Abstract

Background: The loss of kidney function in patients with kidney disease leads to the accumulation of dissolved substances known as uremic toxins, which can negatively impact patient health. Membranes with a high number of pores, such as those in high-flux dialyzers, allow some medium-sized molecules like beta-2 microglobulin to pass through. However, these membranes are less effective at filtering out larger dissolved substances. Larger medium-sized molecules (>25 kD) need to be removed either by convection or through the use of highly permeable membranes such as Medium Cut-Off (MCO) membranes. Therefore, it is important to understand the improvement in uremic toxin clearance when using MCO dialyzers compared to high-flux dialyzers. Purpose: To evaluate and compare the effectiveness of using medium cut-off (MCO) membrane dialyzer with high flux membrane dialyzer. Method: A systematic review with article selection guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The systematic review process begins with the formulation of clinical questions relevant to the topic. Before this, the authors establish PICOS criteria, which stand for: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study design). For this article, P: Patients with chronic kidney disease undergoing hemodialysis, I: Use of dialyzers with medium cut-off (MCO) membrane, C: Use of dialyzer with high flux membrane, O: Increased clearance of uremic toxins of middle to large molecules, such as β2-microglobulin, kappa and lambda free light chains, S: Randomized controlled trials. The article search was conducted systematically using the keywords: "Medium cut-off dialyzer" AND "High flux dialyzer" AND "Uremic toxins" AND "Middle molecules" AND "Large molecules" AND "Clearance Hemodialysis". Results: The use of MCO membranes was more effective in eliminating medium-to-large uremic toxins while maintaining safety and regulation comparable to conventional hemodialysis with high-flux dialyzers. MCO membranes were also more effective even with low blood flow, without significantly increasing albumin loss. Additionally, MCO membranes contributed to reducing inflammation and allowed for shorter recovery times post-hemodialysis treatment. Conclusion: Given its comparable safety to high-flux dialyzers, the use of MCO membranes can be expanded in clinical practice at hemodialysis centers, including those in resource-limited settings.

Abstract

Lost to follow-up (LTFU) in the treatment process for HIV AIDS patients suffering from Tuberculosis is a serious problem that is a challenge for health services. This condition needs to be managed well so it must be treated immediately because the problem of Tuberculosis will have an impact not only on the sufferer himself but also cause an increase in the spread and transmission to other people. Handling the problems of HIV AIDS with tuberculosis infection needs to be managed well to ensure the implementation of the treatment process, to minimize the risk of dropping out. It is hoped that the use of digital applications in the form of telehealth will be able to have an effective function in controlling and integrating the treatment process being undertaken by HIV AIDS sufferers with tuberculosis infection. This systematic review aims to identify and provide a critical assessment. It is also to find evidence based on published articles, and carry out a synthesis of the articles obtained. This method uses a search process carried out through 5 databases (Ebsco Host, Proquest, Medline, PubMed, Cochrane), articles in English, published in 2014-2024 which were assessed by using JBI (Joanna Briggs Institute) as a guide in synthesizing the results searched and reviewed using PRISMA For Systematic reviews. The results obtained were 10 studies that described the use of digital or telehealth applications to combine adherence and loss to follow-up cases in tuberculosis treatment.  The use of telehealth can contribute to overcoming the problem of patients who are lost to follow-up and increasing adherence to HIV AIDS treatment with tuberculosis infection.

Abstract

HIV/AIDS remains a global health challenge, with adherence to antiretroviral therapy (ART) being crucial for its management. This comprehensive concept analysis explores adherence to ART in people living with HIV/AIDS, considering its multifaceted nature and the diverse factors influencing it. The purpose of this study was to explore the concept of adherence and define the operational definition of adherence to antiretroviral therapy in PLWH. Walker and Avant’s eight-step concept analysis was used in this study. The terms "adherence", "compliance", "concordance", and "definition" were used in a literature search that was done using the EBSCOhost, Proquest, Science Direct, and Google Scholar databases. The inclusion criteria were full text, written in English or Indonesian, and open access. This study identified four attributes that constitute the concept of adherence: the ability to follow recommended treatment recommendations, collaborative/cooperative whether forced or not, obedience/loyalty/discipline, adaptability and flexibility. The antecedents of adherence to ARV are patient-related factors, therapy-related factors, environment-related factors and health care system-related factors. The consequences of adherence are increasing health outcomes, reducing the transmission of HIV and health care costs. This concept analysis provides a comprehensive understanding of adherence, emphasizing the need for individualized assessments, patient education, stigma reduction, social support, and targeted interventions. Collaboration among healthcare providers, continuous education, and measurement tool refinement are essential for enhancing adherence rates, improving patient outcomes, and reducing the global burden of HIV/AIDS.

Abstract

Breast cancer is the most common cancer worldwide. Breast cancer has the highest rate around 42.1 per 100,000 in Indonesian. The most common problems experienced are physical problems such as pain. Pain is related to the patient's quality of life. Digital-based guided imagery with virtual reality in pain management are still rarely used. This study aims to determine the effectiveness of guided imagery using digital therapy virtual reality in breast cancer patients. The method uses RCT with a parallel design. 60 respondents who were randomized using block randomization into 30 in both control and intervention groups. Conducted at Dharmais Cancer Hospital using virtual reality for 10-15 minutes in intervention group and digital leaflets to control group. Measurements using the NRS were carried out every day for 7 days. The results showed that guided imagery with virtual reality was able to reduce the pain scale in breast cancer patients compared to the control group with P = 0.000 (? 0.05). The use of virtual reality can be used as a reference for oncology nurses in providing non-pharmacological therapy to reduce pain and stress in patients especially breast cancer patients.

Abstract

ABSTRACT Acute Coronary Syndrome (ACS), as the acute manifestation of Coronary Heart Disease (CHD), remains a major contributor to global morbidity and mortality, accounting for 85% of deaths from heart attacks and strokes (Universitas Muhammadiyah Magelang, 2025). In Indonesia, while the specific CHD prevalence was reported at 0.85% in 2023 (SKI, 2023), the incidence of ACS cases is high, exemplified by 840 cases reported in the ED of NCCHK Jakarta (January–May 2025). Recurrent ACS is a serious threat, primarily driven by poor patient adherence to secondary prevention regimens (Universitas Muhammadiyah Magelang, 2025) and atherosclerosis progression(Universitas Hasanuddin, 2025). This is compounded by profound physical (chronic fatigue, pain) and psychological impacts (anxiety, PTSD) (Jackson, 2021; Lee & Chen, 2023). Given the complexity of medication management challenges (Smith, 2021) and the need for holistic healthcare support (Brown, 2020), this study aims to explore the subjective experiences and preventive behaviors of patients facing recurrent ACS. Method: This qualitative study utilized an exploratory phenomenological approach to describe preventive behaviors and analyze risk factors for recurrent heart attacks in ACS patients. Results: Five key themes were identified: 1) Physical Impact, including chest pain, weakness, and reduced functional capacity. 2) Psychosocial Impact, encompassing anxiety, fear, restlessness, and thoughts of death. 3) Post-Attack Medication Management, highlighting adherence barriers and treatment ineffectiveness due to irregular medication consumption. 4) Behavioral Changes, involving the adoption of positive healthy behaviors. 5) Healthcare Support, indicating a strong need for more optimal, comprehensive, and continuous health education from healthcare providers. Conclusion: The study identifies five core themes detailing the complex post-recurrent ACS experience, dominated by significant physical and psychological burdens. Non-adherence to treatment exacerbates this experience, underscoring the necessity for more holistic, patient-centered healthcare support.  Keywords: Patient, Experience, Recurrent ACS     ABSTRAK Sindrom Koroner Akut (SKA), sebagai manifestasi akut Penyakit Jantung Koroner (PJK), merupakan kontributor signifikan terhadap morbiditas dan mortalitas global, menyumbang 85% kematian akibat serangan jantung dan stroke (Universitas Muhammadiyah Magelang, 2025). Abstract

Cancer patients usually experience disturbing nausea and vomiting, although they have consumed some medicine before, during, and after chemotherapy. Chemotherapy-induced nausea and Vomiting (CINV) is nausea and vomiting experienced by patients as a result of chemotherapy. CINV is the most significant effect of chemotherapy from the patient’s perspective. CINV is reported as acute, delayed, and combined from both cases. CINV that are not managed properly can cause some problems, such as fluid and electrolyte imbalance, interrupt the next chemotherapy cycle, and decrease productivity and quality of life. Rehospitalization costs caused by CINV in the health care system are high. By the CINV management that is being improved in this time, some patients still experienced nausea and vomiting. Nowadays, non-pharmacological therapy like relaxation techniques is very suggested for patients to relieve CINV. Twenty-five minutes of an audio relaxation technique is effective in decreasing nausea and vomiting in cancer patients. One of the audio relaxation techniques is Progressive Muscle Relaxation (PMR). The case report was taken from one of the type A hospitals in Jakarta, Indonesia . It consisted of 20 cancer patients who underwent chemotherapy. After doing PMR exercise guided by an educational video on a tablet for 4 days after chemotherapy, it was found that there was a 35% decrease in nausea and vomiting scores; they felt comfortable and more relaxed. The interesting educational video of PMR supports the patients to do the exercise well when done in the hospital or at home. Therefore, preventing or reducing the potential problem caused by nausea and vomiting induced by chemotherapy can increase the nursing care quality for the cancer patient. The results of this case report indicate that progressive muscle relaxation can reduce nausea and vomiting due to chemotherapy.

Abstract

Background: Patients undergoing cardiac surgery often experience decreased self efficacy in managing their health condition, which can complicate recovery and reduce quality of life. Structured discharge planning programs provide ongoing support and instruction, which can help patients improve this self-efficacy. Purpose: To examine the role of structured education within discharge planning programs in improving self-efficacy in cardiac surgery patients. Method: This systematic review used PRISMA guidelines and included data published between 2020 and 2025 from the following databases: PubMed (n = 404), Science Direct (n = 24), ClinicalKey for Nursing (n = 769), Sage Journals (n = 47), ProQuest (n = 1063), and Wiley (n = 97). Results: Nine of the ten studies showed that patients who received structured discharge planning interventions, whether through direct instruction, technology (phone, WhatsApp, and augmented reality/AR), or home care, had higher levels of self efficacy. Furthermore, this intervention reduced readmission rates, depression, anxiety, and fatigue. Conclusion: A structured discharge planning program is effective in improving patient self efficacy after cardiac surgery and provides additional benefits on psychological aspects and quality of life. This program can be consistently implemented in patients undergoing cardiac surgery. Suggestion: Further research should be conducted in hospitals in Indonesia to determine the success of structured discharge planning in improving the self efficacy of cardiac surgery patients by linking local culture, family roles, and health systems.   Keywords: Cardiac Surgery; Discharge Planning Program; Patients; Self Efficacy; Structured Education.   Pendahuluan: Pasien yang menjalani operasi jantung sering mengalami penurunan efikasi diri dalam mengelola kondisi kesehatan pasien sendiri, yang dapat mempersulit pemulihan dan menurunkan kualitas hidup.
