Abstract

Cancer patients often experience a variety of signs and symptoms that can significantly impact their quality of life. This research aims to apply the Theory of Planned Behavior to nursing care for cancer patients. This approach emphasizes the importance of attitudes, subjective norms, and perceived behavioral control in forming nursing diagnoses. n the context of cancer patients, understanding the factors of the Theory of Planned Behavior, namely attitudes toward the behavior, subjective norms, and perceived behavioral control, can help identify effective interventions to improve medication adherence, the adoption of a healthy lifestyle, and participation in follow-up care. This framework can increase understanding and improve patient care. The research method used is a case study approach. The research results show that the application of the Theory of Planned Behavior allows nurses to provide holistic care involving various aspects of cancer patients' lives including physical, emotional, social, spiritual and cultural. By understanding the concept of the Theory of Planned Behavior, nurses can recognize that changing a patient's behavioral habits requires planning, where from an accepting attitude to becoming a habit of carrying out therapy requires a process and needs support from both family and health workers so that goals and outcomes can be achieved.

Abstract

Background: The loss of kidney function in patients with kidney disease leads to the accumulation of dissolved substances known as uremic toxins, which can negatively impact patient health. Membranes with a high number of pores, such as those in high-flux dialyzers, allow some medium-sized molecules like beta-2 microglobulin to pass through. However, these membranes are less effective at filtering out larger dissolved substances. Larger medium-sized molecules (>25 kD) need to be removed either by convection or through the use of highly permeable membranes such as Medium Cut-Off (MCO) membranes. Therefore, it is important to understand the improvement in uremic toxin clearance when using MCO dialyzers compared to high-flux dialyzers. Purpose: To evaluate and compare the effectiveness of using medium cut-off (MCO) membrane dialyzer with high flux membrane dialyzer. Method: A systematic review with article selection guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The systematic review process begins with the formulation of clinical questions relevant to the topic. Before this, the authors establish PICOS criteria, which stand for: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study design). For this article, P: Patients with chronic kidney disease undergoing hemodialysis, I: Use of dialyzers with medium cut-off (MCO) membrane, C: Use of dialyzer with high flux membrane, O: Increased clearance of uremic toxins of middle to large molecules, such as β2-microglobulin, kappa and lambda free light chains, S: Randomized controlled trials. The article search was conducted systematically using the keywords: "Medium cut-off dialyzer" AND "High flux dialyzer" AND "Uremic toxins" AND "Middle molecules" AND "Large molecules" AND "Clearance Hemodialysis". Results: The use of MCO membranes was more effective in eliminating medium-to-large uremic toxins while maintaining safety and regulation comparable to conventional hemodialysis with high-flux dialyzers. MCO membranes were also more effective even with low blood flow, without significantly increasing albumin loss. Additionally, MCO membranes contributed to reducing inflammation and allowed for shorter recovery times post-hemodialysis treatment. Conclusion: Given its comparable safety to high-flux dialyzers, the use of MCO membranes can be expanded in clinical practice at hemodialysis centers, including those in resource-limited settings.

Abstract

Lost to follow-up (LTFU) in the treatment process for HIV AIDS patients suffering from Tuberculosis is a serious problem that is a challenge for health services. This condition needs to be managed well so it must be treated immediately because the problem of Tuberculosis will have an impact not only on the sufferer himself but also cause an increase in the spread and transmission to other people. Handling the problems of HIV AIDS with tuberculosis infection needs to be managed well to ensure the implementation of the treatment process, to minimize the risk of dropping out. It is hoped that the use of digital applications in the form of telehealth will be able to have an effective function in controlling and integrating the treatment process being undertaken by HIV AIDS sufferers with tuberculosis infection. This systematic review aims to identify and provide a critical assessment. It is also to find evidence based on published articles, and carry out a synthesis of the articles obtained. This method uses a search process carried out through 5 databases (Ebsco Host, Proquest, Medline, PubMed, Cochrane), articles in English, published in 2014-2024 which were assessed by using JBI (Joanna Briggs Institute) as a guide in synthesizing the results searched and reviewed using PRISMA For Systematic reviews. The results obtained were 10 studies that described the use of digital or telehealth applications to combine adherence and loss to follow-up cases in tuberculosis treatment.  The use of telehealth can contribute to overcoming the problem of patients who are lost to follow-up and increasing adherence to HIV AIDS treatment with tuberculosis infection.

Abstract

HIV/AIDS remains a global health challenge, with adherence to antiretroviral therapy (ART) being crucial for its management. This comprehensive concept analysis explores adherence to ART in people living with HIV/AIDS, considering its multifaceted nature and the diverse factors influencing it. The purpose of this study was to explore the concept of adherence and define the operational definition of adherence to antiretroviral therapy in PLWH. Walker and Avant’s eight-step concept analysis was used in this study. The terms "adherence", "compliance", "concordance", and "definition" were used in a literature search that was done using the EBSCOhost, Proquest, Science Direct, and Google Scholar databases. The inclusion criteria were full text, written in English or Indonesian, and open access. This study identified four attributes that constitute the concept of adherence: the ability to follow recommended treatment recommendations, collaborative/cooperative whether forced or not, obedience/loyalty/discipline, adaptability and flexibility. The antecedents of adherence to ARV are patient-related factors, therapy-related factors, environment-related factors and health care system-related factors. The consequences of adherence are increasing health outcomes, reducing the transmission of HIV and health care costs. This concept analysis provides a comprehensive understanding of adherence, emphasizing the need for individualized assessments, patient education, stigma reduction, social support, and targeted interventions. Collaboration among healthcare providers, continuous education, and measurement tool refinement are essential for enhancing adherence rates, improving patient outcomes, and reducing the global burden of HIV/AIDS.
