Abstract

Accurate hemodynamic assessment is critical for guiding fluid therapy and avoiding fluid overload in patients with heart failure (HF). Leg elevation maneuvers, including passive leg raising (PLR) and leg lifting (PLL), have emerged as functional diagnostic interventions. Still, their multifaceted roles across different HF phenotypes have not been fully synthesized. This literature review aimed to evaluate the effect of leg elevation on preload responsiveness and its broader clinical applications in adult patients with HF. A comprehensive search of online studies published between 2019 and 2025 was conducted across multiple databases (Sage Journals, Scopus, ScienceDirect, PubMed, Wiley Online Library, and ProQuest). Keywords included “heart failure”, “passive leg raising”, and “hemodynamics.” A total of 1551 articles were identified, and 5 articles were thoroughly reviewed. Five articles that met the inclusion criteria were systematically analyzed to synthesize evidence on the impact of PLR on key hemodynamic parameters. Leg elevation serves distinct, context-dependent roles. PLR/PLL is a provocative test in patients with suspected HF with preserved ejection fraction (HFpEF), confirming latent diastolic dysfunction with a significant PCWP increase. A blunted increase in the stroke volume index (SVi) during PLR/PLL signifies exhausted preload reserve and is an independent predictor of poor clinical outcomes in patients with HF with reduced ejection fraction (HFrEF). A significant decrease in PPV during the maneuver accurately predicts fluid responsiveness, which is particularly crucial for vulnerable patients, such as those with heart failure, as it remains accurate even with low-tidal-volume ventilation. Leg elevation is a versatile and reliable non-invasive maneuver with a multifaceted role in the management of HF. Beyond simply predicting fluid responsiveness, it functions as a crucial diagnostic tool for HFpEF, a prognostic tool for HFrEF, and an accurate predictor of fluid needs in critical care settings. Integrating PLR/PLL into clinical practice allows for precise, physiology-based decision-making, fluid management optimization, and improved patient outcomes.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a major global health burden and is projected by the World Health Organization (WHO) to become the third leading cause of mortality by 2030. This trend highlights the urgent need for multidisciplinary healthcare management to maintain patient stability and improve outcomes. Objective to evaluate the effectiveness of non-pharmacological breathing exercise techniques and patient positioning strategies in reducing dyspnea, enhancing lung function, improving nocturnal symptoms, and supporting overall quality of life in individuals with COPD. This systematic review followed PRISMA 2020 guidelines. Literature searches were conducted in PubMed, Scopus, ProQuest, EBSCO, SAGE Journals, ScienceDirect, and ClinicalKey using MeSH terms, keywords, and Boolean operators. The search strategy used combinations of the following main keywords: COPD, breathing exercise, breathing technique, positioning, pulmonary rehabilitation, dyspnea, nocturnal symptoms, sleep disturbances. Inclusion criteria were Randomized Controlled Trials (RCTs), adult participants (≥19 years) and publication years 2020–2025. A total of 2,695 articles were identified. After screening, removal of duplicates, and full-text assessment, 14 studies met the inclusion criteria. Analysis of the 14 included studies demonstrated that a variety of non-pharmacological breathing techniques particularly when combined with optimal patient positioning, pulmonary rehabilitation, supportive devices, complementary therapies, and virtual-reality–based interventions consistently improved lung function, functional capacity, dyspnea, and health-related quality of life in patients with COPD. None of the included studies reported adverse effects related to the interventions. Breathing exercise techniques and therapeutic positioning are effective non-pharmacological strategies for managing COPD symptoms.

Abstract

Background: Sleep quality is an important yet understudied element influencing postoperative recovery in intensive care units. Considering that there is a lack of consistent knowledge of the quality of sleep, it is difficult to establish appropriate tests and therapies. Objective: To do a conceptual analysis of sleep quality in postoperative intensive care units patients utilizing the Walker and Avant framework, delineating its qualities, antecedents, consequences, and empirical referents. Methods: This study used the Walker and Avant eight-step process for idea analysis, which included a systematic literature evaluation of papers published between 2020 and 2024 in databases such as PubMed, Scopus, ScienceDirect, and ProQuest. Data was evaluated to determine important defining qualities, causes, outcomes, and measurable indicators of sleep quality. Results: The analysis identified four defining attributes of sleep quality: sleep architecture, sleep disturbances, subjective experience, and physiological indicators. Antecedents included environmental noise, lighting, pain, anxiety, and frequent medical interventions. Poor sleep quality was associated with adverse outcomes such as increased delirium risk, delayed wound healing, and prolonged intensive care units      stays. Empirical referents included patient-reported tools like the Pittsburgh Sleep Quality Index, polysomnography, and actigraphy. Conclusion: Sleep quality is a multidimensional concept central to postoperative care in intensive care unit. Addressing environmental and psychological factors through targeted interventions can improve sleep quality and enhance recovery outcomes.

Abstract

Background: A cancer diagnosis often has a significant impact on patients’ psychological aspects, particularly on self-acceptance. Purpose: To identify and summarize various factors influencing self-acceptance in cancer patients. Method: The selection process followed the PRISMA guidelines. Literature searches were conducted in PubMed, Scopus, and Google Scholar databases for studies published between 2010 and 2025, including English-language articles with quantitative study designs (cross-sectional, cohort, case-control, or case report), involving cancer patients aged ≥18 years. Results: Nine studies met the criteria for further analysis. Factors influencing self-acceptance included sociodemographic factors (age, gender, education, income, residence, marital status, employment status, social support, educational level, economic status), disease-related factors (TNM stage, metastasis status, disease duration), and psychosocial factors (social support, meaning-making, fear of recurrence). Conclusion: Self-acceptance in cancer patients is influenced by sociodemographic, disease-related, and psychosocial factors. These findings are important for developing more targeted psychosocial interventions to improve the well-being of cancer patients.

Abstract

Background: Malnutrition is one of the major complications among patients with chronic obstructive pulmonary disease (COPD) due to increased energy requirements and chronic systemic inflammation. This condition may worsen respiratory function, increase the risk of exacerbations, and reduce patients’ quality of life. Nutritional education plays an important role in preventing malnutrition; however, its implementation by nurses remains suboptimal. Purpose: To identify the relationship between nurses’ knowledge and attitudes and the implementation of nutritional education for malnutrition prevention among patients with COPD. Method: A cross-sectional study was conducted among 65 respondents providing nursing care to patients with COPD at Dr. H. Abdul Moeloek Regional General Hospital, Lampung, using a total sampling technique. Data were analyzed using Spearman’s correlation test and multiple linear regression. Results: There was a significant relationship between knowledge and the implementation of nutritional education (r=0.443, p<0.001), as well as between attitude and the implementation of nutritional education (r=0.718, p<0.001). No significant relationships were found between age, gender, educational level, length of employment, history of nutrition training, or experience in providing care for patients with COPD and the implementation of nutritional education (p>0.05). Attitude was identified as the variable most strongly associated with the implementation of nutritional education. Conclusions: Nurses’ knowledge and attitudes were significantly associated with the implementation of nutritional education as an effort to prevent malnutrition among patients with COPD. Attitude was the most dominant factor associated with the implementation of nutritional education.
