphasizing the need for individualized assessments, patient education, stigma reduction, social support, and targeted interventions. Collaboration among healthcare providers, continuous education, and measurement tool refinement are essential for enhancing adherence rates, improving patient outcomes, and reducing the global burden of HIV/AIDS.

Abstract

Breast cancer is the most common cancer worldwide. Breast cancer has the highest rate around 42.1 per 100,000 in Indonesian. The most common problems experienced are physical problems such as pain. Pain is related to the patient's quality of life. Digital-based guided imagery with virtual reality in pain management are still rarely used. This study aims to determine the effectiveness of guided imagery using digital therapy virtual reality in breast cancer patients. The method uses RCT with a parallel design. 60 respondents who were randomized using block randomization into 30 in both control and intervention groups. Conducted at Dharmais Cancer Hospital using virtual reality for 10-15 minutes in intervention group and digital leaflets to control group. Measurements using the NRS were carried out every day for 7 days. The results showed that guided imagery with virtual reality was able to reduce the pain scale in breast cancer patients compared to the control group with P = 0.000 (? 0.05). The use of virtual reality can be used as a reference for oncology nurses in providing non-pharmacological therapy to reduce pain and stress in patients especially breast cancer patients.

Abstract

Spiritual care for cancer pain is an intervention to address the spiritual needs of patients with cancer in managing pain. Despite numerous studies conducted in this area, the diversity of spiritual care interventions for cancer pain remains a challenge. Objective: This literature review aimed to identify spiritual care interventions for cancer patients in managing pain. Method: The database used Scopus, Pubmed, Proquest, Science Direct, and Google Scholar. The keywords used were “spiritual care” OR “spiritual therapies” OR “spiritual healing” OR “faith healing” OR “spiritual intervention” OR “religious intervention” AND “cancer pain” OR “cancer-related pain” OR “neoplasm-related pain” OR “oncology pain. The search was limited to articles published between 2011 and 2021, resulting in the identification of 6642 articles, which only 10 met the inclusion criteria. The selected articles underwent critical appraisal using JBI tools. Results: This literature review showed that the types of spiritual care interventions consist of yoga, psychoreligious intervention, mindfulness, music therapy, and therapeutic touch, and the primary outcomes of spiritual care are pain levels. Conclusions: This literature review showed the interventions were very diverse in all types of cancer. Therefore, a spiritual care intervention study on managing pain with a specific type of cancer is needed.

Abstract

Sifon is a post-circumcision ritual of sexual intercourse in the Atoin Meto tribe with women who are not their partners. Sifon women have various reasons for doing this practice such as fulfilling their economic and daily needs. Most of Sifon women are the heads of families. In addition to having an impact on the risk of contracting HIV, AIDS and STD Infection, Sifon also has other impacts on women such as social stigma and shameful accusation. Objective:The purpose of this study is to explore the psychological experiences of Sifon women in Timor Island.Method:This study used a phenomenological approach and purposive sampling technique. Participants were with inclusion criteria. They have to be more than 18 years old and have practiced Sifon in the last 6 (six) months. Data collection was carried out from March to July 2020 in Kupang Regency and South Central Timor Regency, East Nusa Tenggara Province. The data was processed using Collaizi method.Results: There were 13 women who were successfully interviewed and 7 of them shared their psychological experiences. Most of them are farmers, have elementary school education and become the head of their families. Based on the results of data analysis, the researcher found 3 themes: (1) feeling shameful, (2) feeling humiliated and not worthed, (3) having trauma.Conclusions:The decision to become a Sifon woman was made with various considerations, including meeting economic needs and earming money for all family members. It is necessary to take a culturally sensitive approach and deeper psychological assistance with Sifon women to help improve their wellbeing. The findings of this study show that most of these Sifon women are heads of families.

Abstract

Patients with Chronic Obstructive Pulmonary Disease (COPD) are at high risk of weight loss. This weight loss can exacerbate malnutrition, which is closely associated with worsened lung function, increased hospitalization rates, decreased physical activity tolerance, reduced quality of life, and higher mortality. The Mini Nutritional Assessment (MNA) is a widely used questionnaire that provides clear information regarding nutritional status and helps assess malnutrition in patients. Objective to evaluate the effectiveness of the Mini Nutritional Assessment (MNA) in assessing nutritional status and predicting malnutrition in COPD patients. This article is written using a systematic review method, with a search strategy conducted across four databases, yielding a total of 871 articles. These articles were then screened based on inclusion and exclusion criteria, resulting in 7 articles from 2014-2024. The 7 selected articles were subjected to critical appraisal using the JBI Critical Appraisal Checklist. The review of the articles generally indicates that the MNA intervention is effective in assessing malnutrition in COPD patients. The MNA can be optimally used to predict malnutrition in COPD patients, particularly among the elderly.

Abstract

HIV AIDS cases continue to increase every year. In 2023, out of 1,414 HIV AIDS cases in North Kalimantan, Indonesia, 500 cases were in Tarakan region. This study aimed to reveal the situation of the extent of efforts of commercial sex workers (CSWs) in preventing HIV AIDS transmission in coastal and border areas of Tarakan. This research was qualitative with a descriptive phenomenological approach. This study involved 15 CSWs in the localization of Tarakan area as the participants. The data analysis used thematic analysis by Braun and Clarke. This study produced four themes, namely: (1) health checks were carried out on an irregular basis, (2) customers who refused to use condoms were still served, (3) the use of antibiotics to prevent HIV AIDS transmission, and (4) limiting customers. The prevention of HIV AIDS transmission by CSWs had not been maximized due to the limitations of economic background and negotiation skills and a lack of understanding of HIV AIDS. A more comprehensive educational approach was needed to raise awareness of the importance of self-prevention from infectious diseases to work, so that they can work for a long time.

Abstract

Aims: Critical patients in ICU and HCU often experience poor sleep due to excessive stress and various symptoms like environmental factors, pain, noise, anxiety, and physical discomfort. Foot massage and natural music interventions can help meet their sleep needs, providing a relaxing effect and improving physical and psychological conditions. The purpose of this study is to determine the effect of providing combination therapy of foot massage and natural music therapy in Dr. Hospital's peacock room Salamun 2023. Methods:  This type of research is pre-experimental with a pre-post test of one group and uses the Richard Champbell Sleep Questionnaire (RCSQ). Then the data was processed using a paired sample T-test. The sample in this study is 16 respondents. Results: The results of the statistical test obtained a p-value of 0.01, meaning that there was an effect between the pretest and posttest of the combination of foot massage and natural music therapy on the quality of sleep in critical patients in the peacock room at Dr. Salamun Hospital, Bandung City, 2023. Conclusions : the conclusion of this study is that there is an effect of foot massage therapy and natural music on the sleep quality of critical patients as shown by the results of measuring the quality of critical patients before and after consecutive interventions with massage of both feet combined with natural music. For 10 minutes it has been proven to be able to improve sleep quality in critically ill patients so that this intervention can be applied as a therapy to improve sleep quality.

Abstract

ABSTRACT Acute Coronary Syndrome (ACS), as the acute manifestation of Coronary Heart Disease (CHD), remains a major contributor to global morbidity and mortality, accounting for 85% of deaths from heart attacks and strokes (Universitas Muhammadiyah Magelang, 2025). In Indonesia, while the specific CHD prevalence was reported at 0.85% in 2023 (SKI, 2023), the incidence of ACS cases is high, exemplified by 840 cases reported in the ED of NCCHK Jakarta (January–May 2025). Recurrent ACS is a serious threat, primarily driven by poor patient adherence to secondary prevention regimens (Universitas Muhammadiyah Magelang, 2025) and atherosclerosis progression(Universitas Hasanuddin, 2025). This is compounded by profound physical (chronic fatigue, pain) and psychological impacts (anxiety, PTSD) (Jackson, 2021; Lee & Chen, 2023). Given the complexity of medication management challenges (Smith, 2021) and the need for holistic healthcare support (Brown, 2020), this study aims to explore the subjective experiences and preventive behaviors of patients facing recurrent ACS. Method: This qualitative study utilized an exploratory phenomenological approach to describe preventive behaviors and analyze risk factors for recurrent heart attacks in ACS patients. Results: Five key themes were identified: 1) Physical Impact, including chest pain, weakness, and reduced functional capacity. 2) Psychosocial Impact, encompassing anxiety, fear, restlessness, and thoughts of death. 3) Post-Attack Medication Management, highlighting adherence barriers and treatment ineffectiveness due to irregular medication consumption. 4) Behavioral Changes, involving the adoption of positive healthy behaviors. 5) Healthcare Support, indicating a strong need for more optimal, comprehensive, and continuous health education from healthcare providers. Conclusion: The study identifies five core themes detailing the complex post-recurrent ACS experience, dominated by significant physical and psychological burdens. Non-adherence to treatment exacerbates this experience, underscoring the necessity for more holistic, patient-centered healthcare support.  Keywords: Patient, Experience, Recurrent ACS     ABSTRAK Sindrom Koroner Akut (SKA), sebagai manifestasi akut Penyakit Jantung Koroner (PJK), merupakan kontributor signifikan terhadap morbiditas dan mortalitas global, menyumbang 85% kematian akibat serangan jantung dan stroke (Universitas Muhammadiyah Magelang, 2025). Di Indonesia, meskipun prevalensi PJK spesifik berada pada 0,85% pada tahun 2023 (Survei Kesehatan Indonesia, 2023).Angka kejadian SKA di indonesiacukup tinggi dimana  berdasarkan Data Laporan Unit Gawat Darurat  di National Cardiovascular Center Harapan Kita (NCCHK) Jakarta terdapat 840 kasus SKA periode bulan Januari 2025 sampai Mei 2025.Serangan SKA berulang menjadi ancaman serius, terutama akibat ketidakpatuhan pasien terhadap regimen pengobatan sekunder (Universitas Muhammadiyah Magelang, 2025) dan progresivitas aterosklerosis (Universitas Hasanuddin, 2025), yang diperparah oleh dampak fisik (kelelahan kronis) dan psikologis mendalam, termasuk kecemasan, depresi, dan PTSD (Jackson, 2021; Lee & Chen, 2023). Mengingat kompleksitas tantangan manajemen pengobatan (Smith, 2021) dan kebutuhan akan dukungan layanan kesehatan holistik (Brown, 2020).  Penelitian kualitatif fenomenologi dengan eksploratif gambaran prilaku pencegahan  terhadap risiko serangan jantung berulang dan menganalisa faktor risiko serangan jantung berulang pada pasien SKA. Tema pertama yang teridentifikasi adalah dampak fisik, berupa gejala fisik dan penurunan kapasitas fungsional, gejala nyeri dada, lemas, kaku leher, sesak nafas. Tema kedua yang teridentifikasi adalah dampak psikososial serangan jantung berulang, yaitu perasaan cemas, takut, gelisah, hingga munculnya pikiran akan kematian Temaketiga Managemen pengobatan paska serangan meliputi kepatuhan pengobatan dan inefektif pengobatan, ketidakteraturan dalam konsumsi obat merupakan hambatan utama dalam efektivitas pengobatan pada pasien Sindrom Koroner Akut (SKA). Tema keempat perubahan perilaku, yaitu melakukan perubahan perilaku positif. Temakelima dukungan layanan kesehatan paska serangan berulang, yaitu menunjukkan kebutuhan yang kuat terhadap dukungan layanan kesehatan yang lebih optimal dalam bentuk edukasi yang komprehensif dan berkelanjutan. Penelitian ini mengidentifikasi lima tema kunci yang menggambarkan pengalaman kompleks paska serangan jantung berulang, dimulai dari dampak fisik yang signifikan dan psikososial. Ketidakpatuhan terhadap pengobatan merupakan keseluruhan pengalaman ini diperburuk oleh tantangan dalam manajemen pengobatan dan kebutuhan akan dukungan layanan kesehatan yang lebih holistik yang  berpusat pada pasien. Kata Kunci:  Pasien, Pengalaman , SKA berulang
